Objective To investigate the protective effect and mechanisms of NF-?B decoy oli- godeoxynucleotides(ODNs)on rat liver graft following ischemia-reperfusion injury.Methods Ani- mals were randomly divided into 3 groups(n=8 in each group):control group,ischemia-reperfusion (IR)group,and decoy ODNs group,in which donor grafts were transfected with 120?g NF-?B decoy ODNs before graft procurement.Following 2 h of reperfusion,Kupffer cells(KCs)were isolated,and NF-?B biding activity was detected with electrophoretic mobility shift assay(EMSA),and TNF-?、IL- 6 mRNA expression was assayed by using RT-PCR method.Meanwhile,the liver and serum samples were collected,the histopathologic change of liver was examined by light microscope,and liver func- tion was analyzed.Results The NF-?B biding activity,TNF-?,IL-6 mRNA expression,and the ALT,TBIL levels in serum were significantly increased following 2 h of reperfusion in IR group as compared with those in control group(P＜0.01).A large amount of degeneration and necrosis in hep- atocytes accompanied with obvious congestion in hepatic sinusoid occurred.However,these tested in- dexes were significantly ameliorated in decoy ODNs group as compared with those in IR group(P＜0.01),and the architecture of hepatic lobules was remained.Conclusions KCs NF-?B activation fol- lowing reperfusion plays an important role in IRI in liver transplantation.Decoy strategy shows appar- ent effect on suppression of NF-?B activation,and thus inhibits the production of downstream cyto- kines,which protects the liver graft from IRI.

Objective To investigate the mechanisms of fluconazole resistance in clinical and experimental induced isolates of C.glabrata.Methods Efflux of rhodamine 6G was performed to evaluate the effects of efflux pumps.The expression levels of transporter genes CDR1,CDR2,SNQ2 and ERG11 were examined by real-time RT-PCR.Meanwhile,sequence of PDR1 was determined by PCR based DNA sequencing.Results Efflux pumps of all fluconazole-resistant isolates had stronger effects than that of susceptible isolates,consistently with significant upregulation of CDR1,but no obvious difference was found in CDR2 or SNQ2.Also,no notable change in the expression level of ERG11 between susceptible and resistant isolates.PDR1 mutations existed in both clinical and experimental induced isolates of C.glabrata,among which P927S,L543P and S947L haven't been reported previously.Conclusion Mutations of PDR1 were induced by fluconazole both in vivo andin vitro,which will result in overexpression of CDR1 and strengthen the effect of efflux pump.

Objective To assess the application value of REP-PCR in genotyping of candida glabrata strains in clinical pratice. MethodsFrom 2009 to 2010, thirty-eight candida glabrata strains were isolated from Shanghai Ruijin Hospitals, Shanghai Renji Hospital, Shanghai Huashan Hospital, Anhui Medical University Hospital, Shenzhen People's Hospital. Six loci in housekeeping genes (FKS, LEU2,NMT1, TRP1, UGP1 and URA3 ) were amplified and sequenced. The sequences were compared with the MIST database and allele profile and sequence type (ST) were obtained. With primers Ca21, Ca22 and Com21 used to amplify the adjacent variable gene regions,the amplicons were analyzed through electrophoresis to generate different REP-PCR types. Finally, the results of these two genotyping methods were compared. ResultsFor REP-PCR, Ca22-Com21 has the best genotyping effect. REP-PCR and MLST have the same genotyping results. Five REP-PCR types were found in 38 candida glabrsta isolates. Type A,B, C, D and E strains from REP-PCR were genotyped as ST 7, 3, 19, 45 and new type respectively byMIST. REP-PCR saves time compared with MIST. Conclusions REP-PCR offers a simple and rapid method for molecular typing, with similar discriminatory power with MIST. Therefore, REP-PCR can be the preferred choice in laboratory, especially for a large number of isolates.

Objective To clarify the helical CT features and anatomic distribution of recurrence and metastasis of postoperative esophageal cancer.Methods Fifty patients ( 43 men , 7 women , ages 42~76 , mean 58 years ) with postoperative esophageal cancer underwent helical CT examination.On the basis of axial CT imaging , CT features were evaluated , including types , anatomic distribution and time of recurrence in patients with postoperative esophageal cancer .Results Among 50 patients , 31 patients developed recurrence and metastasis. Of 31 patients , 12 patients had recurrence of upper gastrointestinal tract , including esophagogastric anastomoses (n = 8), esophageal stump (n =1) , and intrathoracic stomach (n=3). Lymph node metastasis were investigated in 28 cases , which included right upper paratracheal (2R, n=11), and subcarinal right(7+8, n=11) ; 13 patients possessed lymph node metastasis of two and over-two regions ; Five patients had abdominal para-aortic lymph node metastasis. Metastasis of lung, liver and pancreas were discovered in 8, 7 and 1, respectively. There were six with pleural fluid . Objective To investigate the biological characteristics of neural stem cells from the striatum of human embryos. Methods Neural stem cells were isolated from the striatum of 16-20 weeks human embryos.These cells were cultured to proliferate and then differentiate without mitogens or inductive factors.At various time points the progeny of neural stem cells differentiation were analyzed.Using immunocytochemistry stainings,the biological characteristics of neural stem cells were examined. Results Neural stem cells in the striatum of 16-20 weeks human embryos proliferated rapidly in vitro within the first one month of culture at an average doubling time of 3-4 days.Upon mitogen withdrawal,the differentiation of neural stem cells to neurons exceeded 50%.After 8 weeks in culture,however,the proliferation speed of neural stem cells lowered significantly.The proportion of neurons induced by mitogen removal was under 20%.There were about 20% of cells within neurosphere continue to divide.Conclusion The proliferation and self-renewal ability of neural stem cells in the striatum of 16-20 weeks human embryo is robust.In vitro,the rate of proliferation went down with time,and the ability of differentiation to multiple neural cells varied.The different mitogen factors in media have different effects on neural stem cell.Within neurospheres,neural stem cells are not homogenous,as only a portion of cells can divide.

