1.A Case of Hereditary Trichilemmal Cyst with Proliferating Trichilemmal Tumor.

We report a case of hereditary trichilemmal cyst with proliferating trichilemmal tumor in a 64-year-oid man, who has generalized 157, variable sized firm cutaneous nodules for 40 years. The cystic nodules are chiefly located on the scalp and trunk which showed characteristic histopathologieal findings of trichilemmal cyst and proliferating trichilemmal tumors. His son also has multiple trichilemmal cysts and his mother seems to have had clinically similar lesions. The patient and his son were treated with surgical excision and therapeutic trial of oral isotretinoin for 2 months. Isotretionoin was ineffective in the patient, but the lesions of his son were decreased in size.

3.Comparison of Dietary Behavior of Eating Alone in Single Households by Status of Workers and Age

OBJECTIVES: This study compared the dietary behaviors of single-person households when eating alone according to the employment pattern and age. METHODS: A total of 566 people aged 20~59 years old were collected from the status of workers and classified into three groups according to their employment pattern (regular, non-regular workers and business owner). The subjects were collected by purposive quota sampling on a Gallup panel from June to November in 2017. The dietary behavior and perception of eating alone of the subjects were surveyed via online and self-reported questionnaires. RESULTS: The frequency of eating alone was significantly higher in the regular group than the non-regular group and business group (p<0.01). The place of eating alone was significantly higher in the regular and non-regular group in the convenience store, and business group in the office (p<0.001). Ramen, the menu when eating alone, was significantly higher in the non-regular group than the other groups (p<0.01). The preference for eating alone was lower in the older age group (p<0.05). The young aged group (aged 20~30) ate more fast food and felt more convenience than the older aged group aged 40~50 years (p<0.05). CONCLUSIONS: Single-person households with a non-regular job have poorer dietary behavior in eating alone than those who had regular employment. In a situation of an increasing number of non-regular workers aged in their 20s and 30s, there is a high likelihood of social problems, such as health and poverty. This study highlights the need for a healthy food selection environment to improve the dietary life of single-person households with non-regular jobs for the diverse types of single-person households.

4.Hinged Turnover Flap for Reconstruction of Nasal Ala Defect.

Surgical remedy of nasal ala present a challenge to the reconstructive surgeon. As ala have anatomic complexity and a paired structure, reconstruction must be precise and aesthetically acceptable. The reconstruction of full-thickness ala defects often require staged surgical procedures, and they can introduce operational risks and consume extraordinary time and cost. However, a hinged turnover flap can offer a one-stage repair of full-thickness ala defects. With this procedure, satisfactory ala reconstruction was achieved in one step, and good aesthetic and functional results were obtained.

Pyruvate dehydrogenase complex deficiency (PDCD) is one of the most common neurodegenerative
disorders associated with abnormal mitochondrial metabolism. Pyruvate dehydrogenase complex
plays an important role in glucose metabolism and generation of energy from carbohydrates. Potential
therapies for PDCD, include thiamine and ketogenic diet (KD), have been used with varying degrees
of success. However, the KD is too restrictive, and its serious complications, particularly in early
age of neonate or infancy are important drawbacks. Recently, the modified Atkins diet (MAD) for
intractable epilepsy has provided balanced nutrients. The complications can be expected to be less
frequent and well controlled. In this report, we describe an infant with PDCD confirmed by PDHA1
gene mutation through high-throughput sequencing technique of whole exome sequencing, who failed
to continue the KD, but made good progress on MAD.
6.Hyperprolactinemia after taking Levosulpiride and its Causality Assessment: An Adverse Event Reported by a Community Pharmacy.

Levosulpiride is one of the most frequently prescribed medicines in Korea. An adverse drug reaction (ADR) after taking levosulpiride was reported at a community pharmacy in Korea. A 31-year-old woman reported the symptoms of lactation and amenorrhea after taking levosulpiride; an evaluation of whether these symptoms were caused by the medication was therefore necessary. Several tools can be used to determine if the ADR resulted from the administered drug or other factors, including the World Health Organization-Uppsala Monitoring Centre (WHO-UMC) criteria, the Naranjo scale, and the Korean causality assessment algorithm (Ver. 2). The causality was evaluated as “possible” by the WHO-UMC and Naranjo scales, but as “probable” by the Korean causality assessment algorithm (Ver. 2). In conclusion, the information provided did not indicate definite causality and there were slight differences in the results obtained from each assessment method.

7.Serum Low-Density Lipoprotein Cholesterol Levels and Depressive Mood in Korean Adults: A Nationwide Population-Based Study

Background:
The association between serum cholesterol levels and depression has been studied extensively; however, results are inconsistent. This study aimed to investigate the association between low-density lipoprotein cholesterol (LDL-C) levels and depressive symptoms among Korean adults.
Methods:
This cross-sectional study used data obtained from the 2007–2015 Korea National Health and Nutrition Examination Survey. In total, 40,904 adults were included in the final analysis. Participants were categorized into five groups according to their LDL-C levels, using the Korean guidelines for dyslipidemia. Symptoms of depression were evaluated using a self-reported questionnaire. Weighted logistic regression was used to examine the relationship between LDL-C levels and self-reported depressive symptoms.
Results:
Compared with the intermediate category, the lowest (<70 mg/dL) and highest (≥160 mg/dL) LDL-C categories were associated with depressive symptoms, after adjusting for potential confounding factors (odds ratio [OR], 1.191; 95% confidence interval [CI], 1.008–1.409; OR, 1.241; 95% CI, 1.073–1.435, respectively). The highest LDL-C category was positively associated with depressive symptoms in those who were middle-aged, female, had a low body mass index, and taking or not taking dyslipidemia medications.
Conclusion
A U-shaped association was identified between LDL-C categories and self-reported depressive symptoms. Our findings suggest that LDL-C levels that are too low or too high are associated with self-reported depressive symptoms. Further prospective studies are needed to determine the causal relationship of this association.
8.Clinical outcome of surgical resection for multifocal T2-T3 hepatocellular carcinoma up to 3 nodules: a comparative analysis with a single nodule

