1.Research on classification of prescriptions in Treatment Record of Hundreds of Selected Ancient Prescriptions based on hierarchical clustering

Objective:Based on systematic idea of clustering methods, a novel approach for classifying prescriptions was explored in the light of the method of classifying analogous prescriptions.Methods:A total of 581 ancient prescriptions were selected from Treatment Record of Hundreds of Selected Ancient Prescriptions, and the standardized names of Traditional Chinese Medicines in these prescriptions were entered in Microsoft Excel 2007, then imported to SPSS 24.0 to generate dendrograms using the hierarchical clustering function. The classification of 581 selected ancient prescriptions was analyzed. Results:All 581 prescriptions could be classified into 86 categories through repeated clustering, the largest group has 29 prescriptions, the smallest group has 2 prescriptions, and the average group has about 6.75 prescriptions. In general, the later the intercepted group, the lower the similarity of its internal prescriptions.Conclusion:This method could realize the classification of prescriptions in the light of the method of classifying analogous prescriptions, which may help to break the original thinking bondage and further deepen the understanding of compatibility rules of prescriptions. However, its disadvantage lied in that the theoretical clues would be reduced when analyzing the compatibility rules of prescriptions and the issues of drug dosage, nature, taste and meridian categories were not specially considered.
2.Internal fixation combined with bone grafting through the injured pedicle for the treatment of thoracolumbar burst fractures via a paraspinal intermuscular approach under MAST Quadrant retractor.

OBJECTIVETo study clinical effects of short-segment fixation and injured vertebra bone grafting through injured pedicle for the treatment of thoracolumbar burst fractures under MAST Quadrant retractor via a paraspinal muscle approach.
METHODSThe data of 42 patients with thoracolumbar burst fractures treated from June 2009 to September 2012 were reviewed. There were 19 males and 23 females, with an average age of (55.2±11.9) years old. The mean injury time was (5.8±4.3) days. Fracture segments included T10 in 3 cases, T11 in 6 cases, T12 in 13 cases, L1 in 9 cases, L2 in 7 cases, and L3 in 4 cases. According to Denis classification, 9 patients were type A, 21 patients were type B, 5 patiens were type C, 5 patients were type D, and 2 patients were type E. All the patients were treated with short-segment pedicle screw-rod system fixation under MAST Quadrant via the paraspinal muscle approach. The operative time, blood loss, complications and the height of vertebra, kyphosis Cobb angle, VAS scores, JOA scores were measured before and after treatment.
RESULTSAfter treatment, the vertebral height and kyphosis Cobb angle were restored. Compared with preoperative results, postoperative vertebral height and kyphosis Cobb angle, VAS scores and JOA scores were all improved. But there was no statistically significance in vertebral height, kyphosis Cobb angle between postoperative at 1 week and 1 year.
CONCLUSIONInternal fixation combined with injured vertebra bone grafting through the injured pedicle for the treatment of thoracolumbar burst fractures via the paraspinal intermuscular approach under MAST Quadrant is a safe, minimally invasive, effective and satisfactory method.

3.Clinical features and surgical treatment for posterior cortex epilepsy

Objective To characterize the clinical features and assess the role of surgery in posterior cortex epilepsy. Methods A retrospective analysis of clinical data was performed in 43 patients with posterior cortex epilepsy. The diagnosis Was established by means of a standard presurgical evaluation, including ictal semiology, MRI, interictal and ictal scalp video-EEG, and additional intracranial EEG monitoring in selected cases. Results The 43 patients included 11 parietal lobe epilepsy, 13 occipital lobe epilepsy, and 19 patients with seizures originating from other part of posterior cortex. Thirty-three patients (76.7%)experienced at least one type of aura, such as visual aura, somatosensory aura, dizziness and so on. The common ictal manifestations included deviation, automatisms, tonic posture and so on. Intracranial EEG monitoring was preformed in 22 selected cases. Transient contralateral hemiparesis occurred in 2 patients, mixed aphasia in 1 patient, and they recovered in 3 weeks after surgery. Visual and visual field deficits were observed in 5 patients, and they did not fully recovered. All patients were followed-up 1 to 5 years, and 27(62.8%)became seizure free (Engel' S I class). Conclusions Some of the specific auras or ictal manifestations may indicate posterior cortex epilepsy. Favorable surgical outcome has been achieved in many of the patients.
4.Protection of total paeony glycoside on cardiomyocytic injury in neonatal rats cultured in vitro

