1.The effects of human dental pulp stem cells-conditioned medium on the osteogenic differentiation of human dental follicle stem cells in vitro

Objective:To evaluate the effects of human dental pulp stem cells-conditioned medium(HDPSCs-CM)on the osteogenic differentiation of human dental follicle stem cells(HDFSCs) in vitro.Methods:HDFSCs were in vitro cultured,purified and identified.CCK-8 assay was applied to evaluate the HDFSCs viability after 7 days cultured by HDPSCs-CM;the morphological changes of HDFSCs were observed;the osteogenic differentiation was studied by alkaline phosphatase(ALP) staining and alizarin red staining.The mRNA expression of POSTN,Col-Ⅰ,ALP,BSP and OPN was detected by Real-Time PCR.Results:Induced by HDPSCs-CM,HDFSCs exhibited several characteristics of cementoblast or osteoblast lineages.In the induction group the viability of HDFSCs was inhibited(P<0.05 or P<0.01),ALP staining was stronger and there were more mineralized nodules,the expression levels of POSTN,Col-Ⅰ,ALP,BSP and OPN were upregulated(P<0.05 or P<0.01).Conclusion:HDPSCs-CM can promote the osteogenic differentiation of HDFSCs.
2.Influence of different routes of nutrition on the respiratory muscle strength and outcome of elderly patients in respiratory intensive care unit

Objective To compare the influences of different routes of nutrition on the outcome and respiratory muscle strength of elderly patients in respiratory intensive care unit (RICU). Methods Totally 147 elderly patients in RICU were equally randomized into combined nutrition group ( combinination of parenteral nutrition and enteral nutrition), total enteral nutrition (EN) group, and total parenteral nutrition (PN) group. The changes of energy metabolism, respiratory muscle strength, and short-term outcome were observed or determined.Results Plasma albumin (ALB), hemoglobin (Hb), creatinine (Cr) levels, and nitrogen balance significantly increased in all group 7 days later ( all P < 0. 01 ), while blood urea nitrogen (BUN) significantly decreased ( P <0.01 ). The increase of ALB, Hb, Cr levels, and nitrogen balance were significantly more remarkable than that in EN group and PN group (P<0.05 or P<0.01). The decrease of BUN level in the combined nutrition group was significantly more remarkable than that in EN group ( P < 0. 05 ). After nutritional support, the maximum inspiratory pressure in the combined nutrition group (P =0. 021 ) and EN group (P = 0. 011 ) became significantly higher, and occlusion pressure at 0.1 second inspiration level in the combined nutrition group became significantly lower ( P =0. 025). The incidences of infectious and non-infectious complications in PN group were significantly higher than those in EN group (P = 0. 002 and 0. 017, respectively) and combined nutrition group ( P = 0. 005 and 0. 004, respectively). Gastric retention was more common in EN group than that in PN group ( P = 0. 035). The weaning time,length of RICU stay, length of hospital stay and 20-day-mortality were significantly decreased in the combined nutrition group than those in the other two groups ( all P < 0. 05 ). Conclusion The effectiveness of the combined application of PN and EN is superior to PN or EN alone in improving nutritional status and respiratory muscle strength as well as in improving the short-term outcome in elderly patients in RICU..

Objective To discuss the anxiety and panic disorder in the patients with cardiovascular diseases. Method To analyze the incidental rate, diagnostic criteria, clinical manifestations and treatment in the anxiety and panic disorders with cardiovascular diseases. Result The incidental rate of anxiety and panic attack in the patients with cardiovascular diseases is high. The common clinical manifestation including chest pain, hypertension, tachycardia, arrhythmia and symptoms similar with left heart failure. Conclusion Physicians ought to improve their ability in identifying the anxiety and panic disorders with cardiovascular diseases.

5.Necessary and Feasibility of Fistulous Tissue-Saving Anorectoplasty for Low Anorectal Malformation with Fistula in Children

Objective To investigate the necessary and feasibility of fistulous tissue-saving anorectoplasty for low anorectal malformation with fistula in children.Methods The clinical data of 67 cases of low anorectal malformation with fistula were analyzed retrospectively, there were 59 girls with rectovestibular fistula, 8 boys with rectoperineal fistula. The patients′ average age were 10 months, ran -ging from 3 months to 16 years. The clinical manifestations of all patients were intractable constipation, or out of position of anus. Cut-back procedure had been performed on 4 patients in infancy. The anterior sagittal anorectoplasty was performed in all cases.Results In all cases returned for follow-up at 3 and 6 months after operation, the longest was 8 years. The postoperative course of 65 patiens was uneventful without wound infection, and had a normal-looking perineum, other 2 cases with anal rupture in operation were with sagittal wound infection resulted from perforation in repairing anus, and formed finally rectoperineal fistula with a normal-looking anus. Sixty-four cases (95.5%) had good continence by 5-6 score, another 3 cases (1 boy, 2 girls) with intractable constipation defecated with help of Glycerinum.Conclusions The fistulous tissue-saving anterior sagittal anorectoplasty is a reliable and feasible procedure for low anorectal malformation with fistula. This operation provides good results.
6.Stroke-associated Infection:382 Serial Case Study

