2.Ultrasound diagnosis types of placenta previa during the late pregnancy and its relationship with the perinatal outcome

Objective To investigate ultrasound diagnosis different types of placenta previa (PP)during the late pregnancy and its relationship with the perinatal outcome.Methods The clinical data of 170 pregnant women with PP and 170 healthy pregnant women who underwent ultrasound were retrospectively analyzed.PP-related risk factors were analyzed,the perinatal outcome of different types of PP were compared.Results Among PP pregnant women,complete PP was in 82 cases,partial PP was in 36 cases,marginal PP was in 32 cases,low-lying placenta was in 20 cases.The rate of age > 30 years old,parity,gravidity,the number of abortion and the number of cesarean section in PP pregnant women were significantly higher than those in healthy pregnant women [59.4%(101/170) vs.40.0%(68/170),(1.5 ± 0.6) times vs.(1.3 ± 0.4) times,(2.4 ± 0.5) times vs.(1.7 ± 0.1) times,(1.7 ± 0.1) times vs.(1.1 ± 0.3) times,(1.3 ±0.3) times vs.(1.0 ± 0.1) times,P < 0.01].Parity,gravidity,the number of abortion,the number of cesarean section,age,and the incidence rate of cesarean section,postpartum hemorrhage,placental adherence,premature delivery,neonatal weight < 2.5 kg,5 min Apgar score < 7 scores in complete PP pregnant women were significantly higher than those in partial PP,marginal PP and low-lying placenta pregnant women (P<0.01).Conclusions Ultrasound can dynamically observe the development process of the PP.There is a certain correlation with the severity of risk factors of PP,early intervention can improve the prognosis.
4.Changes of Plasma Concentration of Calcitonin Gene-related Peptide and Substance P after Electrical Stimulation on Dura Mater Adjacent to Sinus Sagittalis Superior in Rats

ObjectiveTo analyze the changes of plasma concentration of calcitonin gene-related peptide (CGRP) and substance P (SP) after electrical stimulation on dura mater adjacent to sinus sagittalis superior (SSS) in rats.Methods13 rats were randomly divided into control group(n=6) and stimulation group(n=7) in which the dura matter adjacent to SSS was stimulated electrically with square wave of 20 Hz, 4 mA and 250 μs. The plasma concentrations of CGRP and SP in both groups were compared by means of radioimmunity method.ResultsThere was significant difference in the plasma concentration of CGRP between control and stimulation groups (50.60±7.16 pg/ml vs 42.20±4.82 pg/ml, P=0.029). However, there was no significant difference in the plasma concentration of SP between two groups (308.59±43.07 pg/ml vs 264.60±60.28 pg/ml, P=0.165).ConclusionPlasma concentration of CGRP decreased after electrical stimulation on dura mater in rats but the concentrations of SP unchanged.
5.Analysis of influencing factors for synchronous colorectal liver metastasis: a report of 3 172 cases

