1.A RCT study on oral lidocaine plasmagel on improvement of the postoperative odynophagia in patients receiving trans-oesophageal echocardiography examination for cardiac surgery

Objective To investigate the effect of oral lidocaine plasmagel on the postoperative odynophagia in patients receiving trans-oesophageal echocardiography (TEE) examination for cardiac surgery.Methods Sixty patients for cardiac surgery were divided randomly into the control group (C group) and the oral lidocaine plasmagel group (L group) with 30 cases in each group.Patients in L group received 2% oral lidocaine plasmagel 20 ml before anesthesia,while C group did not.The incidence of postoperative odynophagia was compared between two groups.Results Postoperative odynophagia pain score were significantly higher in C Group than that in L Group.Conclusions Oral lidocaine plasmagel can alleviate the postoperative odynophagia in patients receiving trans-oesophageal echocardiography examination for cardiac surgery.
2.Investigation on Contamination Situation of Centralized Air Condition Systems in Hotels in Shenzhen City

Objective To know the contamination situation of the centralized air conditioning-ventilating systems in hotels in Shenzhen City, Guangzhou province and to ensure the indoor air quality when the centralized air conditioning-ventilating systems were at working. Methods 22 hotels were selected to determine the total count of bacteria and fungi in internal surface of pipes and air blow, the dust volume of internal surfaces of pipes, inhalable particulate matter(PM10) in air blow, and Legionellae in cooling tower water. Results The eligibility percent of dust volume, the total number of bacteria and fungi in internal surface was 94.76 %, 95.71% and 91.90 % respectively,the eligibility percent of PM10, the total count of bacteria and fungi in air blow was 42.54%, 94.76% and 84.29% respectively, and the detected rate of Legionellae was 57.50%. Conclusion The pollution in some degrees has been observed in the centralized air conditioning-ventilating systems in hotels of Shenzhen City, so it is considered as the important measure to improve indoor air quality and ensure people health to strengthen management of centralized air conditioning-ventilating systems in the usage.
4.Assessing Sports Injury Risks of Chinese Rugby Athletes Using Functional Movement Screen

Objective To explore the effect of applying functional movement screen (FMS) in the sport injury risk assessment of Chinese rugby athletes.Methods Rrugby athletes of Chinese national and provincial teams were selected and their data were collected using the standard FMS test.Their non-impact injury of the lower limbs and trunk were tracked and recorded.FMS diagnostic value and diagnostic cut-off value were evaluated using the receiver operating characteristic curve (ROC) and odds ratio (OR).Results The area under curve of all,male and female rugby athletes was 0.780 (P=0.000),0.877 (P=0.001) and 0.7130 (P=0.013) respectively,with significant differences from AUC=0.5.FMS score optimal cut-off point of all,male and female rugby athletes corresponding to the maxi mum Youden index was 13.5,15.5 and 13.5 respectively.Among all,male and female rugby athletes,the injury rates of the positive group (with FMS score less than the corresponding optimal cutoff point) were significantly higher than that of the negative group (with FMS total score greater than a corresponding cut-off point)(P<0.01),and OR value of the positive group was 25.85 (95%CI:3.34~200.23),25 (95%CI:2.36~264.80) and 14.22 (95%CI:1.76~114.92) respectively.Conclusion In China,the average FMS score of rugby athletes had a strong correlation with non-contact sport injury,which might become an assessment index of non-contact sport injury risks.There is a significant difference in FMS score optimal cut-off points between the male and female rugby athletes,with that of the female being 13.5 points and the male being 15.5 points.
5.Influence of bone marrow mesenchymal stem cell transplantation on growth-associated protein-43 expression in rats with cerebral infarction

