2.Advanced in solubilization methods of water-insoluble natural drugs.

With the development of natural products, the research activities on the solubilization methods of water-insoluble natural products have been carried out worldwide. Big molecular weight and poor solubility of most natural active ingredients lead to a very poor oral absorption and low bioavailability, which has extremely limited their development in pharmaceutical fields and clinical application. As a result, it is necessary to find out a suitable technique to improve the solubility and enhance the oral bioavailability of insoluble natural drugs. Based on the related references published in these years, this review introduced some new techniques to improve the solubility and bioavailability of natural drugs, including prodrugs, inclusion complex, solid dispersion, cocrystals, osmotic pump, liquisolid compacts, micronization, self-microemulsifying, nanosuspensions, lipsomes, polymeric micelles and so on, and summarized the theory, characteristics, application range, application examples, problems and development direction of each technique.

3.Effects of healthcare cooperative intervention on patients with psychiatric symptoms secondary to aortic dis-section

Objective Aortic dissection is accute in occurrence along with fast change , which is inclined to to induce psy-chotic symptoms and affect the lives of patients .The study was to investigate the effects of healthcare cooperative intervention on pa-tients with psychiatric symptoms secondary to aortic dissection . Methods We established medical cooperation organization structure to clarify the division of responsibility along with a quick assessment form , a process chart and a green channel for emergencey treat-ment.The above method was applied to intervene 32 patients with psychiatric symptoms secondary to aortic dissection and observe the diagnosis time , the time from diagnosis to surgery , the remission time of psychiatric symptoms , the incidence of adverse events and the length of stay. Results 32 patients were diagnosed within 30min followed by emergency surgery within 30min after diagnosis, which was 1-2 hours shorter than the previous diagnosis and surgery preparation time .5-21 d after surgery , psychiatric symptoms have been effectively controlled.Extubation, falling out of bed, self-injury or injury and other adverse events did not occur .No patient died, and the average length of stay was 13.5d, which was 1.75d shorter than the length of stay of previous similar patients . Conclusion The healthcare cooperation method can quickly identify patients with psychiatric symptoms secondary to aortic dissection , prevent adverse events and shorten the length of stay .
4.Stady on the expression of miRNA-21 in renal tubular epithelial cells

To describe the relationship between miRNA-21 and renal fibrosis.Methods NRK-52E cells cultured in vitro,then constructed fibrosis model induced by transforming growth factorβ1 ( TGF-β1 ) , then bone morphogenetic protein-7 ( BMP-7 )-containing lentivirus vector was used to transfect the NRK-52E cells to anti-fibrosis, SMAD1, SMAD6, FN1, rno-miR-21-5p gene expression were detected.Results After TGF-β1 treatment, the BMP-7 mRNA and protein expressions were significantly decreased, FN1 mRNA and protein expressions were significantly increased ( P<0.05 ) . After transfected by BMP-7-containing lentivirus vector, BMP-7 mRNA expression significantly increased(P<0.05), FN1, SMAD6 and rno-miRNA-21-5p mRNA decreased, SMAD1 mRNA increased(P<0.05).Conclusion Epithelial-to-mesenchymal transition (EMT) is positively correlated with the miRNA21 and TGF-beta 1, its maybe related to the changes of BMP-7, SMAD1 and SMAD6.
5.Effect of dexmedetomidine on rabbit heart rate: in vitro and in vivo experiments

Objective To evaluate the effect of dexmedetomidine on heart rate (HR) of rabbits through in vitro and in vivo experiments, and investigate the mechanism by which dexmedetomidine lowered HR.Methods In vitro experiment Healthy adult rabbits of both sexes, weighing 2.0-2.5 kg, aged 8-10 weeks, were studied.The 24 isolated hearts passively perfused in a Langendorff apparatus were randomly divided into 3 groups (n =8 each) using a random number table: control group (group C) , and dexmedetomidine 3 and 30 ng/ml groups (D1 and D2 groups).The isolated hearts were continuously perfused with K-H solution for 45 min in group C.After 15 min of equilibration, the isolated hearts were perfused for 30 min with K-H solution containing dexmedetomidine 3 and 30 ng/ml in D1 and D2 groups, respectively.At 15 min of equilibration, and at 15 and 30 min of perfusion with K-H solution containing dexmedetomidine, HR and left ventricular systolic pressure (LVSP) were recorded.In vivo experiment Twenty-five healthy adult rabbits of both sexes, weighing 2.0-2.5 kg, aged 8-10 weeks, were randomly divided into 5 groups (n=5 each) using a random number table: dexmedetomidine 3, 6, 9, 12, and 15 μg/kg groups (D3, D6, D9, D12, D15groups), to receive the corresponding doses of dexmedetomidine which was intravenously infused over 10 min.HR and mean arterial pressure were monitored and recorded before administration (T0) , and at 15 and 40 min after administration (T1,2).The correlation between doses of dexmedetomidine and change rate of HR was tested by Spearman correlation analysis.Results In vitro experiment Compared with group C, no significant changes were found in HR and LVSP at each time point in D1 and D2 groups (P>0.05).In vivo experiment Compared with those at T0 , HR at T1 in D6 and D9 groups, HR at T1,2 in D12 and D15 groups, and mean arterial pressure at T1,2in D6, D9, and D12 groups were significantly decreased (P<0.05) , and no significant change was found in HR at each time point in group D3 (P>0.05).The correlation coefficient between doses of dexmedetomidine and change rate of HR was 0.944 (P<0.05).Conclusion The mechanism by which dexmedetomidine lowers HR of rabbits is not related to direct inhibition of sinoatrial nodes, but associated with the balance of autonomic nervous system.
6.Interventional therapy for post-anastomotic stenosis in congenital esophageal atresia

