1.Effects of recombinant hemoglobin on balance of oxygen supply and demand in coronary heart disease rats

Objective To discuss the influence of two recombinant hemoglobin (rHb1.1 and rHb2.0) and human serum albumin (HSA) on oxygen supply and demand balance in rat with coronary heart disease (CHD). Methods Male Wistar rats were randomly divided into normal control group, CHD model group, HSA treatment group, rHb1.1 treatment group and rHb2.0 treatment group, 20 rats in each group. Rat model of CHD was established by high fat diet combined with pituitrin injection. The mean arterial pressure (MAP) decreased to 40 mmHg (1 mmHg = 0.133 kPa) after femoral arterial blood was drawn from the femoral arteries, and the rats were resuscitated with 13.4% HSA, rHb1.1 and rHb2.0, respectively, at the rate of 60 mL·kg-1·h-1 (20 mL/kg). The changes of electrocardiogram (ECG) ST-segment were calculated before model reproduction and at 12 hours after the last time injection of pituitrin. MAP, heart rate (HR), superior mesenteric artery blood flow (QSMA) and arterial blood gas analysis were recorded at 0, 30, 60, 90 and 120 minutes after the administration. The blood was collected after 12-hour fasting, and serum total cholesterol (TC) and triglyceride (TG) were determined by enzymatic method. The pathological changes in cardiac tissue were observed with light microscope. Results Compared with the normal control group, the changes of ECG ST-segment and TC, TG of model group were significantly increased. Compared with the model group, rHb can significantly reduce the value of ST segment changes, and HSA has no such effect; rHb short-term infusion has no significant effect on blood lipids, but can reduce myocardial pathological changes. Compared with the normal control group, the MAP of the model group decreased significantly, the HR was increased, the QSMA was slowed down, the pH value, the residual alkali (BE), the arterial carbon dioxide partial pressure (PaCO2) and HCO3- were decreased significantly. MAP in rHb1.1 group and rHb2.0 group were significantly higher than those in HSA group. Values of MAP were significantly higher in rHb2.0 group than those in rHb1.1 group at 90 minutes and 120 minutes (mmHg: 80.9±3.3 vs. 69.4±4.9, 79.2±4.0 vs. 69.1±3.7, both P < 0.05). The HR of HSA, rHb1.1 and rHb2.0 decreased to normal in 30 minutes after administration, significantly lower than those in the model group (bpm: 534±46, 518±28, 526±37 vs. 609±52, all P < 0.05). In the rHb2.0 group, the QSMA increased significantly at 60, 90 and 120 minutes compared with the model group (qv·mL-1·min-1: 5.6±0.4 vs. 3.9±0.6, 6.2±0.6 vs. 4.1±0.4, 6.9±0.7 vs. 4.0±0.3, all P < 0.05), but there was no significant difference between the HSA group and the rHb1.1 group. The pH, BE, PaCO2 did not return to the normal level after administration of HSA; pH, PaCO2 and HCO3- in the rHb1.1 group returned to normal level at 60 minutes after administration, and BE returned to normal level at 90 minutes after administration. Each index in rHb2.0 group can restore to normal levels 30 minutes ahead of. Conclusion Recombinant hemoglobin can significantly improve the oxygen supply and demand balance of rats with CHD model, can quickly and effectively correct the hypoxic state of blood metabolic acidosis, and rHb2.0 has better effect than rHb1.1.
2.Construction of the Expression Vector of Viruslike Particles Containing FMDV IRES RNA

