1.Effect of Acupoint Thread-embedding at Neiguan (PC6) and Zusanli (ST36) on Anxiety and Sleep Disorders After Heroin Withdrawal

Objective To observe the effect of acupoint thread-embedding at Neiguan (PC6) and Zusanli (ST36) on anxiety and sleep disorders after heroin withdrawal. Method Sixty-seven patients with heroin addiction were randomized into a Neiguan group (25 cases), a Zusanli group (22 cases) and a Neiguan plus Zusanli group (20 cases), to receive corresponding treatments once every 20 d, for 4 sessions in total. The Self-rating Anxiety Scale (SAS) and Pittsburgh Sleep Quality Index (PSQI) were adopted to observe the anxiety and sleep condition before and after the treatment. Result After 60 d thread-embedding treatment, the total and standard SAS scores, sleep initiation time and total PSQI score were significantly decreased in the 3 groups (P<0.05), and the sleep durations were significantly extended (P<0.05); in comparing the improvement of anxiety, Neiguan plus Zusanli group > Neiguan group >Zusanli group (P<0.05); in comparing the improvement of sleep, Neiguan plus Zusanli group was superior to both Zusanli group and Neiguan group (P<0.05), while there was no significant difference between Zusanli group and Neiguan group (P>0.05). Conclusion It's effective to use thread-embedding at Neiguan and Zusanli independently or together for anxiety and sleep disorders after heroin withdrawal, but the efficacy is more significant when the two points are used in combination, indicating the synergy of acupoints.
2.Clinical outcomes of implant-retained overdentures:A retrospective study

Objective:To evaluate the clinical outcomes of implant-retained overdentures.Methods:57 patients treated by implant-retained overdentures were included.Parameters for peri-implant tissue conditions (e.g.peri-implant probing depth,plaque index,bleeding on probing,mucosal hyperplasia,peri-implant marginal bone loss) and prosthetic complications were examined and recorded.The precentage of satisfaction of the patients was assessed using the visual analog scale (VAS).Results:After an average follow-up of (48±11.3) months,the survival rate of the implants was 98.1%,the marginal bone loss was (1.38±0.74) mm.There was no statistically difference among the different attachment groups(bar,magnet and ball) regarding the peri-implant marginal bone loss or bleeding on probing(P>0.05).The peri-implant probing depth and plaque index in patients with magnet and ball attachments were lower than those in patients with bar attachments(P<0.05).The major complications were the upper abutment fracture,prostheses fracture and screw loosening.Most patients were satisfied with their prostheses and there was no statistically significant difference between the attachment types(P>0.05),except that magnet and ball attachments were much easier to clean compared with bar attachments(P<0.05).Conclusion:Implant-retained overdenture is a successful and satisfactory treatment option for patients with edentulous jaw.The patients should been given regular clinical examinations to keep peri-implant tissue health and reduce the complications,especially those with bar attachments.
3.The prognostic value of CD14+HLA-DRlow/-for evaluating the severity of acute pancreatitis

Objective To investigate the prognostic value of CD 4+CD25+/high CD127 low/-and CD14+HLA-DRlow/-for evaluating the severity of acute pancreatitis .Methods The percentages of CD4+CD25+/highCD127low/-and CD14+HLA-DRlow/-and the CD64 index were measured by flow cytometry in pa-tients with acute pancreatitis ( including 43 cases of mild acute pancreatitis and 24 cases of severe acute pan-creatitis).Moreover, the levels of C-reactive protein (CRP), acute physiology and chronic health evaluationⅡ( APACHEⅡ) score and CT severity index ( CTSI ) were detected for a correlation analysis .Results The percentages of CD4+CD25+/highCD127low/-and CD14+HLA-DRlow/-and the CD64 index in patients with severe and mild acute pancreatitis were significantly higher than those in healthy subjects .Patients with se-vere acute pancreatitis showed higher percentages of CD 14+HLA-DRlow/-than patients with mild acute pan-creatitis.With the disease progression, the CD64 index and the levels of CD4+CD25+/highCD127low/-, CD14+HLA-DRlow/-and CRP were significantly dropped after an initial increase in patients with mild acute pancrea -titis, while these indexes were continuously elevated in patients with severe acute pancreatitis .The percent-age of CD14+HLA-DRlow/-was positively correlated with CD64 index, CRP level, APACHEⅡ score and CTSI.Conclusion CD14+HLA-DRlow/-level was closely related to the severity of acute pancreatitis , which could be used as immune parameter for the estimation of the clinical severity of acute pancreatitis .
4.Application of array CGH in genetic diagnosis of clinical complex chromosomal abnormalities

