1.Prevalence of CTX-M Extended-spectrum Beta-lactamases in Clinical Isolates of Enterobacteriaceae in Korea.

The evolution and dissemination of extended-spectrum beta-lactamases (ESBL) have compromised the clinical use of third-generation cephalosporins worldwide. Although most ESBLs belong to the TEM and SHV beta-lactamase families, the members of CTX-M, a novel ESBL family, are increasing worldwide in Gram-negative bacteria. We examined the prevalence of CTX-M ESBL in clinical isolates of the family Enterobacteriaceae collected from three university hospitals located in three different cities in Korea. Among a total of 603 isolates collected, 163 isolates (27.0%) revealed > or =2 microgram/ ml of MIC against cefotaxime, and 93 isolates (15.4%) produced ESBL confirmed by the double disk synergy test. Among 93 ESBL-producing isolates, blaCTX-M genes were detected in 41 isolates by PCR method and they included 1 isolate of C. freundii, 3 of E. aerogenes, 2 of E. cloacae, 17 of E. coli, 9 of K. pneumoniae, and 9 of S. marcescens. Thus, the overall prevalence of CTX-M ESBL-producing isolates among the family Enterobacteriaceae was 6.8% (41 of 603 isolates) and the proportion of CTX-M-producers among the ESBL-producing isolates was 44.1% (41 of 93 isolates). Further determination of the blaCTX-M subtype by nucleotide sequencing revealed blaCTX-M-3 in 17, blaCTX-M-15 in 11, blaCTX-M-14 in 9, and blaCTX-M-9 in 4 isolates. To our knowledge, this is the first report on the dissemination of CTX-M ESBL among clinical isolates of the family Enterobacteriaceae in Korea.

3.Clinical Nurses' HPV-related Knowledge and Perception of Cancer Causes: HPV Vaccinated vs. Not Vaccinated.

PURPOSE: The purpose of this study was to compare knowledge level of those clinical nurses who received HPV vaccine and those who did not and their perception of the relatedness of HPV vaccine to causes of cervical cancer. METHODS: A total of 249 clinical nurses were surveyed from June to July, 2009. The questionnaire originally developed by Kim & Ahn (2007) examined HPV-related knowledge originally and the tool for perception of the causes of cervical cancer was originally developed by Kim (1993). The total number of subjects equaled to: vaccination group of 52 (20.9%) and non-vaccination group of 197 (79.1%). RESULTS: Vaccination group showed significantly higher score of both knowledge of HPV vaccination and the perception of the cause of cervical cancer in comparison to the nonvaccination group at (p<.05). Among 4 subscales of the perception of causes of cervical cancer, destiny and constitution subscale scores were significantly different between the two groups at (p<.05). CONCLUSION: Clinical nurses need to constantly update with current knowledge of HPV and be prepared with currently changing cancer prevention strategies, especially in cervical cancer.

4.Factors Influencing Professional Quality of Life in Intensive Care Unit Nurses of University Hospitals

Purpose:
: This study aims to identify job stress, emotional intelligence, and exhaustion of the nurses in comprehensive nursing service units and then verify the moderating and mediating effects of emotional intelligence on the relationship between job stress and exhaustion.
Methods:
: Participants are 118 nurses working in two general hospitals. The collected data are analyzed using the SPSS WIN 27.0 and AMOS 18.0 programs by assessing frequency and percentage, mean and standard deviation, independent t-test and one-way ANOVA, Scheffé test, Pearson correlation coefficient, and hierarchical multiple regression analysis and bootstrapping.
Results:
: Exhaustion has a statistically significant positive correlation with job stress (r=.40, p <.001) and a statistically significant negative correlation with emotional intelligence (r=-.26, p =.004). A partial mediating effect of emotional intelligence is found between job stress and exhaustion, however, there is no moderating effect.
Conclusion
: It is expected that to reduce the exhaustion of nurses in comprehensive nursing service units, it is necessary to assess job stress and emotional intelligence, and strengthen emotional intelligence along with job stress intervention.
5.Congenital Antral Web in Premature Baby.