BACKGROUND:Growth-associated protein-43 is the primary substance for constructing plasticity of central nervous system, which is recognized as the first molecular marker for studying growth and repairing injury of nerves. OBJECTIVE: To explore the expression of growth-associated protein-43 in rats with focal cerebral ischemia following bone marrow mesenchymal stem cells (MSCs) transplantation.DEISNG, TIME AND SETTING: A randomized, controlled, animal study was performed at the Laboratory of Department of Neurology, Renmin Hospital of Wuhan University and Doctoral Scientific Research Work Station of C-BONS Pharmacy, Hubei, China, from March 2006 to October 2007.MATERIALS: Sixty adult Sprague Dawley rats, with clean grade, irrespective of genders, weighing (180±20) g, were randomly divided into the model, sham operated, and MSCs transplantation groups, with 20 animals in each group. METHODS: Additional 4 Sprague Dawiey rats, aged 2 months, were selected to isolate and culture MSCs, which was labeled with 5-Bromo-2,-deoxyuridine (BrdU). In the sham operated group, the right internal carotids were deligated after anaesthesia. The remained rats were prepared for models of right middle cerebral artery ischemia. At 24 hours after model preparation, 20 μL culture solution containing 5×105/L MSCs was injected into the left lateral ventricle of rats in the MSCs group, the same volume of phosphate buffer saline was injected in the model group.MAIN OUTCOME MEASURES: The rats were sacrificed prior to and at days 7, 14, and 21 after transplantation. The expression of growth associated protein-43 at the infarcted areas, the survival and migration of transplanted cells were examined by immunohistochemistry, at the same time, neurological deficit scores were recorded.RESULTS: The transplanted MSCs migrated from the left lateral ventricle to the infracted areas. Seven days after transplantation, the expression of BrdU-positive cells was found, reached a peak at day 14, and gradually decreased, until disappeared at 28 days after transplantation. Results of immunohistochemistry image analysis showed that immunological activity of growth associated protein-43 around the infarcted area of the MSCs group was obviously greater than that of the model group at days 7 and 14 after transplantation (P<0.06). There was no neurological deficit in the sham operated group. Moreover, with time prolonged, the neurological deficit scores gradually decreased in the model and MSCs groups, which were significantly lower in the MSCs group compared to the model group at day 14 after transplantation (P<0.05).CONCLUSION: MSCs transplantation up-regulates the expression of growth associated protein-43 around the infarcted area, which is consistent with the recovery of neurological function.

Humans are exposed to the ubiquitous radiofrequency (RF, 100 kHz-300 GHz) electromagnetic fields because of the mushroom development of wireless communications,raising concerns over the possible hazards of RF radiations.Epidemiological investigation has showed that chronic use of cellphones increases the risk of brain tumors.Since genetic damage is closely related to tumors, researchers have been trying to find out whether cellphones and other RF devices are genotoxic.However, the investigations have yielded both negative and positive results.This review summarized the recent in vitro and in vivo researches about genotoxicity of RF radiations and proposed a possible mechanism by which of RF radiations cause genetic damage.
7.Effect of ulinastatin on the expression of neutrophil CD_(11b)/CD_(18) in patients during orthotopic liver transplantation

Objective To investigate the effects of ulinastatin on the expression of neutrophil CD_(11b) and CD_(18) during orthotopic liver transplantation(OLT).Methods Forty patients with liver diseases,ASAⅢorⅣ, undergoing OLT were randomly divided into two groups.Ulinastatin group(n=20)received intravenous infusion of ulinastatin 3?10~5 IU in 100 ml normal saline after skin incision and repeated every 4 hours thereafter.Control group received same amount of normal saline instead(n=20).Blood samples were taken immediately before skin incision,120 min after skin incision,30 min of anhepatic phase,60 min of neohepatic phase and at the end of operation for measurement of CD_(11b) and CD_(18) expression of neutrophil by flow cytometry.Results CD_(11b)/CD_(18) expression was increased significantly in control group 60 min of neohepatic phase and at the end of operation compared to the level before skin incision,but in ulinastatin group there was no significant change in CD_(11b)/CD_(18) expression during the whole procedures(P>0.05).CD_(11b)/CD_(18)expression was significantly lower 60 min of neohepatic phase and at the end of operation in ulinastatin group than in control group(P<0.01).Conclusion Ulinastatin can inhibit the increase in CD_(11b)/CD_(18)expression during OLT,and be helpful for reducing the inflammatory response.
8.Relationship between peripheral neuropathy and retinal nerve fiber layer thickness in elderly patients with type 2 diabetes