Objective To evaluate application feasibility of Array CGH in genetic diagnosis of clinical complex chromosomal abnormalities.Methods Two patients of genetic counseling and two patients of prenatal diagnosis were selected from Xiamen Maternity & Child Health Care Hospital during the period of December 2010 to December 2011.Under aseptic conditions 2-4 ml peripheral blood was collected in EDTA and 2-3 ml Cord Blood was collected through cordocentesis after genetic counseling and preoperative examination.G-banded chromosome analysis and genome DNA extraction were carried out on the four cases.The whole genome of four cases were scanned and analyzed by Array CGH.The results of Array CGH were confirmed by FISH.Results Array CGH detected different kinds of duplications and deletions in several chromosomes.Most of these duplications and deletions were not detected by karyotype analysis.The results of Array CGH showed duplication of 4p16.3-4p15.31,deletion of 4p16.3 in the first case,duplication of Xp11.22-Xq11.1 in the second case,duplication of 4p16.3-4p15.32,deletion of 2q37.3 in the third case and duplication of 2q21.2-2q32.1,deletion of 2q14.3-2q21.1 in the fourth case.These duplications and deletions were confirmed by FISH.Conclusions Compared with conventional cytogenetic analysis,Array CGH can not only accurately detect micro deletion and micro duplication with high resolution and sensitivity but also identify breakpoints precisely.Array CGH can provide the basis for clinical genetic diagnosis.
5.Investigation of metabolic syndrome in the elderly population

Objective To investigate the distribution of elderly metabolic syndrome(MS)and its components in the elderly population,and to analyze its correlation with the common chronic diseases in the elderly.Methods The general situation,history of diseases and health examination results were collected in 362 elderly with an average age of 79.2 years visiting Beijing Hospital.They were grouped into 3 groups of the young old age(65-74 years old),the middle old age(75-84 years old)and the older old age(85-98 years).The prevalence distribution of MS and its correlation with the common chronic diseases in the elderly were analyzed.Results MS prevalence in average was 28.4% (103 cases)in the elderly population,with 18.6% (67 cases) in the young old group,26.3% (95 cases)in middle old group,and 33.7% (122 cases)in older old group.The tendency was rising up with age increase,and the MS prevalence was higher in older old group than in other groups (x2 =8.57,P =0.03).Correlation was observed between genesis of MS and common geriatric chronic diseases.The prevalence differences in hypertension,prostatic hyperplasia,diabetes mellitus,and coronary heart disease were statistically significant higher in MS group than in non-MS group(P =0.02,0.01,0.02,0.03,respectively).Conclusions MS prevalence of elderly population is relatively high,and has significant correlation with other chronic diseases.We need to recognize the harm of MS,to conduct regular monitoring and management according to risk level,and practice patient educations to reduce MS negative effects on elderly life quality and health.
6.Effect of constant magnetic field on the osteopontin gene expression in rat aorta smooth muscle cells