The Coat protein and Maturase gene of E.coli bacteriophage MS2 was amplified by PCR,then the gene was cloned into pET32a to construct the intermediate vector pET32aCP.The conservative sequence of FMDV internal ribosome entry site(IRES) was cloned into the downstream of pET32aCP bacteriophage gene to construct the prokaryotic expression vector pCPES.The recombinant plasmid pCPES transformed into E.coli strain BL21(DE3) was induced to express with 1mmol/L IPTG.The expression products were purified by sucrose density gradient centrifugation.The expression products observed by TEM were circular viruslike particles,and the diameter of these particles was about 26nm.The stability of viruslike particles was detected,and the viruslike particles was identified by RTPCR.The results showed that the viruslike particles contain the FMDV IRES RNA and have good stability.The viruslike particles have great prospect as the standard and quality control in the area of RNA virus detection.
3.Comparison of postoperative analgesic effect of flurbiprofen axetil and parecoxib sodium in patients with humeral shaft fracture in painless ward

Objective:To compare postoperative analgesic effect of flurbiprofen axetil and parecoxib sodium in patients with humeral shaft fracture in painless ward.Methods:All of 200 hospitalized humeral fractures patients were retrospectively studied in the painless ward of the Forth Medical Center of PLA General Hospital from January 2017 to September 2019 , the clinical effects of flurbiprofen axetil and parecoxib were compared.Results:Postoperative visual analogue scale (VAS) scores after 3 d of two groups of patients were significantly lower, compared with preoperative results: (4.26±0.96) scores vs. (6.09±1.38) scores, (4.04±1.19) scores vs. (6.04±1.11) scores, and the differences were statistical significantly ( P<0.01). Postoperative VAS score after 3 d of two groups had no statistical significance ( P>0.05). Two groups had different degree of adverse reactions after operation, and flurbiprofen axetil group had singnificant gastrointestinal adverse reaction: 22 patients vs. 3 patients( P<0.05). The number of patients in the parecoxib group were more than that in the flubiprofen axetil group without troubled sleep: 20 patients vs. 8 patients. Two groups of patients were satisfied with the pain care during hospitalization. Conclusions:Two analgestic drugs can obtain obvious analgesic effect in the treatment of bones surgery. The side effects and sleep disturbance in the flurbiprofen axetil group are higher than those in the parecoxib group. Good pain control can improve patients satisfaction with pain care.
4.Significance of id4 promoter methylation in monitoring AML patients with completely remission.

The study was purposed to investigate the significance of id4 gene promoter methylation in monitoring AML patients with complete remission (CR). Methylation specific-PCR (MS-PCR) were used to detect the status of promoter methylation of id4 gene in bone marrow samples from AML patients with CR who had accepted induction with DA or IA and 4 to 5 consolidation chemotherapy with Ara-C. The results showed that in the all 32 patients, 15 were found to show id4 promoter methylation and 7 out of the 15 were found relapsed or tendency to relapse in the following-up period. While all the 17 patients with id4 unmethylation were still in their CR status in the same period. In conclusion, id4 promoter methylation might be a predictor for relapse of AML patients with CR in certain degree.

5.Preliminary study on difference of Id4 gene methylation in various types of myelodysplastic syndromes.

The aim of this study was to investigate the difference of Id4 gene promoter methylation in patients with different subtypes of myelodysplastic syndromes (MDS). By using MS-PCR method, the methylation status of Id4 gene was detected in 50 patients with different subtypes of MDS. Id4 methylation was also detected in bone marrow samples from patients with iron deficiency anemia which served as control. The results showed that Id4 gene was unmethylated in all of bone marrow samples from controls. In various subtypes of MDS patients, the rate of Id4 gene methylation was different. No Id4 methylation was found in 6 cases of RA, 2 cases of RARS and 4 cases of MDS-U. Id4 methylations was found in 2 out of 18 patients with RCMD. Id4 methylation in 3 out of 12 patients with RAEBI and other 3 out 8 patients with RAEBII were found. In groups with blast ratio lower or higher than 5%, the incidence of Id4 gene methylation were 6.7% and 30% respectively, so that there was significant difference. In tentative conclusion, Id4 gene methylation possibly is found in MDS patients with higher ratio of blast cells.

6.Preventive effect of integrative medical therapy on children Henoch-Schonleln purpura with renal impairment.

