1.Associations between Breast Density on Mammography and Lifestyle Related Disease.

BACKGROUND: Dense breast reduced the sensitivity of mammography in breast cancer screening and known as an independent risk factor of breast cancer. The relationship between breast density and age, body mass index has studied. However, there are few studies on the relationship between breast density and lifestyle related disease. In this study, we investigated the relationship between mammographic breast density and lifestyle related disease. METHODS: Retrospective cross sectional research was carried out from people who visited a single health screening center in Busan from January 2015 to December 2015. We investigated age, past history of the subjects and measured their height, weight, blood pressure and waist circumference. The biochemical test was carried out using their blood. All patients underwent mammography. The breast density on mammography determined by the basis of American College of Radiology Breast Imaging Reporting and Data System (ACR BI-RADS) breast composition and 996 people was recruited. RESULTS: In the distribution of breast density, 16.3% of women (n=160) had dense breast. Age (under 49), body mass index (BMI) (underweight) were positively correlated with the BI-RADS composition category 3, 4 but the number of lifestyle related disease were negatively correlated (age ρ=0.17, BMI ρ=0.39, the number of lifestyle related disease ρ=-0.21). The odds ratio (OR) of dense breast increased with decreasing lifestyle related disease severity (OR=3.06, 95% confidence interval: 1.13-8.22, P=0.027). CONCLUSIONS: This study showed that the number of lifestyle related disease was negatively correlated with mammographic density. The OR of dense breast increased with decreasing lifestyle related disease severity. Therefore, primary physicians should consider negative correlation between breast density and lifestyle related disease in breast cancer screening.

2.A case of severe transient hyperammonemia in a newborn.

Transient hyperammonemia in a newborn is an overwhelming disease manifested by hyperammonemic coma. The majority of affected newborns are premature and have mild respiratory syndrome. The diagnosis may be difficult to determine. This metabolic disorder is primarily characterized by severe hyperammonemia in the postnatal period, coma, absence of abnormal organic aciduria and normal activity of the enzymes of the urea cycle. Hyperammonemic coma may develop within 2-3 days of life, although its etiology is unknown. Laboratory studies reveal marked hyperammonemia (>4,000 micromol/L). The degree of neurologic impairment and developmental delay in this disorder depends on the duration of hyperammonemic coma. Moreover, the infant may succumb to the disease if treatment is not started immediately and continued vigorously. Hyperammonemic coma as a medical emergency requires dialysis therapy. Here, we report a case of severe transient hyperammonemia in a preterm infant (35 week of gestation) presented with respiratory distress, seizure, and deep coma within 48 hours and required ventilatory assistance and marked elevated plasma ammonia levels. He survived with aggressive therapy including peritoneal dialysis, and was followed 2 years later without sequelae.

3.Association of Psychosocial Factors in Developing Childhood Depression and ADHD in a Community Low Income Family Children.

OVJECTIVES: As the attention devoted to children's mental health increases, medical costs and burdens mount as well. In the present study, we evaluated the association between socioeconomic status (SES) and major child psychiatric symptoms. METHODS: The subjects of this study were children of recruited from a mental health screening program in the Seoul Sungbuk mental health center over the course of 3 months. To establish the SES of each child, we collected data about each child's medical insurance, years of parental education, household income, family structure and housing. 149 children& parents completed questionnaires including the Childhood Depression Inventory (CDI) and the Korean Attention Deficit Hyperactivity Disorder Rating Scale (K-ARS). RESULTS: The mean K-ARS-P score was 12.1+/-11.1 and the suspected prevalence of ADHD was 20.8% (n=31). The mean CDI score was 12.9+/-7.9, and the prevalence of suspected depression was 16.8% (25). Depressive symptoms and ADHD symptoms were both more severe than those observed in a previous epidemiologic study in Korea. Depressive symptoms were more closely associated with family SES status. CONCLUSION: SES status is one of the most important risk factors in the development of major child psychiatric symptoms. In our study we found that depressive symptoms in particular were most tightly associated with psychosocial factors. Evaluation of the risk factors, early screening and intervention for low SES children would be valuable mental health management tactics to implement in a community mental health system.

