1.Influence of atrial fibrillation type on plasma brain natriuretic peptide level in male patients

Objective To explore the effect of different type of atrial fibrillation(AF) on plasma brain natriuretic peptide(BNP) level in male patients.Methods Fifty-eight hospitalized male patients with AF were enrolled,including 35 cases with paroxysmal AF,14 cases with persistent AF,and 9 cases with permanent AF.All subjects had the examination of echocardiography/electrocardiogram within 24 h after admission.Venous blood sample were collected in a quiet prostration state on the next early morning for BNP detection.The plasma BNP level was determined by radioimmunology-assay(RIA).Results The average level of plasma BNP in male patients with permanent AF(2343.25 ± 820.24)μg/L was significantly higher than those with persistent AF([576.43 ± 223.07]μg/L,t=8.74,P<0.001) and paroxysmal AF([632.74 ± 103.93]μg/L,t=8.73,P<0.001),but there was no significant difference between the last two groups(t=0.59,P>0.05).Conclusion BNP is a sensitive indicator of early heart function impairment related with AF,particularly elevates in male patients with permanent AF.
2.Risk factors of anastomotic leakage and its significance on the survival of rectal cancer patients after low anterior resection

Objective To identify risk factors for anastomotic leakage after low anterior resection (LAR) in rectal cancer patients and study its impact on patients'long-term prognosis.Methods Chnical data were analyzed from 720 patients who underwent low anterior resection(LAR) for rectal cancer between 1996 and 2006.Results Anastomotic leakage after anterior resection occurred in 44 cases(6.1%).The median time of anastomotic leakage after operation was 5.6 days.Muhivariate analysis showed male patients.history of preoperative chemoradiation therapy,diabetes,cancer distance from anus less than 7 cm and hand-sewed anastomosis were independent risk factors predisposing anastomosis leakage (P<0.05).Tumor local recurrence rate was 13.6% in patients suffering from leakage and 5.9% for those without leakage (χ2= 4.116,P<0.05).The distant metastasis rates were 25.0 and 14.1 percent for the leakage and noaleakage groups,respectively(χ2=3.943,P<0.05).The survival rates were 56.8 and 72.5 percent in the leakage and nonleakage groups,respectively(χ2=4.979,P<0.05).Conclusion Sex,preoperative chemoradiation therapy,diabetes,cancer distance from anus less than 7 cm and hand-sewed anastomosis were found to be independent risk factors for anastomotic leakage after rectal cancer surgery.Anastomotic leakage was associated with poor prognosis.

Objective To observe the distribution of aerobic bacteria in conjunctival sac of patients with proliferative diabetic retinopathy and the sensitive antimicrobial agents in order to master the distribution of bacteria in the conjunctival sac and select sensitive antibiotics to decrease the infection rate and improve the therapeutic effect. Methods From March 2013 to August 2016, 248 patients with proliferative diabetic retinopathy were treated with aerobic bacterial culture and antibiotic susceptibility test. The distribution characteristics of aerobic bacteria in conjunctival sac and the sensitivity of antibiotics were observed. Results There were 112 strains in this study, the positive rate was 45.16% (including 8 cases of mixed infection), including 104 strains of gram-positive bacteria, 92.86% of gram-negative bacteria and 7.14% of gram-negative bacteria, the difference was statistically significant differences (P<0.05). Among them, the positive rate of staphylococcus epidermidis higher than other types of strains, the difference were all significant differences (P all<0.05). The sensitivity of vancomycin to gram-positive bacteria 100% was higher than that of, ofloxacin 13.46%,ciprofloxacin 21.15%,gentamicin 69.23% and rifampicin 88.46%, (P all<0.05). The sensitivity of gentamicin to gram-negative bacteria 87.50% was higher than that of rifampicin 0 and vancomycin 12.50%, (P all<0.05). Positive rate of culture results and staphylococcal multidrug resistance rate were 72.94%,31.76%, the incidence of >65 years of age were higher than ≤ 65 years old 30.67%,12.27%, male 51.61%,23.12% than female 25.81%,6.45%, there are basic diseases 69.15%,31.91% were higher than those without underlying diseases30.54%,11.04%, (P<0.05). Staphylococcus number composition ratio, the difference was not statistically significant. Conclusion To strengthen the observation of aerobic bacteria and the sensitivity of commonly used antimicrobial agents in patients with proliferative diabetic retinopathy, especially the high risk population, it is helpful to master the distribution characteristics of bacterial conjunctival sac and sensitive antimicrobial agents, so as to achieve the preoperative conjunctival sac Bacteria and reduce the rate of infection and other purposes.
4.Surgical strategy for treatment of nonparasitic hepatic cysts