Objective To explore the occurrence of surgical site infection(SSI)in patients in different levels of hospitals.Methods SSI among patients in 47 hospitals at 0:00-24:00 of May 16,2012 were investigated by medi-cal record reviewing,doctor inquiry,and bed-side visiting.Results A total of 5 977 surgical patients were investiga-ted,SSI prevalence rate was 1 .76%.SSI prevalence rate in secondary hospitals was higher than tertiary hospitals (χ2 =9.337,P =0.002);SSI prevalence rates in clean-contaminated and contaminated incision in secondary hospi-tals were both higher than tertiary hospitals (χ2 =4.315,8.129,both P <0.05);departments with high SSI preva-lence rates were general surgery,orthopedic,and neurosurgery;the major isolated pathogens were Escherichia coli , Staphylococcus aureus ,and coagulase negative Staphylococcus .Conclusion SSI rates of different types of incision and different departments are varied,corresponding prevention and control measures should be taken.

Objective To observe the relationship of awake bispectral index (BISawake) and the postoperative cognitive dysfunction (POCD). Methods 100 patients aged 60~75 years with ASA status Ⅰ~Ⅱ, underwent elective lower abdominal surgery with intravenous anesthesia (propofol, remifentanyl and rocuronium). The BIS was adjusted in 40~60 during the surgery. Their cognitive function was assessed with the neuropsychological test battery commended by International study of postoperative cognitive dysfunction (ISPOCD). Results 8 patients withdrew for unwilling or serious complications. The BISawake was more than 65 in 68 patients (group A), and less than 65 in 24 patients (group B). The difference of the verbal learning immediate and delayed of Hopkins Verbal Learning Test-Revised was more in the group B than in the group A. Conclusion Lower BISawake may be associated with the decline of postoperative cognitive function.

Objective To analyze the expression characteristics of chemotherapeutic related genes (ERCC1 ,BRCA1 ,APE1 , RRM1 ,TUBB3 and TS) in non-small cell lung cancer(NSCLC) and their associations with the pathological types to provide the ex-perimental evidences for clinical individual chemotherapy .Methods The immunohistochemical SP method was used to determine the expression of ERCC1 ,BRCA1 ,APE1 ,RRM1 ,TUBB3 and TS genes in 733 patients with NSCLC .Chi-square test was performed to analyze the gene expression characteristics among different pathological types .Spearman relationship was conducted to evaluate the associations among the six genes and different pathological types .Results The expression of BRCA1 ,TUBB3 and TS had sta-tistical differences among three pathological types (P<0 .01);and these three genes showed a relationship with the pathological type (r=0 .107 ,-0 .229 and 0 .168 ,respectively ,P<0 .01) .ERCC1 vs .BRCA1 ,ERCC1 vs .APE1 and APE1 vs .RRM1 showed the pos-itive correlation(r=0 .214 ,0 .316 and 0 .222 ,respectively ,P<0 .01) .Conclusion The chemotherapeutic related gene expressions of NSCLC are related with the clinical pathological types .And the expressions of multiple markers are associated with each other .The combined detection of the chemotherapeutic related markers and the pathological types would be helpful to the decision making of individual chemotherapy in clinic .

Objective To explore the efficacy of Chen's cholangiojejunostojmy for tumors around biliary-intestinal anastomosis.Methods The clinical data on 11 patients operated from Jan 2012 to Dec 2014 were retrospectively analyzed.Results All these 11 patients underwent Chen's cholangiojejunostomy.The postoperative liver function significantly improved,and the symptoms of jaundice completely disappeared after operation [ALT (182.0 ±110.6) U/L vs (68.3±33.3) U/L,TBil (316.9 ±153.9) μmol/L vs (60.3 ± 25.8) μmol/L,DBil (184.1 ± 89.6) μmol/L vs (28.6 ± 12.4) μmol/L,P ＜ 0.01;AST (195.5 ± 206.9) U/L vs (48.6 ± 21.2) U/L,GT (806.7 ± 480.0) U/L vs (204.0 ± 99.1) U/L,ALP (612.8 ±424.6) U/L vs (277.5 ± 68.7) U/L,P ＜ 0.05].The level of CA19-9 also significantly decreased [(1 369.75 ± 1 812.18) kU/L vs (71.0 ± 46.5) kU/L,P ＜ 0.05].There were no significant differences in the CA125,CA15-3,CEA levels [CA125 (35.3 ± 26.0) kU/L vs (29.4 ± 23.5) kU/L,CA15-3 (19.4±12.3) kU/L vs (17.9±10.7) kU/L,CEA (8.4 ±7.7) μg/L vs (7.8 ±6.6) μg/L,P ＞ 0.05].There was no perioperative death.All the patients had a smooth perioperative recovery,except in 1 patient who developed bile leakage and another patient who had episodic attacks of cholangitis.There were no recurrent or metastatic tumors detected on follow-up.Conclusion Chen's cholangiojejunostomy was effective in the treatment of bile duct obstruction caused by tumors around biliary-intestinal anastomosis.
