2.Analysis of pathogen distribution and drug resistance of 1676 cases of urinary tract infection

Objective To retrospectively analyze the pathogenic bacterial distribution and drug resistance in the patients with u-rinary tract infection in our hospital to provide a basis for clinically rational use of antibacterial drugs .Methods The midstream u-rine of the patients with urinary tract infection in our hospital during 2013-2015 was performed the bacterial culture .The bacterial identification and drug susceptibility test were performed by using the Vitek-2 Compact automatic microorganism identification in-strumen .The data were statistically analyzed by adopting the WHONET 5 .6 software .Results Among 11130 urine culture sam-ples ,1676 strains of pathogenic bacteria were cultured with the detection rate of 15 .1% ;among them ,1332 strains were Gram-negative bacteria and accounted for 79 .5% ,275 strains were Gram-positive bacteria and accounted for 16 .4% and 69 strains were fungi and accounted for 4 .1% .The top 3 pathogenic bacteria of detection rate were in turn Escherichia coli ,Klebsiella pneumoniae and Enterococcus faecalis .Six hundreds and eleven strains of ESBLs bacteria were detected ,in which Escherichia coli ,Klebsiella pneumoniae accounted for 88 .5% and 10 .2% respectively .Escherichia coli had higher sensitivity to carbapenems ,fosfomycin ,ami-kacin ,etc .(>95 .0% ) .Klebsiella pneumoniae had higher sensitivity to carbapenems (>90 .0% ) .No vancomycin ,teicoplanin and linezolid resistant Enterococcus faecalis was detected .One strain of vancomycin resistant enterococcus faecium was detected .These pathogens mainly derived from the urology surgery department ,the constituent ratio of female was higher than that of male .Conclu-sion The pathogens of urinary tract infection in this hospital is dominated by Gram-negative bacteria ,Escherichia coli is mainly pathogenic bacterium .Multidrug resistant species and number are continuously increased .Clinicians should rationally and correctly use the antibacterial drugs based on the drug sensitivity test results for improving the effect of clinical anti-infection treatment .
3.Inhibition of Propionibacterium acnes by Effective Components of Traditional Chinese Medicines

Objective To investigate the inhibitory effects of Propionibacterium acnes (P.acnes) by 22 effective components of traditional Chinese medicines. Methods The susceptibility tests of P.acnes to 22 effective components of traditional Chinese medicines were performed. Results P.acnes was highly sensitive to both eugenol and cineole, and moderately sensitive to baicalin, emodin and others. The number of bacteria per oil field decreased obviously under microscopy and bacteriolysis was found after the treatment with those components. Conclusion Eugenol, cineole and other components of traditional Chinese medicines can inhibit P.acnes in vitro.
4.Prevalence of Excessive Daytime Sleepiness and Associated Factors in 3085 University Teachers and Staffs in Kunming

Objective To investigate the prevalence of excessive daytime sleepiness and explore the associated factors in university teachers and staffs in Kunming.Methods Epworth sleepiness scale was used as the tool of diagnosis. We consecutively collected the data of teachers and staffs on duty in Yunnan Nationalities University,Yunnan Agricultural University,Yunnan Normal University,Kunming Medical University and Yunnan University of Finance and Economics within 8 months.Results The prevalence of excessive daytime sleepiness in university teachers and staffs was 12.45%, the prevalence of male was 16.82% and female was 7.70%, respectively. The results of Logistic regression showed that the main associated factors of excessive daytime sleepiness were male (OR=1.898),age (30 years and less was as control group,the ORs of 31-40 year group, 41-50 years group and more than 51 years group were 1.480, 2.366 and 2.719, respectively), family history of snoring (OR=3.371), consumption sedative (OR=1.815), memory failing (OR=2.044), dizzy and feeble (OR=2. 076),choking (OR=2.143),report apnea (OR=2.684),etc. Conclusions The prevalence of excessive daytime sleepiness is high in the university teachers and staffs that can partially represent the residents of Kunming city. The associated factors of excessive daytime sleepiness hint that excessive daytime sleepiness may be caused by sleep apnea/hyperpnoea syndrome in the sample population,hence the people with excessive sleepiness need further sleep monitoring .
5.Medial epicanthoplasty using skin redraping and orbicularis oculi muscle disconnection

