1.Relationship of CD+4CDnull28 T cells with left ventricular hypertrophy in end-stage renal disease patients

Objective To determine CD+4CDnull28 T cells and their relationship with the microinflammatory state and left ventricular hypertrophy in end-stage renal disease (ESRD) patients.Methods One hundred and twenty ESRD patients were selected,including sixty non-dialytic stage 5 chronic kidney disease patients(CKD5 stage) and thirty maintenance dialysis patients(CKD5D stage).In addition,thirty healthy adults from the hospital medical center were selected as the control group.Flow cytometry was used to detect the frequencies of T lymphocyte subsets in peripheral blood.All enrolled subjects underwent Doppler echocardiography investigations.Results(1)Compared with the control group,the high sensitivity C-reactive protein and the frequency of CD+4CDnull28 T cells were significantly expanded in CKD5 and CKD5D paients (F=14.887,9.659,all P<0.05).(2)In ninety ESRD patients,the incidence of LVH increased in CKD5 and CKD5D patients (76.67% vs.86.67%).(3)Linear correlation in CKD5 patients showed that:LVMI was positively correlated to duration of hypertension,SBP,P3+,lg(CRP+1),lgiPTH,lg(CD+4CDnull28+1) (r=0.332,0.291,0.269,0.310,0.304,0.396,all P<0.05),as well as negatively correlated to Hb(r=-0.285,P<0.05).Multiple linear regression analysis showed that LVMI was independently associated with lg(CD+4CDnull28+1) and duration of hypertension(F=6.589,P<0.000).ConclusionThere is microinflammatory state in the ESRD patients regardless of whether or not they have dialysis treatment,and they have the high incidence of LVH and exhibit a substantially increased frequency of circulating CD+4CDnull28 T cells.The frequency of CD+4CDnull28 T cells and duration of hypertension are the independent predictors of LVH in non-dialytic stage 5 CKD patients,indicates that these cells may play a critical role in the development of cardiovascular disease.

3.Complications of Visceral and Vascular Injury in Laparoscopic Surgery for Gynecologic Diseases

Objective To study the incidence of complications of visceral and vascular injury in laparoscopic surgery for gynecologic diseases,and to discuss the ways to decrease the incidence. Methods The data of 2684 patients who received laparoscopic surgery from Januray 2003 to December 2005 in the Department of Obstetrics and Gynecology were reviewed retrospectively.The incidence and treatment of complications of visceral and vascular injury were observed. Results The total incidence rate of complications was 2.53%(n=68),and that of the visceral and vascular injury was 0.37%(n=10).Four cases of injury were related with trocar punctures(injury of omental blood vessel,n=2;postperitoneal vessel,n=2),three took place during the operation (severe bleeding,n=1;bladder injury,n=2),and the other three were observed in the postoperative stage(ureter injury,n=2;intestinal fistula,n=1). Conclusion The complications in laparoscopic surgery for gynecologic diseases are increased with the extension and difficulty of operation,and are closely related with the experience of the surgeons.Proper candidates for the surgery and established operative technique are the key factors in decreasing the incidence.
5.Factors Affecting Distortion Product Otoacoustic Emisson in Newb orn Hearing Screening

Objective To seek the factors affecting distortion product otoac oustic emisson in newborn hearing screening. Methods All newborns in our hospital received hearing sc reening by DPOAE and wer e investigated for the factors such as: birth history, pregnancy history, family history, the social position of the family. All data were analysed by SPSS. Results 82.8%(3 242/3 944) newborns passed the hearing sc reening. Pass rates were h igher in right ears, female baby, monocyesis, normal or high birth weight and fo rceps delivery. The Results of the first hearing screening were correlated with birth weight,sex and fetus number by logistic regression. Conclusion The factors suc h as sex, ways of labor, fetus number, fetus age, and birth weight can affect t he results of first hearing screening significantly.
6.AN ANALYSIS OF THE FEATURES OF PRE-HOSPITAL SUDDEN DEATH

In order to decrease the mortality of pre-hospital sudden death events(SDEs),their features especially those of cardiac sudden death were analyzed. 1030 pre-hospital emergent cases from 1973~1999 were analysed systematically. The total SDEs occurred in 106 cases, among the 68 cases of cardiovascular diseases, 89.7% had coronary heart diseases(CHD), AMI or OMI with re-infarction. There were two peak age stages of SDEs: the first was 46-55 years old stage(non-retaired age stage,22.6%), and the second stage was 66-75 years old(27.3%). The pre-hospital SDEs are the most emergent cases. Great attention must be paid to those with the history of cardiovascular diseases, especially those with the history of coronary cardiac events
7.Expression of liver X receptor in the liver tissues of rat with multiple organ dysfunction syndrome induced by wound infection

Objective To study the expression of liver X receptor (LXR) during the progress of multiple organ dysfunction syndrome (MODS) caused by wound infection, and its role in the lipid metabolism during MODS. Method The MODS models caused by wound infection were produced. One hundred Wistar rats were randomly divided into wound group (group T, n = 30) and MODS caused by wound infection group (group M, n = SO). The closed multi-fracture and extensive soft-tissue injury was produced in rats using jaw. After 12 hours, a thirty percent total body surface area Ⅲ (TBSA Ⅲ ) bum was induced in group M,and the rats were contaminated by pseudomonas aeruginosa. The levels of endotoxin ( LPS), alanine transaminase (ALT), free fatty acid ( FFA), TG, HDL, VLDL were determined before injuries and post injuries 24, 48, 96, 120 hours, and at each time point in each group, only 10 rats were determined. And at the same tune, real-time quantitative reverse transcription polymerase chain reaction technique was used to measure the gene expression of LXR. Chi-square test was used. Results The blood ALT and LPS levels in rats were increased slowly, the levels of FFA reached peak at 96 hours post injuries (P < 0.05), the level of TG increased first and then decreased to the lowest at 96 hours, HDL dropped slowly, and VLDL decreased first and then increased. The gene expression of LXR increased slightly at 24 hours and then gradually decreased in group T, and the gene expression of LXR decreased more obviously in group M, and there was significant difference compared with group T. Conclusions ALT increased slowly, and was positively correlated with IPS post injuries. This indicated that infection aggravated the liver injury. The gene expression of LXRα increased in the early stage and then decreased in the late stage, LPS and LXRα were negatively correlated, which may cause the change of LXRα. The decreased expression of LXRα during MODS may contribute to dysfunction of cholesterol transport and fatty acid synthesis.
8.Lesion Extraction from B-type Ultrasound Image Using Subordinate Degree Region Level Set Method.

B-type ultrasound images have important applications in medical diagnosis. However, the widely spread intensity inhomogeneity, low-scale contrast, constructed defect, noise and blurred edges all make it difficult to implement automatic segmentation of lesion in the images. Based on region level set method, a subordinate degree region level set model was proposed, in which subordinate degree probability of each pixel was defined to reflect the pixel subjection grade to target and background respectively. Pixels were classified to either target or background by calculation of their subordinate degree probabilities, and edge contour was obtained by region level set iterations. In this paper, lesion segmentation is regarded as local segmentation of specific area, and the calculation is restrained to the local sphere abide by the contour, which greatly reduce the calculation complexity. Experiments on B-type ultrasound images showed improved results of the proposed method compared to those of some popular level set methods.