Background/Aims:
Although the Barcelona Clinic Liver Cancer staging system seems to underestimate the impact of curative-intent surgical resection for multifocal hepatocellular carcinoma (HCC), recent studies have indicated favorable results for the surgical resection of multiple HCC. This study aimed to assess clinical outcomes and feasibility of surgical resection for multifocal HCC with up to three nodules compared with single tumor cases.
Methods:
Patients who underwent surgical resection for HCC with up to three nodules between 2009 and 2020 were included, and those with the American Joint Committee on Cancer (AJCC) 8th edition, T1 and T4 stages were excluded to reduce differences in disease distribution and severity. Finally, 81 and 52 patients were included in the single and multiple treatment groups, respectively. Short- and long-term outcomes including recurrence-free survival (RFS) and overall survival (OS), were evaluated.
Results:
All patients were classified as Child-Pugh class A. RFS and OS were not significantly different between the two groups (P=0.176 and P=0.966, respectively). Multivariate analysis revealed that transfusion and intrahepatic metastasis were significantly associated with recurrence (P=0.046 and P=0.005, respectively). Additionally, intrahepatic metastasis was significantly associated with OS (hazard ratio, 1.989; 95% confidence interval, 1.040-3.802; P=0.038).
Conclusions
Since there was no significant difference in survival between the single and multiple groups among patients with AJCC 8th stage T2 and T3, surgical resection with curative intent could be considered with acceptable long-term survival for selected patients with multiple HCC of up to three nodules.
9.Clinicopathologic Features, Survival Rates and Prognostic Factors of Gastric Cancer in Young Age Group under 30 Years of Age.

PURPOSE: The purpose of this report is to clarify the clinicopathologic features, survival rates and prognostic factors of gastric cancers in young age group. MATERIALS AND METHODS: We reviewed 133 patients with gastric cancer under 30 years of age who underwent surgery from January 1986 to December 1995 at the Department of Surgery, Seoul National University Hospital. We have analyzed the sex, age, Borrmann type, histologic differentiation, Lauren classification, depth of invasion, lymph node metastasis, distant metastasis, location of tumor, size of tumor, curability and extent of operation. The 5 year survival rates and prognostic factors were also evaluated. The Kaplan-Meier method, Log-Rank test and Cox hazards proportional mode were used for the statistical analyses. RESULTS: The overall 5 year survival rate was 55.3% in all 133 patients in contrast to 67.4% in 108 patients who underwent gastric resections. Significant prognostic factors were Borrmann type, histologic differentiation, depth of invasion, lymph node metastasis, distant metastasis, location of tumor, size of tumor, curability and extent of operation in univariate analysis. Lymph node metastasis was the only significant independent prognostic factor in multivariate analysis. CONCLUSION: In our study, the 5 year survival rate in young age group was comparable to that of prevalent age group. This is attributed to the relatively higher incidence of early gastric cancer and more frequent curative resections in this young age group.

10.Impact of an Emergency Department Isolation Policy for Patients With Suspected COVID-19 on Door-toElectrocardiography Time and Clinical Outcomes in Patients With Acute Myocardial Infarction

Background:
Rapid electrocardiography diagnosis within 10 minutes of presentation is critical for acute myocardial infarction (AMI) patients in the emergency department (ED).However, the coronavirus disease 2019 (COVID-19) pandemic has significantly impacted the emergency care system. Screening for COVID-19 symptoms and implementing isolation policies in EDs may delay the door-to-electrocardiography (DTE) time.
Methods:
We conducted a cross-sectional study of 1,458 AMI patients who presented to a single ED in South Korea from January 2019 to December 2021. We used multivariate logistic regression analysis to assess the impact of COVID-19 pandemic and ED isolation policies on DTE time and clinical outcomes.
Results:
We found that the mean DTE time increased significantly from 5.5 to 11.9 minutes (P < 0.01) in ST segment elevation myocardial infarction (STEMI) patients and 22.3 to 26.7 minutes (P < 0.01) in non-ST segment elevation myocardial infarction (NSTEMI) patients.Isolated patients had a longer mean DTE time compared to non-isolated patients in both STEMI (9.2 vs. 24.4 minutes) and NSTEMI (22.4 vs. 61.7 minutes) groups (P < 0.01). The adjusted odds ratio (aOR) for the effect of COVID-19 duration on DTE ≥ 10 minutes was 1.93 (95% confidence interval [CI], 1.51–2.47), and the aOR for isolation status was 5.62 (95% CI, 3.54–8.93) in all patients. We did not find a significant association between in-hospital mortality and the duration of COVID-19 (aOR, 0.9; 95% CI, 0.52–1.56) or isolation status (aOR, 1.62; 95% CI, 0.71–3.68).
Conclusion
Our study showed that ED screening or isolation policies in response to the COVID-19 pandemic could lead to delays in DTE time. Timely evaluation and treatment of emergency patients during pandemics are essential to prevent potential delays that may impact their clinical outcomes.
