5.A Brief Introduction to Several Novel Antifungal Drugs
Microbiology 1992;0(03):-
The introduction of lipid-formula Amphotericin B,Voriconazole, and Echinocandins, including Caspofungin and Micafungin, reflect the trend that the antifungal drugs are being developed along to a direction with high effect, broad spectrum, and low toxicity. These drugs undoubtedly provide novel useful approaches to the treatment of invasive fungal infections.Compared to the traditional antifungal drug, such as amphotericin B and Fluconazole, these drugs have the character of good effect and low adverse effect, which will be very important to prophylaxis and treatment for systemic fungal infections. The information concerning for these issues is being reviewed in this paper.

BACKGROUND: The active ingredients of Chinese herbs, such as hongjingtian (rhodiola root, Radix Rhodiolae), srenshen (ginseng, Radix Ginseng), and danggui (Chinese angelica root, Radix Angelicae Sinensis),were found to be effective to inhibit the hypoxia injury.OBJECTIVE: To observe the protecting effect of xionggui diwan (Dripping Pills) on the hippocampal neurons from hypoxia by whole-cell recording patch-clamp technique.DESIGN: An auto-control observation on cells.SETTING: The 88 Hospital of Chinese PLA and Academy of Military Medical Science of Chinese PLA.MATERIALS: The experiment was performed during March to August 2002 in the Institute of Toxicology and Pharmacology of Academy of Military Medical Sciences of Chinese PLA. The neonatal Wistar rats, clean degree, were used. The hippocampal neuron cells out of 9 rats were cultured and divided into the control groups of before and after hypoxia-oxygenation,and xionggui diwan 1 × 10-6 mol/L and 10 × 10-6 mol/L groups. (Xionggui diwan was provided by the 88 Hospital of Chinese PLA and its active ingredient was the volatile ingredient of chuanxiong and danggui.)METHODS: While hypoxia-reoxygenation groups were not given any medicine intervention as control, the treatment of xionggui diwan 1×10-6mol/L was subdivided into xionggui diwan 1 × 10-6 mol/L + non-hypoxia and xionggui diwan 1 × 10-6 mol/L + hypoxia groups, and the treatment of xionggui diwan 10 × 10-6 mol/L was subdivided into xionggui diwan 10 × 10-6 mol/L + non-hypoxia and xionggui diwan 10 × 10-6 mol/L + hypoxia groups. The patch-clamp technique was performed to determine the Na+ and K+ currents of rat hippocampal neurons cultured in vitro before and after hypoxia-reoxygenation.MAIN OUTCOME MEASURES: The Na+ and K+ current amplitude of rat hippocampal neurons cultured in vitro.RESULTS:① The Na+ current amplitude of hippocampal neurons: After hypoxia-reoxygenation, it was significantly lower than that before [(-3.8±1.0, -10.3±0.5) nA, t=3.49, P < 0.01]. ② The K+ current amplitude of hippocampal neurons: After hypoxia-reoxygenation, the activation thresholdof K+ current was changed from -40 mV to -20 mV.③The Na+ and K+ currents of hippocampal neurons before and after hypoxia-reoxygenation with xionggui diwan pretreatment: Before hypoxia-reoxygenation, the ampli tudes of Ⅰ Na and J K of hippocampal neurons at the threshold point were very similar, without significant difference. After hypoxia-reoxygenation,those amplitudes of xionggui diwan 10.0 × 10-6 mol/L group were higher than that of xiongguidiwan 1.0 × 10-6 mol/L group [(-58.5±1.9, -6.9±1.4,-3.8±1.0) nA, t=7.51,P < 0.05].CONCLUSION: It was suggested that xionggui diwan propect the function of hippocampal neurons from hypoxia.

Chinese Journal of Tissue Engineering Research 2005;9(33):175-177
BACKGROUND: Both animal experiments and clinical practice have confirmed that mild or moderate hypothermia is effective in reducing secondary brain injury, but its effect on homeostasis is not very clear.OBJECTIVE: To investigate the effect of a combined therapy of mild hypothermia and hibernation on the homeostasis of patients with severe brain injury.DESIGN: A randomized controlled study.SETTING: Neurosurgical Institute of Beijing; Neurosurgical Department of the First Clinical Medical College Affiliated to Harbin Medical University;and Neurological Department of the Second Clinical Medical College Affiliated to Harbin Medical University.PARTICIPANTS: The study was conducted at the Department of Neurosurgery, the First Affiliated Hospital of Harbin Medical University, from June to December 2002. Totally 24 patients (aged 35-60 years) with severe cerebral hemorrhage or brain injury were randomly divided into combined therapy group and normothermia group. Their Glasgow Coma Scale scores ranged from 3 to 8. The subjects signed the informed consent.METHODS: Within 10 hours of their injury, patients in hypothermia and hibernation combination group were given half dosage of No. 1 hibernation cocktail (chlorpromazine 25 mg, pethidine hydrochloride 50 mg, and promethazine 25 mg), and were cooled by cooling blankets to make their body temperature dropped to 32-34 ℃ (rectal temperature). Their temperature was kept within this range for 5 days, at 35 ℃ for 24 hours, and then was slowly increased to their normal level. The body temperature of patients in normothermia group was maintained at 37-38 ℃. The mean arterial pressure and heart rate of all patients were measured continuously by HP monitor. On the 3"d and 7th days of hospitalization, intracranial pressure and creatine phosphate kinase were measured via lumbar puncture.Femoral artery puncture was performed to check the partial pressure of arterial O2 and CO2. Platelets count and blood electrolytes K+ and Na+ concentration of each patient were measured, too. On the 7th day Glasgow Outcome Scale scores of each patient and mortality of each group were recorded.activity of creatine phosphokinase, platelets count, blood K+ and Na+ conand CO2 of patients in combined therapy group on the 3rd and 7th days of hospitalization.intracranial pressure, creatine phosphokinase and platelets count: The decreased values of intracranial pressure, creatine phosphokinase and platelet number in combined therapy group were all significantly higher than those in normothermia group [(104.09±54.90), (58.75±25.33) mm H2O; (26.95±19.22), (10.17±7.18) μkat/L; (89.82±46.36)×109/L, (48.83±44.59)×109/L,the mean arterial pressure, blood electrolytes, and partial pressure of artewas significantly lower than that of normothermia group (25.0%, 66.6%,P <0.05).CONCLUSION: This combined therapy of hypothermia and hibernation can effectively decrease intracranial pressure and creatine phosphokinase, but has no significant effects on the mean arterial pressure, blood electrolytes concentration, and partial pressure of arterial O2 and CO2. It has the risk of disturbing the patients' hematopoiesis.
