1.Clinical and finite element analysis of alcohol-deactivated autograft-prosthesis composite after resection of bone giant cell tumor in distal femur

Objective To evaluate the clinical outcome of alcohol-deactivated autograft-prosthesis composite after resection of bone giant cell tumor in distal femur.Methods From January 2007 to October 2008,5 patients with bone giant cell tumor in distal femur were treated with alcohol-deactivated autograftprosthesis composite,including 3 males and 2 females with an average age of 29.6 years(range,22-40).Three patients were diagnosed with postoperative recurrence,and 2 with pathological fracture.All patients were of Campanacci Ⅲ.Three-dimensional finite element models with 40% bone defect in distal femur were established based on CT images of a healthy volunteer.Three times of body mass load corresponding to the normal walking gait cycle was applied.The influence on stress distribution of femur-cement and prosthesis stem was analyzed.Results All patients were followed up for average 37 months,there was no infection,recurrence,loosening and limb length inequality.The bony healing time was 6 to 11 months.The mean MSTS function score was 25.7(range,25-27).The mean ISOLS graft score was 31.4 (range,28-35).The finite element analysis showed that for the short-term model,the maximum stress was 145.82 MPa in the proximal femur,40.90 MPa in the medial side of 1/4 proximal cement,and 389.24 MPa in the proximal prosthesis stem.The maximum stress was not exceeding the fatigue strength in three sites.For the long-term model,with the bone healing,the maximum stress on three sites decreased to 139.05,36.95,and 253.65 MPa,respectively.Conclusion These results suggest that the alcohol-deactivated autograft-prosthesis composite after resection ot bone giant cell tumor in distal femur can reduce the tumor recurrence and improve the short-term limb function,It is stable in short term and can reduce stress shielding in long term.
2.Radioprotective effect of calorie restriction in Hela cells and SD rats

Objective To explore the effect of low calorie metabolism on the survival of HeLa cells exposed to X-rays,and the influence of starvation on the antioxidative factors in the blood of rats after irradiation.Methods MTT method was used to evaluate the impact of different concentration glucose on the proliferation of HeLa cells.Colony formation assay was employed to detect the influence of glucose ( 1,5,10 and 25 mmol/L) on radiosensitivity of HeLa cells. Flow cytometry assay was used to analyze distribution of cell cycle and apoptosis.60 male SD rats were randomly divided into 6 groups with 10 rats each.Rats in every two groups were fed ad libitum,fasted for 24 h and fasted for 48 h,respectively.Rats in one group of each approach were respectively exposed to whole-body X-rays at 11 Gy. At 2 h after irradiation,all of rats were sacrificed and their venous blood was collected.Elisa kits were used to detect superoxide dismutase (SOD) and total antioxidant capacity (T-AOC).Results An increased viability was observed in HeLa cells treated with the glucose at low concentration ( <25 mmol/L),while HeLa cell growth was inhibited by glucose at doses of >25 mmol/L. Relevant to cells treated with 1 mmoL/L glucose,SERs (sensitive enhancement ratio) in cells exposed to 5,10 and 25 mmol/L glucose were 1.07,1.10 and 1.23,respectively. A reduction of G2/M and S arrests and apeptosis caused by 6 Gy X-ray irradiation were observed [(49.68 ±1.88)% and (35.54±1.45)% at G2/M phase,(16.88 ±1.22)% and (10.23 ±1.65)% atS phase,t=10.42,5.61,P<0.05]and in the cells treated with 1 mmol/L glucose compared with cells treated with 25 mmol/L glucose [ ( 25.50 ± 0.95 ) % and (7.56 ± 1.07 ) %,t =21.72,P <0.05 ].Without irradiation,calorie restriction exhibited a negligible influence on SOD and T-AOC in rats.However,after 11 Gy irradiation,compared with rats fed ad libitum,the levels of SOD and T-AOC were significantly increased in rats with calorie restriction ( t =40.32,42.78, P < 0.05 ).Conclusions Calorie restriction has a certain radioprotective effect in vivo and in vitro.
3.A mini-plate fixation for the treatment of children's ulna and radius distal unstable fractures: A 18-case analysis

OBJECTIVE: To observe the clinical effect of mini-plate fixation on the treatment of children unstable fractures of the distal ulna and radius. METHODS: A total of 18 children with unstable fracture of the distal ulna and radius (15 boys and 3 girls, aging 4.1-13.3 years, with the mean age of 9.8 years) were selected from Department of Orthopaedics, Wuhan Children and Women's Hospital, Tongji Medical College, Huazhong University of Science and Technology from August 2006 to September 2009. The fracture occurred on right side (n=9) and left side (n=9). All children were treated with mini-plate fixation which was instead of the traditional pinning fixation. Before operation, stainless steel or titanium plates of adult digital bone, metacarpal bone, and metatarsal bone were chosen according to fracture types and sites. The distance from fracture line to epiphyseal plate was measured to get ready the T-, L-, clover or straight plates. Joint function, degree of forearm rotation loss, bone healing, and complications were monitored postoperatively and within following up. RESULTS: All cases were followed up in out-patient for an average 26.4 months. An average 10°limitation was measured on forearm pronation function of all children, but the function was recovered by doing exercises which did not influence daily activities. Supination, flexion and extension of wrist joint were generally normal, and the fracture was generally healed. Complications including re-displacement, refracture, malunion, delayed union, bone bridge formation, vascular and nerve injury were not found. CONCLUSION: Patients who were not satisfied with the reduction, re-displacement, or old distal radius fracture needed an open reduction. Micro-plate was fixed to avoid epiphyseal injury of distal forearm and reduce postoperative complications. An ideal mini-plate may replace the traditional pinning fixation.
4.Effects of particulate montmorillonite on tissue lead levels, erythropoiesis,ALA-D activity,and lead-induced lipid peroxidation in liver of pigs

