1.Clinical evaluation for esophogeal metallic stent selection

Objective To select properly the esophageal metallic stent to treat the dysphagia and strictures of advanced esophageal cancer. Methods To place esophageal metallic stent for 72 cases of advanced esophageal cancer. Results Dysphagia were markedly improved or completely alleviated together with esophageal and mediastinal fistula sealed completely after the procedure. Conclusions The complications will be reduced by the proper selection of esophageal metallic stent and also for resuming of food intake.
2.Treatment of proximal and middle one-third of humeral fractures with tibial helical plate

Objective To explore the fixation method of tibial helical plate for treatment of proximal and middle one-third of humeral fractures and evaluate the clinical outcome. Methods A review was performed on 15 patients with proximal and middle one-third of humeral fractures treated by tibial helical plate from May 2004 to February 2009. There were 10 males and five females, at age range of 19-65 years ( average 38.0 years). The surgical method was summarized and the shoulder function evaluated.Results The follow-up lasted for 7-24 months (mean 13 months) , which showed bony union in all patients, with mean union time of 4.7 months. There was no implant loosening or failure. According to Neer 's criteria, the result was excellent in seven patients and good in six, with excellence rate of 86.7%.Conclusions Open reduction and internal fixation using tibial helical plate is simple and effective for treatment of proximal and middle one-third of humeral fractures.

4.Enhanced recovery after surgery in perio perativem anagement of hepatectomy:a Meta-analysis Chen

Objective To conduct a systematic review on the safety and efficacy of enhanced recov -ery after surgery ( ERAS) in perioperative management of hepatectomy .Methods A literature search was conducted on databases which included the PubMed , Embase, Cochrane Library, Sinomed, Wangfang, VIP and CNKI for randomized controlled trials ( RCTs) on application of ERAS in patients after hepatectomy . The data collection ended in August 2015.A meta-analysis was performed using RevMan 5.3 software.Re-sults Eleven RCTs which included 1074 patients were selected into this study .There were 530 patients in the ERAS group and 544 patients in the control group .On Meta-analysis, when compared with the control group, the ERAS group had significantly shorter length of hospital day (WMD=-2.36, 95%CI: -3.19~-1.54 , P<0.05 ) , shorter time for functional recovery ( WMD=-2.30 , 95%CI: -3.77 ~-0.83 , P<0.05), lower total complication rate (RR=0.65, 95%CI:0.52~0.80, P<0.05), and significantly decreased rates of postoperative pulmonary infection (RR=0.36, 95%CI:0.14~0.91, P<0.05) and nausea and vomiting (RR=0.48, 95%CI:0.26~0.89, P<0.05).There were no significant differences between the two groups on the rates of postoperative bleeding , biliary fistula, abdominal infection, delayed incisional healing, wound infection and urinary tract infection (P>0.05).The ERAS group had significant-ly lower hospitalization cost (SMD=-1.61, 95%CI:-2.42~-0.80, P<0.05), but the differences between the two groups on mortality and re-admission rates were not significant (P>0.05).When compared with the control group , the drainage tube removal time ( WMD=-2.83 , 95% CI:-3.92~-1.76 , P<0.05), time to first mobilization (SMD=-2.34, 95%CI:-2.98~-1.70, P<0.05), time to first feeding ( SMD=-5.08 , 95%CI: -9.33~-0.83 , P<0.05) , time to passage of first flatus ( SMD=-3.60, 95%CI:-4.85~-2.34, P<0.05) in the ERAS group were significantly shorter , but there was no significant difference on the time to the first bowel motion ( P>0.05 ) .Conclusions ERAS in the peri-operative management of hepatectomy was safe and beneficial .
5.Effects of hyperbaric oxygenation therapy on the concentrations of sICAM-1,sVCAM-1,sE-selectin and matrix metalloproteinase-9 in patients with cerebral infarction

Objective To investigate the effects of hyperbaric oxygenation(HBO)therapy on serum con- centrations of soluble intercellular adhesion molecule-1(slCAM-1),soluble vascular cell adhesion molecule-1(sV- CAM-1),soluble E-selectin(sE-selectin)and matrix metalloproteinase-9(MMP-9)in patients with aeute cerebral infarction and their clinical implications.Methods One hundred and twelve cases of cerehral infarction in the ca- rotid artery system were assigned into two groups.Patients in the routine treatment group(RT group,n=62)were treated with routine clinical treatment regime,whereas those in the HBO group(n=50)were treated with H BO ther- apy in additioo to routine clinical treatment.Thirty age- and sex-matched normal subjects were recruited and served as controls.The serum concentrations of sICAM-1,sVCAM-1,sE-selectin and MMP-9 were measured hy using ELISA method before and 10 days after treatment.The assessment of neurological deficits using National Institutes of Health Stroke Scale(NIHSS)score was conducted before treatment,and at 10 and 30 days after treatment,and the therapeutic efficacy was evaluated.Results The concentrations of sICAM-1,sVCAM-1,sE-selectin and MMP-9 of the patients were significantly higher than those of the control subjects.These parameters were all decreased signifi- cantly after treatment in the two patient groups.Moreover,these parameters were lower in the HBO group than those in the RT group after treatment.The NIHSS scores of HBO group were significantly lower than that of the RT group at the 30th clay post-treatment.The effective rate of HBO group was higher than that of RT group.Conclusion HBO therapy can decrease the serum levels of sICAM-1,sVCAM-1,sE-seleetin and MMP-9,which might be one of the mechanisms of HBO in the treatment of cerebral infarction.