OBJECTIVETo observe the clinical effect of integrative medicinal therapy in treating children Henoch-Schonlein purpura (HSP) and its preventive effect on complicated renal impairment.
METHODSOne hundred and twenty children with HSP were equally randomized into two groups, the treated group and the control group. Both were treated with conventional Western medical therapy, but Sanhuang Qingxue Yin (SQY, a Chinese herbal drug) was given additionally to the treated group. Besides, a group consisted of 30 healthy children was set up as a normal control. Changes of symptoms, physical signs, routine urine, plasma endothelin-1 (ET-1) and urinary levels of beta2-microglobulin (beta2-MG), albumin (ALB) and immunoglobulin G (IgG) before and after treatment were observed, and the recurrence was monitored.
RESULTSThe cure rate and the total effective rate in the treated group were 80.0% and 98.3%, while those in the control group were 61.7% and 88.3%, showing significant differences between groups (P < 0.05); the disappearance time of clinical symptoms was shorter in the treated group than in the control group, also showing a significant difference (P < 0.01); after 1-month treatment, levels of plasma ET-1, and urinary beta2-MG, ALB and IgG were improved in the treated group, reaching the levels opproximate to those in the normal control (P > 0.05), significant difference was shown as compared with those in the control group and with those before treatment respectively (P < 0.01, P < 0.05). The recurrent rate was 13.33% in the treated group and 30.0% in the control group, and they were statistically different (P < 0.05).
CONCLUSIONThe integrative medicinal therapy is good for treating HSP in children, it could not only obviously relieve clinical symptoms, shorten the illness course and reduce the recurrent rate, but also effectively prevent the occurrence of renal impairment.

7.Analysis of iodine nutritional status and thyroid function of pregnant and lactating women in urban areas of Wuwei City of Gansu Province

Objective To find out the iodine nutritional status and thyroid function of pregnant and lactating women in the urban areas of Wuwei City of Gansu Province,and to provide a scientific basis for iodine supplementation.Methods Blood and urine samples of pregnant (52 persons) and lactating women (59 persons) were collected in 2009.Urinary iodine content was measured with arsenic cerium catalytic spectrophotometric.Serum free three triiodothyronine(FT3),free thyroid hormone(FT4),triiodo thyronine(TT3),serum total thyroxine (TT4) and thyroid-stimulating hormone (TSH) were detected with direct chemiluminescence immunoassay.Results The medians urinary iodine of pregnant and lactating women were 274.68,217.88 μg/L.The rates of low urinary iodine (pregnant women below 150 μg/L,lactating women lower than 100 μg/L) were 9.62% (5/52) and 6.78% (4/59).Serum TT3,TT4 levels in pregnant women[(2.48 ± 0.59),(132.18 ± 33.36)nmoL/L] were higher than that in the lactating women[(2.16 ± 0.49),(108.79 ± 28.36)nmol/L,t =-3.123,-3.971,all P < 0.05].Thyroid dysfunction incidence rates of pregnant and lactating women were 17.31% (9/52) and 8.47% (5/59).Thyroid dysfunction was mostly subclinical hypothyroidism.Conclusions Overall iodine nutrition of pregnant and lactating women is in good condition,some individuals have the trend of hypothyroidism.It is necessary to carry out routine monitoring of urinary iodine and thyroid function.
8.Effect evaluation of health education on iodine deficiency disorders prevention in Linxia Hui autonomous prefecture of Gansu province