ObjectiveTo explore a new simple epicanthoplasty method fit for various types of epicanthus with inconspicuous scar after operation.Methods As the latest studies showed that the anatomical structure of epicanthus is mainly due to the eyelid orbicularis muscle's dislocation at the starting point of the medial canthal tendon,which affects the skin appearance,rather than the traditional view that was merely the excess skin,we released the adhesions between the skin and the inner canthus orbicularis muscle,and corrected the dislocation of orbicularis muscle that was key to the operation,incised along with the line A′→B→A″→ C in the field of interior canthus,separated the subcutaneous fascia tissue,relaxed the subcutaneous fibrous cord,disconnect the orbicularis muscle's attachment point in the inner canthal fold,and then sutured and folded the inner canthal ligament.After sewing up point A′ and point A″,the released skin retracted naturally and formed a new inner canthal contour.ResultsFrom June 2008 to June 2009,57 cases of epicanthus patients accepted this surgery.After operation,the appearance of canthus was nice.Eye fissure got widen,and the scar was not obvious.Also there was a high satisfactory rate.ConclusionsThis method has a broad range of applications.It is suitable for various types of epicanthus correction.
6.Study of Fanggan Decoction,s death prevention on mouse and inhibition effects on Influenza A virus

Objective:To study the Fanggan Decoction,s death prevention on mouse and inhibition effects on Influenza A virus in vivo.Methods:After setting up the model of mouse infected with Influenza A virus(H1N1),we observed the death prevention with Fanggan Decoction,done hemagglutination test and detected the dynamic contents of virus with Real-Time Fluorescence Quantitative RT-PCR.Results:Fanggan Decoction can prevent the death of infected mouse and delay the survival time.The death rate was 66.67%,33.33% and 25% respectively in low,middle and high dose of Fanggan Decoction groups and the average survival time was respectively 8.75 days,11.41 days and 12.33 days.Virus contents reached peak on the 5th day,while compared with the model group,virus contents were lower in each Fanggan Decoction groups,especial in the middle and high dose groups.Conclusion:Fanggan Decoction had good effect in inhibiting Influenza A virus,and can prevent the death of infected mouse,delay the survival time,while get better antivirus dose-effect relationship at double dose.
7.Abnormal DNA-binding abilities of NF-?B and glucocorticoid receptor in cortex of kindey from rats with Adriamycin-induced nephrosis

Objective To explore the role of transcription factors such as nuclear factor-kappa B(NF-?B) and glucocorticoid receptor(GR) in the pathogenesis of Adriamycin(ADM)-induced nephrosis in rats and the therapeutic effects of dexamethasone(Dex) and cyclosporin(CsA) on these animals.Methods The DNA-binding abilities of NF-?B and GR in cortex of kindey were examined with electrophoretic mobility shift assay(EMSA) and isotopic radioautography on the 7th,14th,21th and 28th day after a single intravenous injection of ADM,and the therapeutic effects of Dex and CsA were estimated.The biochemistry parameters from blood and urine of rats and the urine protein excretion were also measured.Results The NF-?B DNA-binding ability was significantly increased after 7 days and achieved maximum after 28 days(P0.05).Conclusion The DNA-binding ability of NF-?B is abnormally increased and that of GR is decreased in cells from cortex of kindey in Adriamycin-induced nephrotic rats.The NF-?B DNA-binding ability can be inhibited and the urine protein excretion is decreased by the treatment of CsA.
8.Effect of vitrectomy combined medication hyperplastic on patients with diabetic retinopathy