Objective To report the results of balloon catheter dilation in 13 infants with anastomotic stenosis after surgical correction of esophageal atresia. Methods Thirteen cases of anastomotic stenosis after surgical correction of esophageal atresia were enrolled, age ranging from 3 to 10 years. Barium esophagography before balloon dilation showed the diameter of esophageal stenosis ranging 1 to 3 mm and the stenosis was dilated by using balloons of 6 to 12 mm diameter. Results All the 13 cases obtained complete resolution of the stenosis after dilation for twice or thrice. There was no esophageal perforation or other complications with a period of clinical symptomless of 6 to 30 months. Conclusions Ballon catheter dilation is a simple, safe and reliable method for treating anastomotic esophageal stenosis in infants.
7.Clinical evaluation of urinary interventional therapy in children

Objective To evaluate the role of interventional therapy in Wilm's tumor, renal vascular lesions and ureter stricture in children. Methods Thirty-eight cases were analysed, including 18 cases of Wilm's tumor treated with preprocedural chemotherapy and embolization through renal artery, and 13 cases of unknown-reason hypertension with selective renal arterial angiography for venous blood renin including undergone some patients PTA. Six cases of ureter stricture were dilated with balloon and 1 with hematurea by small aerial embolization. Results After embolization, Wilm's tumors showed reduction in vascularization and reduction in size too. The surrounding tissue of the tumor had a clear demarcation with the normal tissue, decreasing the dissemination to blood and distal metastasis. Renal vascular hypertension was clearly diagnosed and treated, and the increase of renin was helpful to the diagnosis of unknown-reason hypertension. And the obstruction of ureter was improved. Conclusions Interventional therapy is feasible in the application of urinary system diseases with great significance clinically.
8.Clinical application of peripheral angiography in infants.

Objective To evaluate the application and safety of angiography in infants.Methods Eighteen infants with diagnosis of clinics,CT and MRI as hemangioma, hepatic and renal tumor and cerebral vascular diseases were undertaken digital subtraction angiography(DSA). Results All the cases were found to have anomalous lesions including hemangioma racemosum in 3 cases, capillary hemangioma in 1, hepatoblastoma in 3, renal Wilm's tumor in 1, renal rhabdomyosarcoma in 1, liver arteriovenous fistula (AVF) in 1, Galenic arteriovenous malformation (AVM) in 1,Sturge-Weber Syndrome in 1, AVM OR AVF in the craniofacial region in 2 and pre-operation cerebral angiography of PNET in 2. Conclusions DSA in infants is safe and reliable with extensive clinical application.
9.Interventional treatment of intracranial arteriovenous fistula in infants

Objective To evaluate the efficacy and safety of embolization therapy with NBCA for intracranial arteriovenous fistula (AVF) in infants. Methods Four patients with cerebral arteriovenous fistula were reported, including 3 males and 1 female with nonspecific symptoms and signs, and the diagnosis was made by DSA. Three of them were treated by embolization therapy with NBCA. Results Three of the cases were treated by NBCA, neither death nor complication occurred. Conclusion Intracranial AVF is rare in childhood. Endovascular treatment with NBCA is effective and safe, but the long-term effect has to be comfirmed by follow-up study. 10.The performance of △POP in the assessment of fluid responsiveness in septic shock patients in emergency department

Objective Respiratory variations in the pulse oximetry plethysmographic waveform amplitude (△POP) have been popularly studied as a dynamic indicators for fluid responsiveness assessment.The authors hypothesized that △POP can indicate fluid responsiveness in septic shock patient in emergency department.Methods A prospective study of 28 patients with septic shock was carried out in Emergency Room and Emergency Intensive Care Unit from 1 October,2010 to 30 September,2011.Hemodynamic data including cardiac index,stroke volume Variation (SVV) and △POP were recorded before and after volume expansion treatment.Fluid responsiveness was defined as an increase in cardiac index of 15% or greater.Results Changes in △POP after volume expansion were greater in responders than that in non-responders (P < 0.01).There was a significant relation between △POP and SVV before volume expansion (r =0.900,P < 0.0001).Conclusions △POPcan indicate fluid responsiveness non-invasively in septic shock patient in emergency department.This marker has potential clinical application with high sensitivity and reliability.
