1.The monitoring of intra-abdominal pressure in critically ill patients

Objective To monitor intra-abdominal pressure (IAP) in critically ill patients.Methods A prospective cohort study was conducted.IAP was measured through the bladder technique.Patients were screened for intra-abdominal hypertension (IAH,IAP ≥ 12 mmHg,1 mmHg=0.133 kPa) upon ICU admission.The patients with IAH/abdominal compartment syndrome (ACS) were given appropriate treatment and management for IAH and/or ACS.Mean arterial pressure (MAP),IAP,abdominal perfusion pressure (APP),filtration gradient (FG) and serum creatinine (Cr) were determined in patients with or without IAH,as well as in survivors and non-survivors.Results The entire protocol of IAP measurement was completed in 88 patients.Number of IAH and ACS patients was 25 (28.4%) and 2 (2.3%),respectively.The number of survivors was 80 (90.9%),with 8 (9.1%) non-survivors.Compared with non-IAH patients,IAP and SCr were increased in IAH patients [IAP (mmHg):14.16 ± 2.43 vs.8.13 ± 2.28,t=10.984,P=0.000; SCr (μmol/L):126.72 ± 83.02 vs.73.41 ± 37.59,t=3.087,P=0.005],with a lower FG (mmHg:59.32 ± 17.08 vs.70.24 ± 15.03,t=-2.956,P=0.004).There were no significant differences in MAP and APP between IAH group and non-IAH group [MAP (mmHg):79.18 ± 12.33 vs.88.71 ± 17.34,t=-1.368,P=0.190; APP (mmHg):73.40 ± 16.11 vs.78.37 ± 14.32,t=-1.415,P=0.161].Compared with survivors,non-survivors showed significantly lower APP and FG [APP (mmHg):60.88 ± 14.58 vs.78.56 ± 14.06,t=3.382,P=0.001 ; FG (mmHg):50.38 ± 16.18 vs.68.81 ± 15.44,t=3.208,P=0.002],and higher SCr (μmol/L:129.12 ±83.62 vs.84.36 ± 55.15,t=-2.082,P=0.040).There was no significant difference in IAP and MAP between survivors and non-survivors [MAP (mmHg):71.00 ± 25.46 vs.84.38 ± 13.53,t =1.224,P=0.238 ; IAP (mmHg):10.62 ±5.34 vs.9.76 ± 3.40,t=-0.647,P=0.519].Conclusions Earlier IAP measurements in critically ill patients are essential for the detection of IAH/ACS and renal injury.With appropriate management of IAH/ACS,significant decrease in morbidity and mortality of patients has been achieved.
3.Optimizing of the Extraction Process for Total Flavonoids from Flos Lonicerae by Uniform Design

OBJECTIVE: To optimize the extraction process of total flavonoids from flos lonicerae. METHODS: The time and temperature of extracting, the amount of solvent and the extraction times of total flavonoids from flos lonicerae was optimized using the uniform design. With the yield total flavonoids as markers to conduct U11(116) experiment and optimize the technology. RESULSTS: The optimum extraction process was adding 10 times 5% alcohol extracting for 1 h (only one time) at 85 ℃. CONCLUSION: This extraction process is simple, reliable and serves as a theoretic basis for the extraction process of total flavonoids from flos lonicerae.
4.The in Vitro Antibacterial Activity of the Crude Extract of Total Flavonoids from Lonicera japonica

OBJECTIVE: To study the in vitro antibacterial activity of the crude extract of total flavonoids from Lonicera japonica against the main pathogenic bacteria.METHODS: With DM130 macroporous resin as stationary phase,the different components(component A contained more sugar,and content B less sugar;C,D and E components were eluted by 20%,40%,and 60% ethanol,respectively) were obtained by gradient elution of the aqueous extract from Lonicera japonica using different concentration of ethanol,then the MIC of different components on main pathogenic bacteria were detected.RESULTS:The antibacterial activity of component B against staphylococcus aureus was more ponent with its MIC at about 1 mg?mL-1.CONCLUSION:Component B has ponent antibacterial action on main pathogenic bacteria.

Objectives:To study the postoperative nutritional support in patients with pancreatic disease. Methods:Based on the kind of pancreatic disease,twenty two patients were randomly divided into PN+EN group and TPN group during 20 days after operation.Body weight,the restoring time of intestinal movement,the complications,the blood amylase,the serum glutamic pyruvic transaminase,the blood urea nitrogen,the blood sugar,the serum albumin(Alb),transferritin(TFN) and prealbumin(PAB) were compared on the first,fifth,tenth and twentieth day after operation. Through defining the connotation of division and cooperation among medical institutions, this paper introduces the current status for division and cooperation among medical institutions of Britain, Germany, Singapore and USA. The advantages include excellent general practices, optimal health resource allocation, effective first treat-ment in community health facilities, and effective two-way referral. We analyzed the influence of some new policies on the division and cooperation among medical institutions such as medical association, managed care and the General Practices’ service contract. This provided a lot of experience and revelations, including the dynamics to promote the division and cooperation, the separation of outpatient services and inpatient services, privatization of primary medical institutions, division and cooperation between medical institutions and nursing, rehabilitation institutions, distinguis-hing the division and cooperation from hospitals alliance, and division and cooperation among medical institutions can not resolve the all issues in the medical care supply system.

Objective To study the effects of granulocyte-macrophage colony stimulating factor (GM-CSF) on the expression of vascular endotllelial growth factor (VEGF) in human dermal fibroblast. Methods In vitro human dermal fibroblasts in good status were incubated with GM-CSF (GM-CSF group) or non-GM-CSF (control group) culture medium for different periods of time. The mRNA, protein expression of VEGF in derma fibroblast were determined by reverse transcription-polymerase chain reaction (RT-PCR) and Western blotting, respectively, and the secretion of VEGF in supernatant was measured by enzyme linked immunosorbant assay (ELISA). Results The expression of VEGF mRNA from dermal fibroblasts was increased significantly after l or more hours of incubation with GM-CSF comparing with the control (P<0.05). 6 hours of stimulation by GM-CSF caused maximal expression of VEGF mRNA. The expression of VEGF protein in dermal fibroblasts was increased from 12 hours and was peaked at 24 hour after stimulation by GM-CSF. VEGF protein from the supernatant of the dermal fibroblasts was also raised persistently from 12 hour after stimulation by GM-CSF and was improved remarkably compared with the control. Conclusions GM-CSF can up-regulate directly the expression of VEGF in human derma fibroblast, which may be one of the mechanisms that GM-CSF accelerates neovascularization in wound healing.