Objective:To investigate the influencing factors for synchronous colorectal liver metastasis (synCRLM).Methods:The retrospective case-control study was conducted. The clinicopathological data of 3 172 patients with primary colorectal cancer (CRC) who were admitted to the Affiliated Hospital of Qingdao University from January 2010 to January 2016 were collected. There were 1 946 males and 1 226 females, aged (63±12)years, with a range from 21 to 97 years. Observation indicators: (1) general data analysis; (2) clinicopathological data analysis; (3) analysis of influencing factors for synCRLM. Measurement data with normal distribution were represented as Mean±SD. Count data were represented as absolute numbers. The influencing factors for synCRLM were analyzed after excluding missing data of tumor differentiation degree, tumor diameter, pathological T stage and N stage. Univariate analysis was conducted by chi-square test or Logistic regression model. Multivariate analysis was conducted by Logistic regression model. Results:(1) General data analysis: among the 3 172 patients, cases with age ≤29 years, from 30 to 39 years, from 40 to 49 years, from 50 to 59 years, from 60 to 69 years, from 70 to 79 years, and ≥80 years were 15, 82, 342, 774, 965, 759 and 235, respectively. There were 2 972 patients in Qingdao, 172 cases in Yantai and 28 cases in Weihai. Of the 2 972 patients in Qingdao, there were 422 cases in Shinan District, 658 cases in Shibei District, 457 cases in Huangdao District, 144 cases in Laoshan District, 188 cases in Licang District, 205 cases in Chengyang District, 252 cases in Jimo District, 221 cases in Jiaozhou City, 255 cases in Pingdu City, 170 cases in Laixi City. (2) Clinico-pathological data analysis: among the 3 172 patients, there were 1 639 cases of colon cancer including 972 cases with left colon cancer and 667 cases with right colon cancer, 1 533 cases of rectal cancer. There were 2 981 cases of adenocarcinoma, 165 cases of mucinous adenocarcinoma, 10 cases of signet ring cell carcinoma and 16 cases of other types including carcinoid tumor, squamous carcinoma, tubular adenocarcinoma, etc.There were 162 cases with highly differentiated adenocarcinoma, 5 cases with highly-moderately differentiated adenocarcinoma, 2 338 cases with moderately differentiated adenocarcinoma, 80 cases with moderately-poorly differentiated adeno-carcinoma, 396 cases with poorly differentiated adenocarcinoma and 191 cases missing tumor differentiation data. There were 708 cases with tumor diameter <4 cm, 1 957 cases with tumor diameter ≥4 cm and 507 cases missing tumor diameter data. There were 486 cases in T1 or T2 stage of pathological T stage, 2 169 cases in T3 or T4 stage of pathological T stage and 517 cases missing tumor pathological T staging data. There were 1 563 cases in N0 stage of pathological N staging, 1 062 cases in N1 or N2 stage of pathological N staging and 547 cases missing tumor pathological N staging data. There were 2 895 cases without synCRLM and 277 cases with synCRLM. There were 2 799 cases without diabetes and 373 cases with diabetes. There were 2 931 cases without fatty liver and 241 cases with fatty liver. There were 2 989 cases negative for hepatitis B surface antigen (HBsAg) and 183 cases positive for HBsAg. (3) Analysis of influencing factors for synCRLM. Results of univariate analysis showed that gender, tumor location, tumor differentiation degree, tumor diameter, pathological T stage, fatty liver, HBsAg were related factors for synCRLM in primary colorectal cancer ( χ2=7.400, 7.577, 7.111, 4.513, 12.125, 5.686, 5.919, P<0.05), and neutrophils counts, lymphocytes counts, platelet counts, alanine aminotransferase (ALT), aspartate aminotrans-ferase (AST), total bilirubin, γ-glutamyltransferase (GGT), triacylglycerol (TG), total cholesterol (TC), carcinoembryonic antigen (CEA), and CA19-9 were related factors for synCRLM in primary colorectal cancer ( odds ratio=1.101, 0.807, 1.002, 1.017, 1.023, 1.027, 1.012, 0.686, 1.169, 1.007, 1.004, 95% confidence interval as 1.048-1.156, 0.678-0.960, 1.001-1.004, 1.011-1.024, 1.016-1.031, 1.011-1.044, 1.009-1.015, 0.541-0.869, 1.047-1.306, 1.006-1.008, 1.003-1.004, P<0.05). Results of multivariate analysis showed that cases as male, case with positive HBsAg, AST, GGT, TC, CEA and CA19-9 were independent risk factors for synCRLM in primary colorectal cancer ( odds ratio=1.503, 2.492, 1.018, 1.007, 1.301, 1.005, 1.003, 95% confidence interval as 1.038-2.178, 1.443-4.304, 1.003-1.034, 1.003-1.011, 1.112-1.522, 1.003-1.006, 1.002-1.003, P<0.05), and lymphocytes, ALT and TG were independent protective factors for synCRLM in primary colorectal cancer ( odds ratio=0.777, 0.983, 0.602, 95% confidence interval as 0.608-0.993, 0.966-0.999, 0.421-0.862, P<0.05). Conclusion:Cases as male, case with posotive HBsAg, AST, GGT, TC, CEA and CA19-9 are independent risk factors for synCRLM in primary colorectal cancer, while lymphocytes, ALT and TG are independent protective factors for synCRLM in primary colorectal cancer.
6.Establishment of adventitious root culture system and scale-up fermentation of Tripterygium wilfordii.

Using MS as basic medium, supplemented with 1.0 mg · L(-1) IBA, the adventitious roots of Tripterygium wilfordii were induced, and the good adventitious root culture system was established by leaves or callus induced by leaves as explants. The adventitious roots were also induced with 2.0-4.0 mg · L(-1) NAA and the good adventitious root culture system established by using suspension cells from callus as materials to induce adventitious root. The content of triptolide of three adventitious roots culture system were exceeded in the natural root bark. The content of triptolide of AR3 adventitious roots was the highest about 5.3 times as that in the natural root bark. By using 5 L stirred fermentor during pilot enlarge cultivation, compared with 250 mL flask cultivation, the adventitious roots increment and secondary metabolites content per liter medium showed no significant difference. The accomplishment of this analysis laid a foundation by tissue culture production of the secondary metabolites of T. wilfordii.

8.Weekly docetaxel combined with cisplatin versus weekly docetaxel combined with oxaliplatin in the treatment of advanced gastric cancer