Objective To define the indications and evaluate the results of various surgical treatment options in patients with nonparasitic hepatic cysts. Methods The clinical data of 284 patients with nonparasitic hepatic cysts treated in our hospital from January 1995 to December 2005 were retrospectively analyzed. Results Percutaneous aspiration and sclerotherapy were performed in 161 cases and complications occurred in 9 (5.59%), recurrence in 53 (32.92%) and the mortality was 0%.Open surgery was conducted in 71 cases and the complications occurred in 16 (22.54%), recurrence in 8 cases (11.27%) and the mortality was 2.82%. Laparoscopic surgery was employed in 52 cases and complications occurred in 7 ( 13.46%), recurrence in 6 (11.54%) and the mortality was 1.92%.Conclusion There is currently no general agreement in the literature concerning when nonparasitic hepatic cysts should be treated. Laparoscopic surgery was more favorable than other therapeutic options.However, we should choose individually suitable methods according to different clinical symptom of patients.
5.Study the FOLR1 mRNA expression of 40 esophageal cancerous tissues and relationship between ABO blood group

ObjectiveTo investigate the expressions of folate receptor alpha (FOLR1) in 40 esophageal cancerous tissues and its relevance of ABO blood group.MethodsABO blood groups were analyzed in 40 patients.Immunohistochemistry (IHC) and real-time quantitative reverse transcription PCR detection were used to detect the expression of folate receptor in cancer and adjacent tissues.Results20%of esophageal cancer was suspected positive (+), 50% of cancer adjacent tissue was suspected positive (+), 10% were positive (+), the difference was statistically significant (x2 = 14.48, P <0.01).Real Time RT-PCR analysis showed FOLRI low expression in esophageal cancer and adjacent tissues, compared to normal esophageal mucosa, the differences were statistically significant (F1 =53.247, F2 = 116.500, P <0.01).ABO blood type and FOLRi in esophageal cancer patients were not significant correlated (P =0.647, P >0.05).ConclusionsFOLRI expression was decreased in the esophageal lesions compared with adjacent tissues.Detection of FOLRl expression level in esophageal cancerous tissues and the paired adjacent tissues was helpful for judging the extent of disease and guiding surgery treatment.The FOLRI expression level in esophageal carcinoma tissues had no relationship with ABO blood types.
6.Viral dynamics during entecavtr therapy in patients with chronic hepatitis B

Objective To analyze the viral dynamics and clinical significance during the antiviral treatment by a mathematical model.Methods Six chronic hepatitis B patients were evaluated with a kinetic model(Neumann model)during dose of 0.5 mg/d oral entecavir.Blood samples were drawn for HBV DNA measurement at week 0,2,4,12,24.Non-linear modeling was used to fit individual patient data.Results The median effectiveness in blocking viral production was 99.970%(n=6).The median half-life of viral turn-over was 1.6 d(n=6).The median half-life of infected hepatocytes was 21.3 d(n=5).Compared with the other patients,the c(virions are cleared at a rate)、ε(effectiveness)、δ(infected cell are lost at a rate)value of one patient(eg.6)were all lower and the half-life of virus and infected cells were higher,and eg.6 developed viral break-through after 38 weeks of follow-up.Conclusions Viral load decay showed a biphasic pattern during entecavir therapy which can be described with a mathematical model.The model relates processes of viral infection and replication as well as drug efficacy to model parameters.It indicates the prediction of bio-mathematical model during antiviral treatment.
7.Study on the relationship between plasma brain natriuretic peptide level and the location and prognosis of acute myocardial infarction in elderly people

