1.Effects of Modified Wendan Decoction Combined with Amlodipine Tablets on TCM Constitution and Vascular Endothelial Growth Factor of Phlegm-damp Hypertension Patients

Objective To observe the clinical efficacy of modified Wendan Decoction combined with amlodipine tablets in treating hypertensive patients with phlegm-damp constitution, and the impact on patients' constitution and vascular endothelial growth factor (VEGF). Methods Totally 126 patients were randomly divided into two groups, 63 cases in each group. The treatment group was given modified Wendan Decoction combined with amlodipine tablets, while the control group received amlodipine tablets, for 12 weeks. The blood pressures, TCM phlegm-damp scores, and serum VEGF levels in both groups were observed before and after treatment. Antihypertensive effect between the two groups was compared. Results TCM phlegm-damp scores decreased significantly in the treatment group after treatment (P<0.01), while that was not significant in the control group (P>0.05). The treatment group was superior to the control group (P<0.01). The systolic blood pressure, diastolic blood pressure, and serum VEGF level significantly decreased after treatment (P<0.01), and the indicators of treatment group were better than those of the control group (P<0.05). The total efficiency of treatment group and control group were 92.1%(58/63) and 76.2%(48/63), respectively, the difference was significant (P<0.05). Conclusion Modified Wendan Decoction combined with amlodipine tablets can significantly improve biased constitution in hypertension patients with phlegm-damp constitution, lower serum VEGF level, and improve endothelial function, with good antihypertensive effect.
2.Experimental study on acute hypertensive cerebral edema at 7.0T MR

Objective To verify the feasibility of a rat model of acute hypertension, and to observe the imaging findings at 7.0T MR scanner of the model.Methods In experimental group, the rats were scanned at 7.0T Bruker MR scanner when phenylephrine was injected continuously to evoke an acute hypertension state.The real time blood pressure was monitored through a femoral arterial catheter connected to a pressure transducer.The control group was infused with saline.T2WI, DWI and T1 mapping were performed in the two rat groups.ADC maps and T1 maps were acquired after image post-processing, and a voxel wise analysis and a ROI analysis were applied.The brain morphology change was evaluated by HE staining.The blood-brain barrier permeability was evaluated by Evans blue staining.Results The T2 images and DWI images of the experiment group showed no abnormal signal intensity changes observed by naked eyes.But ROI analysis of the ADC maps showed that the brain ADC values of the experiment group was higher than the control group (t=3.291,P<0.01, the cortex;t=2.186,P<0.05, the subcortex).Voxel wise analysis of the ADC maps showed vasogenic edema mainly distributed in the parietal and occipital regions,subcortical nuclei (caudate nucleus),thalamus,brainstem and cerebellum,which mainly located in the posterior portions of cerebral hemispheres.T1 image analysis did not show any significant difference between two groups (P>0.05).The blue dye region was only performed in the experimental group,the results of HE staining corresponded with vasogenic brain edema.Conclusion Continuous infusing of phenylephrine hydrochloride can induce a rat model of acute hypertension.Vasogenic edema and blood-brain barrier permeability change can be observed in the rat model, and the imaging distribution of vasogenic edema can be detected by MRI.
3.Curative Effects of Budesonide Suspension Combined with Ubertaline Solution for Nebulization on Infantile Asthma

Objective To study the clinic effects and safety of budesonide suspension combined with tubertaline solution for nebulization therapy on infantile asthma.Methods Seventy-five cases with asthma were divided into 3 groups randomly,double-blinded and controlled,each group having 25 cases.Group A inhaling tubertaline solution(bricanyl),while group B inhaling pulmicort respules combined with bricanyl,group C inhaling budesonide suspension(pulmicort respules)only.Results In group A,16 cases was cure,3 cases was markedly improved,6 cases was failure,the total efficacy rate was 76 percent.In group B,22 cases was cure,2 cases was markedly improved,1 case failed,the total efficacy rate was 96 percent.While in group C,14 cases was cure,4 cases was markedly improved,7 cases failed,the total efficacy rate was 72 percent.The difference of efficacy rate between group A and group B was significant(P0.05).Conclusion Pulmicort respules combined with bricanyl for nebulising inhalation is effective and safety on infantile asthma and its effect is better than inhaling bude-sonide or tubertaline only.
4.Study on Selection of Preparation Procedure of Buqiyangxuecuiru Oral Liquid

