1. Bevacizumab (Avastin) inhibits corneal neovascularization in rabbits

Objective: To evaluate the effect of subconjunctival injection of Bevacizumab (Avastin) on corneal neovascularization(CNV) in rabbits. Methods: Forty-eight New Zealand rabbits were randomly divided into three groups: normal control group (group A, 3 rabbits), neovascularization group (group B, 9 rabbits) and Bevacizumab treatment group (group C, 36 rabbits). Group C was further divided into group C1 (1 day small dose group), C2 (1 day high dose group), C3 (14 days small dose group) and C4 (14 days high dose group), with 9 rabbits each group. CNV model was made by suture in group B and C. Animals in group C were injected subconjunctivally with 0.1 ml or 0.2 ml Bevacizumab (25 mg/ml) in the left eyes. The growth of CNV was observed every day after operation and the neovascularization areas calculated. Expression of VEGF in the cornea was detected by immunohistochemistry on day 7, 14 and 28 after suture. VEGF content in the aqueous humor was determined by ELISA assay. Results: CNV growth in group C1 and C2 was inhibited significantly compared with that in group B on day 7, 14 and 28 (P<0.01). On day 28 the growth in group C3 and C4 was significantly inhibited compared with that in group B (P<0.01), that in group C1 was significantly inhibited than that in group C3, and that in group C2 was significantly inhibited compared with that in group C4 (P<0.01). VEGF levels in the cornea and aqueous humor in group B increased as time passed by, and they were positively associated with CNV area (P<0.01). On day 7, 14, and 28, the VEGF levels in the cornea and aqueous humor in C1 and C2 groups were significantly lower than that in group B (P<0.01). On day 28, those in group C3 and C4 were significantly lower than those in group B (P<0.01); those in group C1 were significantly lower than those in group C3 (P<0.01); and those in group C2 were significantly lower than those in group C4 (P<0.01). The CNV area and VEGF levels in the cornea and aqueou humor were similar between C1 and C2 groups and between C3 and C4 groups. Conclusion: Subconjunctival administration of Bevacizumab can effectively inhibit corneal neovascularization in experimental CNV model, and early administration has a better outcome than late administration. Bevacizumab may exert its effect by down-regulating VEGF in cornea and aqueous humor.
2. Pentacam anterior segment analysis system in evaluating the efficacy of phacoemulsification for angle-closure glaucoma

Objective: To observe the changes in the anatomy of anterior segment before and after phacoemulsification for treatment of angle-closure glaucoma by Pentacam system, so as to assess the efficacy of phacoemulsification combined with artificial lens implantation(ALI) in treatment of early- and middle-stage angle-closure glaucoma. Methods: Forty-two patients with early- and middle-stage angle-closure glaucoma(44 eyes) were included in the present study. The central anterior chamber depth, anterior chamber volume and peripheral anterior chamber angle were examined by Pentacam system pre-operatively and 1 day, 1 week and 3 months post operatively. Meanwhile, A ultrasound was used to examine the anterior chamber depth before and 3 months after operation, and the results were compared with those by Pentacam system. Results: Pentacam system could clearly demonstrate the 3-dimensional structures of the anterior eye segment before and after operation. The central anterior chamber depth, anterior chamber volume and anterior chamber angle were increased significantly after operation(P<0.01); the intraocular pressure was significantly decreased(P<0.01); and the visual acuity was improved(P<0.01). The values of central anterior chamber depth and the dispersion coefficient obtained by Pentacam were smaller than those by A ultrasound. Conclusion: Pentacam system can directly demonstrate the increase of anterior chamber depth, anterior chamber volume, and anterior chamber angle after phacoemulsification combined with ALI in treatment of early- and middle-stage angle-closure glaucoma. It is a safe method for early examination after operation.
3.IL-3 Gene-Modified Bone Marrow Stromal Cells Accelerate Hematopoiesis Recovery of Mice Received High Dose Chemotherapy

