1.Expression and significance of IL-4 and IL-10 in anaphylactic shock

Objective To establish animal model of guinea pig anaphylactic shock ,explore the expression of IL‐4 and IL‐10 in guinea pig serum ,provide scientific and objective evidence for medicolegal and clinical diagnosis of anaphylactic shock as well .Meth‐ods After 40 guinea pigs were classified into the experimental and the control groups ,in which the experimental group was 30 and the control group was 10 ,Elisa method was used to detect IL‐4 and IL‐10 by extracting serum of guinea pigs in the experiment group and the control group .Results The IL‐4 levels of serum in experimental group was higher than that in control group (P<0 .01);the IL‐10 levels of serum in experimental group was higher than that in control group (P<0 .01) .Conclusion The level of serum IL‐4 and IL‐10 in the experiment group are obviously higher than that of the control group .This provides an evidence for an‐aphylactic shock forensic identification and clinical diagnosis .
2. Vitamin E Succinate inhibits growth and induces apoptosis of prostate cancer cell line PC-3

Objective: To study the inhibitory and apoptosis-inducing effect of vitamin E succinate (VES) on androgen-independent prostate cancer cell line PC-3 in vitro. Methods: VES was dissolved with ethanol to obtain VES solution. PC-3 cells of logarithmic growth phase were treated with various concentrations of VES solution (25, 50, 75, 100, and 125mg/L); cells in control group were treated with 1.25% ethanol. MTT method was used to measure the viability and inhibitory rate of cells in each group 24h, 48h and 72h after VES treatment; flow cytometry was employed to determine the apoptosis rate of the PC-3 cells. Results: The viability of cells in the experimental groups was significantly lower than that in the control group (P<0.05). The viability of cells in the experimental groups was negatively correlated with the concentration and exposure period of VES solution; the viability of cells in the control group was positively correlated with the exposure period of VES solution. The apoptosis rate of cells in the experimental groups was much higher than that in the control group; the rate in the experimental group was positively correlated with the concentration and exposure period of VES solution. The optimal induction of apoptosis was achieved af ter 48 h exposure to 75 mg/L VES solution, with a apoptosis rate above 80%. Conclusion: VES can inhibit the proliferation of PC-3 cells and can induce apoptosis of them, which casts new lights on prevention and treatment of prostate cancer.
3.Systemic lupus erythematosus with secondary antiphospholipid syndrome: a retrospective clinicopathological analysis of 11 cases

Objective To analyze the clinicopathological characteris tics of systemic lupus erythematosus (SLE) with secondary antiphospholid syndrome (APS) . Methods Data of 11 cases of SLE with secondary APS (SLE with APS) admitted to Peking Union Medical College Hospital from January 2000 to March 2010 were retrospectively analyzed. Kidney biopsy was performed on all of these patients. Differences of clinicopathology and outcomes between SLE with and without APS were compared. Results Renal involvement was found in all the SLE with APS patients. The prominent clinical manifestations included hypertension (54.5%), nephrotic level of proteinuria (24 h proteinuria ≥3.5 g)(72.7%) and renal insufficiency (45.5%). Diastolic blood pressure, mean arterial pressure and glomerular filtration rate in SLE with APS were significantly higher than those in SLE without APS (all P<0.05). In 8 out of 11 cases (72.7%), APS nephropathy (APSN) in kidney biopsy was found, characterized by small vessel vaso-occlusive lesions. These included thrombotic microangiopathy (TMA), fibrous intimal hyperplasia (FIH), focal cortical atrophy (FCA) and tubular thyroidization. Among those, 5 cases (45.5%) had chronic APSN and 4 (36.4%) had acute APSN (one case had acute and chronic APSN at the same time). The incidences of APSN and acute APSN in the SLE with APS group were significantly higher than those in SLE without APS group (P<0.05). Conclusions The major renal manifestations of SLE with APS are hypertension, nephrotic level of proteinuria and renal insufficiency. Other than lupus nephritis, also a high incidence of APSN is found in SLE with APS patients.
4.Pulsed radio-frequency stimulation combined with nerve block for cervicogenic headache

Objective To observe the clinical effect on cervicogenic headache (CEH) of pulsed radiofrequency stimulation (PRF) applied to the C2 dorsal root ganglion combined with nerve block.Methods A total of 78 cases diagnosed as CEH were randomly divided into a combined treatment group,a PRF treatment group and a nerve block group.The combined treatment group was given both PRF applied to the C2 dorsal root ganglion and blocking therapy.The other two groups were given only one treatment or the other.All the treatments were once weekly for 3 weeks.Before treatment and 1,3 and 6 months after treatment,all of the patients' headaches were evaluated using a visual analogue scale (VAS).Results At 1,3 and 6 months after treatment,the average VAS scores of all three groups had decreased significantly.The VAS ratings dropped the most in the combined treatment group,followed by the PRF group and then the nerve block group.All the intergroup differences were statistically significant.The combined treatment group's cure rate (88%) was significantly better than that of the PRF group (81%),which was significantly better than that of the nerve block group (54%).Conclusion Combining PRF applied to the C2dorsal root ganglion with nerve block therapy has a synergistic effect on CEH.The curative effect of the combined treatment was better than either PRF or blocking alone.