Objective To study the incidence,risk factors and prognosis of stroke-associated infection(SAI) in acute stroke patients.Methods 382 serial acute cerebral stroke patients were retrospectively surveyed.Results The incidence of SAI was 29.3% in all cases,mainly involved lower respiratory tract(63.0%) and urinary tract(28.4%).Escherichia coli,Coagulase negative staph and Streptococcus viridans were the main pathogenic organisms.Developments of SAI were closely related with aging,diabetes mellitus,the application of antimicrobial agents and invasive procedure.The mortality was 10.7%(12/112) in the cases with SAI,but 1.9%(5/270) in the cases without SAI.Conclusion SAI is caused by various factors in acute stroke patients.Controlling SAI can help to succeed in treating stroke.
7.Analysis of diagnosis and treatments of acute mesenteric ischemic disease of 15 cases

Objective To investigate the clinical features, diagnosis and treatment of acute mesenteric isehe-mic disease. Methods Clinical data of 15 patients with acute mosenteric ischemic disease were analyzed retrospective-ly. Results Operation was performed on 14 patients and 1 patient received conservative treatment, 9 cases recovered and 5 died postoperatively. Conclusion Color ultrasound and selective angiography are effective examinations for early diagnosis of acute mesenterie ischemic disease. Early resection of necrotic bowel and application of anticoagulant post-operatively are effective to reduce the mortality. Conservative treatment is still effective for elective patient.
8.The Preemptive Analgesic Effect of Flurbiprofen on the Level of Cytokine in Patients Undergoing Neurosurgery

Objective:To investigate the preemptive analgesic effect of flurbiprofen on inflammatory cytokine in patients undergoing cerebral operation.Methods:Forty ASA Ⅰ-Ⅱpatients undergoing cerebral surgery were randomly divided into control group(group C,n=20)and flurbiprofen group(group F,n=20).Flurbipmfen of 10 mL was given intravenously 15 min before skin incision.The values of VAS scores were recorded at 2 h,24 h and 48 h after operation.Blood samples were taken for determination of serum levels of TNF-α,IL-6 and IL-10 immediately before induction of anesthesia(T1,baseline),at 2 h of operation (T2),immediately after the end of operation (T3) and 24 h after operation (T4).The parameters of anaesthesia time,hemorrhage volume,urine volume,transfusion volume and the dosage of remifentanil and propofol were recorded during the period of anaesthesia.Results:The VAS score was lower in group F than that in group C at 2 h and 24 h after operation (P<0.05),but there was no difference at 48 h after operation between the two groups(P>0.05).The dosages of remifentanil and propofol were lower during anaesthesia in group F than that in group C(P<0.05).Compared with T1,TNF-α was significantly increased at T2 and T3 in both groups,and the peak value at T2 (P<0.05).TNF-α was decreased at T2-T4 in group F than that in group C(P<0.05).Compared with T1,the levels of IL-6 and IL-10 were significantly increased at T2-T4 in both groups,and the peak value at T3(P<O.05).The value of IL-6 was decreased and the value of IL-10 was increased at T2-T4 in group F than those in group C (P<0.05).Conclusion:Flurbiprofen was suitable as a preemptive analgesia and could suppress stress reaction of inflammatory in neurosurgery.

Objective To investigate the role of monoclonal antibody of insulin-like growth factor Ⅰ receptor (IGF-ⅠR) in the proliferation and apoptosis of papillary thyroid carcinoma cell line CGTH W-3, and the effect of IGF-ⅠR monoclonal antibody on papillary thyroid carcinoma cells and normal follicular cells in primary culture. Methods MTT and TUNEL method were used to determine the proliferation and apoptosis of cells respectively after IGF-ⅠR monoclonal antibody intervention. Results The inhibitory effect on cell proliferation was enhanced with the increasing IGF-ⅠR monoclonal antibody concentration and duration of action. IGF-ⅠR monoclonal antibody induced apoptosis of CGTH W-3 cells remarkably even in the concentration of 50 nmol/L. Primary papillary thyroid carcinoma cells and normal follicular cells were inhibited by 100 nmol/L IGF-ⅠR monoclonal antibody, and the degree of inhibition was significantly higher in carcinoma cells than that in normal follicular cells (P
