1.One case of postoperative nursing care for patient undergoing domino liver transplantation with two living donors

Objective To probe into the methods of postoperative nursing care for the patient undergoing domino liver transplantation.Methods The key of postoperative care included:monitoring of vital signs,the condition of cruor and the state of drainage rigorously; keeping the balance of intake and output; taking the immunosuppressive drugs correctly; proper diet; prevention of complication;giving psychological nursing.Results The patients passed the perioperative period successfully and were all cured.Conclusions To aim at the characteristic of this operation,implement holistic nursing care,is an important link to assure the patients of domino liver transplantation to recover smoothly after the operation.

3.Rehabilitation exercise and restenosis following coronary artery stent implantation

OBJECTIVE:To explore the effect of rehabilitation training on restenosis following stent implantation in patients with coronary atherosclerotic heart disease METHODS:A computer.based online search of VIP database was performed to search articles regarding restenosis following stent implantation and rehabilitation treatment for patients with coronary atherosclerotic heart disease,published between January 2000 and October 2009,with key words"coronary artery,stent implantation,intracoronary restenosis,rehabilitation exercise".The data were collected,and the references of each article were reviewed A total of 20 articles were included.RESULTS:Intracoronary stent has become an effective treatment for coronary atherosclerotic heart disease,and intracoronary restenosis has been focused in clinical studies.Prophase treatment following stent implantation is very important for coronary atherosclerotic heart disease.Rehabilitation exercise in combination with antiplatelet anticoagulatiOn and lipid treatment can reconstruct coronary arterial blood transport and significantly reduce incidence of restenosis and thrombus.CONCLUSION:Regular rehabilitation exercise following stent implantation can ameliorate degree of vascular restenosis.
4.The effect of PARP inhibitor on the delayed development cerebral vasospasm after subarachnoid hemorrhage and inflammatory factors

ABSTRACT:Objective To investigate the effects of poly ADP-ribose polymerase (PARP ) inhibitor 3-aminobenzene (3-AB)on the delayed development cerebral vasospasm (DCVS)after subarachnoid hemorrhage (SAH)and on the inflammatory factors,namely monocyte chemotactic protein 1 (MCP-1 )and hypersensitive c-reactive protein (hsCRP),and to explore the relationship between these and the signaling pathway of NF-kappa B (NF-κB).Methods Eighty male Sprague-Dawley rats were randomly divided into four groups:normal group (n =8),sham-operation group (n =8),SAH model group (n =32)and 3-AB group (n =32).We established 64 SAH model animals by double injection of blood into the cisterna magna.Half of the SAH model animals were treated with 3-AB by intraperitoneal injection (30 mg/kg).These rats were killed to obtain specimens respectively at days 3, 5,7 and 14 after the second blood injection.The morphological changes of basilar arteries were observed under the light microscope.The contents of PARP,MCP-1 and hsCRP in brain tissues were detected with enzyme-linked immunosorbent assay (ELISA).The expression of NF-κB in basilar arteries was determined by immunohistochemistry.
Results Compared with those in the sham-operation group,the degree of basilar artery spasm reached the peak [(30.47±3.89)%]at day 5 after established SAH model;the thickness and diameter of basilar artery were (1 6.44 ±1.32)μm and (1 78.21 ± 1 1.13)μm,respectively.Cerebral blood flow was reduced by nearly 60% (P <0.01 ). The expression of NF-κB in the cytoplasm and nucleus and PARP content in brain tissue were both increased significantly (P < 0.01 ).MCP-1 [(365.29 ± 28.08 )pg/mL ] and hsCRP [(402.1 6 ± 48.99 )ng/mL ] were significantly enhanced (P <0.01).Compared with the SAH group,after 5 days’intervention with 3-AB,there was obvious alleviation in the spasm degree of basilar artery [(22.65±3.21)%],the thickness [(14.89±1.27)μm]and diameter [(1 98.56±10.91)μm],respectively (P <0.01).Cerebral blood flow was significantly enhanced,but the expression of NF-κB in the cytoplasm and nucleus was decreased and PARP in brain tissue was significantly decreased (P < 0.01 ).MCP-1 [(126.5 1 ± 18.67 )pg/mL]and hsCRP [(285.39 ± 39.07 )ng/mL]in brain tissue were significantly declined,respectively (P <0.01).Conclusion PARP inhibitor 3-AB can alleviate DCVS and inhibit the inflammatory response in brain tissue after SAH.The mechanism may be related to NF-κB signaling pathway.
5.Application of Simulator Image Workstation in Making and Verification of Integrated Blocks

Objective To apply simulator image workstation to making and verification of integrated blocks.Methods The outline of blocks drawn by doctors at simulator image workstation were transferred to the auto-cutting machine.The blocks were fabricated and were verified by the simulator.Then the qualified ones were verified by the linear accelerator.Results Of 1 371 blocks selected to verify by the simulator,1 107 (80.7%) blocks were qualified;Of 1 371 blocks qualified by the simulator,1 345(98.1%) blocks were qualified by the linear accelerator.Conclusion Simulator image workstation can improve efficiency and reduce the error in making blocks.
6.Clinic Efficacy of Tongqiao Biyan Granules in the Treatment of Sinusitis

