Objective To observe the clinical therapeutic effect of combination of traditional Chinese medicine (TCM) and western medicine for treatment for acute coronary syndrome (ACS). Methods Sixty patients with ACS were randomly divided into 2 groups: the treatment group and the control group. The combined treatment group was treated with conventional therapy (anti-platelet, anticoagulation, therapies for lowering blood-lipids and blood pressure, dilatation of coronary artery and protection of vascular endothelial function) and TCM complex prescription named Suxinkang granules (复方苏欣康颗粒), the control group was treated with conventional therapy and drop pills of salvia miltiorrhiza (复方丹参滴丸). Two weeks of treatment constituted one therapeutic course, and after one therapeutic course, the clinical symptoms, manifestations of electrocardiogram (ECG) and dynamic ECG changes in 24 hours were recorded. Results The total effective rate of combined treatment group was significantly higher than that of the control group90.0%(27/30) vs. 83.3%(25/30), P0.05). Conclusion The TCM complex prescription Suxinkang has a certain curative effect on ACS and it is an effective method in combination of traditional Chinese and western medicine for treatment of ACS.

Objective To investigate the effects of Huanglong decoction on mRNA and protein expression levels of intestinal epithelial tight junction protein Claudin-1 and Occludin in rat with sepsis. Methods Fifty-five Wistar rats were randomly divided into sham operation group (n=10), model group (n=15), western medicine treatment group (15 rats) and combined traditional Chinese and western medicine group (n=15). In sepsis model group, cecal ligation with puncture (CLP) was reproduced. In the sham operation group, the steps of operation performed were similar to those in the model group, but without CLP. Normal saline 2 mL was given to sham operation group and model group by gavage;meropenem 50 mg was given to the western medicine treatment group by intra-peritoneal injection; in the combined traditional Chinese and western medicine treatment group, besides meropenem 50 mg was injected intra-peritoneally, 2 mL Huanglong decoction (containing raw herbal medicine 0.68 g/mL) was administered by gavage [ingredients of the decoction: rhubarb 15 g (put into water the latest), Glauber salt 10 g, fructus aurantii immaturus 15 g, mangnoliae officinalis 10 g, ginseng 10 g, Angelica sinensis 20 g, Scutellaria baicalensis 15 g, Radix Glycyrrhizae 10 g, concentrated to a decoction 150 mL], in the four groups, the respective drugs or normal saline were given twice a day. After 4 days, the rats were sacrificed, and a partial ileal tissue close to the ileocecal region about 10 cm in length was taken; and mRNA expression levels of Claudin-1 and Occludin were detected by reverse transcription-polymerase chain reaction (RT-PCR), and protein expression levels of Claudin-1 and Occludin were detected by Western Blot. Results Compared with the sham operation group, the expression levels of Claudin-1 and Occludin mRNA [integrated absorbance (IA) ratio] and protein (gray value ratio) were significantly lowered in model group, western medicine group and combined therapy group (Claudin-1 mRNA: 0.327±0.029, 0.387±0.035, 0.420±0.053 vs. 0.561±0.035, Occludin mRNA: 0.364±0.058, 0.422±0.033, 0.442±0.048 vs. 0.616±0.036, Claudin-1 protein: 0.307±0.015, 0.337±0.017, 0.355±0.027 vs. 0.426±0.017, Occludin protein: 0.326±0.028, 0.355±0.017, 0.366±0.025 vs. 0.455±0.018, P<0.05 or P<0.01). Compared with the model group, the expression levels of Claudin-1 and Occludin mRNA and protein in western medicine group and combined therapy group were significantly higher (P < 0.05 or P < 0.01); the up-regulation was more marked in combined therapy group (all P < 0.05). Conclusion The protective effect of Huanglong decoction on intestinal mucosal barrier in rats with sepsis is possibly related to the up-regulation of expression levels of Claudin-1 and Occludin mRNA and protein.

