1.Clinical study on combination of traditional Chinese and western medicine in the correction of breech position

Objective To compare the clinical efficacy of moxibustion Zhiyin with knee chest decubitus,ear acupoint magnetic bead press combined with knee chest decubitus and knee chest decubitus in correction of breech position,thus to provide more safe and effective correction method for the clinical treatment.Methods 150 pregnant women with breech were selected.They were randomly divided into observation group Ⅰ,observation group Ⅱ and control group according to the digital table,50 cases in each group.The control group was treated with knee chest decubitus to correct breech position,the observation group Ⅰ was given moxibustion Zhiyin combined with knee chest position,the observation group Ⅱ was given ear acupoint magnetic bead press combined with knee chest decubitus.Results The effective rates of the observation group Ⅰ and observation group Ⅱ were 96%,92%,respectively,which were higher than 76% of the control group,the difference were statistically significant (χ2=5.09,4.18,all P<0.05).The effective rate between the observation group Ⅰ and observation group Ⅱ showed no significant difference (P>0.05).Conclusion Moxibustion Zhiyin combined with knee chest decubitus and ear acupoint magnetic bead press combined with knee chest decubitus in the correction of breech position have short course of treatment,high success rate,and are more economical and convenient,with high safety and good curative effect,which are worthy of promotion.
2. Research progress on Ganoderma lucidum polysaccharide in prevention and treatment of central nervous system diseases

Ganoderma lucidum is a traditional folk common rare medicinal herb, which plays an important role in maintaining human health. Among them, G. lucidum polysaccharide is one of the main bioactive components of G. lucidum, which has various pharmacological effects such as anti-cancer, immunomodulatory, antibacterial, antioxidant, and scavenging free radicals. Due to the complex etiology and pathogenesis of central nervous system diseases, the clinical manifestations are diverse, and the individual response to treatment varies widely, which brings great challenges to clinical treatment. The characteristics and mechanism of the central neuroprotective effect of G. lucidum polysaccharide are reviewed in this paper from the aspects of Alzheimer's disease, anti-epileptic, modulating microglia inflammatory response, etc., in order to provide relevant evidence for its clinical application.
3.Medium-term follow-up of clinically insignificant residual fragments after minimally invasive percutaneous nephrolithotomy

Objective To discuss the outcomes of the clinically insignificant residual fragments after minimally invasive percutaneous nephrolithotomy. Methods 75 patients (11%) with CIRF among 655 who underwent initial MPCNL from January 2008 to December 2010 were diagnosed by CT scan.Clinical data of 68 patients (39 male and 29 female) were analyzed retrospectively.Previous open surgery hadbeen performed in 13 and ESWL in 20 cases.The median residual fragment size was 1.8 mm.The anatomical distribution of CIRF was 9 at upper pole,14 at middle,34 at lower,9 at renal ureteropelvic junction and 2 at upper and lower pole.Stone analysis showed 40 cases of calcium oxalate calculi,15 of calcium oxalate calculi mixed with carbonate calculi,2 calcium oxalate calculi mixed with uric acid,3 calcium oxalate calculi mixed with struuvite stone,3 struuvite stone,2 uric acid stone and 3 carbonate apatite mixed with struvite stone.Mean follow up was 23 months (12-36).Follow-up consisted of physical examination,serum routine,urine routine and CT imaging. Results 14(21%) patients (3 upper pole,1 middle pole,4 lower pole and 6 ureteropelvic junction) had symptomatic episodes,including 9 hematuria,2 renal colic pain,5 lower urinary tract symptoms,12 with size of CIRF > 4 rmm.8 patients required surgical procedures.5 patients (1 middle,2 upper pole and 2 renal pelvis) underwent ESWL.3 patients with ureteral CIRF were performed ureteroscopic lithotripsy.The CIRF were clear after surgeries.4 paticnts with CIRF > 4 mm did not have symptoms.These patients were recommended to conservational treatments.2 patients with ureteral CIRF had renal colic pains.The stones were excluded after spasmolytic analgesic treatments.27% (3/11)CIRF located in upper pole had symptom,compared with 4% (1/14) in middle pole,11% (4/36) in lower pole and 67% (6/9) in ureteropelvic junction. Conclusions CIRF can be located variously in the kidney and ureter.Most CIRF are calcium oxalate calculi and locate in the lower pole.Patients with the history of previous open surgery or SWL are more likely to get CIRF.Medium-term follow-up of CIRF revealed that CIRF located in the renal ureteropelvis junction are more likely to have clinical symptoms.
4.The clinical effects for ultrasound guided paravertebral nerve block anesthesia of percutaneous nephrolithotomy