Objective To explore the relationship between plasma N-terminal pro-B-type natriuretic peptide (NT-proBNP) level and the location and prognosis of acute ST-segment elevation myocardial infarction (STEMI) in elderly people.Methods From May 2012 to April 2014, the 137 elderly patients (aged≥60 years) with acute STEMI within 24 h after symptoms onset admitted in cardiovascular department of Ningbo First Hospital were selected as acute myocardial infarction group (AMI group), and 40 elderly healthy people (age≥60 years) as control group.The plasma NT-proBNP levels were detected by enzyme linked immunosorbent assay(ELISA), and compared between the two groups.The patients in AMI group were classified into four subgroups: anterior AMI group (A group, n=44), anterolateral AMI group (B group, n=21), anterior septal AMI group (C group,n=18) and inferior AMI group (D group, n=54), and the changes of plasma NT-proBNP levels in different infarction location were explored.Meanwhile, all patients were followed up for 30 days and 1 year to observe major adverse cardiac events (MACEs) and mortality rate, then to explore the relationship between plasma NT-proBNP level and prognosis in elderly people with STEMI.Results The plasma NT-proBNP levels were significantly higher in AMI group than in control group [(1 406.2±1 322.5)ng/L vs.(63.7±18.5) ng/L, P<0.01].The changes of plasma NT-proBNP levels in high-to-low sequence were from A group to B group to C group to D group and to control group (P<0.05).At 30 days, the plasma NT-proBNP levels were higher in death group than in survival group (P<0.05), and were higher in MACE group than in non-MACE group (P<0.05).At one year, there were no significant differences in plasma NT-proBNP levels between death group and survival group (P>0.05), and the plasma NT-proBNP levels were higher in MACE group than in non-MACE group (P<0.05).Conclusions In elderly STEMI patients, there is a correlation between plasma NT-proBNP levels and infarct location, with the highest NT-proBNP level in anterior AMI.The plasma NT-proBNP level could predict MACE.
8.Correlation between the expression of periostin and Epithelial-mesenchymal transition markers in colorectal cancer

Objective To investigate the correlation between the Periostin and Epithelial-Mesenchymal Transition(EMT) markers(E-cadheirn and N-cadheirn) in colorectal cancer(CRC),and analyze the relationship between the expression of Periostin and clinicopathological parameters.Methods The expression of Periostin,Ecadheirn and N-cadheirn in 106 cases with primary CRC tumors and corresponding normal tissues were detected by Immunohistoehemistry and the results were analyzed.Results The expression of Periostin,E-cadheirn and N-cad-heirn in tumors were significantly high expression than those in corresponding normal tissues(62/106 vs 21/106,x2 =34.027,P < 0.05 ;43/106 vs 89/106,x2 =42.480,P < 0.05 ; 66/106 vs 19/106,x2 =43.382,P < 0.05).The expression of Periostin in tumors was associated significantly with tumor differentiation,tumor invasion,lymph node metastasis and distance metastasis (x2 =7.752,P =0.007; x2 =5.008,P =0.031 ; x2 =10.227,P =0.002;x2 =8.001,P =0.006).It was negatively correlated with E-cadherin expression (r =-0.435,P < 0.001),but positively correlated with N-cadherin expression (r =0.213,P =0.028).Conclusion Periostin may promote the occurrence and development of CRC by participating in EMT program.
9.Exploration of research innovation capacity building for young talents in research-oriented hospitals

Objective To analyze youth talents' scientific research projects in research oriented hospital for recent five years,provide scientific basis for hospitals to strengthen the youth talents' capacity building of research innovation.Methods Identify those youth talents who have been funded in the First Affiliated Hospital of Xinjiang Medical University,and statistically analyze the demographic information,granted scientific research projects and hospital funding situations.Results There are 140 young talents who have got scientific research projects from 2011 to 2015 in the hospital.The national project number and the trend of approval rate has no obvious change;the provincial level project number and approval rate showed a trend of a significant increase in the last two years;Setting up the hospital fund promoted the youth talents' scientific research ability,but there are still spaces for further improvement in the funding field and founded group.Conclusions There has been a group of youth talents to stand out from the crowd during the construction and the development of the researchoriented hospital,but youth talents remains scarce for the national key projects and talent projects,and that will put forward new requirements for the hospital in the future to strengthen the youth talents' capacity building of research innovation,which needs to gradually establish a suitable cultivation mode and mechanism for youth talents' growth.
10.Management practice of academic and discipline leader in research-oriented hospitals

Objective Promote the development of disciplines and enforce the construction and training of talent echelon through selection and assessment of discipline and academy leaders.Methods Conduct classified management on different disciplines and academy leaders concerning theirworking positions.The specific measurements include setting up selection and evaluation systems separately with different focuses,implementing dynamic management,establishing performance oriented mechanisms of promotion and demotion during the tenure period.Results The construction of talent echelon plays a significant role in capacity building of young talents,team building and development of all disciplines.Conclusions Systematic measurements could promote the development of disciplines and cultivation of reserved talents,including attaching importance on talent echelon establishment,defining suitable function of hospital,strengthening talent team building;establish a well-functioning talent management mechanism and effective incentive mechanisms.