4.Effect of a desensitizer on microtensile bond strength of different adhesives.

This study evaluated the influence of a desensitizer(MS coat) on microtensile bond strength of different adhesives: a three-step adhesive(All-Bond 2), a two-step adhesive(Single Bond), a one-step adhesive(One-up Bond F). Non-caries extracted human molars were used. Dentin surface was obtained by horizontal section on midportion of crown using a water-cooled low speed diamond saw. Teeth were randomly divided into 6 group. AMO(MS coat + All Bond)-, SMO(MS coat + Single Bond)- and OMO(MS coat + One-up Bond F)-dentin surface were treated with 17% EDTA before bonded adhesive. AMX-, SMX- and OMX-dentin surface were bonded with All-Bond 2, Single Bond and One-up Bond F, respectively, with no previous treatment with MS coat and 17% EDTA. About 1cm high resin composite(Z-250(TM)) were incrementally build-up on the treated surface. The specimens for the microtensile test were serially sectioned perpendicular to the adhesive layer to obtain 0.7x0.7 mm sticks. 30 sticks were prepared from each group. After that, tensile bond strength for each stick was measured with Microtensile Tester at a 1mm/min crosshead speed. Fractured dentin surfaces were observed under the SEM. The results were statistically analysed by using a One-way ANOVA and Tukey's test(p<0.05). Value in MPa were: AMO-44.35+/-13.21; SMO-39.35+/-13.32; OMO-31.07+/-10.25; AMX-49.22+/-16.38; SMX-56.02+/-13.35; OMX-72.93+/-16.19. Application of MS coat reduced microtensile bond strengths of both Single Bond and One-up Bond F, whereas microtensile bond strengths of All-Bond 2 were not affected significantly.

5.Isolation and characterization of a cDNA encoding a mammalian cathepsin L-like cysteine proteinase from Acanthamoeba healyi.

We have cloned a cDNA encoding a cysteine proteinase of the Acanthamoeba healyi OC-3A strain isolated from the brain of a granulomatous amoebic encephalitis patient. A DNA probe for an A. healyi cDNA library screening was amplified by PCR using degenerate oligonucleotide primers designed on the basis of conserved amino acids franking the active sites of cysteine and asparagine residues that are conserved in the eukaryotic cysteine proteinases. Cysteine proteinase gene of A. healyi (AhCP1) was composed of 330 amino acids with signal sequence, a proposed pro-domain and a predicted active site made up of the catalytic residues, Cys(25), His(159), and Asn(175). Deduced amino acid sequence analysis indicated that AhCP1 belongs to ERFNIN subfamily of C1 peptidases. By Northern blot analysis, no direct correlation was observed between AhCP1 mRNA expression and virulence of Acanthamoeba, but the gene was expressed at higher level in amoebae isolated from soil than those from clinical samples. These findings raise the possibility that Ahcp1 protein may play a role in protein metabolism and digestion of phagocytosed bacteria or host tissue debris rather than in invasion of amoebae into host tissue.

6.Risk factors influencing to mortality and recurrence after first cerebral infarction.