Objective:To evaluate the clinical efficacy and safety of Tongqiao Biyan granules in the treatment of sinusitis. Meth-ods:Totally 240 cases of sinusitis were randomly divided into two groups, each group had 120 cases. The patients in observation group were treated with 1 bag of Tongqiao Biyuan granules, 3 times a day, while the patients in control group were treated with busesonide nasal spray 100 μg each side nasal cavity, once a day. After 3 weeks,curative effect and adverse drug reaction of the two groups were observed. Results:After the treatment, nasal congestion, runny nose, headache, cough and other signs and symptoms were improved more significantly in the observation group than those in the control group (P<0. 05), the total effective rate of the observation group was 92. 5%, while that of the control group was 79. 2%, the difference was significant (P<0. 05). During the treatment, the two groups showed no obvous adverse drug reactions. Conclusion: The effect of Tongqiao Biyuan granules in the treatment of sinusitis is significant with no obvious adverse drug reactions, which should be recommended to use in clinic.
7.Determination of Nucleosides in Cordyceps sinensis Preparation by Dual-wavelength TLC-scanning

OBJECTIVE:To establish a dualwavelength TLC-scanning method for the determination of nucleosides in the preparation of Cordyceps sinensis.METHODS: The sample was analyzed on silica GF254 thin layer plate using 1% CMC-Na as adhesive and chloroform-ethylacetate-isopropanol-water-ammonia(8∶2∶6∶0.5∶0.12) as developing agent and detected by dualwavelength reflection sawtooth scanning(?s=254 nm and ?R=300 nm).RESULTS:Adenosine,Uracil and Uridnine had good linearity in the ranges of 11.16~58.03 ?g(r= 0.998 5),1.04~6.24 ?g(r=0.991 5) and 0.8~4.8 ?g(r=0.991 2) respectively,and their average recovery rates were 95.559%,96.366% and 95.270% and RSD were 0.74%,0.73% and 0.98% respectively(n=6).CONCLUSION: The method is simple,accurate,reproducible,and can be used as the quality control of Cordyceps sinensis preparation.
8.Advances of Lentiviral Vectors Design and Application

As an efficient gene transfer vehicle lentiviral vector has been widely used in therapy research. Comparing with other retrovirus vectors, lentiviral vectors have the unique ablility of transfecting nondividing cells and terminal differentiated cells. In addition, lentiviral vectors can accommodate two or more promoters and can carry larger foreign gene insertions. Now the new generation of lentiviral vectors encoding transcriptional control sequence provides effective means for the regulation of foreign gene expression. The development of lentiviral vectors and its application in the gene therapy field were summarized.
9.Comparison of operative effect between McKeown minimally invasive approach and the left chest-neck incision approach esophagectomy in mid-to-distal esophageal cancer

Objective To compare the outcome between McKeown minimally invasive approach and left chest-neck incision approach esophagectomy in mid-to-distal esophageal cancer.Methods The clinical data of 148 patients with mid-to-distal esophageal cancer from January 2009 to October 2013 were analyzed retrospectively.Seventy-eight patients (observation group) were performed with McKeown minimally invasive approach esophagectomy and 70 patients (control group) were performed with left chest-neck incision approach esophagectomy.The peroperative period status,postoperative complications and lymph nodes transfer between two groups were compared.Results All the operations were performed successfully.One patient in control group was dead because of acute myocardial infarction.The blood loss in observation group was significantly lower than that in control group[(89.2 ± 40.7) ml vs.(361.6 ± 81.5) ml] (P < 0.05),the number of lymph nodes harvested and lymph node of thorax were significantly higher than those in control group[(22.8 ± 5.6) lymph nodes vs.(15.7 ± 3.4) lymph nodes,(14.7 ± 4.6) lymph nodes vs.(9.1 ± 3.6)lymph nodes] (P < 0.05),the hospital stay was significantly shorter than that in control group [(11.6 ± 3.5)d vs.(14.0 ±6.6) d] (P <0.05).There were significant differences between two groups with regard to pneumonia,atelectasis,pleural effusion and delayed gastric emptying [5.1% (4/78) vs.12.9% (9/70),3.8% (3/78) vs.11.4%(8/70),2.6%(2/78) vs.10.0%(7/70),9.0%(7/78) vs.1.4%(1/70)] (P <0.05).The transfer rate at the side of right recurrent laryngeal,left recurrent laryngeal and upper mediastinal lymph node in observation group were significantly higher than those in control group(P < 0.05).Conclusion McKeown minimally invasive approach esophagectomy is feasible and safe which has lower blood loss,lower respiratory complication,shorter hospital stay and more number of lymph nodes harvested.
10.Comparison of effects of thoracoscopy combined with laparoscopy surgery and the conventional thoracotomy in treatment of esophageal cancer

Objective To analyze effects comparison of thoracoscopy combined with laparoscopy surgery and the conventional thoracotomy in treatment of esophageal cancer.Methods 88 patients with esophageal carcinoma in the past two years in our department were randomly divided into the two groups according to the digital form,each group had 44 cases.The control group was given radical operation as the treatment for esophageal carcinoma,while the observation group used thoracoscope combined with laparoscope radical operation for treatment.The clinical efficacy of the two groups was compared.Results The operation time of the observation group was slightly higher than that of the control group (t =1.372,P > 0.05),the number of lymph node dissection in the observation group was more than that of the control group (t =1.573,P > 0.05).The intraoperative blood loss [(85.6 ± 30.7) mL],postoperative extubation time [(4.1 ± 1.3) d],chest drainage volume [(550.0 ± 30.5) mL] and postoperative hospital stay [(10.8 ± 2.2)d] in the observation group were significantly lower than those in the control group [(220.4 ± 58.7)mL,(7.5 ±1.8)d,(1 130.0 ±50.8)mL,(17.1 ±3.8)d,(t =18.847,6.486,40.649,8.938,all P <0.05)].The incidence of postoperative complications in observation group(6.82%)was significantly lower than that of control group (27.27%) (x2 =5.143,P < 0.05).Conclusion Thoracoscope and laparoscope combined surgery in treatment of esophageal carcinoma can shorten time of being in hospital,decrease blood loss,reduce the incidence of complications,receive high safety and remove lymph node more thoroughly.
