2.The treatment of acute vascular injury: report of 132 cases
Chinese Journal of General Surgery 2000;0(11):-
ObjectiveTo review the treatment of 132 patients with vascular injuries admitted to our hospital from Jun,1985 to May,2001.Methods Clinical data were analyzed according to the cause, site, type, and surgical modality of vascular injuries. ResultsThe mortality rate in this group was 6.8%, amputation rate 8.3%, and patency rate of primary surgical treatment was 91.9%.ConclusionsSurgical exploration is the mainstay of diagnostic approaches for acute vascular injury; flexible measures should be taken to ensure high patent rate of the injuried blood vessel, lower mortality and high salvage of limbs.

Objective To explore the application effect of Sandwich teaching method in clinical teaching in the department of neurosurgery. Methods Totally 144 clinical medicine students were divided into 2 groups:Sandwich teaching group (n=72) and traditional teaching group (n=72).The analysis of test and the questionnaire of the students were conducted after the course.Comparison between the two groups was made using independent sample t test and measurement data were expressed as as ± s.P<0.05 signifies statistically significant differences.Rank sum test was used to compare the distribution of the test scores. Results The test score was statistically higher in Sandwich teaching group than control group [(76.38 ± 10.84) vs.(69.67 ± 10.50)].And the main distribution of the test scores was 70~90 in Sandwich teaching group while 60~80 in control group.Questionnaire showed that the scores of Sandwich teaching group were statistically higher than those ofthe control group in five aspects such as enhancing self-learning ability, improving the ability to analyze and solve problems, and so on.Conclusions Sandwich teaching method achieves good teaching effect and it is worth recommending in clinical teachifig.

Objective To explore the feasibility of nasolabial sulcus flap transfer with autologous free skin graft to repair the alar defects after malignant tumor resection. Methods From January 2012 to January 2015,9 patients with malignant tumor were treated in the hospital. After complete tumor removal, the defect area being reconstructed was 1.5 cm × 1.3 cm to 2.5 cm × 2.5 cm.The defects of 9 patients were all restored with nasolabial sulcus flap combined with autologous free skin graft. Results The 9 patients were followed up for 6-18 months postoperatively. The nasolabial sulcus flap and autologous free skin graft were survived completely in all cases.Symmetrical alae were noted with slight edema within nasal cavity but without difficult ventilation.Scar was repaired in phase-two surgery. Conclusions Nasolabial sulcus flap combined with autologous free skin graft is an optional way in alar defects restoration.Further with secondary morphologic plasty, satisfactory surgical outcome can be achieved.

Objective To investigate the efficiency of ring butterfly sewing in lower uterine segment aided with tourniquet in treating pernicious placenta previa combined with placenta percreta. Methods Twelve pregnant women diagnosed with pernicious placenta previa combined with placenta percreta by prenatal ultrasound in Peking University First Hospital from April 1, 2012 to November 30, 2014, were enrolled. All of them received elective cesarean delivery using ring butterfly sewing in lower uterine segment aided with tourniquet. The efficiency of this novel technique, in term of blood loss and vital signs during operation, blood transfusion, and postoperative recovery, was analyzed. Results Twelve cases were diagnosed with placenta percreta during operation, with placenta invading the perimetrium in seven cases and urinary bladder being involved in one case. Ring butterfly sewing in lower uterine segment aided with tourniquet during elective cesarean delivery was all successful in the 12 cases. The median amount of blood loss was 1 000(400-2 000) ml during operation and the amount of blood loss was lower than 1 000 ml in three cases. Two cases received no blood transfusion, and the median blood transfusion in the other 10 cases were 400(400-1 200) ml. The mean operation time was (75±22) min (43-131) min. Eleven cases had normal lochia and temperature after operation and the other one had placenta invading urinary bladder and partial placenta in situ, and developed fever at day 3 after operation, but recovered after antibiotic treatment, and no placenta tissue was seen by ultrasound at day 59 after operation. Conclusions The ring butterfly sewing in lower uterine segment aided with tourniquet is highly effective for the pernicious placenta previa combined with placenta percreta, and it is a simple, fast and fertility sparing technique with low blood loss and without usage of special equipment.

Objective To evaluate catheter-directed thrombolysis through popliteal vein for the treatment of lower extremity deep venous thrombosis (DVT). Method A total of 62 patients with single symptomatic lower limb DVT were analyzed. The popliteal vein of symptomatic limb was punctured, under Doppler guidance a 5-F sheath was advanced into the deep vein. A catheter was advanced beyond the thrombus. After a bolus of 200 000 IU urokinase, 100 000 IU/h urokinase was infused for up to three days, meanwhile 400 IU heparin was administered subcutaneously every 12h. Clinical efficacy grade was evaluated by measuring the perimeter of legs and lysis grade was calculated based on venographic results. Results In addition to total or partial removal of the thrombus collateral circulation increased, the limbs edema reduction rate was 78% and the thrombolysis rate was 67%. Thrombolysis rate in acute, and relapsing cases was significantly higher than in chronic group (75%,and 71% vs. 49%,P
