1.Development strategies of hospitals under the market economic system

Only by working out and implementing scientific and rational developmnent strategies can hospitals contend for the initiative and have a leading edge in the fierce market competition. At present, it is imperative for hospitals to develop the new century-oriented market competition strategy in view of their respective current conditions and circumstances, the talent developnent strategy based on the reform of the personnel system, the strategy of scientific and technological development, and the strategy of the socialization of rear services. Hospital managers ought to promote the construction of their hospitals in light of the above strategies. Nevertheless, in implementing hospital development strategies, it is important to guarantee the continuity and stability of the overall development strategies and not to make unpredictable changes. Only by doing so can the development of hospitals be pushed forward via working out hospital development strategies.

3.Treatment of cervical disc herniation using coblation technology

[Objective]To retrospectively analyze the results of plasma-mediated coblation in treating cervical disc herniation.[Method]From January 2003 to April 2006,127 symptomatic cases(197 disc levels)with cervical contained disc herniation were treated with plasma-mediated coblation technology,including 81 males and 46 females,their average age was 49 years,the treated disc levels were 17 in C3、4,61 in C4、5,90 in C5、6 and 29 in C6、7 ,respectively.All patients were reported to have persistent cervical or unilateral arm pain for a minimum of three months and failed previous conservative treatment.MRI showed herniated cervical disc causing focal compression of the nerve root.An average percentage of symptom relief(VAS score)and subjective satisfaction were administered to measure symptoms at all visits.[Result]In the immediate post-operative period,124 patients reported significant amelioration of symptoms,3 patients complained of arm numbness did not change clinical status,one week after operation,percentage of symptom relief was 64%,perfect rate of subjective satisfaction was 91%.Patients were followed up for average 18 months(range 6~31 months)postoperatively.Percentage of symptom relief was 47%,perfect rate of subjective satisfaction was 62%.In the last follow-up,7 patients were observed a stabilization of clinical status over 2 years and in 15 patients the status was similar to preoperation.The other patients referred some residual symptoms and were still under follow-up.There were no complications observed such as hemorrhages,infections and nerve root injurys due to the procedure.[Conclusion]Our data indicate coblation procedure appearing to be less injury,safety,is a promising treatment option for patients with contained cervical disc herniation who have failed conservative therapies and are not considered candidates for open surgery,also supplementary therapy perioperatively in patients treated with cervical open surgery.
4.DALP Analysis on genetic diversity of Sinopodophyllum hexandrum in Yunnan Province

Objective To study the genetic diversity of Sinopodophyllum hexandrum so as to provide basic molecular genetic evidence for protecting and exploiting the resource of S.hexandrum.Methods Six populations of S.hexandrum were analyzed by DALP molecular markers.To make up the genetic diversity correlation data by PopGene 1.31 software and cluster by UPGMA method,and establish the dendrogram.The tree was subsequently visualized with Treeview software.Results Five primer groups were screened and a total of 150 DNA fragments were amplified,among which 104 were polymorphic,i.e.the percentage of polymorphic bands(PPB) was 69.33%,the average number of DNA polymorphic band amplified by each primer group was 20.8 and the average PPB was 14.22%.In the six populations,the total observed number of alleles(Na) was 1.693 3 and the average Na was 1.142 2;The total effective number of alleles(Ne) was 1.417 1 and the average Ne was 1.083 8;The total Nei′s gene diversity(H) was 0.244 3 and the average H was 0.048 8;The total Shannon′s information index(I) was 0.364 3 and the average I was 0.073 0;The total gene diversity(Ht) was 0.244 3 while the gene diversity within a population(Hs) was 0.048 7.The coefficient of gene differentiation(Gst) was 0.800 5 between populations,namely 80.05% genetic variation occurring between populations and the rest 19.95% within population,and estimated gene flow(Nm) was 0.124 6.Conclusion The result indicats that S.hexandrum has much larger genetic differentiation among populations and gene flow has been blocked.This might be a result of species breeding system and population habitat.

6.Lumbar protection and lumbago prevention for the army drivers worked in plateau

Objective To investigate the long-term protective effects of the waistband onto lumbar spins.Based on the practical situation,to search for some effective and convenient methods,such as wearing waistband and mutual massage,to prevent from lumbago for the army drivers worked in plateau.Methods 372 army drivers,young males,healthy and all worked in plateau,ware involved as the treatment group in the present study.Another 545 army drivers with the similar physical qualities,also worked in plateau,were selected as the control group.For the treatment group,a kind of special waistband was employed for the drivers,and mutual massage on hack of each other was performed among them;while in control group the army drivers were given no measures for lumbar protection and lumbago pre- vention.All the drivers in the both groups ware in normal driving conditions.The therapeutic effects of the waistband and the lumbago symptoms were investigated by epidemiologic survey 16 months later,Results The epidemiologic survey results showed that the incidence of lumbago in the drivers of control group(72.8%)was significantly higher than that in the treatment group(30.9%,P

8.Effect of Motor Vehicle Exhaust of Main Urban Roadside on the Inflammatory Response in the Airway of Rats

Objective To evaluate the inflammatory response in the airway of healthy rats following exposure to motor vehicle exhaust. Methods Sixty healthy SD rats(30 males and 30 females,aged 6 weeks) were randomly divided into 6 groups. The rats in the exposed groups were placed by the main traffic road and those in the control group were placed in the normal laboratory environment. The bronchoalveolar lavage fluid(BALF) was analyzed on the 14th,30th and 90th day of exposure .WBC,NO,TNF-?,and IL-8 in BALF of the rats in each group were tested. Results The NO content [(9.75?4.78) ?mol/L] in BALF of the rats in the group exposed for 30 days was obviously higher than that in the control group [(4.40?1.45) ?mol/L],but there was no any obvious difference between the two groups in terms of the content of TNF-? and IL-8 in BALF. Conclusion This study demonstrates that exposure to vehicle exhaust can induce inflammatory response in healthy rats,the on-the-spot experiment on animal exposure can be used to observe early respiratory tract inflammation and 30 days of exposure is the sensitive period for the change of the inflammatory indicators.

AIM: To investigate the influences of native and oxidized lipoprotein(a) on human arterial smooth muscle cell (SMC) proliferation, change of intracellular free calcium concentration ([Ca 2+ ] i) and the protective effect of sodium ferulate(SF). METHODS: Lp(a) was oxidized by Cu 2+ and the extent of oxidation was assessed by the MDA content.Human SMC were incubated in culture media with SF for 12 h, then exposed to Lp(a) and oxidized-Lp(a), respectively. MTT colorimetry and flow cytometry were used to evaluated the proliferation of SMC and flurorescent indicator Fura-2/AM was used to determined [Ca 2+ ] i. RESULTS: ox-Lp(a) significantly promoted proliferation of SMC and increased[Ca 2+ ] i compared with Lp(a). SF(40,80 mg/L) remarkedly inhibited SMC proliferation and decreased the rising of [Ca 2+ ] i induced by ox-Lp(a) in a dose-dependent manner, but no effect on SMC proliferation and the increase in [Ca 2+ ] i induced by Lp(a).CONCLUSION: ox-Lp(a) induces the strong growth-promoting effect in SMC through increasing in [Ca 2+ ] i, which might be one of the cellular mechanisms responsible for the higher atherogenic potential of ox-Lp(a) compared with Lp(a), and this process can be prevented by inhibiting of oxidation by SF.
