1.Clinical Observation of Vitamin A Acid Combined with Triple-drug in the Treatment of Oral Leukoplakia

OBJECTIVE:To observe the efficacy and safety of vitamin A acid combined with vitamin A,vitamin E,celecoxib in the treatment of oral leukoplakia. METHODS:104 patients with oral leukoplakia were randomly divided into control group(52 cases) and observation group (52 cases). Control group received 1 Vitamin A soft capsule,3 times a day+1 Vitamin E soft cap-sule,3 times a day+1 Celecoxib capsule,twice a day. Observation group additionally received 1 Vitamin A acid tablet,twice a day. 4-week was regarded as 1 treatment course,it lasted for 3 courses. Clinical efficacy,and leukoplakia number,maximum diam-eter of leukoplakia,immunoglobulin A(IgA),IgM,IgG before and after treatment,and the incidence of adverse reactions in 2 groups were observed. RESULTS:The total effective rate in observation group was significantly higher than control group,the dif-ference was statistically significant(P<0.05). Before treatment,there were no significant differences in leukoplakia number,maxi-mum diameter of leukoplakia,immunoglobulin levels in 2 groups(P>0.05). After treatment,leukoplakia number,maximum diam-eter of leukoplakia,IgA and IgM in 2 groups were significantly lower than before,and observation group was lower than control group,IgG was significantly higher than before,and observation group was higher than control group,the differences were statisti-cally significant (P<0.05). And there was no significant difference in the incidence of adverse reactions in 2 groups (P>0.05). CONCLUSIONS:Vitamin A acid combined with vitamin A,vitamin E,celecoxib shows obvious efficacy in the treatment of oral leukoplakia,it can effectively reduce clinical symptoms,improve immune function,with good safety.

Osteoarthritis is a chronic disabling disease that tends to occur in middle-aged and elderly population, the main pathological characteristics are the dynamic imbalance between the synthesis and degradation of articular chondrocytes, extracellular matrix and subchondral bone. The occurrence and development is related to many factors. This article reviews the structure, the function and the interaction between α2-macroglobulins and a disintegrin and metallo-proteinase with throm-bospondin motifs in the development of osteoarthritis.
5.Analysis of correlation between dry eye and diabetic retinopathy in type 2 diabetic patients

?AIM:To investigate the correlation between dry eye and different degrees of diabetic retinopathy ( DR ) in type 2 diabetic patients.?METHODS: In the cross-sectional study, 340 patients (340 eyes) with type 2 diabetes were enrolled. Tear film function tests including tear meniscus height, tear film breakup time ( BUT ) , fluorescein staining, Schirmer Ⅰtest were performed followed by surveying questionnaires about dry eye. Retinal status was evaluated by retinal color photography and indirect ophthalmoscopy exam with dilated pupils to evaluate DR and whether companied by macular edema.?RESULTS:The prevalence of dry eye was 49. 41%. The mean duration of diabetes in patients with dry eye was 11.15±7.07a, while 6.92±5.45a without dry eye(P<0.01). Dry eye had the positive correlation to the development of DR. The incidence of dry eye in people with mild nonproliferative diabetic retinopathy ( NPDR ) , moderate NPDR, severe NPDR and proliferative diabetic retinopathy (PDR) was 1. 097 times, 1. 724 times, 2. 86 times and 5. 43 times respectively, compared with people without DR. The occurrence of dry eye in people with macular edema increased by 3. 697 times compared with people without macular edema.?CONCLUSION: Dry eye was more prevalent in people with type 2 diabetes. The incidence of dry eye increased gradually with the occurrence and development of diabetic retinopathy.
6.The Use of Pulsed Dye Laser for the Treatment of Xanthelasma Palpebrum.

Xanthelasma palpebrum is the most common type of xanthoma that develops mainly on the inner epicanthus of upper eyelids with symmetrical distribution. The lesion tends to be permanent, progressive and cosmetically disfiguring. The recommended treatment has been surgical excision, local treatment with chemicals, and carbon dioxide laser. All of the mentioned treatment have some disadvantages including scarring, pigmentary change and need for local anesthesia. We describe the treatment of xanthelasma palpebrum with the pulsed dye laser which coagulates the hyperpermeable vessels so that the lipid leakage could be blocked and prevent recurrence and further progression.