Background and purpose:It has confirmed that docetaxel, in combination with cisplatin or oxaliplatin have good effect in the treatment of advanced gastric cancer. This study in order to observe the clinical efficacy and adverse reaction of weekly docetaxel combined with cisplatin (DP) versus weekly docetaxel combined with oxaliplatin (DO) as the firrst-line treatment of advanced gastric cancer.Methods:A total number of 76 cases of advanced gastric cancer were randomly assigned 2 arms, 38 per arm. DP regimen group (docetaxel 35 mg/m2,ivgtt,dl,d8 combined with cisplatin 60 mg/m2, ivgtt,dl,repeated every 3 weeks) and DO regimen group(docetaxel 35 mg/m2,ivgtt,dl,d8 combined with oxaliplatin 120 mg/m2 ivgtt,dl,repeated every 3 weeks). Results:No significant difference was found between DP regimen group and DO regimen group on the objective RR(37%vs 41%),PFS (4.9vs 4.4 months) and OS (9.7vs 12.3 months). The main grade 3 or 4 toxicity in the DP and DO groups was neutropenia, DO was less associated with nausea and vomiting, but more associated with peripheral neuropathy than DP group. No signiifcant difference was found between DP regimen group and DO regimen group on the anemia, thrombocytopenia, diarrhea, alopecia (P>0.05).Conclusion:Weekly docetaxel combined with cisplatin (DP) shows similar efifcacy and toxicity compared with weekly docetaxel combined with oxaliplatin (DO) as the ifrst-line treatment of advanced gastric cancer and worthy of further study.
9.Screw plate fixation, intramedullary fixation and artificial femoral head replacement in the repair of femoral intertrochanteric fractures in the elderly: choice and comparison

BACKGROUND:Nowadays,the internal fixation materials for the clinical treatment of femoral intertrochanteric fractures are broadly divided into two categories: intramedulary fixation system and extramedulary fixation system.Artificial femoral head replacement can be selected for unstable intertrochanteric fractures.However,the effects of diverse treatments usualy lack of macroscopical evaluation.OBJECTIVE:To investigate and compare the clinical effects of different surgical treatments for femoral intertrochanteric fractures in the elderly.METHODS:168 elderly patients with intertrochanteric fractures,who were treated in the Department of Orthopedics,Beijing Municipal Corps Hospital of Chinese People's Armed Police Forces from June 2005 to June 2010,were enroled in this study.They were treated by screw plate fixation system (dynamic hip screw,dynamic condylar screw),intramedulary fixation system (Gamma 3 nail,expandable intramedulary nail) and artificial femoral head replacement,including 69 males and 99 females,at the age of 75-94 years,averagely 81.4 years.According to Evans type,there were 20 cases of type Ⅰ,43 cases of type Ⅱ,57 cases of type Ⅲ,33 cases of typeⅣ and 15 cases of type Ⅴ.Operative time,blood loss,weight-bearing ambulation time,and hip function were compared and evaluated in patients of each group.RESULTS AND CONCLUSION:After treatment,patients were folowed up for 14-39 months.At 12 months after treatment,Harris score was used to assess hip function in the affected side.Fracture was healed within 12 months.There was no significant difference in excelent and good rate among four groups (P>0.05).Operative time and blood loss were significantly better in the expandable intramedulary nail group than in the other groups (P< 0.05).Weight-bearing ambulation time was significantly better in the artificial femoral head group than in the other groups (P< 0.01).Results suggest that according to fracture typing,in combination with osteoporosis in elderly patients,suitable repair method could improve therapeutic effects for femoral intertrochanteric fractures,and reduce postoperative complications.Dynamic hip screw is firstly selected for Evens types Ⅰ and Ⅱ stable intertrochanteric fractures.The intramedulary fixation system can be selected for unstable intertrochanteric fractures as Evans types Ⅲ-Ⅴ.Artificial hip arthroplasty is good for elderly patients with unstable intertrochanteric fractures as wel as severe osteoporosis.
10.Changes of motilin concentrations in plasma and upper gastrointestinal tract in rat model of acute incisional pain

Objective To observe changes of motilin(MTL) levels in gastric body, duodenum and plasma in rat model of acute incisional pain.Methods A total of 156 healthy male adult SD rats,weighing 180-220 g,were randomized into two groups:control group (group C,n=78) and incisional pain group (group P,n=78),Rats in P group received incision on the right plantaris. Values of paw withdrawal mechanical threshold (PWMT) and paw withdrawal thermal latency (PWTL) at dif?ferent time points of 24 hours before operation (T0) and 1 hour (T1),6 hours (T2),24 hours(T3),48 hours (T4) and 72 hours (T5) after operation were measured in six rats chosen randomly from each group. Twelve rats were chosen from each group at T 0-5, and sacrificed. The MTL levels in plasma, the mucosal tissues of gastric body and duodenum were detected by ELISA. Re?sults Compared with group C, PWMT and PWTL were significantly decreased at T1-4 in group P. The MTL levels were sig?nificantly decreased in plasma and gastric body (P<0.05).The MTL level was significantly increased at T1-4 in duodenum (P<0.05),and no significant changes were found at T0 and T5 in P group(P>0.05). The plasma MTL levels were positively correlated with PWMT and PWTL (r=0.952,r=0.879,respectively,P<0.01) in P group. The MTL levels in gastric body were positively correlated with PWMT and PWTL(r=0.970,r=0.931,respectively,P<0.01) in P group. The MTL levels were neg?atively correlated with PWMT and PWTL(r=-0.991,r=-0.975,respectively,P<0.01) in duodenum in P group. Conclu?sion The MTL levels in plasma and gastric body are decreased in rat model of acute incisional pain, and increased in duo?denum.