Sixty barrows (Duroc × Labdrace × Yorkshine) were randomly assigned to two groups by weight of 33 kg,each of which was replicated three times with ten pigs. Half of the pigs were fed with diets containing 10 mg/kg lead and 0.5% particulate montmorillonite,the other half pigs were fed only with diets supplemented 10 mg/kg lead as control groups for 100 days.The results showed that the addition of particulate montmorillonite to the diet significantly decreased lead concentration in tissues such as blood,brain,liver,bone,kidney and hair and enhanced the erythropoiesis as measured by increasing numbers of RBC,hemoglobin and hematocrit values ,and elevated ALA-D activity in liver. The damage of lead to the liver was evident in the increases in hepatic concentration of malondialdehyde (+ 17.08% );decreases in the antioxidant enzymes catalase(-85.73 % ),superoxide dismutase ( - 52.17% ) and glutathione peroxidase ( - 47.56% ). Concomitant use of particulate montmorillonite in the diets completely ameliorated the lead-induced oxidative damage. It indicated that particulate montmorillonite is possessed of the potential therapeutic activity against lead poison.
5.Investigation and Analysis on Degree of Job Datisfaction of Grassroots Medical Personnel and Influencing Factor from the Perspective of New Health Care Reform

Objective:To understand the present situation of the grassroots medical personnel satisfaction during the implementation of new health care reform , and compared with the survey of 2011 , to further analysis the main factors of affecting the job satisfaction .Methods:Conducted a questionnaire survey to 326 employees of basic medi-cal institutions of Weifang city , Dongying city and Yantai city in Shandong Province , and study with the method of qualitative analysis and quantitative analysis .Results:The number of satisfied with the current basic medical work only is 5.2%, and the overall satisfaction rate is 38.04%.The overall satisfaction to compensation rate is 31.3%. The overall satisfaction to the relationship between doctors and patients is 36 .19%.The overall satisfaction to the hospital management system and performance appraisal is respectively 44 .17%and 40 .79%.Conclusions:Com-pared with the investigation results in 2011 , the overall satisfaction of the grassroots medical personnel has im-proved, but the overall satisfaction is not high .It puts forward constructive countermeasures: strengthen grass -roots hospital ability and talent building;strengthen the policy stability;improve the grassroots personnel′s reasona-ble salary;improve the occupation environment;protect the doctors′interests.
6.Investigation and Analysis on Satisfaction Degree and Influence Factor in Grassroots Hospital Patients during New Health Care Reform

Objective:To understand the present situation of the grass-roots hospital patient satisfaction and influence factors after the implementation of new health care reform, and to put forward the rationalization proposals of improving degree of satisfaction. Methods:Conducted a questionnaire survey to 310 Grassroots hospital patients in Shandong province, and make data statistics and analysis. Results:After the implementation of new health care reform, satisfaction degree of medical services is relatively high in the mass, but there are also some problems. Through the investigation and analysis, the factors influencing patients satisfaction mainly include medical treatment cost, The doctor's technical level, the doctor's service attitude, diagnosis and treatment effect, The cost perform-ance of paying the medical costs and getting medical service price, The situation of meeting the work -up with medical equipment. Conclusion:From the perspective of new health care reform, the ascension of hospital patient satisfaction need to improve the level of basic hospital's medical technology, improve the medical service attitude, strengthen the management and control of medical expenses, and strengthen the government's financial investment.
8.Integrative pharmacology: new paradigm of modernization of Chinese medicine.

Chinese medicinal formulae( CMF) were often used in the clinics of traditional Chinese medicine (TCM) which were critical for modernization of Chinese medicine to shed light on the interaction between CMF and biological organisms. In current studies, correlation between system and part, macroscopic actions and microcosmic mechanism, ADME process and pharmacologic actions were often neglected. Thus, we put forward integrative pharmacology, which could integrate the correlation between CMF and biological organisms from multi-levels and multi-dimensional views. Integrative pharmacology would reveal the molecular mechanism of CMF for ailments treatment and screen out effective material systematically, which would be the new paradigm of TCM research.

9.Effect of Kechuanning on Expression of IL-5mRNA in Lung Tissue of Bronchial Asthmatic Rats

Objective To observe effect of Kechuanning on the expression of IL-5mRNA in lung tissue of bronchial asthmatic rats.Methods Forty rats were randomly divided into 5 groups:the normal group,the model group,Kechuanning high-dose group (27 g/kg),Kechuanning low-dose group (13.5 g/kg),Guilong Kechuanning control group (0.45 g/kg),8 rats in each group.The bronchial asthmatic model was established by egg protein sensibilization and inhalation provocation.The rats of each treatment group were lavage administration each day from the first time of provocation to execution.After 4 weeks of treatment,the rats were killed and lung tissue were taken to dying of HE to be observed.The expression of IL-5mRNA in lung tissue were determined by RT-PCR.Result Compared with the normal group,the thickness of bronchus wall and bronchus smooth muscle,the numbers of eosinophile granulocyte and leukomonocyte,and expression of IL-5mRNA in lung tissue were increased (P
