2.Applied anatomy and cast observation in lingual arterial chemoembolization

Objective: To provide anatomical basis for the chemotherapy of lingual arterial embolization in clinic. Methods: The origin, course, branch distribution and anastomosis of the lingual artery were observed in 15 cases of adult head specimens. Results: As one of principal branches, the lingual artery arises from the external carotid artery at the level of the major cornu of hyoeides, approaches to the carotid bifurcation. With original diameter of (2.4?0.3) mm, it runs upward and passes deepwards to the hyoglossus muscle, gives off the dorsal lingual artery and the terminal branch -profunda lingual artery. Limited in each side of intrinsic muscles, two profunda lingual arteries creep tortuously along the muscular fibers and anastomose freely to structure submucous arterial rete. Conclusion: With concentrated origins, wide vascular diameter, constant course and enrichment of submucous arterial rete, the lingual artery is an ideal blood vessel for arterial chemoembolization.

4.Relationship between methylation of Ras association domain family-1A and protein expression in ;endometriosis

Objective To analyze the methylation status and protein expression of Ras association domain family- 1A (RASSF1A) in endometriosis (EMS). Methods The ectopic and corresponding eutopic endometrium tissues were collected from 45 women with EMS and normal endometrium tissues of 20 women without EMS. The methylation status of RASSF1A was examined by methylation specific PCR (MSP). Immunohistochemistry was performed to measure the level of RASSF1A in endometrium tissues. Results The RASSF1A protein expression rate in ectopic endometrium, eutopic endometrium, and normal endometrium was 37.78%(17/45), 60.00%(27/45) and 85.00%(17/20), and there was significant difference (χ2 = 13.136, P = 0.001). The frequency of aberrant methylation of RASSF1A was 55.56%(25/45), 33.33%(15/45) and 0 in ectopic endometrium , eutopic endometrium, and normal endometrium, and there was significant difference (χ2 =18.770, P = 0.000). The frequency of aberrant methylation of RASSF1A had no significant differnce throughout the menstrual cycle in ectopic endometrium and eutopic endometrium: 66.67%(14/21) vs. 45.83%(11/24), 38.10%(8/21) vs. 29.17%(7/24), P>0.05. In ectopic endometrium, the frequency of aberrant methylation of RASSF1A inⅢ-Ⅲstage was significantly higher than that in Ⅰ-Ⅱstage (χ2=5.940, P=0.015). In ectopic endometrium and eutopic endometrium, the RASSF1A protein expression had negative correlation with aberrant methylation of RASSF1A (r =- 0.594、- 0.577, P<0.01). Conclusions Epigenetic inactivation of RASSF1A through aberrant promoter methylation may be strongly correlated with the formation and progression of EMS, and assessment of RASSF1A methylation status in eutopic endometrium may be a potentially useful biomarker to enhance the early detection of EMS.
5.The significance of RASSF2A promoter methylation in the early diagnosis of epithelial ovarian cancer

Objective To evaluate the methylation status of RASSF2A in epithelial ovarian cancer (EOC) tissues and plasma, and to explore the correlations in two kinds of samples with clinicopathological characteristics. Methods The frequency of methylation of the RASSF2A gene in tissues and corresponding plasma samples from 60 EOC patients, 20 patients with benign ovarian tumors and 10 patients with normal ovarian tissues were detected respectively. The methylation-specific PCR was used to determine the methylation status. Results The frequency of aberrant methylation of RASSF2A was 58.3% (35/60) in EOC tissues and 43.3%(26/60) in corresponding plasma samples respectively, however, such hypermethylation was not detected in the benign ovarian tumors and normal ovarian samples, and there was significant difference between them (P<0.05). Besides, RASSF2A methylation levels in serum of EOC patients correlated reasonably well with methylation status in tumor samples (r=0.739, P=0.000). No significant differences were observed between the promoter hypermethylated status of the RASSF2A and clinicopathological characteristics (P>0.05). Conclusions The promoter methylation of RASSF2A gene is an early and frequent epigenetic event in EOC. Detection of promoter hypermethylation in plasma specimens can help the early diagnosis of EOC.
6.Mandibular distraction combined with orthognathic techniques for the correction of adult hemifacial microsomia.

OBJECTIVETo study the combination of Mandibular distraction and orthognathic techniques for the reconstruction of adult hemifacial microsomia.
METHODSThe three-dimensional CT reconstruction data was used with Mimics for preoperation design. The osteotomy location, distraction vector, distraction distance were decided before operation with a surgical guider. At the first stage, internal distractor was implanted after ostetomy through an extra-oral approach. The distraction begun 5-7 days after operation with a frequency of 1 mm/day. After distraction, the distractor was maintained for 3-6 months. At the second stage, the distractor was removed. Le Fort I osteotomy was performed in order to correct the cross-bite and improve the facial contour. Usually, bone graft was inserted into the gap after Le Fort I osteotomy. The genioplasty was also performed if necessary.
RESULTS9 cases of adult hemifacial microsomia with severe mandibular deviation were treated. The facial asymmetry were improved greatly. 1 patient suffered an wound infection in the maxillary region after Le Fort I osteotomy and healed uneventfully with wound irrigation.
CONCLUSIONSMandibular distraction combined with orthognathic surgery is an effective procedure for adult hemifacial microsomia with complicated mandibular hypoplasia.