BACKGROUND:Constant magnetic field of proper intensity can inhibit the proliferation of vascular smooth muscle cells,and it can be used to inhibit the restenosis following percutaneous coronary intervention.OBJECTIVE: To observe the effect of constant magnetic field of different intensities on the expression of osteopontin gene in rat aorta smooth muscle cells, so as to investigate whether magnetic field can be used to prevent and treat restenosis following percutaneous coronary intervention.DESIGN: A randomly grouping and controlled observation.SETTING: Department of Cardiology, Xijing Hospital of the Fourth Military Medical University of Chinese PLA.MATERIALS: The experiments were carried out in the laboratory of Department of Cardiology (Military Institute of Cardiovascular Disease), Xijing Hospital of the Fourth Military Medical University of Chinese PLA from February to December in 2006. Male pure SD rats of 200-250 g were used.METHODS : Rat aorta smooth muscle cells were cultured in vitro in DMEM medium containing fetal bovine serum (0.1 in volume serum), and then the cells were randomly divided into control group, constant magnetic field of 1, 5, 10 and 50 Gs groups, those in the control group were not treated with magnetic field, and those in the other groups were treated with magnetic field and cultured for another 48 hours. Reverse transcription-polymerase chain reaction and Western blotting were combined with absorbance (A) scanning analysis to observe the effect of constant magnetic field on the expressions of osteopontin and its mRNA in smooth muscle cells.MAIN OUTCOME MEASURES: Expressions of osteopontin and its mRNA in smooth muscle cells.RESULTS: The expressions of osteopontin and osteopontin genes in the constant magnetic field groups were significantly decreased as compared with that in the control group (P < 0.05), there were also significant differences among the constant magnetic field groups of different intensities (P < 0.05). It was indicated that the stimulation of constant magnetic field was in an intensity-dependent manner, and the expressions of osteopontin and osteopontin mRNA were enhanced as the intensity of magnetic field was increased.CONCLUSION: Constant magnetic field of proper intensity can inhibit the osteopontin expression in vascular smooth muscle cells on the gene level, and magnetic field may play a role in preventing and treating the restenosis following percutaneous coronary intervention.
7.Research on training effect of the standardized training program for resident physicians by using Kirkpatrick's model

Objective To evaluate the effect of the standardized training program for resident physicians in Ruijin Hospital,and analyse the problems so as to provide reference to improve the training.Methods Questionnaire surveys were conducted among 113 resident physicians,31 teachers and 43 head nurses in Rujin Hospital by using the simple random sampling and 300.The data was analysed by Kirkpatrick's model in four layers including reaction layer,learning layer,behaviors layer and results layer.Data of reation layer was analysed by ANOVA and data of behaviors layer was compared by paired t-test.Results Reaction layer:the resident physicians' overall satisfaction score for the training is 3.45.Learning layer:all resident doctors participating in the training passed all the exams organised by the hospital.Behaviors layer:Residents made a great progress in many aspects after the standardized training program,and the difference was statistically significant (P<0.01) according to the analysis of all questionnaires written by residents,clinical teachers and head nurses.Results layer:both the patient complaint rate and the accident rate of the 113 resident physicians was 0 while they all passed the National Medical Licensing Examination and the employment rate was 100%.Conclusions the standardized training program for resident physicians in Ruijin Hospital gets fairly good effect.Resident doctors' quality and ability in many aspects are improved and the overall satisfaction for the training is high,but in salary,benefits and sense of belonging to the hospital,the satisfaction is relatively low.
8.Design, synthesis, and PPARalpha/gamma agonistic activity of novel tetrahydroisoquinoline derivatives.