Antral web is a rare cause of gastric outlet obstruction in neonate. It is a 2-4 mm thin mucous membrane that can be found anywhere from 1 to 7 cm proximal to the pylorus. The baby was born at gestational age of 32(+1) weeks with 1,880 g as 2nd baby of dizygotic twin. After birth, the baby had constant non-bilious vomiting without feeding while he didn't show abdominal distension or discoloration. The infantogram showed distended stomach with distal small bowel gas. Upper gastrointestinal series revealed that the antrum was abruptly narrowed at 1 cm proximal to pylorus. We performed laparotomy at the 10th day after birth and excised the 2 mm-thick web circumferentially. He began milk feeding after 6 days and discharged uneventfully at postoperative 35 days with corrected age of 38(+4) weeks with body weight 2,420 g. The antral web should be considered in the case of non-bilious vomiting in neonate.

6.Determination of Genomospecies and Characterization of Antimicrobial Resistance of Multi-drug Resistant Acinetobacter spp. Isolates.

Acinetobacter species are non-fermentative Gram-negative coccobacilli and they have emerged as important nosocomial pathogens which are associated with the significant multidrug resistance in recent years. Carbapenem-resistant A. baumannii (CRAB) and pandrug-resistant A. baumannii (PDRAB) were reported in 1991 and 1998, respectively. Fiftyeight isolates of Acinetobacter species recovered from a university hospital between August 2004 and March 2005 were investigated for the existence of CRAB, PDRAB, extended-spectrum beta-lactamase (ESBL)-producing Acinetobacter and examined for their phenotypic and genotypic characteristics. Genomospecies of Acinetobacter species were determined by amplified rDNA restriction analysis (ARDRA) and antimicrobial susceptibility test was performed with 13 kinds of antimicrobial agents. Metallo-beta-lactamase (MBL) producers were screened by modified hodge test and confirmed by imipenem-EDTA disk synergy test. Detection of blaIMP-1, blaVIM-2, blaTEM, and blaPER-1 was performed by PCR. Genomic DNAs were analyzed by pulsed-field gel electrophoresis (PFGE). Among 58 isolates of Acinteobacter species, 40 isolates were identified as genospecies 2 (A. baumannii), 9 were 13TU, 5 were A. phenon 6/ct, and 4 were Acinetobacter genospecies 3 by ARDRA. Thirteen isolates were confirmed as MBL-producers and blaIMP-1 and blaVIM-2 were carried by 5 and 8 isolates of them, respectively. MBL-producers were mostly 13TU, A. phenon 6/ct 13TU, and Acinetobacter genospecies 3 and they were susceptible to ciprofloxacin and ampicillin-sulbactam. BlaPER-1 was carried by thirteen isolates and 12 isolates of them were PDRAB showing resistance to all antimicrobial agents tested, including ceftazidime, cefepime, aztreonam, ciprofloxacin, amikacin, gentamicin, ampicillin-sulbactam, and imipenem. In conclusion, most MBL-producers belonged to 13TU, A. phenon 6/ct 13TU, and Acinetobacter genospecies 3 which were susceptible to ciprofloxacin and ampicillin-sulbactam, whereas 12 of 13 PER-1-producers were PDRAB originated from the same clone.

8.Endothelial Cyst of the Adrenal Gland: Report of a case.

