Retroperitoneal Schwannoma is relatively rare and benign tumor originating from Schwann cell. Schwannomas arising in the retroperitoneum are infrequently reported. The majority of Schwannoma is found incidentally and misdiagnosed for other benign conditions both clinically and radiologically. We report a case of a benign retroperitoneal pelvic Schwannoma which was preoperatively misdiagnosed as an ovarian tumor with brief review of literatures.

Ovarian granulosa cell tumor is a uncommon low-grade feminizing malignancy. Its natural history shows slow growth without pain. It often reveals to be very large sized mass when diagnosed, and is confined to the involved ovary in many cases. Complete surgical removal is primary treatment and recurrences thereafter are relatively common. Postoperative adjuvant therapy including reoperation, chemotherapy, radiation therapy or a combination therapy has been used with various degrees of success. We experienced a case of huge ovarian granulosa cell tumor occupying whole abdominal cavity by rapid growth, which is presented with a brief review of corresponding literatures.

3.Radiographic Evaluation on the Forefoot: Based on the Two Different Data (1982 and 2004).

PURPOSE: We compared the bony arrangements of the forefoot in 2 different years, 1982 and 2004, to determine any changes with time period of 22 years. MATERIALS AND METHODS: The radiographs of 200 normal Korean adults, 100 male and 100 female volunteers, were evaluated both in 1982 and 2004. The radiographic results were evaluated with as follows; hallux valgus angle (HVA), intermetatarsal angle (IMA), morphology of metatarsal head and relative anterior protrusion of the metatarsals and the phalanges. The mean ages were 38 years (23-52years) in 1982, and 37 years (24-50years) in 2004. RESULTS: The mean of HVA decreased from 15.6 degrees in 1982 to 14.3 degrees in 2004 (p=0.047), and the mean of IMA increased from 8.0 degrees in 1982 to 9.4 degrees in 2004 (p=0.031). The morphology of metatarsal head and relative anterior protrusion of the metatarsals and the phalanges were not different between the two study years. CONCLUSION: Comparing with those of 1982 measurements, we found an increase of IMA and a decrease of HVA. A prospective study may be needed to illuminate course of the changes.

5.Does Real-time Compound Imaging Improve Evaluation of Breast Cancer Compared to Conventional Sonography? .

PURPOSE: Real-time compound imaging obtains multiple coplanar tomographic ultrasound images and combines them into a single compound image, reducing acoustic artifacts and noise. The purpose of this study is to determine if real-time compound imaging improves evaluation of breast cancer compared to conventional sonography. METHODS: From May 2000 to July 2001, we scanned the same axial plane with conventional sonography and real-time compound imaging in 520 patients with solid breast nodules. Twenty-eight cancers in 25 patients which were confirmed pathologically were included in this study. Twenty-five of 28 cases were invasive ductal carcinoma and the remaining three were ductal carcinoma in situ. Each image pair consisted of a conventional ultrasound and a real-time compound image with a stationary probe, to maintain an identical projection. The evaluating points were 1) contrast between cancer and normal breast tissue, 2) depiction of margin, 3) clarity of internal echotexture, 4) clarity of posterior echo pattern, and 5) clarity of internal microcalcifi-cation. Two radiologists graded for quality of images on a 5-point scale and in a blinded fashion and Wilcoxon rank test was used for comparison between conventional and real-time compound images. RESULTS: For reviewer 1/reviewer 2, compound image showed grade improvements in 1) contrast (1.4?0.5/1.4?0.7), 2) margin (1.4?0.5/1.8?0.4), 3) internal echotexture (1.0?0.5/1.4?0.7), 4) posterior echo pattern (?0.9?0.7/?0.8?0.7), and 5) internal microcalcification (1.8?0.5/1.8?0.5). In all evaluating points, there was statistically significant difference between conventional and compound images (P<0.05). CONCLUSION: Real-time compound imaging improves contrast, depiction of margin, and clarity of internal echotexture and internal microcalcification of the breast cancer. But compound image is not effective to evaluate posterior echo pattern of the breast cancer.

Viral load and the duration of viral shedding of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) are important determinants of the transmission of coronavirus disease 2019.In this study, we examined the effects of viral doses on the lung and spleen of K18-hACE2 transgenic mice by temporal histological and transcriptional analyses. Approximately, 1×105 plaque-forming units (PFU) of SARS-CoV-2 induced strong host responses in the lungs from 2 days post inoculation (dpi) which did not recover until the mice died, whereas responses to the virus were obvious at 5 days, recovering to the basal state by 14 dpi at 1×102 PFU. Further, flow cytometry showed that number of CD8+ T cells continuously increased in 1×102 PFU-virusinfected lungs from 2 dpi, but not in 1×105 PFU-virus-infected lungs. In spleens, responses to the virus were prominent from 2 dpi, and number of B cells was significantly decreased at 1×105PFU; however, 1×102 PFU of virus induced very weak responses from 2 dpi which recovered by 10 dpi. Although the defense responses returned to normal and the mice survived, lung histology showed evidence of fibrosis, suggesting sequelae of SARS-CoV-2 infection. Our findings indicate that specific effectors of the immune response in the lung and spleen were either increased or depleted in response to doses of SARS-CoV-2. This study demonstrated that the response of local and systemic immune effectors to a viral infection varies with viral dose, which either exacerbates the severity of the infection or accelerates its elimination.