BACKGROUND: Cerebral infarction is one of the common cause of death in Korea. There are many studies about risk factors of cerebral infarction in the world and Korea, and there are some studies about risk factors of mortality and recurrence after first cerebral infarction in the world, but there are no reliable epidemiologic studies about risk factors of mortality and recurrence after first cerebral infarction in Korea. So this study was designed to investigate the risk factors of mortality and recurrence after first cerebral infarction in Korea. METHODS: A cohort of 257 patients with diagnosing first cerebral infarction at our hospital were selected and followed for at least 5 years. 30 days, 1 year, 5 year cumulative mortality and recurrence was calculated. Risk factors classified at the time of first cerebral infarction, and then entered into a Cox proportional harzards model for mortality and for recurrence. RESULTS: The cumulative risk of mortality after first cerebral infarction was 13% at 30days, 21% at 1year, 49% at 5years. The cumulative risk of recurrence after first cerebral infarction was 4% at 30days, 11% at 1year, 24% at 5years. The significant risk factors of mortality after first cerebral infarction were age(> OR =65), heavy alcohol drinking history, heart failure, untreated diabetes mellitus history, untreated hypertension history, high admission Hct(> OR =50%), high admission glucose(> OR =200), atrial fibrillation at admission, under drowsy mental state at admission, embolic infarction type, high admission systolic blood pressure(> OR =160), high admission diastolic blood pressure(> OR =100). The significant risk factors of recurrence were age(<65), men, heavy alcohol drinking history, ischemic heart disease, untreated DM history, high admission glucose(> OR =200). CONCLUSION: Diabetes mellitus, hypertension, heavy alcohol drinking, cardiovascular disease were important factor of cerebral infarction prognosis on our study. So primary care physicians focus on these factors when treating cerebral infarction patient.

7.Clinical Features and Factors Associated with the Frequency of Phototherapy in Premature Birth Gestation < 35 Weeks and Birth Weight < or =2,500 g.

OBJECTIVES: Clinical features according to the frequency of phototherapy and clinical risk factors on the number of phototherapy were investigated in premature births with gestation <35 weeks and birth weight < or =2,500 g. METHODS: The 186 infants with gestation <35 weeks and birth weight < or =2,500 g were admitted to the neonatal intensive care unit of Kosin University Gospel Hospital from March 2009 to August 2010. The 171 infants were alive and had jaundice requiring phototherapy. Phototherapy was usually started to 50-70% of the maximal bilirubin level. They were divided into two groups according to the frequency of phototherapy as single phototherapy group (group I) and multiple phototherapy group (group II). We retrospectively reviewed the medical records of all patients. RESULTS: The mean gestational age and birth weight of group I were 31.0+/-2.9 weeks and 1,596+/-485 g and those of group II were 31.1+/-2.6 weeks and 1,592+/-430 g. Compared with group I, albumin and Apgar score at 1 minute of group II were significantly higher and the day of peak bilirubin was also late. Duration of phototherapy in group II was statistically longer than that group I but duration of ventilator and aminophylline use for apnea was significantly shorter. The frequency of antibiotic use, incidence of bronchopulmonary dysplasia (BPD), and intraventricular hemorrhage (IVH) of group II were significantly lower than those of group I. CONCLUSIONS: The day of peak bilirubin was late and the frequency of antibiotic use, incidence of BPD, and IVH were low in group II. The aggressive phototherapy may be considered in premature births with jaundice.

8.The Expression of Heat Shock Protein 60 kDa in Tissues and Cell Lines of Breast Cancer.

PURPOSE: Breast cancer has been reported as the most common cancer of women in the United States, Western Europe and Korea and about 210,000 and 10,000 women in United States and Korea every year, respectively are diagnosed with it. Breast cancer is curable with an early diagnosis, and many researchers have made efforts to find a marker for this malady, heat shock protein (HSP) consists of 6 groups, it is highly preserved throughout both the prokaryotic and eukaryotic cells and it acts as a molecular chaperone that's involved in protein folding. HSPs have been recently reported to be related with breast cancer. In this study, we investigated the changes of expression of HSP60 in tissues and cell lines of breast cancer. METHODS: We obtained breast cancer tissues and normal tissues from breast cancer patients, and we purchased several cancer cell lines from American tissue culture correction. We treated the tissues and the cell lines of human breast cancer with heat shock protein. Proteins and mRNAs were isolated from the tissues and the cells and then we performed Western blotting, reverse transcriptase-Polymerase chain Reaction and fluorescence activated cell sorter analysis on them. RESULTS: On Western blot, HSP60 was more overexpressed in the tissues and the cell lines of breast cancer than in the normal breast tissues and cell lines. The expression of HSP60 showed 2 types of molecular weight differences in the tissues and cell lines of breast cancer, and specifically, low HSP60 was over-expressed in the cancer tissues. There was no difference between the breast cancer cell lines and the normal cell lines in the expressions of HSP60 mRNA, according to the treatment with heat shock. Also, there was no relationship between phosphorylation and the structural difference of HSP60 protein, according to HSP60 protein's molecular weight. The expression of HSP60 has been mostly reported at the mitochondria; however, in this study, it was more predominantly detected at the cytoplasm than at the mitochondria in the breast cancer cell lines. CONCLUSION: We conclude that HSP60 may be used as a diagnostic marker for breast cancer. Detailed investigation of the usefulness and significance of the HSP60 expression as a prognostic factor is required in further studies.