Objective: To select the optimum extraction procedure for Buqiyangxuecuiru Oral Liquid. Methods: The water extraction, water extraction and alcohol precipitation, and alcohol percolation procedures were designed. Taking contents of ferulic acid and astragalus saponins I, and the lactation quantity of normal lactation mice as markers, the rational preparation procedure was selected. Results: The oral liquid prepared by the procedure of alcohol percolation. Both ferulic acid and astragalus saponins I contents in this preparation were the highest, and it could remarkably increase the lactation quantity of normal mice. Conclusion: The alcohol percolation procedure was the optimum extraction procedure for Buqiyangxuecuira Oral Liquid.
5.Establishment of TaqMan real-time quantitative PCR assay based on vvhA gene for quick detection of Vibrio vulnificus

Objective To establish a TaqMan real-time fluorescent quantitative PCR to detect Vibrio vulnificus based on hemolysin gene(vvhA)that coding cytolysin.Method By using software Primer Express, the PCR primers and TaqMan probe,which located in the conserved region of vvhA gene sequence,were designed for establishment of a TaqMan real-time fluorescent quantitative PCR to detect 100 bp amplicon from V.vulnificus DNA.A recombinant plasmid pMD19-vvhA100 as a positive control during detection was constructed using gene cloning technique.Minimal amplification cycles(Ct value)and fluorescence intensity enhancement (△Rn value)were used as observing index to optimize the reaction conditions of the TaqMan real-time fluorescent quantitative PCR.The DNAs with different concentrations from V.vulnificus and other eight bacteria and pMD19- vvhA100 were applied as templates to determine the specificity,sensitivity and reappearance of the TaqMan real- time fluorescent quantitative PCR.ICR mice were intraperitoneally,subcutaneously and orally infected with V. vulnificus,respectively.The detection effect of the TaqMan real-time fluorescent quantitative PCR was measured using the specimens of peripheral blood,subcutaneous tissue and intestinal content collected from the infected mice.Results The established TaqMan real-time fluorescent quantitative PCR showed positive results only for V. vulnificus DNA and pMD19-vvhA100.The detection effectiveness of the TaqMan real-time fluorescent quantitative PCR was as high as 0.01 ng of V.vulnificus DNA or 103 copies of pMD19-vvhA100.The SD values of the detection results repeated for three times using pMD19-vvhA100 with different concentrations were lease than 0.79. The detection results of TaqMan real-time fluorescent quantitative PCR were positive for all the specimens of peripheral blood and subcutaneous tissue.Conclusions The TaqMan real-time fluorescent quantitative PCR established in this study for V.vulnificus vvhA gene detection has advantages such as quickness,stability, sensitivity and specificity,indicating this method can be used for clinical laboratory diagnosis of septicemia and wound infection caused by V.vulnificus.
6.Significance of Monitoring Serum Level of Gentamycin in Children with Serious Illness

Objective To explore the serum level of gentamycin for orally in children with serious illness.Methods The serum level of gentamycin in 41 children who were in serious illness [multiple organ dysfunction(MODS)group with 21 cases and non-MODS group with 20 cases ] were monitored and the patients were treated with select decontamination of the digestive tract(SDD) from October 2004 to April 2005.Dosage:10 mg/(kg?d),orally taken three times(every 8 hours) one day.The blood after taking the drug one hour later in the fourth day was selected and the serum level of getamycin was monitored.Results Thirty-six children of 41 cases serum level of gentamycin were negative and 5 children(4 in MODS group and 1 in non-MODS group) who had alimentary tract hemorrhage were masccline in serum after taking gentamycin one hour later in the forth day.The absorption of gentamycin from enteric after orally was not(rela)-ted to MODS.There were statistics value between the gestrintestinal tract ulcer and serum level of gentamycin.Conclusions The safety for treating the children in serious illness with gentamycin for SDD is obvious.But we suggest to monitor the serum level of gentamycin for who has severe alimentary tract hemorrhage together with insufficiency of liver and kindey.
7.Bacterial culture and drug resistance of vaginal discharge in perinatal pregnant women

Objective To investigate the distribution and drug susceptibility of vaginal pathogens in pregnant women during perinatal period.Methods Vaginal discharge specimens of 10 800 women (5 400 were pregnant women during perinatal period,5 400 were non-pregnant women) were performed bacterial culture and drug susceptibility testing.Results The isolation rate of pathogens from 5 400 perinatal pregnant women was 26.00% (n =1 404),including 759 strains of fungi,611 strains of gram positive cocci,32 strains of gram-negative bacilli,2 strains of Neisseria gonorrhoeae;among 5 400 non-pregnant women,the isolation rate of pathogens was 7.87% (n=425),including 232 strains of fungi,182 strains of gram-positive cocci,5 strains of gram negative bacilli,and 6 strains of Neisseria gonorrhoeae.Resistance rates of Streptococcus agalactiae isolated from perinatal pregnant women to erythromycin and clindamycin were 84.85% and 80.81 % respectively,resistance rate of Staphylococcus aureus to erythromycin was 40.91 %,resistance rates of Escherichia coli to tetracycline and sulfamethoxazole/trimethoprim were 69.23 % and 53.85 % respectively,resistance rate of Neisseria gonorrhoeae to penicillin and sulfamethoxazole/trimethoprim was 100.00%.Conclusion Vaginal infection rate in perinatal pregnant women is higher than non-pregnant women,screening of vaginal pathogens in perinatal pregnant women should be strengthened,so as to ensure the safety of mothers and infants.
8.Analysis of prognostic factors in patients with cervical squamous cell carcinoma of stage Ⅰb and Ⅱa