Objective To assess the clinical safety and feasibility for ultrasound guided paravertebral block anesthesia of percutaneous nephrolithotomy.Methods Between December 2015 to June 2016,180 patients with renal or ureteral calculi were enrolled and evaluated with uhrasonography and CT scan.Of all the 180 patients,108 males and 82 females.Their mean age was 39 years (23-71 years).The clinical characteristics of the patients in each group,such as age,gender,BMI index,ASA status,mean arterial pressure and disease type had no significant differences (P > 0.05).These patients were randomized into general anesthesia group (G group),combined spinal epidural anesthesia group (C group) and paravertebral nerve block anesthesia group (P group).G group:35 males and 25 females.Their mean age was (40.1 ± 11.8) years and BMI was (25.1 ± 3.8) kg/m2;Renal calculi 52 cases,ureteral calculi 8 cases,Average maximum stone diameter was (2.6 ± 0.8)cm.C group:38 males and 22 females.Their mean age was (39.7 ± 12.4) years and BMI was (24.6 ± 4.1) kg/m2;Renal calculi 54 cases,ureteral calculi 6 cases,Average maximum stone diameter was (2.4 ± 0.8) cm.P group:35 males and 25 females.Their mean age was (38.9 ± 12.7) years and BMI was (25.4 ± 4.0) kg/m2;Renal calculi 51 cases,ureteral calculi 9 cases,Average maximum stone diameter was (2.5 ± 0.7) cm.Periprocedural Vital signs,complications,the times of anal discharging gas and postoperative feeding,hospitalized day and expense in these three groups were evaluated.Results Major intraoperative or postoperative complications did not occur in all of the patients.Mean arterial pressure decreased during preoperative changing positions was observed in group G (mean decreased 8.8 mmHg)and group C (mean decreased 1.9 mmHg),with significant difference in intra-group (P < 0.05).Postoperative nausea and vomiting was observed in 8 and 2 patients of group G and group P,respectively (P < 0.05).Postoperative pain was observed in 2 and 7 patients of group C and group P,respectively (P > 0.05).In addition,group P had early post operation feeding time [(6.4 ± 2.4) h],shorter hospitalized day [(4.5 ± 1.1) d] and lower hospitalized expense compared with other groups (P < 0.05).Conclusions Ultrasound guided paravertebral block can provide safe and reliable surgical anesthesia for percutaneous nephrolithotomy.
5.The expressions and their relationship of PTTG,P27 and CDK4 in glioma

Objective To investigate the role of expressions of pituitary tumor transforming gene(PTTG) and p27 in glioma,and explore the relationship between the expressions of them and cell cycle regulation.Methods Specimens of 40 patient with gliomas were divided into gradeⅠ:8 cases,gradeⅡ:10 cases,gradeⅢ:13 cases, gradeⅣ:9 cases,PTTGmRNA and p27mRNA were detected by reverse transcription polymerase chain reaction (RT-PCR),and their expressions of PTTG,P27,CDK4 protein were detected by immunostaining assay using strep- tavidin-peroxidase(SP) method.Results The expressions of PTTGmRNA in gradeⅠ~Ⅳwere (0.907?0.065),(1.109?0.083),(1.312?0.089),(1.499?0.215) respectively,there was significant difference among the groups(P

Objective To analyze comparatively the outcome of modified Manipal and DeVega tricuspid annuloplaaty.Methods From Oct.2001 to Aug.2004,the consecutive 123 patients operated with modified Manipal tricuspid annuloplasty for tricuspid re- gurgitation at the time of left cardiac valve replacement(group A)were elected in this study.The other 174 patients operated with De Vega tricuspid annuloplasty at the time of left cardiac valve replacement were elected randomly for control(group B).There were no significant differences of the patient data before surgery between two groups.Results There were overall 11 early deaths(4 in group A,7 in group B),The overall in-hospital mortality rate was 3.7 %.215 of the 286 surviving patients were followed;the overall follow up was 75.2 %.Mean follow-up was(28.4?9.1)months(range from 13 to 49 months).There were ten late deaths and the late mortality rate was 4.7%.There was no significant differences in the patients with 3+~4+ tricuspid regurgitation at follow-up 18,30 and 42 months in group A;but there was an increase of the patients with 3+~4+ tricuspid regurgitation with an incremental follow up term in group B(P