Objective To evaluate the effect of health education on the target population of iodine deficiency disorders (IDD) in Linxia Hui autonomous prefecture.Methods During 2006-2010,IDD health education was carried out in the target population of Linxia Hui autonomous prefecture,and IDD knowledge of 5th grade pupil and 20-50 years old child-bearing women were surveyed by stratified sampling and questionnaire survey methods to evaluate the effect of health education.Results After health education,IDD knowledge passing rate rose from 55.6% (548/986) and 18.0%(118/657) to 90.0%(1103/1225) and 86.2%(687/797) among pupils and women,respectively.The awareness rates of iodine deficiency in causing goiter,mental retardation and birth born silly children,and the key role of iodized salt in controling IDD were increased from 79.2% (781/986),76.5% (754/986),51.7% (510/986) and 82.3% (811/986) to 88.8% (1088/1225),85.6% (1049/1225),68.7% (842/1225) and 90.0% (1103/1225) among pupils,and from 48.6% (319/657),28.3% (186/657),15.4% (101/657) and 39.6% (260/657) to 86.2% (687/797),70.0% (558/797),46.3% (369/797) and 87.3% (696/797) among women,and all the difference were significant,respectively.Belief rate held by students that health education can prevent iodine deficiency disorders was 87.3% (1069/1225),and 52.9% (648/1225) of the students told their parents about IDD.The rate of consciously purchase of qualified iodized salt was 87.1% (694/797) after health education among women of childbearing age,and the consumption rate of iodized salt was 64.6%(515/797).More than 90% of the people in relevant departments knew iodine deficiency disorders related knowledge,in particular,had a clear understanding of intellectual hazards caused by iodine deficiency,and actively supported and participated in health education in the prevention and control of iodine deficiency disorders.Conclusions Comprehensive health education measures taken through schools and religious organizations has promoted a shift in the behavior,beliefs and attitudes of its consciously purchase and consumption of iodized salt,and integrated knowledge,attitude and practice (KAP).It is a good model of health education through religious organizations in ethnic minority areas.It has promoted the health education feasibility and accessibility,and the effect is more durable.
9.Methylation of Id4 gene promoter in acute leukemia.

This study was purpose to investigate the difference of Id4 gene promoter methylation between healthy individuals and acute leukemia patients. MS-PCR methods were used to detect the status of Id4 gene methylation in healthy individuals and acute leukemia patients. The results showed that Id4 gene was unmethylated in bone marrow samples from healthy individuals. In new diagnosed AML and ALL patients, the rate of Id4 gene methylation was 84% and 86% respectively. Id4 gene methylation was found in all 8 cases of relapsed acute leukemias. In 14 ALL-CR patients with Id4 gene methylation, 8 patients relapsed within 12 months, while in 9 ALL-CR patients with Id4 gene unmethylation only 1 patient relapsed within 12 months. In AML-CR patients, the 12-months relapse rate in patients with Id4 gene methylation was 62.5%, while it was 10% in Id4 gene unmethylation patients, there was significant difference between them. The rate of Id4 gene methylation in ALL-CR patients was 64.3%, while it was 28.6% in AML-CR patients, there was significant difference between them. In the all 39 new diagnosed AL patients, Id4 gene methylation could be detected in 33 patients. In all 8 relapsed AL patients Id4 gene methylation was found, out of 58 AL-CR cases, 24 patients was found with Id4 gene methylation. It is concluded that as compared with healthy individuals, Id4 gene in acute leukemia patients was methylated in different degrees. The percentage of Id4 gene methylation in AL-CR group is lower than that in AL-non remission group. The change of Id4 gene methylation is thought to be associated with occurrence of acute leukemia.

10.Preliminary study on Id4 as a reporter for all patients before relapse.

This study was purposed to investigate the feasibility of inhibitor of DNA banding 4 (Id4) as a reporter for acute lymphoblastic leukemia (ALL) patients before their clinical relapse. Id4 promoter at methylation status was detected by MS-PCR in bone marrow samples from the ALL patients who had been in their complete remission (CR) for 6 to 8 months with follow-up period of at least three months. The results showed that Id4 methylation were found in 20 out of all 32 patients (62.5%). The remainder 12 patients were fourd Id4 unmethylated, 1 case out of whom relapsed within the next 3-month of follow-up (8.33%), while 10 out of 20 patients (50%) with Id4 methylation relapsed within the same follow-up period. Existence of high risk factors at de novo diagnosis didn't correlated with the Id4 methylation status. In conclusion, Id4 could be regard as a reporter for ALL patients before their relapse.