7.FLUORESCENT STAINING METHOD DEMONSTRATING CELLS IN DIFFERENT STAGES OF DIFFERENTIATION

Fluorescent staining method is a sensitive cytochemical staining method, it can reflect minimal changes of biochemical components of cells. The 7690-Xu fluorescent staining solution prepared with thioflavine can show the heterogeneo- us fluorescence of cells in different stages of differentiation. The younger cells with low degree of differentiation show blue fluorescence, the lower the degree the deeper the color. Along with the differentiation of cells, the blue color of fluorescence deflects to the red color of spectrum gradually, the blue color of fluorescence of cytoplasm changes gradually to bluish, bluish-green, and finally yellow color. The blue color of fluorescence of nucleus changes through greyish- blue to orangeyellow or orange-red.The cytoplasm of cells in various organs with high degree of differentiation appears yellow, and the nucleus orange-yellow or orange-red in color.
8.Everolimus after liver transplantation: a Meta-analysis

Objective To compare everolimus (EVR) with calcineurin inhibitor (CNI) minimization or withdrawal on renal function of liver transplant patients with standard CNI therapy.Methods A search was conducted on databases which included the PubMed,Embase and Cochrane library for randomized controlled trials (RCTs) comparing EVR with CNI with minimization or withdrawal (the EVR group) with standard CNI therapy (the standard CNIs group) on renal function of liver transplant patients.A metaanalysis was performed using RevMan 5.3 software.Results Five RCTs which included 1 264 patients were selected into this study.There were 790 patients in the EVR group and 474 patients in the standard CNIs group.On meta-analysis,the EVR group had significantly better renal function (SMD =0.36,95% CI 0.09 ~ 0.64,P < 0.05),but higher rates of infection (RR =1.37,95% CI 1.08 ~ 1.74,P < 0.05),dyslipidemia (RR =2.46,95% CI 1.79 ~ 3.38) and leukopenia (RR =2.37,95% CI 1.32 ~ 4.26).No significant differences were found on the mortality and the acute rejection rates between the two groups (all P < 0.05).Conclusions EVR with CNI minimization or withdrawal after liver transplantation provided effective immunosuppression and improved patients' renal function.The treatment increased the rates of infection,dyslipidemia and leukopenia.
9.Characteristics of acupoints selection of moxibustion for primary dysmenorrhea based on data mining technology.

By using data mining technology, the characteristics of acupoints selection of modern moxibustion for primary dysmenorrhea (PD) were analyzed. The modern literature regarding moxibustion for PDl was coll6cted~ from a comprehensive retrieval in Chinese Biomedical Medicine (CBM), China National Knowledge Infrastructure (CNKI), VIP and PubMed. The prescription database of moxihustion was established, and the association rule4 method of data mining technology was adopted to analyze characteristics and rules of acupoints selection of modern moxibustion for PD. As a result, a total of 35 acupoints were involved with a total frequency of 399. The acupoints, with higher selected frequency included Guanyuan (CV 4, 90 times), Sanyinjiao (SP 6, 47 times), Shenque (CV 8, 43 times), Zhongji (CV 3, 3 times) and Qihai (CV 6, 28 tirmes); the ratio of yin-meridian acupoints to1 yang-meridian acupoints was approximately 341; the utilization of the specific acupoints accountedfor68. 6% (24/35); the crossing acupoints were used mostly (203 "times), featured with high acupoints selected, especially Guanyuan (CV 4), Sanyinjiao CSP 6) and Zhongji (CV 3); the front-mu points (130 times), he-sa points (19 times) and lower he-sea points (18 times) were frequently used. The results reveal thiat acupoints selection of moxibustion of PD focuses on the local acupoints and comnbination with the distal acupoints. The yin-meridian acupoints are preferred and the specific acupoints are the main part of the prescription. This conclusion can also be used to optimize the choosing of acupoints in clinic.

10.Problems and solutions on transformation of scientific research achievements of acupuncture.

With more and more attention and investment on acupuncture scientific researches, considerable outcomes and achievements has been acquired, but the shortcoming of low transformation rate of acupuncture research achievements is gradually exposed. Nowadays there is no related report on this problem, so based on achievement translational research in other areas and practical situation of acupuncture, the existing problems and solutions are analyzed. As a result, the existing problems include (1) the research content is mainly basic research and clinical research but less acupuncture device research, leading to limited transformation efficiency; (2) the evaluation system and transformation pattern are still needed to be perfect. The solutions are (1) to properly evaluate the research achievements of acupuncture, (2) to advocate the concept and method of translational medicine, (3) to reform the policy and system, and (4) to establish valid platforms covering research, outcomes and transformation.