Objective Evaluate the efficiency of scientific research output of the 54 departments in a hospital,to put forward improvement suggestions based on the evaluation results.Methods Select appropriate indicators of scientific input and output,use the Data Envelopment Analysis method to evaluate and analyze the efficiency.Results According to the analysis of DEA,calculate the values of overall efficiency,technical efficiency,scale efficiency and scale income.Then compare and analyze the relative efficiency of different units scientific output,to identify the relatively superior department a mong the various categories.Conclusions According to the evaluation results,to find out the input surplus and insufficient output of each decision units.Then we will put forward suggestions on hospital resource allocation to optimize the scientific input and output,to improve the competitiveness of the hospital,and to activate the potential of each department's scientific research.
8.Report of 66 cases of adrenal incidentalomas

Objective To investigate the clinical features,diagnosis and therapy of adrenal incidentalomas. Methods The data of 66 cases of adrenal incidentalomas were retrospectively analyzed. Results The accuracy rates of localization detected by B-Ultrasound,CT scan and MRI were 84.3%,100% and 100% respectively. The accuracy rates of qualitative diagnosis by B-Ultrasound, CT scan and MRI were 30.0%,75.5% and 79.2% respectively. There were 60 cases underwent operation and the postoperative pathological examination confirmed that there were 31 cases of adrenal cortical adenoma, 6 cases adrenal cyst, 5 myelolipoma, 5 pheochromocytoma, 3 adrenal cortical carcinoma, 3 metastatic carcinoma, 3 ganglioneuroma, 1 adrenal neurilemmoma, 1 adrenal hematoma, and 1 nodular hyperplasia of adrenal cortex. Conclusions Ultrasonography is the first choice of scanning adrenal incidentaloma. However the CT scan and MRI are satisfactory in localization of the tumour. The qualitative diagnosis of adrenal incidentaloma will depend on the diameter, imaging features of tumors, the age of patients, and endocrine examination.
9.Significance of Changes of Serum Beta 2-Microglobulin,Cytocine Levels in Childhood Idiopathic Thrombocytopenic Purpura

Objective To study the changes and significance of serum beta 2-microglobulin(?2-MG),cytocine interleukin-2(IL-2) in the clinical progress of childhood idiopathic thrombocytopenic purpura(ITP).Methods One hundred and ten patients of childhood ITP were chosen for the test group(70 cases of primary ITP and 40 cases of recurrent ITP),and 110 normal children for the control group.Levels of serum ?2-MG and IL-2 were determined by radioimmunoassay kit and analysed with two sample t-test.Results Among the patients,the serum IL-2 levels were significantly lower and serum ?2-MG levels were significantly higher than those in control group(P0.05).Conclusion Determining serum IL-2 and ?2-MG levels has important significance to reflect the progress of disease and direct treatment.
10.Therapeutic Effect of Electroencephalograph Biofeedback on Cognitive Function of Children with Attention-Deficit Hyperactivity Disorder

Objective To explore the therapeutic effect of electroencephalograph(EEG)biofeedback on clinical symptoms and cognitive function of children with attention deficit hyperactivity disorder(ADHD).Methods Twenty children aged(10.4?2.2)years old who were diagnosed as ADHD according to American Diagnostic and Statistical Manual of mental disorders the 4th ed received EEG biofeedback treatment.The treatment was administered by Autogenic A620 machine,? wave(4-8 Hz)was suppressed and sensory motor rate wave was strengthened(12-15 Hz)at the same time.The treatment was total 40 times.Before and after treatment,Conner's Behavior Rating Scale-parent questionnaire was administered to assess the clinical symptoms,wechsler memory scale(WMS)to assess the long-term memory,short-term memory and instantaneous memory,number cancellation test and stroop test were used to assess the cognitive process speed and selective attention,and continuous performance test(CPT)was administered to assess the attention and impulsiveness.The children in healthy control group who compared with ADHD children by one to one came from the primary or middle school near the hospital,the age difference between ADHD child and healthy control group was within 6 months.All the measurements were administered to the healthy control group.Results After the EEG biofeedback treatment,the score of behavior factor in Conners Scale decreased significantly(P0.05).Conclusions Forty sessions EEG biofeedback can improve clinical symptoms,attention,memory,selective attention and cognitive process speed of ADHD children significantly,some indicators even become normal.