AIM: To observe the comparison of vitrectomy combined drug therapy ( Ranibizumab injection ) and single vitrectomy for proliferatived diabetic retinopathy ( PDR ) and the influence of the curative effect and prognosis of patients.
METHODS:In this study, 112 cases (125 eyes) with PDR were selected and randomly divided into experimental group and control group ( n= 56 ) . Fifty-six cases ( 61 eyes ) in experimental group were injected by drug therapy of 0. 5mg ranibizumab and received vitrectomy;In control group, 56 cases ( 64 eyes ) were received single vitrectomy. The intraoperative and postoperative differences of clinical indicators were analyzed in two groups.
RESULTS: The average operation time, intraoperative electric coagulation hemostasis rate and iatrogenic hiatal incidence of the experimental group were lower than that of the control group:(95. 00±13. 00) min vs (133. 00±14.5) min, 11% vs 34%, 5% vs 20%, respectively (P<0. 05). The anterior chamber and vitreous body cavity hemorrhage rate within postoperative 1mo in the experimental group were lower than that of the control group:7% vs 23%, 5% vs 12%, respectively (P<0. 05). The postoperative visual acuity of experimental group was better than that of the control group:0. 375±0. 210 vs 0. 261±0. 170, respectively (P<0. 05).
CONCLUSION: Patients with PDR injected with ranibizumab in vitreous cavity before vitrectomy can effectively reduce the operation time, less intraoperative blood loss, the incidence of iatrogenic hiatus, and intraoperative and postoperative complications. The postoperative visual acuity was better than before.

Objective:To investigate modulation of a specific HDAC inhibitor,Valproate acid sodium(VPA),on expression of Caspase3,Caspase8,Caspase9 by inhiting HDAC,as well as apoptosis rate of cancer cells treated with VPA and the specific inhibitors of Caspase3,Caspase8,Caspase9.Methods:Heptocellular carcinoma cells-HepG2,gastric carcinoma cells-BGC-823 and breast cancer cells-MCF-7 were cultured with 0.75-4.0 mmol/L of VPA for 48 hrs in vivo,apoptosis was analyzed by flow cytometry with Annexin V/PI assay.The activities and protein expressions of Caspase3,Caspase8,Caspase9 were detected by spectrophotometry and indirect immunofluorescence technique.Results:Contrary to control groups,VPA at concentrations between 0.75 and 4.0 mmol/L induced a significant apoptosis in HepG2,BGC-823,MCF-7 cells(P0.05).The apoptosis rates of cancer cells treated with VPA and specific inhibitors of Caspase3,Caspase9 together was lower than in the groups with VPA treatment singlely(P
10.Study on the perioperative changes of electrogastrogram of thoracic (tube) stomach in patients with esophageal cancer

