1.The clinical analysis of the synchronous multiple primary esophageal carcinoma

Objective To investigate the clinical characteristics,the diagnosis and therapy of synchronous multiple primary esophageal carcinoma.Methods Thirty-two cases of synchronous multiple primary esophageal carcinoma were collected from January 1980 to December 2010 and their clinic data were retrospectively analysised.Results In the whole group,there were totally 66 lesions in 30 cases of double primary lesion and 2 cases of three primary lesion.The length of the lesions were 1 cm to 6.5 cm,and there were 22 lesions in cervical esophagus,10 in upper thoracic esophagus,19 in midthoracic esophagus and 15 in lower thoracic esophagus.Within the 66 lesions,65 lesions were squamous cell carcinoma and 1 was adenocarcinoma.The mucosa between the lesions were normal,with a distance of 4 cm to 9.5 cm,average 7.1 cm.Thirty-two patients with synchronous multiple primary esophageal carcinoma received surgery.Among 32 cases,26 of them were given definite diagnosis before operation.Four of them had exploratory operation,one patient underwent palliative resection,27 patients underwent radical resection.Two cases of the pathologic results of esophageal stump showed carcinoma after operation.The complications occurred in 8 patients.Twenty-eight cases were followed up after operation,the 1,3 and 5-survival rate were 76.9%,43.3 and 14.8%,respectively.Conclusion General pre-operation examination can significant helpful for the definite diagnosis of synchronous multiple primary esophageal carcinoma and surgical treatment is better choice for this disease.
2.Preparation of 68 Ga-DOTA-TATE and its clinical trial in neuroendocrine tumor

Objective To develop an efficient and rapid method to synthesize 68 Ga?DOTA?TATE, and to perform PET/CT imaging on neuroendocrine tumor (NET) bearing mice and one patient. Methods 68GaCl3 was eluted by 0.05 mol/L HCl in 68 Ge?68 Ga generator, then added to pre?adjusted DOTA?TATE buffer, and heated for 15 min at 85 ℃, then filtered with 0.22 μm microfiltration membrane. The radiochemical purity and in vitro stability of 68 Ga?DOTA?TATE were analyzed with radio?HPLC. The biodistribution and PET/CT imaging on HT?29 colon cancer xenografts mice model were performed. After approved by Peking University Cancer Hospital Ethics Committee, a case of NET patient received 148 MBq 68 Ga?DOTA?TATE intravenous?ly, followed by a whole?body scan at 60 min post?injection. Results 68 Ga?DOTA?TATE was prepared in high radiochemical purity (>98%) and with specific activity of 55 GBq/μmol in 25 min. The tracer showed excellent in vitro stability and no acute toxicity. PET/CT imaging on mice model showed positive uptake in tumor tissues. PET/CT imaging on one NET patient showed positive uptake of 68 Ga?DOTA?TATE in primary and metastases lesions. Conclusions A rapid, high radiolabeling yield and high specific activity method to prepare 68 Ga?DOTA?TATE is established. 68 Ga?DOTA?TATE could be specifically uptaken by somatostatin receptor ( SSTR) positive NET with high affinity. It could serve as a useful tool for early diagnosis of SSTR positive NET and has clinical prospective for somatostatin?mediated targeting therapy.
3.Dosimetry verification of virtual wedge using a two-dimensional ionisation chamber array

Objective To investigate the virtual wedge (VW) dosimetric parameters with the ionization chamber array Matrixxenvlution.Methods Using Matrixxenvlution and solid water to measure and calculate Siemens accelerator's VW angle and VW factors of different fields and compare the wedge field dose distribution to that of treatment planning system (TPS) by gamma analysis,summarized the measurement results of 50 times.Results The γ pass rate (3 mm/ 3%) of 15° and 30° VW in both direction were (91.47 ± 1.76)%,(92.99 ± 1.54)% and (93.27 ± 1.24)%,(93.27 ± 1.68)%,respectively,with the increase of filed size and the VW angle,but for 20 cm ×20 cm field and VW 60°,the result was not very good.The largest angle deviation < 2° except small field size and wedge angle,VW factors were approximately equal to 1,the maximum deviation was no more than 0.05,plan dose distribution and the measured dose distribution have good consistency except large field with large wedge angle.Conclusions Matrixxenvlution used in the measurement of VW dosimetric parameters which can obtain all parameters for angle calculation and dose plane analysis with only once positioning,and become more rapid,convenient,economical and practical one of quality assurance tools for VW dose verification.

5.Effect of erythromycin on apoptosis of epithelial cell in nasal polyps.