A main complication of chemotherapy in cancer patients is hematopoiesis suppression. Microenviroment transplantation using bone marrow stromal cells (BMSCs) has been demonstrated to be a potent method in recovery of hematopoiesis in animal models. Based on hematopoiesis-supportive ability of BMSCs and high potency of IL-3 in hematopoiesis stimulation, BMSCs were studied as a cellular delivery system for IL-3 gene transfection to promote hematopoiesis recovery of mice after high dose chemotherapy. BMSCs were transfected with recombinant adenovirous containing murine IL-3 gene(MOI = 10), the level of mIL-3 secreted by gene-modified BMSCs was 110U/ml/10~6 cells/ 24h in vitro. The mice were injected with high dose cyclophosphamide(200mg/kg) i.p. and after 24 hours the IL-3 gene-modified BMSCs(2 x 10~6/mouse) were transplanted intrasplenically. White blood cell counts in peripheral blood of mice received intrasplenic injection of IL-3-BMSCs were kept at a high level within two weeks after chemotherapy. The pathological sections of spleens and bone marrow showed significant recovery of hematopoiesis, compared with that of mice received chemotherapy only. The data indicated the feasibility of IL-3 gene-modified BMSCs transplantation in the acceleration of hematopoiesis recovery after chemotherapy.
4.Soluble growth stimulating gene 2 protein and prealbumin correlates with prognosis in patients with chronic pulmonary heart disease with refractory heart failure

Objective:To study the clinical significance of early detection of soluble growth stimulating gene 2 protein (sST2) and prealbumin (PAB) in patients with chronic pulmonary heart disease (CPHD) complicated with refractory heart failure.Methods:From September 2017 to June 2019, 112 CPHD patients complicated with refractory heart failure were admitted to Hengshui People's Hospital. The selected patients met the revised guidelines for the diagnosis and treatment of chronic obstructive pulmonary disease (2013 revision) and the cardiac function grade Ⅲ-Ⅳ according to the grading criteria of the New York Cardiology Society. Cardiogenic shock, severe liver and kidney dysfunction, malignant tumors, anemia, and autoimmune diseases were excluded. Patients were divided into the high PAB group (≥200 mg/L) and the low PAB group (<200 mg/L) according to the PAB level on admission. The pulmonary artery systolic blood pressure (PASP), pulmonary artery mean pressure (MPAP), and left ventricular ejection fraction (LVEF) were observed in the two groups before and after the treatment. PAB, total bilirubin (TBIL), hypersensitive C reactive protein (hs-CPR), N-terminal B type brain natriuretic peptide (NT-proBNP) and sST2 levels were detected. Measurement data were expressed in terms of mean ± standard deviation, the counting data were compared using χ 2 test. Correlation analysis was conducted using Spearman correlation test. Results:There were 40 cases in the high PAB group and 72 cases in the low PAB group. There were no statistically significant differences in general data between the high PAB and low PAB groups ( P>0.05). Hospitalization time was significantly different between the two groups ( P<0.05). Before the treatment, there were no significant differences in PASP, MPAP, NT-proBNP and LVEF between the two groups ( P>0.05). High sST2 was significantly different between the two groups ( P<0.05). After the treatment, PASP, MPAP, NT-proBNP and sST2 were decreased in both groups, and the improvement was more obvious in the high PAB group than in the low PAB group, with statistical significance ( P<0.05). Before the treatment, the levels of TBIL and hs-CPR were not statistically different between the high PAB and low PAB groups ( P>0.05). However, the levels of TBIL and hs-CPR were beyond the normal range. After the treatment, TBIL and hs-CPR were decreased in both groups, and there was statistically significant difference between the two groups ( P<0.05). Spearman correlation analysis showed that PAB was negatively correlated with sST2 ( r=-0.778, P=0.001). There was a positive correlation between cardiac function and sST2 ( r=0.569, P=0.034), hospitalization time ( r=0.572, P=0.033) in patients with refractory heart failure. The higher the sST2 of CPHD with refractory heart failure, the longer the patient hospitalization time, and the more serious the heart failure was. The area under the combined ROC curve of PAB and sST2 was 0.756. CPHD patients with refractory heart failure had the greatest predictive value. Conclusion:The combined test of sST2 and PAB can evaluate the condition and outcome of CPHD patients with refractory heart failure, and guide the clinic.