BACKGROUND: It is demonstrated in modern pharmacologic study that total paeony glycoside (TPG) provides extensive pharmacologic activities,such as inhibiting aggregation of platelets and erythrocytes, anticoagulation,antithrombsis, anti-arterial sclerosis, protecting heart and liver, anti-tumor,etc.OBJECTIVE: Neonatal rat cardiomyocytes were cultured in vitro and by the changes of superoxide dismutase (SOD) activity, malondialdehyde (MDA) and nitric oxide (NO) contents in cell culture solution, the protection of TPG on injured cardiomyocytes was analyzed.DESIGN: Controlled observation was designed.SETTING: Bioengineering Department in School of Life Science and Technology of Xi 'an Jiaotong University and Institute of Bone Diseases in Medical School of Xi'an Jiaotong University.MATERIALS: The experiment was performed in Bioengineering Department in School of Life Science and Technology of Xi 'an Jiaotong University from February to June in 2003, in which, 44 SD neonatal rats aged 1-3 days were employed. The 48-hour-cultured cardioryocytes were prepared in 42 bottles and randomized into 6 groups, named normal control (normal group), medicated-injury group (injury group), TPG 0.625 mg/L group,TPG 3.125 mg/L group, TPG 15.625 mg/L group and positive control, 7 bottles in each group.METHODS: Cardiomyocytic primary culture was performed under aseptic condition. No any drug was used in normal group, isoprenaline was added in injury group to terminate the concentration at 100 mg/L, in TPG 0.625, 3.125 and 15.625 mg/L groups, 30 minutes after isoprenaline added, RGP at dosages of 0.625, 3.125 and 15.625 mg/L were added respectively; in positive control, 30 minutes after isoprenaline added, coenzyme Q10 was used to terminate the concentration as 100 mg/L.Afterwards, the assay of every index was performed. Xanthine oxidase (XOD) method was used to assay SOD activity, thiobarbituric acid (TBA)method was to assay MDA content and nitrate reductase (NR) method was to assay NO content.cell culture solution in each group.Compared with normal group, the levels of total SOD, CuZn-SOD and MnSOD were reduced remarkably in injury group (P < 0.05 or P < 0.01).The above-indexes in every TPG group and positive control were improved to different extents (P < 0.05 or < 0.01), in which, the protection of TPG 15.625 mg/L group was near to or superior to positive control [(79.50±10.67), (80.30±13.50); (48.24±13.26), (49.73±10.23); (31.26±10.22),in cell culture solution in each group: Those in injury group were higher remarkably than normal group (P < 0.01). MDA and NO contents were all reduced in every TPG group and positive control and dose dependence presented in TPG protection, the higher the dose was, the stronger the action of TPG on protection was, in which, in high-dose group, MDA content was near to normal group [(5.41±1.81), (4.48±0.94) μmol/L, P > 0.05] and NO content was similar to positive control [(81.83± 9.08), (82.41±12.37) mol/L,P > 0.05].CONCLUSION: TPG protects myocardial injury induced by isoprenaline,indicating dose-dependence relationship, which is probably associated with enhanced anti-oxidation of cell, reduced injury of cellular membrane induced by free radical and lipid oxidant.
5.Effectiveness of bioelectrical impedance analysis interventions on volume status in hemodialysis patient:a systematic review

Objective To evaluate the effectiveness of bioelectrical impedance analysis(BIA) interventions on volume status in hemodialysis patient. Methods Searched The Cochrane Library, PubMed, EMbase, EBsco, Wanfang Data, China Biology Medicine, China National Knowledge Infrastructure to collect clinical trials. The retrieval time was from database to July 15, 2016. The studies were selected according to the inclusion and exclusion criteria and were critically appraised by two reviewers. Then the data of included studies were extracted. Meta-analysis was conducted by using RevMan 5.3 software and descriptive analysis. Results Ten clinical trials were included six random control trial, one controlled clinical trail, three longitudinal study, involving 2009 patients. The results of Meta-analysis showed that the effectiveness of BIA interventions on volume management could improve blood pressure (P=0.001), protect the heart function(left ventricular mass index,P=0.002), compared with the clinical evaluation method, the BIA assessment of dry weight of hemodialysis patients could reduce hospitalization rate 14%. Descriptive analysis showed that BIA intervention volume management had certain advantages for the survival benefit of patients, but the current research was still few, had not yet come to a certain conclusion. Conclusions BIA can improve the management of the volume status and dry weight of hemodialysis patients, so as to it can improve the clinical benefit and survival benefit of patients.Better methods and guidelines for assessing DW and using BIA need to be developed.We propose that experienced HD nursing staff be trained in the use of the BIA to help monitor patient over hydration and approximate dry weight in consultation with the nephrologists responsible for the care of these patients so as to obviate excessive residual over hydration between nephrology reviews.
6.Enhancing chondrogenic differentiation in precartilaginous stem cells with 620 nm red light