6.The integral term of the information entropy unadapted for being a measuring index

The information entropy, H = - Pk log Pk, has been proved in this paper to be always positive and invariant for a linear coordinate transformation, so that it can be used as a measuring index. On the contrary, the integral term of the information entropy, H(x) = -f(x) log f(x) dx, is not always positive and invariant for a linear coordinate transformation. Therefore, H(x) can not be used as a measuring index. An example of a probability density function for response of nerve to electric current with different intensities is given to confirm above conclusions.
7.Effects of pulmonary rehabilitation on patients with stable chronic obstructive pulmonary disease

Objective To observe the effects of puhnonary rehabilitation on the symptoms and quality of life of persons with chronic obstructive pulmonary disease (COPD).Methods COPD patients were recruited from a hospital and community health centers and given pulmonary rehabilitation in the community.The rehabilitation program included health education and nutrition guidance,medication,breathing exercises,and physical exercise.Each patient was followed up every 4 weeks by telephone.The outcome measures included pulmonary function (FEV1%,FEV1/FVC%),St George's questionnaire (SGRQ),the Medical Research Council scale (MRC),Borg scoring,evaluation of anxiety and depression,the six-minute walk test (6MWT),and an evaluation of nutritional status.All of the patients were observed for one year.Results A total of 85 patients completed the program and were included in the final statistical analysis.There was no difference in FEV1 % before and after rehabilitation.Six-minute walking distance improved significantly in statistical terms but the average increase was only from (416.1 ±99.84)m to (437.51 ±116.17)m.The SGRQ scores decreased by an average of 4.8 points after a year of rehabilitation,with significant improvements among the moderately and severely impaired.Incidents of COPD exacerbation also decreased significantly.Patients with a low body mass index had the greatest SGRQ score improvments.Few patients showed depression or anxiety in this research population.Conclusions Pulmonary rehabilitation in the community can improve exercise tolerance and the quality of life for persons with COPD,reducing incidents of acute exacerbation.Patients may benefit from such pulmonary rehabilitation regardless of disease severity.The compliance was good despite the infrequent monitoring.Pulmonary rehabilitation in the community is worthy of more widespread use.
8.Application of EWGSOP Consensus in Evaluating Elderly Female Patients with Sarcopenia Ranging in age from 70 to 79 from Communities of Shanghai

Objective To explore the feasibility of using the consensus of European Working Group on Sarcopenia in Older People(EWGSOP) to diagnose sarcopenia among community-dwelling women aged from seventy to seventy-nine in Shanghai.Methods Twenty-two healthy community-dwelling women were recruited as the subject group(group S),and another ten young girls were randomly selected as the control group(group C).The anthropometry,bioelectrical impedance analysis(BIA)and dual energy X-ray absorptionmetry(DEXA)were used to evaluate the circumferences of upper and lower limbs,and the dimension of muscle mass,according to relative appendicular skeletal mass(RASM)and skeletal muscle index(SMI).The grip dynamometer and isokinetic dynamometer were used to measure the muscle strength of upper and lower limbs.The ten meters gait speed,functional reach test,timed up and go test,Berg balance scale and short physical performance battery were all applied to assess the physical performance.The criteria of EWGSOP consensus were applied to diagnose the situation of sarcopenia.Results Firstly,the average biceps circumference and fat mass of upper limbs in group S were significantly higher than group C (P<0.01),while no significant differences were observed in the muscle mass(P>0.05).Secondly,RASM values gained from BIA and DEXA in S group were significantly higher than C group(P<0.05),while the SMI values of the former were significantly lower than the latter(P< 0.01).Thirdly,absolute grip strength,relative grip strength and other bio-mechanical indexes of lower limbs in the s group were significantly lower than the C group(P<0.01);the relative grip strength showed significant correlation with most bio-mechanical indexes of lower limbs.Fourthly,the physical performance in group S declined significantly compared with that of the c group(P<O.05),and no significant correlation was found between muscle strength and physical performance of upper limbs except the absolute grip strength and gait speed.The prevalence was 0% according to the RASM index,while the prevalence was 63.6% and 45.4% according to the BIA and DEXA respectively.Conclusion The assessing pattern from EWGSOP consensus could not be simply copied to applying on Chinese old people due to serious difference between RASM and SMI.The combination of SMI+relative grip strength+ TUG/SPPB/Berg may be better than the classical mode of RASM+absolute grip strength+gait speed in diagnosing sarcopenia.

from a RYR2 mutation who had not only typical electroencephalogram changes, but also epileptiform
discharges in electroencephalogram. We believe the changes were closely related to the RYR2 mutation.

Objective To study the effect of heart function and histopathology after coronary vein bypass grafting.Methods 16 pigs were divided into experimental group and control group.Following 16 pig models of chronic myocardial isehemia of left ventri- cle were established,the left internal mammary artery (LIMA)-great cardiac vein (GCV) anastomasis in 8 pigs were completed 4 week later.After GCV occlusion at proximal,retrograde coronary venous perfusion was afforded.At 8 week,coronary angiography,echo- cardiography and myocardial radioisotope scanning were performed.After harvesting these hearts,histopathologic examination and electron microscope were undertaken.Results All anastomotic vessels of experimental pigs were smooth.LVEF in experimental group was significant higher than that in control group (P
