1.Effects of Multimodal Analgesia on Cytokines and Cortisol Levels in Patients Received Surgical Treatment for Radical Colon Cancer

Objective To investigate effects of multimodal analgesia on cytokines and cortisol levels in patients received surgical treatment for radical colon cancer.Methods Eighty-six patients with radical colon cancer treated with surgery were randomly divided into control group and observation group.The control group only received routine epidural anesthesia and the observation group received multimodal analgesia.Preoperative and postoperative VAS scores,cytokines and cortisol levels were measured.Results Compared with those of the control group,the following 5 indicators decreased significantly in the observation group including VAS scores of after 24 h,48 h,72 h,IL-8 and TNF-α after 24 h,IL-6,IL-8 and TNF-α after 48 h,Cor after 24h,and Cor and ACTH after 48 h (P<0.05).Conclusion Perioperative multi-mode analgesia helps relieve pain,stress and inflammatory cytokines among patients after the surgery for radical colon cancer.

3.Effectiveness and safety of bronchoscope combined with cryotherapeatic equipment in treatment of bronchial foreign body in children

Objective To explore the effectiveness and safety of fiberbronchoscope combined with cryotherapeatic equipment in treatment of bronchial foreign body in children.Methods 72 children with bronchial foreign body received fiberbronchoscope combined with cryotherapeatic equipment, then the complications were observed during and after the operation.Results Bronchial foreign bodies were successfully removed in all the patients. Among the 72 cases, transient hypoxia was the most common complication during the operation which happened in 37 cases and most occurred in the small age group, and decreased with the age increasing; cough or worsening of cough was the most common complication after the operation which happened in 20 cases; No serious complications were found during the follow-up.Conclusion Although it has some complications when bronchial foreign body received the fiberbronchoscope combined with cryotherapeatic equipment, but the operation time is short, and the complication is mild. It has great application value and security.

5.CHEN Yongcan's Experience in Treating Postoperative Complications of Anorectal Disease from the Liver and Spleen

[Objective]To summarize the clinical experience of Director CHEN Yongcan in the treatment of postoperative complications of anorectal disease from the liver and spleen.[Methods]Through learning from teachers,collecting clinical medical cases,reviewing relevant literature,analyzing the relationship between liver and spleen and anorectum,the key pathogenesis and main treatment methods of this disease were proposed,and the clinical experience of Director CHEN in treating postoperative complications of anorectal disease from the liver and spleen was expounded,and three medical cases were attached for evidence.[Results]Director CHEN believes that the postoperative complications of anorectal disease are closely related to liver and spleen disorders,and the disease is located in the anorectum,and the liver and spleen are responsible.The relationship between anorectum and liver and spleen reflects in three aspects:physiological communication,functional connection and five elements correlation,and the key pathogenesis is liver and spleen discord and Qi imbalance,and the treatment advocates starting from the liver and spleen,with the three methods of rising,clearing and nourishing,harmonizing the liver and spleen,regulating the Qi movement,and promoting the recovery of anorectal function after surgery.The cited medical cases included the treatment of postoperative constipation of mixed hemorrhoids by raising the spleen and soothing the liver,the treatment of postoperative abdominal distension of anal fistula by clearing the liver and promoting the spleen,and the treatment of postoperative diarrhea of rectal cancer by nourishing the spleen and softening the liver,all of which achieved good therapeutic effects.[Conclusion]Director CHEN has rich experience in the treatment of postoperative complications of anorectal disease from the liver and spleen,which is practical and worthy of promotion.
6.Clinical features of infective endocarditis in senile patient(with comparison with middle-aged and young cases)

Objective To investigate the clinical features of infective endocarditis(IE)in senile patients.Methods The clinical data of 166IE patients were retrospectively studied,including 37senile cases and 129middle aged and young patients finally diagnosed as IE and treated in the hospital the authors served from Jan.1999to Jan.2009.Analysis and comparison between the two groups were done on original heart diseases,clinical manifestations,laboratory examination,imaging features,treatment,prognosis and outcome.Results Among the primary heart diseases,degenerative heart valve disease(62.1%,23/37)and coronary heart disease(24.3%,9/37)were more common in senile patients,among them the body temperature of febrile cases was usually below 38.5℃ (75.7%,28/37),and the cardiac murmurs could be heard usually in aortic valve area(51.4%,19/37).The incidence of embolization was lower in senile group(13.5%,5/ 39)than in middle aged and young group(31.8%,41/129)with significant difference(P0.05)was found in operative mortality between the two groups.Conclusions The outcome is worse in senile patients with infective IE than in middle aged and young cases.Early diagnosis for senile IE patients was difficult because of atypical clinical features and lower positive rate of blood culture.Echocardiography may play a pivotal role in the diagnosis of senile infective IE.Surgical operation may be a reasonable option for the senile IE patients with operative indication.
7.Inhibition of naringenin complex with β-cyclodextrin on experimental choroidal neovascularization in rats

