1.Study progress of pathogenesis and clinical treatment of gelastic epilepsy

Gelastic epilepsy is a special refractory epilepsy accompany with ictal laughing and/or crying which can be divided into cortical induced gelastic epilepsy and hypothalamic hamartomas induced gelastic epilepsy according to their epileptogenic focus.Currently, domestic scholars had lack of systematic understanding of the mechanism and clinical treatment involved in gelastic epilepsy.Recently, foreign scholars had achieved great progress in the research of gelastic epilepsy which included the ictal laughing mechanism in gelastic epilepsy, the primary molecular mechanism in hypothalamic hamartomas induced gelastic epilepsy, the secondary epileptogenesis mechanism in hypothalamic hamartomas induced gelastic epilepsy and the clinical treatment of gelastic epilepsy.Now, the study progress of pathogenesis and clinical treatment of gelastic epilepsy were reviewed.
2.Remission of severe nephrotic edema by hemodiafiltration and its effect on the prognosis of nephropathy

Objective To evaluate the soothing effects of hemodiafiltration on patients with refractoriness nephrotic edema and its influence on prognosis of nephrotic syndrome.Methods All the 15 cases involved had undergone hemodiafiltration periodically during acme edema and administered with the same standard loading dose of prednisone for 8 weeks.Meanwhile,clinical and lab indexes (urine volume,Cr,Bun,urinary protein,albumin) were measured.Results All patients improved in urine volume and renal function gradually after the treatment of hemodiafiltration for 1 to 3 times.By the 8th week of post-hemodiafiltration,urinary protein 24 h declines obviously[(6.42?2.31)g/d to (0.87?1.24)g/d].Conclusion Hemodiafiltration is an effective treatment in relieving retention of water,increasing the remission rate of protein urine,recovering patients' renal function and improving their prognosis to nephritic syndrome patients with severe refractory edema.
4.Fitting method of the intensity-response curve of electroretinogram and its application

This study presents a fitting method for the intensity-response curve of electroretinogram. The data of b-wave amplitudes from 10 eyes with idiopathic macular hole and their contra-lateral contrast eye (normal) were analyzed. It was observed that the Rmax de creased significantly (P=0.013) in the eyes with idiopathic macular hole.
5.Detection of circulating cardiac troponin I autoantibodies and its clinical significance

Objective To detect the positivity of circulating autoantibodies against cardiac troponin Ⅰ (cTnI) in acute myocardial infarction (AMI) and myocarditis patients and investigate the interference of the antibodies with the detection of cTnI. Methods cTnI ELISA was established for assessment of sera obtained from 121 patients with AMI, 24 with myocarditis and 210 healthy subjects. Binding specificity of cTnI antibody from positive sera by ELISA was confirmed with Western Blot. The recovery of cTnl studies was employed to evaluating the effects of cTnl autoantibodies on cTnI immunoassays. Results Thirteen of the 121 AMI patients (10.74%) and 2 of the 24 myocarditis patients (8.3%) had positive anti-cTnI antibodyas compared with none in the healthy subjects. The recovery of cTnI by adding cTnI-C fusion proteincorresponding to final cTnI concentration of 0.625-100 ug/L to sample with anti-cTnI antibody wasinhibited significantly (Spearman correlation coefficient r=0.943, P=0.005 ). There was no significantchange of recovery of cTnl when adding it to a normal sera(Spearman correlation coefficient r=0.377,P=0.461). When cTnI-C complex corresponding to 20 ug/L cTnI was added, 5 of the 15 sera with anti-cTnIantibody were found with the inhibition of recovery (<80%). Conclusions Autoantibodies against cTnIwere increasing in patients with AMI and myocarditis, yet it appears that these autoantibodies could interferenegatively with the cTnI immunoassay. Thus, it should be paid more attention to cTnI immunoassays.
6.Treatment of diabetic peripheral neuropathy

Diabetic peripheral neuropathy (DPN) is the most common complication of diabetes mellitus in nervous system. However, the pathogenesis is still unkown. Therapeutic options are mainly pathogenetically oriented therapy and symptomatic therapy. With an increasing understanding of the pathogenesis of DPN, many new therapies have emerged. Comprehensive therapy targeting different pathogenesis will be frequently applied in the future. In this article, the common therapy and advance in treatment of DPN are reviewed.
8.The Long-term Efficacy Observation of Whole-breast External Beam Radiation Therapy after Breast Conserving Surgery Coupled with Brachytherapy or Local Electronic Boost Radiation