8.Role of lactoferrin and C-reactive protein in ascites of liver cirrhosis for diagnosis of spontaneous bacterial peritonitis

Objective This study was to investigate the role of lactoferrin and C-reactive protein (CRP)assay in ascitic fluid for diagnosis of spontaneous bacterial peritonitis (SBP)in the patients with decompensated liver cirrhosis.Methods Ascites was collected from the inpatients with decompensated liver cirrhosis before and after treatment from May to December 2011 for anal-ysis of polymorphonuclear cells (PMN)and bacterial culture.The level of lactoferrin and C-reactive protein in the ascites were determined.Results A total of 117 ascites samples were collected from 66 patients.Twenty-six patients met the criteria of SBP with PMN ≥ 250×106/L in ascites,were assigned to SBP group.Of these patients,11 presented with fever 37.3℃ to 38℃, and 11 patients had elevated peripheral blood white cell count > 10 × 109/L.Eleven patients had neutrophil cell percentage >0.75.Only 8 patients in this group had positive bacterial culture.Another 12 patients met the criteria of suspected SBP,and assigned to suspected SBP group.The remaining 28 patients did not satisfy the criteria of SBP,and assigned to non-SBP group.The pretreatment lactoferrin level was (768.46 ± 611 .70)ng/mL and (98.28 ± 56.81 )ng/mL in SBP and non-SBP group,respectively.The pretreatment CRP level was (9.397 ±3.737 )mg/L and (1 .786 ±0.52 )mg/L in SBP and non-SBP group,respectively.The lactoferrin and CRP levels decreased sharply after antibacterial and support-ive treatment in SBP group,which were 657.05 ng/mL and 8.13 mg/L,respectively.The cut-off value of lactoferrin for diagnosis of SBP was 233 ng/mL with sensitivity 96.2% and spe-cificity 97.5%.The cut-off value of CRP for diagnosis of SBP was 4.390 mg/L with sensitivity 92.3% and specificity 92.5%. However,lactoferrin combined with CRP had a sensitivity of 99.70% and specificity of 90.18% for diagnosis of SBP.Conclu-sions Lactoferrin and CRP levels in the ascites of patients with liver cirrhosis are useful for diagnosis of SBP with high specifici-ty and sensitivity.
9.Mechanical strength and degradation in vivo of human hair polylactic acid composite rods

BACKGROUND: Biodegradable internal fixation devices have been used in clinics. However, they can only be used for internal fixation of non weight-bearing bones. A new kind of biodegradable internal fixation device with high initial strength and slow early degradation, which can be used for weight-bearing bones, is required.OBJECTIVE: To test the initial strength and observe the degradation of human hair polylactic acid (HHPLA) composite rods.DESIGN: Repeated measurement designSETTING: Center of Orthopaedic Department, Zhujiang Hospital, Southern Medical University; Department of Human Anatomy of Southern Medical UniversityMATERIALS: The experiment was conducted in the Southern Medical University between September 2002 and October 2003. Totally 54 adult SD rats were recruited.METHODS: ①The shear strength, the bending strength and the bending modulus of 20 HHPLA composite rods were tested with the biomechanical testing system (MTS-858 Mini Bionix) to investigate the initial nechanical feature of HHPLA composite rods. ②A total of 108 samples of HHPLA composite rods, which were developed for internal fixation of fracture, were randomly implanted in bilateral dorsal subcutaneous tissue of 54 SD rats and taken out at weeks 1, 2, 4, 8, 12, 16, 20, 24 and 28 (6 rats at each time point). The weight losses were measured to understand the degradation of HHPLA composite rods in SD rats.MAIN OUTCOME MEASURES: ①Mechanical strength of HHPLA composite rods. ②Degradation in vivo of HHPLA composite rods.RESULTS: Totally 54 rats entered the result analysis. ①HHPLA composite rods has good initial mechanical strength. Its shear strength of the HHPLA rod was 241 MPa, bending strength was 358 MPa, and the bending modulus was 13 GPa. ②HHPLA was fairly degradable in SD rats.Degradation was slower in the earlier period than that in the later period.CONCLUSION: HHPLA composite rods have some fairly good characteristics of initial mechanical strength and degradation in vivo.
10.Prognostic analysis of allogeneic hematopoietic stem cell transplantation in treating patients with malignant hematological diseases

Objective To explore the clinical related prognostic factors of allogeneic hematopoietic stem cell transplantation (allo-HSCT) in treating malignant hematological diseases. Methods From September 1997 to August 2008,a total of 26 patients with hematological diseases were treated with allo-HSCT from HLA identical-sibling and haplo-identical donors in our hospital, including 14 patients with acute leukemias,10 with chronic myeloid leukemias,and 2 myelodysplastic syndromes. Results All patients achieved sustained full donor type engraftment. The cumulative overall survival (OS) was 63.9 %,and cumulative disease free survival (DFS) was 62.6 %. Fifteen patients had graft-versus-host disease (GVHD) (57.7 %),including 8 acute GVHD(aGVHD) (30.8 %) (grade Ⅲ-Ⅳ aGVHD was 15.4 %) and 7 chronic GVHD. GVHD between HLA identical-sibling and haplo-identical donors was different and there was statistic difference between the two groups (P=0.014). 4 patients relapsed,7 patients died. The univariate analysis showed OS were correlated with grade Ⅳ aGVHD (P=0.05) and CMV infection (P=0.027). Conclusion Allo-HSCT is effective for the cure of patients with malignant hematological diseases. The key to improve the efficacy of HSCT is to reduce the incidence of transplant-related complications,especially GVHD and infection.