BACKGROUND: Modified banxia houpu decoction (MBHD) is one of thefamous methods and has been used by Zhang Zhong-jin to treat emotionaldiseases in Han dynasty, and it has been used till now. It can be used totreat depression, anxiety, phychoneurosis and so on.OBJECTIVE: To observe the therapeutic effect of MBHD on treatingheroin abusers with protracted abstinence syndrome and compare the effectwith brown sugar water.DESIGN: Randomized grouping and placebo randomized controlled study.SETTING: School of Medicine, Hubei Institute for Nationalities and theAbstinence Institute of Public Security Bureau of Enshi AutonomousPrepecture.PARTICIPANTS: Totally 187 cases were the abusers of heroin who hadbeen forced to abstinence in the Abstinence Institute of Public SecurityBureau of Enshi Autonomous Prepecture from August 2000 to September2002. There were 101 males and 86 females aged 18-44 years. All pa-tients were known the fact and were randomly divided into control group(n=58), 60-day MBHD group (n=62) and 72-MBHD group (n=67) accordingto double-blind method.METHODS: ① During the abstinence, three groups were treated bylofexidine hydrochloride (LFX) which was produced by the LimitedCompany of Zhengtai Pharmaceuticals Enterprise of Hunan Province (lotnumber: 990619, 10 mg/pill) for 12 days, then they were treated sepa-rated. Give medicine to 60-MBHD group continuously for 60 days (MB-HD: Pinellia Ternate 15 g, Magnolia Bark 30 g, Poria Cocos 20 g, Perilla10 g, Ginger 20 g, Radix Acanthopanacis Senticosi 20 g, Pericarpium CitriReticulate 6 g, Pericarpium Arecae 20 g, Herba Agastachis 10 g. Tradi-tional Chinese medicines were all bought from HuBei Medicine Company;three times a day, one portion for two days), gave the control group red sug-ar water imitate medicine for 60 days (the method was the same as above),72-MBHD group were given the same quantity of MBHD as 60-MBHDgroup when LFX began to detoxifieate, the whole time was 72 days (Thetime before LFX treat 12 days and after 60 days). It took 72 days for thethree groups to be treated by LFX according to single-blind method. ②Heroin protracted abstinence syndrome scale: insomnia, pain syndromes,catarrh syndromes, mental conditions and sexual function. They werescored respectively to 1, 2, 3 according to mild, moderate and severe.③ One year tater collected the urina sanguinis (three times a week con-tinuously) regarded the positive as re-abusers. ④ Measurement andenumeration data were compared with t test and x2 test respectively.MAIN OUTCOME MEASURES: ① Scores of protracted abstinencesyndrome; ② Comparisons of drug re-abusing rate within one year.RESULTS: Totally 187 heroin-dependence patients entered the finalanalysis. ① Scores of protracted abstinence syndrome were lower in 60-day MBHD group and 72-day MBHD group than those in control group(P ＜ 0.01). Scores of pain and catarrh symptoms in 72-day MBHD group were similar to those in 60-day MBHD group (P ＞ 0.05), but scores of insomnia, mental conditions and sexual function were lower than those in 60-day MBHD group (P ＜ 0.01). ② Drug re-abusing rate within one year was lower in 72-day MBHD group than that in control group and 60-day MBHD group [73% (49/67), 95% (55/58), 82% (51/62),P ＜ 0.05],but rate in 60-day MBHD group was similar to that in control group (P ＞ 0.05).CONCLUSION: MBHD can improve the protracted abstinence symptoms of heroin abusers after detoxification. In spite of the complexity of various factors, to effectively control the early stage abstinence symptoms and median stage protracted abstinence symptoms is one of the effective measures to prevent drug re-abusing.

AIM:To observe the therapeutic efficacy of Huoxiangzhengqi Oral Liquid (Herba Agastaches seu Pogostemi, Radix Angelicae Dahuricae, Rhizoma Atractylodis, Rhizoma Pinelliae, Pericarpium Citri Reticulatae, Pericarpium Arecae) and Tablet of Radix et Caulis Acanthopanacis Senticosi on the withdrawal symptoms from heroin. METHODS: 143 cases were randomly divided into three groups, namely control group (n=44), treating group A (n=48) and B (n=51). They were detoxified by using Lofexidine Hydrochloride Tablet (LFX) for 12d, and then, the control group took an imitate preparation, the treating group A took Huoxiangzhengqi Oral Liquid and Tablet of Radix et Caulis Acanthopanacis Senticosi for 60d, But group B took Huoxiangzhengqi Oral Liquid and Tablet of Radix et Caulis Acanthopanacis Senticosi from the time when detoxification began. And then, 10d after the two treating groups stopped taking Huoxiangzhengqi Oral Liquid and Tablet of Radix et Caulis Acanthopanacis Senticosi, the protracted abstinent syndromes of the three group were observed and scored for 7d. Objective To observe the effect of pigplacenta ins tead of Ziheche (P lacenta Hominis) for the treatment of senile dementia. Methods Seventy Kunming mice were divided into blank group, model group, pos itive control group, Zih eche low and high dosage groups, and pigplacenta low and high dosage grou ps, wi th 10 in each. The senile dementia models were established with the D-Galactose subcutaneous injection. The blank group was not administered any medicines. The model group was prescribed normal saline instead of the tested medicine; the pos itive control group was given Naofukang by gavage; the Zih eche low and high dosage groups were given 2g/kg and 4g/kg Ziheche respectivel y by gavage; while the pigplacenta low and high dosage groups were treated wi th p igplacenta 4g/kg and 8g/kg respectively by gavage. After treatment for 6 week s, the behavior experimental dark-avoiding test and step-down test were applied to test the effect of the medicines on the learning memory of mice, and acetylchol inesterase and monoamine levels in brain tissues. 目的观察肌电图导引下应用A型肉毒毒素治疗脑卒中偏瘫患者肢体痉挛的临床疗效。方法65例脑卒中痉挛性偏瘫患者按FMA量表评定分为3组,应用肌电图纪录痉挛肢体在功能位时的运动单元电位数,确定治疗肌肉,每一部位注射5U,0.2ml国产A型肉毒毒素,治疗3天后由康复训练师亲自或指导家属进行肢体功能训练。采用改良AshWorth分度评分评定痉挛状态,FMA量表评定日常生活能力(上下肢分别统计)。结果治疗后,中、轻度运动障碍者恢复至基本正常;严重运动障碍者恢复至明显运动障碍;严重运动障碍合并挛缩者可改善其生存质量;所有患者增高的肌张力均有明显改善。结论在肌电图导引下,A型肉毒毒素作为缓解痉挛药物使用安全、有效,对肢体功能的恢复有显著的辅助作用。