9.The relationship between alcohol consumption and serum liver enzymes in men.

BACKGROUND: It is well known that heavy alcohol consumption may result in various lesions on multiple organs and tissue, especially liver. The aim of this study was to measure prevalence of raised liver derived enzymes in healthy men and possible association with alcohol consumption, smoking and body mass index. METHODS: All subjects were 450 men who received Adult Health Examination at Seoul Hospital of Konkuk medical center from May, 1998 to October, 1998. We carried out telephone survey and analyzed 360 men, excluding men with HBs Ag(+), hepatic problems within 6 months, medication drug that could change serum activity of hepatic enzyme or joints, myscles, cardiac problem and non responder to telephone survey. We examined prevalence of raised liver enzymes and its possible association with alcohol consumption, smoking, and body mass index, applying logistic regression analysis. RESULTS: In 20.3% of study subjects, we found raised levels of lver derived enzymes. If the alcohol consumption was higher than 270 g/week, the odds ratio(OR) for raised liver enzymes, adjusted for body mass index and smoking, increased ; serum gamma glutamyltransferase(gamma GTP)(OR: 9.48), serum aspartate aminotransferase(AST)(OR: 13.09), and serum alanine aminotransferase(ALT)(OR: 7.40). Smoking was not associated with the risk of raised liver enzymes and obesity(body mass index 25 kg/m2) showed a positive association with two enzymes; gamma GTP(OR: 1.92) and ALT(OR: 2.08). CONCLUSION: If alcohol consumption was higher than 270 g/week, the risk of raised liver enzymes increased dramatically and gamma GTP, AST were shown more sensitive than ALT in alcoholic liver disease.

10.Comparative Study of Clinical Outcomes Between 2 Types of Multifocal Aspheric Intraocular Lenses.

PURPOSE: To evaluate the efficacy of the OptiVis(TM) Multifocal intraocular lens (IOL) and compare the clinical outcome between OptiVis(TM) and AcrySof(R) ReSTOR(R) D1. METHODS: We reviewed 20 eyes of 11 patients implanted with OptiVis(TM) and 20 eyes of 10 patients implanted with AcrySof(R) ReSTOR(R) D1. The clinical outcomes of the 2 IOLs were evaluated 1 and 2 months postoperatively and consisted of distant, intermediate, and near visual acuity, depth of focus, contrast sensitivity, wavefront aberration, patient satisfaction, decentration and IOL tilt. RESULTS: Intermediate vision was better in the OptiVis(TM) group. There were no statistical differences between the 2 groups with respect to distant vision, near vision and wavefront aberration, contrast sensitivity, decentration and IOL tilt. Depth of focus was deeper in the OptiVis(TM) group at the intermediate visual acuity zone. There were no statistically significant result differences between postoperative 1 and 2 months. CONCLUSIONS: The OptiVis(TM) multifocal IOL provided satisfactory visual acuity at distance, near, and intermediate with no apparent reduction in contrast sensitivity. The IOL can be effective for improving patient satisfaction after cataract surgery as well as correcting presbyopia.
