1.Effect of telephone follow-up on PICC maintenance

Objective To investigate the effect of telephone follow-up on peripherally inserted central cathter(PICC) maintenance.Methods Two hundred and eighty-nine patients with PICC in our hospital from January to December 2010 served as the control group,given conventional health education on intubation,another 305 patients with PICC from January to December 2011 were set as the observation group,where telephone call follow-up was done on the basis of conventional health education on intubation. The two groups were compared in terms of timely catheter maintenance,catheter-related complications,accidental extubation and satisfaction.Result The observation group was significantly better than the control group in terms of timely catheter maintenance, catheter-related complications,accidental extubation and satisfaction (allP<0.05).Conclusion Telephone follow-up helps to improve patients compliance with PICC catheter maintenance,effectively control the complications and improve the quality of life and satisfaction degree.
2.Research progress in the operational techniques of laparoscopic partial nephrectomy

Compared with open surgery, laparoscopic surgery (LS) provides distinct advantages. However, LS is limited because of the lack of flexibility and tactile feedback. These drawbacks may result in prolonged learning curve and increase in difficulty of sur-gery, particularly for reconstructive LS. This study aimed to determine more efficient methods and resolve several limitations, such as surgical education, LS, surgical procedures (suturing and knotting), iconography, and biotechnology in LS. This review provides a broad overview of studies related to this field.
3.Protective effect of epigallocatechin-3-gallate on intestine ischemia reperfusion injury

Objective To investigate the effect and mechanism of epigallocatechin-3-gallate(EGCG) on intestine ischemia reperfusion injury(IRI) in rats. Methods Forty SD rats were randomly and equally divided into 4 groups:sham group(Sham),intestinal ischemia reperfusion injury group(IRI),EGCG pretreatment group(EGCG) and HLY78 group (Wnt-Ag).The IRI,EGCG and WNT-AG groups were performed the superior mesenteric artery(SMA) ligation for 45 min by non-injury vascular clamp to construct the IRI model.EGCG (50 mg/kg) was administrated by intraperitoneal injection at 45 min before ischemia in EGCG group.The Wnt-Ag group was administrated by intraperitoneal injection of EGCG(50 mg/kg) plus Wnt-Ag (5 mg/kg) at 45 min before ischemia.The IRI group and Sham group were administrated by same dosage of normal saline.The pathological morphology of intestinal tissue was observed by staining at 4 h after reperfusion.The cellular apoptosis was detected by immunohistochemistry.The expressions of tumor necrosis factor-α(TNF-α),interleukin-1(IL-1),interleukin-6(IL-6) in the serum and intestinal tract were examined by ELISA and RT-PCR.The expressions of Wnt,β-catenin,p53,Bax and BCL-2 were measured by Western blot.Results Compared with the Sham group,the expression of IL-6,IL-1,TNF-α,Wnt,β-catenin,Bax,cell apoptosis and pathological change of intestinal tract in the IRI group were significantly increased,while the expression of BCL-2 was significantly decreased.Compared with the IRI group,the expression of IL-6,IL-1,TNF-α,Wnt,β-catenin,p53,Bax,cell apoptosis and the pathological change of intestinal tracrt in the EGCG group were significantly decreased,while the expression of BCL-2 was significantly increased.Compared with the EGCG group,the expression of IL-6,IL-1,TNF-α,Wnt,β-catenin,Bax,cell apoptosis and pathological change of intestinal tract in the Wnt-Ag group were increased,while the expression of BCL-2 was significantly decreased.Conclusion EGCG can alleviate intestine ischemia-reperfusion injury by suppressing inflammation and apoptosis,this protective effect may be mediated by suppressing Wnt/β-catenin signal pathway.
4.The effect of whole course integrated health education on improvement of depressive emotion and psychological disorders in depressive patients

Objective To explore the effect of whole course integrated health education on depressive emotion and self-concept modification in depressive patients and to improve their psychological rehabilitation. Methods Sixties depressive inpatients treated with antidepressive drugs were randomly assigned to receive supportive psychological nursing and general health education (control group 28) or whole course integrated health education (experimental group 32). Hamilton Depression Scale ( HAMD), Tennessee Self-Concept Scale (TSCS) and Symptom Checklist 90(SCL-90) were used before and after intervention to evaluate the clinical effects of two groups. Results Before interventions,the total scores of HAMD,TSCS and SCL-90 were (24.8 ±4. 3) ,(249.68 ±16.23) and(136.39±34.26)in experimental group and (24. 9 ±4. 2) , (248. 74 ± 17. 32) and( 136. 52 ± 35.04) in control group, with no significant difference between two groups (all P < 0.01). After intervention, the total scores of HAMD, TSCS and SCL-90 were (19.7±2.8),(263.55±19.53) and (114.22 ± 18. 75) in experimental group and (22.4 ± 3. 3), (253. 66 ± 18. 37) and (134. 56 ± 32. 64) in control group, with significant difference between two groups (all P<0.01). Conclusions Whole course integrated health education can improve depressive emotion of patients with depression, develop their positive self-concept and boost their mental health level.
5.Clinical analysis of Stenotrophomonas maltophilia septicemia in 15 patients with liver transplantation