Objective To analyze the prognostic factors in patients with cervical squamous cell carcinoma of stage Ⅰb and Ⅱa treated by surgery,and to investigate their guid roles in available post-operation adjuvant therapy. Methods The clinicopathologic records of 306 patients with cervical squamous cell carcinoma of stage Ⅰb and Ⅱa who underwent radical hysterectomy and pelvic lymphadenectomy were retrospectively analyzed, and the prognostic factors were explored by univariate and multivariate methods. Independent prognostic factors were identified by COX proportional hazards regression model. Results The overall 5-year survival rate of these 306 patients was 78.1%. 9.The role of TMPRSS4 in radiation induced metastasis of hepatocellular carcinoma

Objective To investigate the role and mechanism of TMPRSS4 in radiation induced metastasis of hepatocellular carcinoma (HCC).Methods Metastatic model of human HCC was established by orthotopic implantation of histologically intact human HCC tissue into the liver of nude mice.Mice bearing xenografts in liver were killed after radiation and the residual tumors were resected and reimplanted into the liver of normal nude mice.At the end of sixth week,the mice were killed and the histopathological features,tumor volume,intrahepatic and lung metastasis were evaluated.Expression of epithelial-mesenchymal transition (EMT) related genes including N-cadherin,Vimentin,SIP1 and TMPRSS4 were measured by Western blotting and RT-PCR.Results The tumor volume and frequency of lung metastasis of control group was 2.25±0.52 cm3 and 66.7%,respectively.Compared to control group,tumor diameter (1.61±0.51 cm3,P<0.05) and lung metastasis (12.5%,P<0.05) were significantly inhibited 2 days after radiation.Whereas,30 days after radiation,tumor growth recovered (2.60±0.61 cm3,P>0.05) and lung metastasis was enhanced (100%,P<0.05).There were no intrahepatic metastasis in the control group and in the group of reimplantation of HCC 2 days after radiation,while the tumors from those 30 days after radiation showed enhanced intrahepatic metastasis (18 ± 8.05,P< 0.01 ),with overexpression of SIP1,N-cadherin,Vimentin and TMPRSS4,and reduced expression of E-cadherin.Conclusion The metastasis potential of residual HCC after radiation was first inhibited and then promoted.Overexpression of TMPRSS4 plays a critical role in radiation induced long-term metastasis of HCC by facilitating EMT.
10.Treatment of hypertrophic nonunion by using Ilizarov circular fixator

Objective To evalute the effect of Ilizarov technique in the treatment of hypertrophic nonunion.Methods Form June 2008 to December 2010,12 patients with hypertrophic nonunion were treated with Ilizarov technique,including 10 males and 2 females with an average age of 46.5 years.The pathology sites of nonunion were kept as closed as possible without any bone graft during operation.As to patients who had ever been treated with plate or intramedullary nail,the hardware should be removed by minimal invasive approach.These procedures aimed to keep the vascularity of nonunion site intact.Ilizarov apparatus were preoperatively constructed.Distal segment and proximal segment of nonunion were mounted respectively with two external circle using the smooth wires and half pins.The two-circle stabilizing one segment was nominated with transosseous modules.Distal module and proximal one was connected with a pair of axial hinges.The pathology sites were gradually distracted from the seventh day postoperatively,0.25 mm/d.Accompanying with deformity correction,limb length discrepancy (LLD) also were restored simultaneously.Then,all the screws and nuts in the apparatus should be tightened,which was favourable to the callus consolidation.Results All 12 cases of nonunion healed without any bone graft.The fixator wearing time lasted 6-12 months,with an average of 8 months.Correction of deformity and LLD were achieved.The average lengthening was 3.0 cm (range,2.0-5.5 cm),the average correction angle was 23° (range,10°-30°).After 6-18 months follow-up,all the patients restored satisfactory function.Conclusion Hypertrophic nonunion can be treated successfully with Ilizarov technique.The key of successful callus distraction is strictly identifying the indications.