7.Analysis of the usage of TCRβ V/J subfamily and CDR3 repertoire in patients with peripheral T-cell lymphoma unspecified

Objective To analyze the restricted usage of TCRβ V/J subfamily and CDR3 repertoire in patients with peripheral T-cell lymphoma unspecified (U-PTL).Methods The total RNA was respectively extracted from lymph node of U-PTL and reverse transcriptase,then multi-PCR was used to amplify the complete DNA sequence(CDS) of TCR β-chain.The recombinant plasmids were sequenced and sequence was analyzed by using online TCR resources.Results There were 9 TCR β chain CDS obtained from four patients.TCRβ-chain presented specific repertoire skewing in patients with U-PTL.There were restricted usage of BV2,BV4S2,BV14,BV29S1 of BV subfamily and BJ1S4,BJ2S3,BJ2S5,BJ2S7 of BJ subfanily.The clonal proliferation T cells had different CDR3 amino acid sequences.Conclusions There are restricted usage of TCR β V/J subfamily in patients with U-PTL.The sequences of CDR3 in different TCR clone proliferation are mostly different.
8.How to control the leg length in total hip arthroplasty for Crowe Ⅳtype developmental dysplasia of the hip

Objective To investigate the method of controlling leg length in total hip arthroplasty for CroweⅣtype devel?opmental dysplasia of the hip. Methods From February 2006 to February 2011, primary total hip arthroplasty were performed in 11 patients (3 males and 8 females, aged 54 year and ranged from 45 to 65 years) with CroweⅣtype developmental dysplasia of the hip using subtrochanteric femoral osteotomy. Leg length was measured in the method of preoperation plan combining with mea?surement in the course of operation. Five hips were involved in left side, 6 in right side. The average leg length discrepancy were 3.5 cm (ranged, 1.8-6.0 cm). All patients have lumbosacral pain. Clinical and radiographic characteristics were retrospectively re?viewed at the 3, 6 and 12 months after operation. Outpatient follow?up was conducted every year. The lumbosacral pain and degree of patient satisfaction were recorded. Moreover, leg length discrepancy, the bone union, prosthesis subsidence, and the hip Harris score were evaluated. Results Average follow?up was 4.5 years (3-8 years). The average length of resected bone was 2.2 cm (1.5-4.2 cm). The average leg length discrepancy were 1.0 cm (ranged,-1.5-1.5 cm). Lumbosacral pain of 8 patients was eliminated and slight pain was retained in 3 patients. Two patients were satisfied with the result of operation and 9 were extremely satisfied. No nerve injury and nonunion occurred. At the final follow?up, the mean Harris score improved from 45 ± 7.6 before operation to 93±6.6 (P<0.05). The mean union time was 5 months after operation (3-12 months after operation). The prosthesis subsidences were found in one case at the five years after operation and 1 case at the seven years after operation and with subsiding height of 3 mm and 6 mm respectively. No loosening and infection were found in all patients. Conclusion In treating patients with CroweⅣtype developmental hip dysplasia by THA, the length of resected subtrochanteric femoral should be conducted according to pa?tient's tolerance to pain reduced by leg extension. Therefore, the leg length is facilitated to improve the function of joint and to de?crease symptoms reduced by leg extension.
9.Serum proteomic analysis of acute pancreatitis in rats

Objective To investigate the difference of the protein expression in serum of rats with acute edematous pancreatitis (AEP) and acute necrotizing pancreatitis (ANP),and to investigate serum marker for acute pancreatitis severity.Methods The model of AEP and ANP was induced by retrograde injection of 4% or 5% sodium taurocholate into the biliopancreatic duct.Weak cation exchange (WCX2) and surface enhanced laser desorption/ionization time of flight mass spectrometry ( SELDI TOF MS) was used to investigate the difference of the protein expression in serum of rats with AEP and ANP.Results Thirty-eight spectral peak clusters which had significantly different signal intensities between AEP and ANP sera at mass charge ratio between 1000-50 000(P <0.05) were detected.The peak clusters at 9500 and 9700 in the sera of AEP were higher than that in ANP rata.Conclusions Serum analysis with SELDI-TOF MS can detect the difference of the protein expression in rats with AEP and ANP.The decreased expression of the protein of molecular weight of 9500 and 9700 may be a signal of AEP transition into ANP.
10.CT manifestations of pulmonary Langerhans cell histiocytosis

Objective To analyzes the CT manifestations of pulmonary Langerhans cell histiocytosis (PLCH). Methods CT features of 11 patients with PLCH proved pathologically were analyzed retrospectively. Results The main findings in 11 PLCHs were cysts and nodules. Two cases only had cysts,and 1 only had nodules, which most had cavitations. The other 8 cases showed cysts and nodules with 4 cases mainly manifested with cysts and nine mainly manifested with nodules. Two cases had pulmonary interstitial changes. One case only had cysts in the left upper lung field and 10 cases had lesions not only in the upper and middle but in the lower lung field, which 2 cases had more lesions in the lower and costophrenic angle field and 8 cases had less lesions in the costo-phrenic angle field. Three of these 8 cases had more lesions in the superior lobe and apical segment of lower lobe. Conclusion CT manifestations of PLCH are helpful for the early diagnosis.