A series of tetrahydroisoquinoline derivatives were prepared and their peroxisome proliferator-activated receptor (PPAR) alpha/gamma agonistic activities were evaluated to obtain more potent PPAR agonist. All of them were new compounds, and their structures were confirmed by 1H NMR and HR-MS. Three compounds exhibited higher agonistic activities of PPARgamma than that of the comparison, six compounds exhibited higher agonistic activities of PPARalpha than that of the comparison, and compound 8a was discovered as a highly potent PPARalpha/gamma agonist that is much more active than that of WY14643 and rosiglitazone. The development of potent PPAR agonists may offer a new choice for the treatment of diabetes.
9.Hsa-miR-218 inhibits growth of cervical cancer HeLa cells by targeting to LASP1

AIM:To study the effect of hsa-miR-218 on cervical cancer HeLa cell growth and the underlying molecular mechanism .METHODS:The lentivirus expression vector pmiR-218 targeting to hsa-miR-218 was constructed . pmiR-218 was transfected into HeLa cells .The number of viable HeLa cells was counted by the method of Trypan blue ex-clusion.The inhibitory rate of cell activity was detected by WST-8 assay.The expression of LIM and SH3 protein 1 (LASP1) at mRNA and protein levels was determined by real-time PCR and Western blot.The interaction between miR-218 and LASP1 was examined using a luciferase reporter assay .RESULTS:The lentivirus expression vector pmiR-218 tar-geting to hsa-miR-218 was constructed successfully and confirmed by DNA sequencing .Over-expression of miR-218 inhibi-ted the activity of HeLa cells with the inhibitory rates of 15%, 26%and 65%at 24 h, 48 h and 72 h, respectively .The difference between transfection group and blank control /negative control group was statistically significant .The luciferase activity was reduced when co-transfection with miR-218 mimics and LASP1-3’ UTR plasmid.The relative expression of miR-218 was increased after transfection with pmiR-218.Over-expression of miR-218 down-regulated the LASP1 expression at mRNA and protein levels by 25%and 75%respectively.Compared with blank control group and negative control group , the difference was statistically significant (P<0.05).CONCLUSION:pmiR-218 effectively inhibits the growth of HeLa cells in a time-dependent manner.miR-218 targets to the 3’UTR of LASP1, thus down-regulating the expression of LASP1 in HeLa cells .
10.Correlation of expression of Oct-4 and tumorigenesis and prognosis of colorectal cancer

Objective To study dynamic expression of Oct-4 during development of colorectal cancer (CRC) and investigate the association between Oct-4 expression and prognosis of patients with CRC.Methods CRC tissues, matched non-tumor tissues and colonic polyp tissues were collected and Oct-4 expressions were analyzed by quantitative reverse transcription-polymerase chain reaction (qRT-PCR), flow cytometry analysis (FCM) and immunohistochemistry (IHC).Medical records of patients with CRC were reviewed.Clinicopathological analysis was performed to assess the association between Oct-4 expression and certain chnicopathological parameters.Kaplane-Meier survival curve was conducted to evaluate association between Oct-4 expression and survival time of patients with CRC.Results The results of qRT-PCR showed that the relative expressions of Oct-4 mRNA in matched non-tumor, colonic polyp and CRC tissues were (0.23 ±0.16) × 10-5, (1.19 ± 0.76) × 10-5 and (3.79 ± 1.94) × 10-5 respectively, with a significant difference (F =8.633, P =0.001).IHC analysis displayed that the positive expression ratios of Oct-4 in matched non-tumor tissues, colonic polyp tissues and CRC tissues were 4.43%, 12.68% and 40.51% respectively, and a significant difference was discovered among them (x2 =66.311, P < 0.001).Oct-4 expre-ssion in CRC was significantly correlated with pathological stage (x2 =7.248, P =0.007), lymph node metastasis (x2 =4.888, P =0.027), distant metastasis (x2 =5.732, P=0.017) and TNM stage (x2 =4.174, P=0.041).Kaplan-Meier survival curve analysis demonstrated that Oct-4 positive cases had a shorter median survival time (37.0 months) compared with Oct-4 negative cases (76.0 months), with a significant difference (x2 =14.050, P =0.001).Conclusion The expression of Oct-4 is step-wise increased during development of CRC.The abnormal expression of Oct-4 may play certain roles in the development of CRC.Oct-4 may be an important biomarker for the clinical diagnosis, assessment of disease and prognosis in CRC.