Background Choroidal neovascularization (CNV) leads to blindness in many fundus diseases.Study showed that naringenin suppresses CNV,but it presents with poor bioavailability because of its poor solubility in water.β-cyclodextrin (β-CD) can increase the water-solubility of drugs, however, whether the inhibitory effect of naringenin on CNV can be improved after clathrated with β-CD remains unclear.Objective This study was to compare the inhibitory effects of naringenin with naringenin/β-CD compounds on CNV in rats.Methods Naringenin/β-CD clathrate compounds were prepared with saturated solution,the solubility of naringenin in water was calculated based on standard curve.Thirty-two male Brown Norway rats were randomized into normal control group, model control group, naringenin group and naringenin/β-CD group.Laser-induced CNV models were created in the right eyes of rats from the model control group, naringenin group and naringenin/β-CD group.Naringenin and naringenin/β-CD clathrate compounds were intraperitoneally injected at a dose of 20 mg/kg in the rats of naringenin group and naringenin/β-CD group since the day after modeling, respectively, once per day for 4 weeks, and equal volume of DMSO was injected in the same way in the model control group.Fluorescein isothiocyanate dextran (FITC-D) was injected via rat hypoglossal vein for the preparation of flatmounts of choroid in the fourth week,and the areas of CNV were measured and compared among the groups.The retinal pigment epithelium (RPE)-choroid-sclera tissues were isolated from the rats, and the relative expression levels of vascular endothelial growth factor (VEGF) , cyclooxygenase-2 (COX-2),phosphatidylinositol-3-kinase (PI3K),p38mitogen-activated protein kinase (p38MAPK), matrix metalloproteinase (MMP)-2, MMP-9 mRNA and their proteins in RPE-choroid-sclera tissue were detected using real-time PCR and Western blot.Results The solubility of naringenin in water increased by 11.8 folds after encapsulated with β-CD.The CNV areas in the model control group, naringenin group and naringenin/β-CD group were (34.56± 1.67), (20.90± 1.47) and (13.20± 1.38) × 103 μm2 , respectively, showing significant reduces in the naringenin group and naringenin/β-CD group compared with the model control group (t =3.973 ,P<O.05;t =5.532, P<0.01) ,and the CNV area in the naringenin/β-CD group was significantly smaller than that in the naringenin group (t =3.605,P<0.05).The relative expression levels of VEGF, COX-2, PI3K, p38MAPK, MMP-2, MMP-9 mRNA and their proteins were significantly declined in the normal control group,naringenin group and naringenin/β-CD group in comparison with the model control group (all at P<0.05).In addition, the expression levels of VEGF mRNA and COX-2 mRNA and their proteins were significantly lower in the naringenin/β-CD group than those in the naringenin group (all at P<0.05).Conclusions The naringenin/β-CD clathrate compounds can improve the water solubility of naringenin and enhance their inhibitory effect on rats CNV.The inhibitory effect of naringenin on rats CNV probably is associated with anti-inflammatory pathway.

9.Association between prognosis and renal vascular lesions in lupus nephritis patients

Objective To examine the relationship between renal vascular lesions and clinical prognosis in lupus nephritis patients. Methods Renal biopsy specimens and clinical data of 451 patients with lupus nephritis in our hospital from January 1998 to February 2006 were enrolled in this study. According to renal vascular lesions, those patients were classified into six groups: no renal vascular lesion (NVL), immune complex deposits (ICD), lupus vasculopathy (LV),thrombotic microangiopathy (TMA), vasculitis (VAS) and arteriosclerosis (AS). The relationship of renal vascular lesions and the clinical features was retrospectively analyzed. Results (1) In 451 lupus nephritis patients, 247 eases (54.8%) had renal vascular lesion. The incidence of the ICD, LV, TMA, AS and VAS were 8.4%, 9.8%, 6.7%, 29.3% and 0.7%, respectively. (2) The incidence of vascular lesions in IV type lupus nephritis was 69.7%, which was higher than others.(3) Except ICD group, the incidences of renal insufficiency and hypertension were significantlyhigher in vascular lesion groups than those in NVL group (P<0.05). TMA group showed the most severe clinical manifestation, including anemia and depression of platelet. (4) The prognosis of TMAgroup was the worst. The prognosis among other groups was not significantly different.Conclusions Vascular lesions are common in lupus nephritis. TMA patients present not only the most severe clinical manifestation with the poorest prognosis. Other vascular lesions also indicate an elevation of blood pressure and Scr level, but without significant difference in prognosis.
10.Features of 18F-FDG PET/CT Imaging of Primary Lymphoma of Bone

Purpose The clinical manifestation of primary lymphoma of bone (PBL) is lack of specificity,and it is easy to be misdiagnosed.This study is to investigate the characteristics of ~F-fluorodeoxyglucose (~F-FDG) positron emission tomography/computed tomography (PET/CT) of PBL and to improve the understanding of its imaging findings.Materials and Methods Fourteen patients with PLB (9 males and 5 females) proved by pathology in the Affiliated Hospital of Southwest Medical University from January 2011 to October 2015 were enrolled.18F-FDG PET/CT findings of all the patients were retrospectively analyzed.The CT image,PET image and fusion image were analyzed by 3 physicians of medical imaging.The SUVmax value of the lesion was measured through region of interesting.Results Ten patients with unifocal lesion involved 4 cases of femoral bone,2 cases of pelvis,2 cases of vertebrae,1 case of ulna and 1 case of tibia.The other 4 patients had multifocal lesions.The lesions showed different level of increased radioactivity on the PET imaging with SUVmax of 9.85±4.29.On syn-CT images,6 cases of unifocal PLB showed cribriform or fuzzy boundary insect erosion osteolytic bone destruction,5 of which were accompanied by cortical bone destruction.The other 4 cases of unifocal PLB showed mixed bone destruction.Among the 10 cases of uniffocal PLB,7 of them showed soft tissue masses with unclear boundary and no obvious necrosis or cystic change,1 of them showed pathological fracture and periosteum reaction.The 4 cases of multifocal PBL patients showed multiple osteolytic bone destruction,2 of them had localized soft tissue masses.Conclusion 18F-FDG PET/CT manifestations of PBL have certain characteristics,which is helpful for early diagnosis.It may provide important information for clinical treatment and prognosis evaluation.