OBJECTIVETo study the effect of erythromycin on apoptosis and expression of Bax and Bcl-2 of epithelial cell in nasal polyps.
METHODSEpithelial cells from thirty nasal polyps and fifteen inferior turbinates were cultured in Dulbecco Eagle and Ham F12 (1:1) and divided into two groups (one group were treated with Erythromycin). Apoptosis was detected by terminal deoxynucleotidyl transferase mediated dUTP nick end labeling and expression of Bcl-2 and Bax detected by immunohistochemistry at 1, 3, 5 days after culture.
RESULTSThe apoptosis indexes (AI) of epithelial cell in nasal polyps after cultured for 1, 3, 5 days with erythromycin were (33.23 +/- 6.50)%, (38.21 +/- 7.22)% and (52.63 +/- 7.86)% respectively. The AI of epithelial cell in inferior turbinates were (31.02 +/- 5.60)%, (32.13 +/- 7.15)% and (39.64 +/- 7.48)% respectively. There was significant difference between two groups at 5 day after culture (P < 0.05). Expression of Bax and Bcl-2 of epithelial cell was significantly higher in nasal polyps than that in inferior turbinates (P < 0.01). Expression of Bax of epithelial cell cultured with erythromycin was significantly higher than that in the controls (P < 0.05). Apoptosis was clearly promoted after 5 day culture with Erythromycin (P < 0.05).
CONCLUSIONSErythromycin promoted expression of Bax and apoptosis of epithelial cell in nasal polyps.

6.Overexpression of resistin affect 3T3-L1 adipocyte lipid metabolism.

OBJECTIVETo investigate the effect of resistin overexpression on 3T3-L1 adipocyte lipid and glucose metabolism.
METHODSExpression vector for rat resistin gene was constructed and transfected into 3T3-L1 adipocytes. Cell differentiation and lipid accumulation was determined by Oil Red O staining. Differentiation marker genes (pref-1, C/EBPalpha, FAS) and glucose transporter 4 (GLUT4) gene mRNA expressions were evaluated by reverse transcription-PCR (RT-PCR). Triglyceride (TG) and free fatty acids (FFAs) in adipocytes were measured by colorimetric kit.
RESULTS(1) In resistin-overexpressed adipocytes, the lipid droplets presented at the second day which was earlier than the control cells. (2) The expression of C/EBPalpha and FAS genes in resistin-overexpressed adipocytes were up-regulated and the pref-1 was down-regulated compared with that of the control cells. (3) In resistin-overexpressed adipocytes, cellular TG and FFAs levels were significantly increased (P<0.05). (4) There was no difference in the expression of GLUT4 gene between 3T3-L1 adipocytes and resistin-overexpressed adipocytes (P> 0.05).
CONCLUSIONOverexpression of resistin can affect 3T3-L1 adipocyte lipid metabolism and thereby result in obesity and insulin resistance, but have no effect on GLUT4 gene expression.

7.Pemberton acetabuloplasty for treating the developmental dislocation of hip joint.

OBJECTIVETo study advantages of Pemberton acetabuloplasty for the treatment of developmental dislocation of hip joint.
METHODSAmong 106 children (116 hips) with development dislocation of hip joint treated by Pemberton acetabuloplasty, 19 patients (29 hips) were boy, and 87 patients (87 hips) were girl, 46 hips were left side, 50 hips were right side and 20 hips were double side, ranging in age from 18 months to 13 years, with an average of 7 years and 3 months. All the 116 hips were treated with Pemberton acetabuloplasty, or "complete operation", which included the cutting of adductor and iliopsoas, Pemberton acetabuloplasty combined with subtrochanteric femoral shortening and rotation osteotomy. Among the hips, 95 hips were treated with simple Pemberton acetabuloplasty, and the other 21 hips were treated with complete operation. All the patients with bilateral developmental dislocation of hip joint were performed the operation at one side firstly, and one year later the other side were performed and the plate fixation in the first operation were removed.
RESULTSAll the patients were followed-up ranging from 2 to 10 years, with an average of 6 years. According to Muller and Seddon's evaluation criteria, 67 hips got an excellent result, 34 good, 10 fair and 5 bad, the excellent and good rate was 87%.
CONCLUSIONPemberton acetabuloplasty is suitable for the treatment of developmental dislocation of hip joint in children at different ages, and the therapeutic effects are good.

The objective was to identify some biochemical and physical properties for fusion protein IL6D24-PE40KDEL. Edman degradation, SDS-PAGE, peptide mass fingerprinting, Western blot and MTT were used for identification of the protein. The results showed that the sequence of N-terminus is Met-Ile-Asp-Lys-Gln-Ile, Met was added because of prokaryotic expression system; Western blot revealed that the purified protein could react with IL6 and PEA antibody. The purified protein IL6D24-PE40KDEL could kill the multiple myeloma cell lines U266 expressing high affinity IL6R, but it could not kill the cell lines CEM which not expressed IL6R; The molecular weight was 58.7 kD measuring by SDS-PAGE; peptide mass fingerprinting (PMF) confirmed that the construction of IL6D24-PE40KDEL was correct. A novel protein by Peptident database in EXPASY web site was identified. In conclusion, IL6D24-PE40KDEL is a new targeting protein with bioactivity of specific killing effect.