Adrenal cysts are rare lesion that usually present themselves as an incidental finding during surgery, or at the time of autopsy. The cysts are usually small, seldom exceeding 10cm in diameter, and are generally asymptomatic. However, they present a difficult problem in differentiation between benign and malignant lesions of the adrenal gland. In the differential diagnosis, other cystic lesions of the upper abdomen must also be considered, including hepatic, splenic, renal and pancreatic cysts. Herein we report a case of endothelial cyst of lymphangiomatous type of the adrenal gland which was detected in a 44-year-old male patient during a routine health examination by ultrasonography as a pancreatic pseudocyst. Gross examination revealed multiple separate but continuous cysts, measuring 10.6x8x7cm in dimension. Within the wall, compressed adrenal cortex was noted. Microscopically, fibrous wall containing hypertrophied smooth muscle lined by endothelial cells was also noted. We reviewed literatures of the adrenal cyst and report a case.

9.Effusion Cytology of Ki - 1 Positive Anaplastic Large Cell Lymphoma: A Case Report.

Ki-1 positive anaplastic large cell lymphoma is a newly described high-grade lymphoma and is defined by histopathological and immunologic criteria. We experienced a case of systemically involving Ki-1 positive anaplastic large cell lymphoma in a 44 year-old female which initially manifested as pleural effusion. Abdominopelvic CT scan showed the evidence of marked lymphadenopathy in retroperitoneal and both external and inguinal lymph nodes. On cytologic examination of pleural fluid tumor cells revealed pleomorphic large isolated cells with prominent nucleoli and abundant cytoplasms. The nuclei were large with irregular profiles including some deep invaginations. Also. occasional multilobed/multinucleated and binucleated nuclei were seen. Immunohistochemical examination was performed to differentiate from the undifferentiated adenocarcinoma. Hodgkin's disease, non-Hodgkin's lymphoma and malignant histiocytosis. The neoplastic cells were positive for leukocyte common antigen. CD3 CD30(ki-1) but negative for cytokeratin. epithelial membrane antigen. and CD15. A histologic diagnosis of Ki-1 positive anaplastic lymphoma was made by biopsies of the inguinal lymph node, polypoid lesion of the stomach and cecum.

10.Polyphenols of Rubus coreanum Inhibit Catecholamine Secretion from the Perfused Adrenal Medulla of SHRs.

The present study was attempted to investigate whether polyphenolic compounds isolated from wine, which is brewed from Rubus coreanum Miquel (PCRC), may affect the release of catecholamines (CA) from the isolated perfused adrenal medulla of the spontaneously hypertensive rats (SHRs), and to establish its mechanism of action. PCRC (20~180 microgram/ml) perfused into an adrenal vein for 90 min relatively dose-dependently inhibited the CA secretory responses to ACh (5.32 mM), high K+ (56 mM), DMPP (100 micrometer) and McN-A-343 (100 micrometer). PCRC itself did not affect basal CA secretion (data not shown). Also, in the presence of PCRC (60 microgram/ml), the CA secretory responses to veratridine (a selective Na+ channel activator (10 micrometer), Bay-K-8644 (a L-type dihydropyridine Ca2+ channel activator, 10 micrometer), and cyclopiazonic acid (a cytoplasmic Ca2+ -ATPase inhibitor, 10 micrometer) were significantly reduced, respectively. In the simultaneous presence of PCRC (60 microgram/ml) and L-NAME (an inhibitor of NO synthase, 30 micrometer), the inhibitory responses of PCRC on the CA secretion evoked by ACh, high K+, DMPP, and Bay-K-8644 were considerably recovered to the extent of the corresponding control secretion compared with that of PCRC-treatment alone. The level of NO released from adrenal medulla after the treatment of PCRC (60 microgram/ml) was greatly elevated compared with the corresponding basal level. Taken together, these results demonstrate that PCRC inhibits the CA secretion from the isolated perfused adrenal medulla of the SHRs evoked by stimulation of cholinergic receptors as well as by direct membrane-depolarization. It seems that this inhibitory effect of PCRC is mediated by blocking the influx of calcium and sodium into the adrenal medullary chromaffin cells of the SHRs as well as by inhibition of Ca2+ release from the cytoplasmic calcium store at least partly through the increased NO production due to the activation of NO synthase.