Objective To investigate the perioperative characteristics and changing trends of gastric electrical activity of thoracic (tube) stomach in patients with esophageal cancer.Methods The clinical data of 30 patients with esophageal cancer who were admitted to the Sichuan Cancer Hospital between March 2013 and November 2013 were prospectively analyzed.All the eligible patients underwent esophageal cancer resection by Ivor-Lewis according to the inclusion criteria.The electrogastrograms of patients were recorded at preoperative day 1 and at postoperative day 3,7, 11 and 30.The electrogastrograms of patients at preoperative day 1 were used as the control.The parameters of electrogastrogram were analyzed including main frequency, coefficient of dominant frequency instablility, main power, postprandial/preprandial power ratio, percentage of normal gastric slow wave,percentage of slow gastric slow wave and percentage of tachycardia gastric slow wave.Measurement data with normal distritution were presented as x ± s, and measurement data with skew distritution were presented as M (Qn).The postoperative time and pre-and post-prandial electrogastrograms were compared by the repeated measures two-way ANOVA.The comparison between groups were evaluated with the LSD test and analysis of variance.Results Thirty patients were screened for eligibility with a mean age of 62 years (range, 49-75 years), including 26 males and 4 females.The pre-and post-prandial main frequencies were changed from 2.83 ± 0.13 and 3.01 ± 0.17 before operation to 2.66 ± 0.10 and 2.82 ± 0.10 at postoperative day 30 with coherent changing trend.The main frequencies at postoperative each time points were significantly lower than those before operation while postprandial above indicators were higher than preprandial those, showing a significant increasing trend with the passage of postoperative time (F =285.62, P < 0.05).There was no interaction between the time and meal (F =0.22, P > 0.05).The pre-and post-prandial coefficients of dominant frequency instablility were changed from 0.133 ±0.031 and 0.045 ±0.019 before operation to 0.150 ±0.043 and 0.115 ±0.010 at postoperative day 30 with coherent changing trend, and coefficients of dominant frequency instablility at postoperative each time points were significantly higher than those before operation while postprandial above indicators were lower than preprandial those, showing a significant reducing trend with the passage of postoperative time (F =16.51, P < 0.05).The pre-and post-prandial main powers were changed from (85 ± 15) μV and (149 ± 23) μV before operation to (74 ± 9) μμV and (98 ± 10) μV at postoperative day 30, and main powers at postoperative each time points were significantly lower than those before operation, showing a significant increasing trend with the passage of postoperative time (F =48.45, P < 0.05).There was interaction between the time and meal (F =7.39, P < 0.05).The postprandial/preprandial power ratio was changed from 3.00 ± 0.35 before operation to 2.52 ± 0.25 at postoperative day 30, and postprandial/preprandial power ratios at postoperative each time points were significantly lower than those before operation, showing a significant increasing trend with the passage of postoperative time (F =26.66, P < 0.05).The pre-and post-prandial percentages of normal gastric slow wave were changed from 81% ± 6% and 94% ± 5% before operation to 57% ± 5% and 70% ± 5% at postoperative day 30 with coherent changing trend, and percentages of normal gastric slow wave at postoperative each time points were significantly lower than those before operation while postprandial above indicators was lower than preprandial those, showing a significant increasing trend with the passage of postoperative time (F =49.36,P <0.05).There was no interaction between the time and meal (F =0.24, P > 0.05).The pre-and postprandial percentages of slow gastric slow wave were changed from 17% ± 7% and 4% ± 4% before operation to 32%±4% and 21%±4% at postoperative day 30 with coherent changing trend, and percentages of slow gastric slow wave at postoperative each time points were significantly higher than those before operation while preprandial above indicators were higher than postprandial those, showing a significant reducing trend with the passage of postoperative time (F =46.54, P < 0.05).There was interaction between the time and meal (F =18.12, P < 0.05).The pre-and post-prandial tachycardia gastric slow wave percentages were changed from 1.55% (1.04%,2.21%) and 1.95% (1.74%, 4.22%) before operation to 8.97% (5.76%, 12.02%) and 12.41% (8.04%,16.85%) at postoperative day 30 without completely coherent changing trend, and percentages of tachycardia gastric slow wave at postoperative each time points were significantly higher than those before operation while postprandial above indicators were higher than preprandial those, showing a significant difference between before operation and postoperative day 3 (Z =11.47, 13.28, P < 0.05) and no significant difference among the postoperative day 7, 11, 30 (Z =1.88, 0.31, 0.03, P > 0.05).There was no interaction between the time and meal (F=0.85, P<0.05).Conclusions After the esophagectomy, gastric electrical activity of thoracic (tube) stomach is also retained before and after the meal.There are significant differences among the main frequency, main power, coefficients of dominant frequency instablility, postprandial/preprandial power ratio,percentage of normal gastric slow wave, percentage of slow gastric slow wave, percentage of tachycardia gastric slow wave of thoracis (tube) stomach, they have changed dynamically in the perioperative period.