Objective To investigate the relationship between retinal nerve fiber layer thickness and peripheral neuropathy in elderly patients with type 2 diabetes.Methods Clinical data of elderly patients with T2DM in Endocrinology Department in Beijing Hospital were retrospectively collected.Global and sectorial retinal nerve fiber layer(RNFL)thicknesses were measured by using optical coherence tomography(OCT),and never conduction velocity measurements were performed in all patients.Diabetic peripheral neuropathy was diagnosed by the criteria in diabetic neuropathies update(2010,American Diabetes Association).RNFL thickness was compared between diabetic nonperipheral neuropathy group (control group,n =30)and diabetic peripheral neuropathy group (DPN group,n=17)and between different sub-groups.Results The RNFL thicknesses of temporal,nasal,superior and inferior visual fields and the mean RNFL thickness were less in DPN group than in control group,among which there were significant differences in the RNFL thicknesses of superior,inferior visual fields and the mean RNFL thickness [(107.7±27.4)μm vs.(128.1±17.3)μm,(112.9 ±20.8)μm vs.(130.8±21.8)μm,(88.2±15.5)μm vs.(100.5± 11.3)μm,F=7.446,7.468,7.988,respectively,P=0.009,0.009 and 0.007].RNFL thickness was decreased along with the aggravation of DPN from the control group,the subclinical DPN group to the DPN group successively(all P< 0.05).Conclusions Retinal nerve fiber layer thickness is associated with diabetic peripheral neuropathy in elderly patients with type 2 diabetes,and the relationship is more significant in patients with serious DPN.
9.Effect of urinary IgG from nephrotic patients on expression of macrophage migration inhibitory factor in human proximal tubular epithelial cells

Objective To investigate the effect of purified urinary IgG from patients with minimal change nephrotic syndrome and membranous nephropathy on the expression of macrophage migration inhibitory factor (MIF) in human proximal tubular epithelial cells (HK-2). Methods Proteins were precipitated from urine with (NH4)2SO4, and urinary IgG was purified by protein G column chromatography. SDS-PAGE and Western blotting were performed to analyze the urinary IgG. HK-2 cells were incubated with urinary IgG (0,0.5,1.0,2.5,5.0,10.0 mg/ml) from either minimal change nephrotic or membranous nephropathy patients. The expression of MIF mRNA was assessed by RT-PCR, and MIF protein was assessed by Western blotting. Results SDS-PAGE showed that there were four bands, which were all IgG fractions confirmed by Western blotting. The urinary IgG up-regulated the expression of MIF mRNA and protein in HK-2 cells. The expression increased in a dose-dependent manner. Objective To investigate the risk factors for early postoperative cognitive function in the patients with peritoneal surface malignancies after cytoreductive surgery and hyperthermic intra-peritoneal chemotherapy(CRS-HIPEC).Methods Fifty-one patients(21 men and 30 women), ranged from 25 to 65 years,42-80 kg,ASA Ⅰ-Ⅲ,undergoing CRS-HIPEC under combined intrave-nous-inhalational anesthesia,were studied.Patients were assigned into postoperative cognitive dys-function (POCD)group or non-POCD group according to their performances of visual verbal learning test,concept shifting task,letter-digit coding test and stroop color-word test 1 day before operation and 7 days after operation.Years of education,medical history,duration of operation,intraoperative blood loss,frequency of cardiovascular events,amount of fluid infused per hour and VAS scores were recorded.Venous blood samples were taken at five time points:before surgery(T0 ),30 minutes after the beginning of the procedure(T1 ),30 minutes after the beginning of HIPEC(T2 ),at the end of the surgery(T3 )and 24 hours after the surgery(T4 ),to determine the concentration of serum amyloid A (SAA).pH,PaCO 2, Hb,blood glucose were recorded at T0-T2.Then the data was statistically analyzed.Results According to the diagnostic criteria,twenty patients developed POCD 20 (39.2%). There were significant differences between POCD and non-POCD groups on age,gender, pre-operative complications and the origin of tumor(P <0.05).The concentration of SAA increased from T2 and reached the peak at T4 ,and SAA concentration for patients in POCD group was higher than that for patients in non-POCD group(P <0.05).Compared with non-POCD group,the levels of blood glucose were significantly increased in POCD group at T2 (P <0.05).Conclusion CRS-HIPEC resul-ted in exaggerated and prolonged inflammatory response.Advanced age,female,diabetes,hyperten-sion,peritoneal carcinomatosis from ovarian cancer and endometrial cancer are associated with early POCD in the patients undergoing CRS-HIPEC.