Objective To investigate the effect of selectively upward placement of acetabular implants on limb length and post?operative function of developmental dysplasia of the hip patients with shortened legs during total hip arthroplasty (THA). Methods Twenty?six cases of developmental dysplasia of the hip received THA between January 2008 and December 2013, in?cluding 12 cases of Crowe typeⅠ, 8 of Crowe typeⅡ, 6 of Crowe typeⅢ. There were 5 males and 21 females with an average age of 62.7 years (range, 36-80 years). The left hip was involved in 9 cases and the right hip in 17 cases. The preoperative mean Har?ris score was 42.30±12.84, and the preoperative mean WOMAC score was 59.08±13.84 at the last follow?up. The anteroposterior X?ray films and CT scan of the pelvis, anteroposterior and lateral X?ray films of the femur, and TraumaCad analysis were conducted routinely preoperation. More than 70%of the bone?implant interface was covered by appropriate upward distance of acetabular im?plant. Results The follow?up time ranged from 6 to 73 months (mean, 36 months). The Harris score improved to 91.18±7.09, and WOMAC score reduced to 9.85±3.75. According to postoperative measurement, affected limb had been lengthened by 0-5 mm in 8 cases, 6-10 mm in 5 cases, 11-15 mm in 5 cases,>15 mm in 7 cases, and shortening increased 1 mm in 1 case, but the average lengthening was 9.23±7.54 mm. The upward distance of acetabular implant was 0-5 mm in 10 cases, 6-10 mm in 7 cases and>10 mm in 9 cases. The average lengthening was 6.60±6.72 mm in patients having 0-5 mm upward distance, 11.90±5.64 mm in patients having 6-10 mm upward distance and 10.11 ± 9.35 mm in patients having>15 mm upward distance, showing no significant differ?ence. The leg length discrepancy was-3.70±6.43 mm in patients having 0-5 mm upward distance, 1.71±6.24 mm in patients having 6-10 mm upward distance and 0.56 ± 7.70 mm in patients having>15 mm upward distance, showing no significant difference. Con?clusion The limb length could be improved by selectively upward placement of acetabular implants in developmental dysplasia of the hip patients with anatomically abnormal acetabulum during THA, with reasonable preoperative design and corrective operation.

In this study, a rapid and sensitive analytical method was developed for the determination of 10 major compounds (procyanidin B1, catechin, procyanidin B2, rutin, isoquercitrin, kaempferol-3-O-rutinoside, astragalin, quercitrin, quercetin, and kaempferol) in Tetrastigma hemsleyanum by using ultra-performance liquid chromatography coupled with triple-quadrupole tandem mass spectrometry (UPLC-MS/MS) in multiple-reaction monitoring (MRM) mode. UPLC-MS/MS assay with negative ion mode was performed on a Waters CORTECS C18 (2.1 mm x 100 mm, 1.6 μm) with the mobile phase consisting of acetonitrile (A) and 0.1% aqueous formic acid (B) in gradient elution at a flow rate of 0.25 mL · min(-1) and the column temperature was set at 45 °C. Under the optimized chromatographic conditions, good separation for 10 target compounds were obtained including chiral isomer procyanidins B1 and B2 were completely separated within 8.5 min. Satisfactory linearity was achieved with wide linear range and fine determination coefficient (r > 0.996 6), the overall recoveries were ranged from 95.44%-110.40% with the RSD ranging from 2.37%-8.69%.