8.Clinical evaluation of two different -incision phacoemulsifications

AIM:To study and compare the outcomes of coaxial 2. 2 mm phacoemulsiflcation with conventional coaxial 3 mm small-incision cataract surgery.
METHODS: A randomized prospective study was conducted on 100 patients with age - related cataract:coaxial 2. 2 mm micro - incision cataract surgery was performed in 50 cases (50 eyes), and coaxial 3 mm small incision cataract surgery was performed in 50 cases ( 50 eyes) . Statistical analysis was takenwith the data of the two groups. Visual acuity, VF and QOL were compared at intervals of 1wk and 3mo after surgery. In addition, surgically induced astigmatism ( SIA ) was analyzed. Statistic analysis was taken by Student's t-test and Chi-square test.
RESULTS:There was no significant difference on BCVA (t=-1. 366, -1. 688; P=0. 148, 0. 107) between these two groups. One week and 3mo after the surgery, SIA was (0. 46±0.29)D, (0. 43±0. 26)D in the 2. 2 group; and (1. 55±0. 59) D, (0. 89±0. 28) D, in 3. 0 group. The differences between these two groups were statistically significant ( t=-7. 348, -3. 788; P = 0. 000, 0. 000 ) There were no statistically significant differences on VF scores between two group, while it's got a better score in 2. 2 groups on vision adaptation. (t=-3. 348, P<0. 05).
CONCLUSION:Coaxial 2. 2mm micro-incision cataract surgery could significantly reduce SIA and obtain morestable status of VF and QOL. This suggests that the coaxial 2. 2 phacoemulsification surgery implanted AkreosMI60 intraocular lens could get earliervisual rehabilitation postoperation.
9.Beclomethasone dipropionate cream combined with He-Ne laser irradiation treatment of nasal vestibule eczema.

OBJECTIVE:
To observe the clinical efficacy of nasal vestibule eczema by using beclomethasone dipropionate in combination with He-Ne laser therapy.
METHOD:
The 200 cases of nasal vestibule eczema patients were randomly divided into treatment and control groups. The control group received the rub beclomethasone dipropionate cream treatment one time per day. The treatment group supplemented with He-Ne laser irradiation treatment on the basis of the control group on the same treatment, alsoone time per day. Then the results were analyzed.
RESULT:
The total effective rate was 100.0% in treatment group, while 75.0% in the control group. The difference between the two groups was statistically significant (P < 0.05).
CONCLUSION
Beclomethasone dipropionate cream combined with He-Ne laser irradiation therapy on nasal vestibule eczema is significant, and easily to operate, with significant anti-inflammatory, anti-itch, analgesic effects.

10.Clinical observation of characteristic and treatment of posterior Pilon fractures.

OBJECTIVETo explore clinical characteristics and treatment of posterior Pilon fracture.
METHODSFrom January 2011 to January 2013,18 patients with posterior Pilon fracures were treated. Among them, 13 were male and 5 were female, aged from 22 to 63 years old, with an average age of 46. All the patients were closed fractures. Open reduction and internal fixation were performed after swelling subsided, lateral malleolous and posterior Pilon fracture were exposured through lateral approach on healthy side, plates were used to fixed,screws or small plates were used to fix the posterior prominence of medial malleolus after changed to supine position. AOFAS scoring were applied to evaulate clinical effects.
RESULTSAll patients were followed up with an average of 22(ranged, 12 to 48)months. All patients obtained satisfactory reset except one patient. All factures were recovered well with an average healing of 11 weeks. According to AOFAS score at the final following up, 7 cases were excellent,2 cases were moderate, and the total score was 86.8±9.2.
CONCLUSIONPosterior Pilon fracture is not rare in clinical, its mechanism of injury, traumatic anatomy, surgical procedure and prognosis are different from that of classical ankle fracture and Pilon fracture.
