Objective To evaluate short-term therapeutic effects of laser therapy combined with high ligation of the great saphenous vein in the treatment of varicose veins in the lower extremities. Methods Endovenous laser treatment combined with high ligation of the great saphenous vein was performed on 46 patients with varicose veins in the lower limbs (55 limbs). Results The operation time was 30-65 min (mean, 48 min) for each limb. The number of incisions was 1-5 (mean, 2.8) in each limb. Hospital stay after operation was 3-5 d (mean, 3.6 d). Postoperatively, subcutaneous bleeding occurred in 11 limbs (20%), skin burn was seen in 6 limbs (11%). Among the patients, 42 (50 limbs) were followed up for 2-19 months (mean, 13.5 months), during which no recurrence was found. Conclusion Laser treatment combined with high ligation of the great saphenous vein is effective for varicose veins in the lower extremities.

Chinese Journal of General Surgery 2009;24(7):539-542
Objective To evaluate Caspofungin for the treatment of fungous infection liver transplant patients. Methods From 2003 to 2008 clinical data of 27 cases of liver transplant patients with fungal infections were reviewed retrospectively. Before 2005 (control group) fungal infections were treated with amphotericin B or Fluconazole in 13 cases. After 2005, the infection was treated with Caspofungin (observation group) in 14 cases. Liver function (AST, ALT, TBIL) and renal function (BUN, Scr) were evaluated at one and two weeks respectively. Result of treatment was evaluated 7 ~ 14 d after the treatment as for the clinical cure rate, with or without acute rejection. Result The liver function of the observation group compared with that of the control group at one week was as AST(t =8. 03 ,P <0. 01), ALT(t =9. 09, P<0.01), TBIL(t =6.01,P<0.01), and at 2 week as AST(t=5.59,P<0.01), ALT(t =6.60,P< 0. 01), TBIL(t = 8.45,P <0. 01). The renal function of the observation group compared with that of the control group after one week as for BUN(t =6. 51 ,P <0. 01), Scr(t =5.66,P <0. 01). At 2 week,as BUN (t =7.61,P <0.01), Scr(t =6.91,P <0.01). The clinical cure rate of the control group was 7/13 (53%), that of the observation group was 12/14(86%) ; Two cases in the observation group and one in the control group had acute rejection which was successfully managed by methylprednisolone pulse therapy. Conclusion Caspofungin is an ideal alternative therapy for fungal infection after fiver transplantation.

Objective To explore the modified of methods and techniques to establish a rat model of or thotopic liver transplantion effectively by a single operator, and improve the stability. Methods On the basis of two-cuff technique of Kamada, we improved the techniques of perfusing, shearing, triming donor liver, the suturation of the superior and inferior eaval veins, and so on. Operation was performed in 80 rats with modified techniques (experiment group) and conventional techniques (control group), respectively, compare the survival rates of 48 h, 1 week, lmonth were compared between the two groups. Results In contrast to conventional teehniques, the modified techniques reduced the average time of donor operation, recipient operation and anhepatie phase (P < 0. 05), elevated 1-week, 1-month survival rates, remarkably (P < 0. 05 ).Between the Iwo groups, the survival rate of 48 hour had no statistical significance (P > 0. 05). Conclusions The modified techniques may reduce the operation time of donor and receptor effectively raise the quality of donor liver, elevated the survival rate of receptor. II is a stable, reliable and effective method to establish the rat urthotopic liver transplantation model.

Chinese Journal of Hospital Administration 2001;0(08):-
In order for special hospitals to survive and develop in the fierce market competitions, it is imperative for them to enhance their competitiveness, the key to which lies in accelerating personnel training. For this reason, it is necessary to take the following steps: changing concepts, identifying goals, implementing personnel training strategies and taking the initiative in selecting talented personnel; making exertions in personnel training through expanding channels and conducting classifications; creating the right circumstances, optimizing the environment and making a rational use of personnel; adopting innovative mechanisms, perfecting systems and scientifically managing personnel. It is important to take specialty advancement as the guide, personnel training as the prop, and scientific research and teaching as the driving force in achieving the sustainable development of special hospitals.

Objective The study the formative causes of low dense area in lung on CT images during expiration. Methods This group included 15 young volunteers, mean age was 22 years.Thin sections of spiral CT at right middle hilum were scanned at suspended deep inspiration and deep expiration on both supine and prone positions. The density and area of lobes on matching structural images in the middle hilum at deep inhalation and exhalation were measured on the supine and prone. The increasing rate in area and absolute increasing value in attenuation as full inhalation comparing to full exhalation were calculated. Results Changes of distention and density in the same section of the right lower lobe on dependent position were greater than that on nondependent one.Correlation between distention and density of the lobes was high(P

Objective To investigate the role of bone marrow mesenchymal stem cells (MSCs) transplantation in repairing injured intestinal mucosa of acute pancreatitis. Methods MSCs were harvested and cultured from femurs of male SD rats.Twenty female SD rats were divided into three groups, and serve acute pancratitis (SAP) model was induced by intraperitoneal injection of L-arginine (2 g/kg) twice.Twelve hours after SAP model established,MSC transplantation group (n=8) were injected MSCs (5 × 106 cell/rat) through tail vein for three days, and SAP group (n=6) were injected the same volume of saline through tail vein as control.Control group (n=6) were only injected the same volume of saline without any treatment. All the rats were sacrificed at 72 hours after model established. The small intestinal tissues were taken for HE staining and pathological score, the TNF-α mRNA and IL-1β mRNA expression level in small intestine and pancreas were tested by RT-PCR.Y chromo-some (Sry) gene in pancreatic and intestinal tissue was examined by polymerase chain reaction (PCR).Results The relative expression quantity of TNF-a mRNA and IL-1β mRNA in pancreas was significant higher in SAP group and MSC transplantation group than in control group (7.22 ± 1.99,3.46± 1.75 vs 1.32 ± 1.04 ; 2.71 ± 0.56,1.92 ± 0.28 vs 0.61 ± 0.45 ),the difference was statistically significant (F=18.375,F=22.701; P<0.05).Compared with SAP group, the expression quantity of TNF-α mRNA and IL-1β mRNA in pancreas was significantly decreased in MSC transplantation group, the difference was statistically significant (P<0.05).The relative expression quantity of TNF-α mRNA and IL-1β mRNA in small intestine was significantly higher in SAP group and MSC transplantation group than in control group (3.93 ± 1.08,2.13 ± 0.53 vs 0.68 ± 0.42 ; 2.44 ± 1.54,1.02±0.44 vs 0.60±0.14),the difference was statistically significant (F=21.772,F=6.132; P<0.05).The expression of TNF-αmRNA and IL-1β mRNA in MSC transplantation group was lower than that in SAP group, the difference was statistically significant (P<0.05).Compared with SAP group, pathological score indicated that small intestine injure was slighter in MSC transplantation group (3.83±0.28 vs 2.83±0.56),the difference was statistically significant (F=12.013,P<0.05).Sry gene could be detected in the pancreatic and intestinal tissue of MSC transplantation group. Conclusion Allogeneic MSC transplantation group can inhibit Pro-inflammatory cytokines expression in acute pancreatitis, relieve the intestinal mucosa injury and may involve in the intestinal tissue repair.