9.Application of diffusion tensor imaging-based arcuate fasciculus tractography and intraoperative arcuate fasciculus navigation.

OBJECTIVETo investigate the feasibility of applying diffusion tensor imaging (DTI)-based arcuate fasciculus tractography and intraoperative arcuate fasciculus navigation for neurosurgery.
METHODSTotally 85 patients with interhemispheric tumors who had undergone intra-operative magnetic resonance imaging (MRI) and neuronavigation were divided into sinistrocerebral tumor (SCT) group (n = 55) and sham group (n = 30). All patients accepted routine MRI and DTI preoperatively. The results from both DTI-based arcuate fasciculus tractography and neuronavigation were applied to guide the surgery. All patients were followed up at 2-4 weeks and 3-6 months postoperatively.
RESULTSAll patients smoothly received the pre-operative DTI-based arcuate fasciculus tractography. The three dimensional arcuate fasciculus was successfully integrated with the neuronavigation and achieved microscope heads-up display. Long-term follow-up showed that there were only 4 patients suffered from persistent language dysfunction.
CONCLUSIONSThe combined application of DTI-based arcuate fasciculus tractography and intraoperative arcuate fasciculus navigation is feasible for guiding brain surgery. It can improve the surgical outcomes of intracranial tumor involving language functional area. The technology also maximizes the retention of language function and improves the post-operative quality of life.

10.Three dimensional pseudo-continuous arterial spin labeling imaging in clinical staging of nasopharyngeal carcinoma

Objective To investigate the application value of three dimensional pCASL (3D pseudo-continuous arterial spin labeling) with three groups of post-labeling delay (PLD) time in the clinical staging of nasopharyngeal carcinoma. Methods Thirty-one patients with nasopharyngeal carcinoma diagnosed by pathology between August 2016 and January 2018 were analyzed. All patients underwent nasopharyngeal MRI routine plain scan, 3D pCASL and dynamic enhanced scan, 3D pCASL scan in three groups, PLD time was set to 1525 ms, 2025 ms and 2525 ms respectively. The clinical stages (8th Edition UICC/AJCC staging) of patients, the overall clinical stage, T stage and N stage were divided into high and low stage group low stage group:7 cases of overall stageⅠ+Ⅱ, 12 cases of T stage T1+T2, 10 cases of N stage N0+N1, high stage groups:24 cases of overall stageⅢ+Ⅳ, 19 cases of T stage T3+T4, 21 case of N stage N2+N3. The GE AW4.6 workstation is used to process the 3D pCASL data. With reference to the axial T2WI image, irregular ROIs with complete inclusion of tumor were manually drawn to automatically generate the mean blood flow to the Tumor Blood Flow (TBF). In three subgroups, PLD time measurement of TBF values were recorded as the TBF (1.5), the TBF (2.0) and TBF (2.5)which were shown in the TBF color charts. Derived variables were used to assess the TBF values of three groups. Two independent sample t test was used to compare TBF values between high and low stages of nasopharyngeal carcinoma. Spearman correlation analysis was used to analyze the TBF value of nasopharyngeal carcinoma and the each clinical stage. ROC analysis was performed to calculate its diagnostic efficiency. Results On the TBF color map, TBF and perfusion was slightly higher for nasopharyngeal carcinoma. The TBF value of nasopharyngeal carcinoma in three groups at PLD time was TBF(1.5)=(140.59+46.51) ml·100 g-1·min-1, TBF(2.0)=(149.93+48.63) ml · 100 g-1 · min-1, and TBF(2.5)=(157.76+44.83) ml · 100 g-1 · min-1. In the overall clinical stage, T stage and N stage, the TBF value of the high stage group was significantly higher than that of the low stage group(Total clinical stage:t=2.471, 2.774 and 2.6, respectively;T stage:t=1.581, 1.348 and 1.472, respectively;N stage:t=2.083, 2.129 and 2.046, respectively;all P<0.05). TBF value was positively correlated with overall stage, T stage and N stage (when PLD=1.5 s, r=0.474,0.289 and 0.447,respectively;When PLD=2.0 s,r=0.509,0.311 and 0.471,respectively;When PLD=2.5 s,r=0.500, 0.252 and 0.463, respectively;all P<0.05). According to ROC curve analysis, TBF has good diagnostic performance in all clinical stage. Conclusions 3D pCASL, as a non-invasive magnetic resonance perfusion imaging technique, can accurately measure the TBF value of nasopharyngeal carcinoma. The TBF value obtained by PLD=2.0 s had the best correlation with each stage, which has a guiding value for the clinical staging of nasopharyngeal carcinoma.