Objective To investigate the effect of 620 nm red light on chondrogenic differentiation in rat precartilaginous stem cells (PSCs). Methods Rats' PSCs were isolated and purified using magnetically activated cell sorting and cultured in vitro.The PSCs were exposed once to 620 nm wavelength red light from a light-emitting diode (LED) with an irradiation energy of 0.5 J/cm2,1 J/cm2,2 J/cm2 or 4 J/cm2.Any effect was confirmed by Alcian blue staining,immunohistochemistry and observing histomorphological changes under a light microscope,as well as detection using a reverse transcription polymerase chain reaction (RT-PCR). Results After being induced for 14 d,the PSCs exhibited polygonal and round shapes. Alcian blue and type Ⅱ collagen immunohistoehemistry staining showed positive results,but the control group had no significant change.RT-PCR showed that the mRNA expression of Sox9 and type Ⅱ collagen increased significantly compared with the control group. Conclusion Low energy 620 nm red light can enhance chondrogenic differentiation in PSCs significantly.
7.Clinical significance of non - mydriatic fundus photography in screening for preschool children ocular fundus disease

AIM:To observe the incidence of ocular fundus disease in preschool children examined by non-mydriatic fundus camera and evaluate its effectiveness compared with direct inspection shadow mirror.
METHODS: Three thousand eight hundred and ninety-six preschool children from April 2012 to October 2013 were examined by Topcon TRC-NW300 color fluorescence fundus camera and direct inspection shadow mirror, and images were saved immediately.
RESULTS: Detection rate of non - mydriatic fundus photography was higher than that of direct inspection shadow mirror. In 3 896 cases, 41 eyes were detected abnormal fundus accounting for 1. 05%. The retinal myelinated nerve fibers, morning glory syndrome, retinitis pigmentosa, congenital retinoschisis were common, accounted for 24. 39%, 21. 95%, 14. 63%, 12-20% respectively. The children eye diseases were often accompanied by abnormal vision (68. 30%), ametropia (63. 41%), strabismus (19. 51%).
CONCLUSION:Non-mydriatic fundus photography is a mydriatic method without medicine, so it is easy for preschool children to accept. Image results could directly display the fundus lesions. It shows important significance in the screening for preschool children eye diseases.
8.Usefulness of low amplitude spikes with continuous focal periodic discharges on interictal scalp electroencephalogram and their patterns of cortical electroencephalogram

Objective To reveal the influential factors on scalp electroencephalogram (EEG)recording and provide valuable information for localization of the epileptic focus by analyzing the characteristics of spikes with continuous focal periodic discharges on scalp and cortical EEG. Methods Five patients with refractory epilepsy who had low amplitude spikes with continuous focal periodic discharges on interictal scalp EEG were studied. Intracranial EEG recording was also performed in patients. The amplitudes of spikes and cortical areas of spike-wave foci were measured by DaVinci system. Patterns of continuous periodic activity were determined by autocorrelograms,power spectral density and coherence analysis using Matlab and Spike2 software.T-test was employed to compare the mean amplitudes of spikes on the scalp and cortical EEG.Results The amplitudes of spikes recorded on scalp EEG of the 5 patients were:(22.2±4.8),(30.4±7.1),(20.7±3.2),(58.4±10.1),(23.4±3.9) μV.The amplitudes of spikes recorded on cortical EEG of the 5 patients were:(1253.8 ± 199.3),(806.5 ± 161.4),( 1585.7 ±305.7),(922.5 ± 140.6),(736.8 ±70.9) μV.The amplitudes of spikes on scalp EEG were significantly higher than those on cortical EEG ( t =6.394,P < 0.05 ).The cortical areas of spike-wave foci of the 5 patients were:4.0,6.0,3.5,5.5,6.5 cm2.Power spectral density and autocorrelugrams showed 1-3 Hz oscillations on the cortical of spike-wave foci. Cross-correlation and coherence analysis showed synchronization of electrical activity in two contacts of intracranial electrodes. Conclusion The low amplitude spikes with continuous focal periodic discharges on interictal scalp EEG provide valuable information for localization of the epileptic focus.

Aim To investigate the antioxidative effect of total paeony glycoside(TPG) on cardiomyocytes injured.Methods The ischemia and hypoxia injuy model of cultured neonatal rat cardiomyocytes was induced by adding isoprenaline(ISO), and superoxide dismutase(SOD), malonalde hyde(MDA) and nitric oxide(NO) in the culture solution of normal control group; ISO injure group, CoQ 10 positive control group as well as protective group s with high,middle or low-dose TPG were respectively analyzed and compared. Results Compared with normal control group,the enzyme activity o f total SOD, CuZn-SOD and Mn-SOD decreased obviously, and the content of MDA and NO increased markedly in injury group, but in TPG and CoQ 10 groups all of detective indicators had improvement in varying degrees, and the protective effect was better than or close to positive control group in high-dose TPG grou p.Conclusion TPG has protective action on injured cardiomyocyte s induced by ISO in dose-dependent manner. The mechanism relates to the enhance ment of antioxidative effect in cells, and the reduction of membrane damage caus ed by free radical and lipid peroxide.