Objective To summarize the long term effect of the whole-breast external beam radiation therapy after breast conserving surgery coupled with brachytherapy or local electronic boost radiation. Methods The survival rate and cosmetic results were retrospectively analyzed for 96 patients with early stage breast cancer received breast conserving surgery and hormone therapy from January 1995 to December 2003 in Yunnan Tumor Hospital. The enrolled conditions included single lesion, tumor maximum diameter ≤ 3cm, microscopically negative surgical margin, negative axillary lymph node dissection, and pathological type of invasive ductal carcinoma. 53 cases with comprehensive follow-up data were selected. Among them, 23 cases were treated with whole breast tangent conventional fractionated irradiation: 46-50 G/23-25 fractions, local tumor bedarea with high dose-rate brachytherapy: 8-10 Gy/1-2 fractions in postoperative,and 30 cases in control group were treated with electron beam irradiation of 10 Gy/5 fractions. The efficacy and cosmetic results were observed in every three months within two years,in every six months from three to five years, and in every year five years later. Results All the patients were followed up for 217 months (average follow-up 165 months), of which 2 patients died of distant metastasis 62 months and 158 months later after treatment. The excellent cosmetic result was more than 78%, and no local recurrence was observed in the group. The 3-,5-,10-and 15-years survival rates were 100%,100%,95.7%and 88.3%,respectively. In control group,3 patients were died of distant metastasis 46 months,97 months and 87 months later after treatment,also with excellent cosmetic results. There was only one case of local recurrence,and the local control rate was 97.0%. The 3-,5-,10-and 15-years survival rates were 100%,96.7%,93.6%and 82.3%, respectively. There was no significant difference in local recurrence and survival rates between the two groups ( >0.05) . Conclusion Both radiation brachytherapy and electronic boost have the same effect and good cosmetic results. The radiation brachytherapy shows a high quality of life and a short treatment time.
9.Application of antibiotic cement-coated locking plates in induced membrane technique for treating post-traumatic tibial osteomyelitis

Objective To investigate the clinical outcome of post-traumatic tibial osteomyelitis treated by induced membrane technique combined with the antibiotic cement-coated locking plate.Methods A restrospective case series analysis was made on 71 cases of post-traumatic tibial osteomyelitis treated by induced membrane technique from September 2014 to September 2014.There were 55 males and 16 females, aged 18-60 years(mean, 37.4 years).Mean length of bone defect following debridement was 4.5 cm(range, 4-11 cm).Antibiotic cement-coated locking plates were used to provide the stability of the bone defects after one-stage debridement.Bone grafting and exchanging the plates with ntramedullary nails were done during the second-stage surgery.Parameters were monitored dynamically after operation to evaluate infection recurrence and bone healing, including C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), clinical features(sinus formation, redness and swelling, warmth and pain) and X-ray films of the involved limbs.Results All patients were followed up for 13-25 months (mean, 19.5 months).There was no sign of recurring infection and CRP and ESR were normal in all cases after the first stage surgery.However, four cases (6%) had recurrent infection with sinus exudates and their CRP and ESR were abnormal after the second stage surgery, and further treatments were carried out on these patients.Follow-up showed bony union in all patients within mean 5.5 months (range, 4-6 months).Conclusion Induced membrane technique with antibiotic cement-coated locking plates for treatment of post-traumatic tibial osteomyelitis can shorten bone healing time and decrease infection recurrent rate without obvious impact on osteogenic activity.
10.Relations between Relieving Depression and Benefit Findings for Advanced Cancer Patients

This study was aimed to discuss the relations between relieving depression and benefit findings for advanced cancer patients.A total of 80 advanced cancer patients with the anxiety / depression assessment of more than 7 points and less than 15 points,were randomly divided into the intervention group and the control group,in comprehensive hospitals.Finally,76 cases were included with 39 cases in the intervention group,and 37 cases in the control group.The progressive muscle relaxation training (PMRT) was used in the control group.Traditional Chinese medicine (TCM) special technique (i.e.,TCM five-element musicotherapy plus PMRT) was used in the intervention group.The reduced fraction of the Benefit Finding Scale (BFS) and the Hospital Anxiety and Depression Scale (HADS) were used in the effect evaluation after 8-week intervention.The results showed that after 8-week treatment,the reduced fractions of BFS and HADS in the intervention group were obviously higher than the control group with statistical significance.The variation tendencies of BFS and HADS in the intervention group were obviously higher than the control group with statistical significance.There were 8 individual values in BFS of the intervention group were higher than the control group with statistical significance.It was concluded that the benefit findings for advanced cancer patients were related to the application of TCM special techniques,can improve the levels of benefit findings among advanced cancer patients.It enriched and perfected the depression regulation plan of cancer patients by TCM special techniques.