Objective To investigate the related clinical factors and homology of strains in Stenotrophomonas maltophilia (S. Maltophilia) infections in 15 patients with liver transplantation. Methods Fifteen patients with S. Maltophilia infection from September to December 2006 were enrolled and their clinical data were collected. Minimal inhibitory concentrations (MICs) of 10 antimierobial agents against S. Maltophilia were determined by Etest strips. Antibiogram was carried out by resistance analysis assembly with WHONET 5 software. The genomic DNA of all the isolates was digested with Xbal and subjected to pulse-field gel electrophoresis (PFGE). Results All patients received mechanical ventilation during the treatment and had a history of long-term use of extended-spectrum β-lactamases and quinolones. MICs of 10 antimicrobial agents indicated that S. Maltophilia were susceptible to several antimicrobial agents including compound sulfamethoxazole and ciprofloxacin, but the best active agent against these resistant isolates was minocycline in vitro. The results of all 15 S. Maltophilia antibiograms were accordance with PFGE patterns. All 15 S. Maltophilia isolates were classified as 2 PFGE patterns: 9 for pattern A and 6 for pattern B. Conclusion Mechanical ventilation might be associated with the S. Maltophilia septicemia in patients with liver transplantation.
6.Optimization of Extraction Technology of Compound Xiongdan Capsules by Orthogonal Test

Objective:To optimize the extraction technology of compound Xiongdan capsules. Methods: The effects of water a-mount, decoction duration and decoction times on the water extraction technology were investigated by orthogonal design using the ex-tract derivation rate and the content of ginsenoside Rg1 , Re and Rb1 as the indices. Results:The optimal water extraction conditions for ginseng and Nidus vespa were as follows:decocting three times with 8-fold of water, and extracting for one hour each time. The extract was condensed until the relative density was 1.20(80-90℃), and then dried and crushed into fine powder. The other herbs were crushed into fine powder and screened by No. 5 meshes. The above powder was blended and well mixed with starch, and then packed into capsules. Conclusion:The optimized water extraction technology is stable, feasible and reproducible, which provide reference for industrial production.

Objective To study the influence of carboxymethyl-chitosan(CM-chitosan)on chondro- cyte apoptosis induced by recombinant human interleukin-1?(rhIL-1?)and explore its mechanism.Methods Rabbit chondrocytes were isolated and cultured.Chondrocytes were pretreated with different concentrations of CM-chitosan for 1 h,then 10 ng/ml IL-1?were added into the culture medium.After 24 h,the apoptotic rates of chondrocytes were measured by flow cytometry with AnnexinⅤ-FITC and PI staining.The morphology of nuclei was observed by fluorescent microscopy with Hoechst 33342 staining.The mitochondrial membrane po- tentials were tested by confocal laser scanning microsocopy with Rhodamine-123 and ATP contents were mea- sured by luciferase reaction.Results CM-chitosan could inhibit chondrocyte apoptosis and restore the func- tion of mitochondria induced by IL-113 in a dose-dependent manner.Conclusion CM-ehitosan can inhibit chondrocyte apoptosis by protecting mitochondrial function.

9.The clinic application of thoracodorsal artery perforator flap: a report of 16 cases.

OBJECTIVETo investigate the effects of free and pedicled thoracodorsal artery perforator (TDAP) flaps for repairing skin and soft tissue defects in limbs, neck, axillary and shoulder.
METHODSFrom October 2009 to Auguest 2011, 16 TDAP flaps were used to repair skin and tissue defects. Among them, five ipsilateral pedicled flaps were used to repair wounds in neck, axillary and shoulder. 11 free TDAP flaps were used to repair the wounds with bone or tendon exposure. In 12 cases, the flaps were pedicled with thoracodorsal artery and vein-lateral branches-perforators, in 4 cases, pedicled with thoracodorsal artery and vein-serratus anterior muscular branches-perforators. The deep fascia, the latissimus dorsi and thoracodorsal nerve were not included in all flaps. The flaps size ranged from 10 cm x 5 cm to 26 cm x 10 cm.
RESULTSAll 16 flaps survived completely with primary healing both at donor site and recipent area. After a follow-up of 3 to 24 months, all flaps gained good texture and appearance. Only linear scar was left at donor area. The shoulder could move freely.
CONCLUSIONSTDAP flap has good texture, long vascular pedicle,and reliable blood supply, leaving less morbidity at donor site. The latissimus dorsi and thoracodorsal nerve are also preserved. The pedicled TDAP flap is an ideal flap for repairing the ipsilateral skin and soft tissue defects of the neck, shoulder, axillary. The free TDAP flap is suited for repairing skin and soft tissue defects of the extremities.

ObjectiveTo explore the clinical and epidemiological value of semi-quantitative regional cerebral blood flow(rCBF)imaging in children with school phobia. Methods A total of 20 cases diagnosed with school phobia were examined with rCBF.Twelve were males and the other 8 were females.The mean age was(14.2?2.1)years(11-18 years).Semi-quantitative analysis methods were used to investigate the correlations among gender,age and rCBF.
