2.Research progress in the surgical treatment of papillary thyroid microcarcinoma

Thyroid cancer is the most common malignant tumor of the human endocrine system. A papillary thyroid microcarci-noma (PTMC) is usually defined as a papillary thyroid tumor that is less than 10 mm in diameter. The incidence of thyroid cancers has obviously increased in recent years. The surgical treatment of PTMC differs in China and overseas. Aside from conventional surgery, novel surgical methods, such as ultrasound-guided percutaneous ablation, have been used for the surgical treatment of PTMC. This arti-cle mainly discusses the research progress in the surgical treatment of PTMC.

4.The association of 5-HTTLPR with depression and its clinical symptoms

Objective To investigate the association of the polymorphism of serotonin transporter gene linked polymorphic(5-HTTLPR) region with depression and its clinical symptoms in the Han Chinese.Methods PCR-based technique was used to genotype 5-HTTLPR in 64 patients with major depression and 67 unrelated controls,and the association between the polymorphism and clinical symptoms of depression was explored according to HAMD score.Results Fewer patients tended to be of L/L genotype(9.4%) than controls(19.4%),but there was no significant difference.Among women,fewer patients tended to be of L/L genotype(2.9%) than controls(13.9%),but there was no significant difference.The symptom analysis showed that depressive symptoms were significantly associated with 5-HTTLPR genotypes or alleles in the patients of L/L genotype,and that L allele had significantly lower depressive scores compared with those of other genotypes or alleles(P
5.Effects of hypothermia combined with sevoflurane on myocardial monophasic action potential and transmural dispersion of repolarization of ventricles in rabbits

Objective To investigate the effects of hypothermia combined with sevoflurane on myocardial monophasic action potential (MAP) and transmural dispersion of repolarization (TDR) of the left ventricle in rabbits in vitro. Methods Adult rabbits weighing 1.5-2.0 kg were sacrificed after heparinized and anesthetized.The hearts were immediately removed and perfused with K-H soluation saturated with 95%O2-5%CO2 at 37℃ in a Langendorff apparatus. Forty-eight isolated hearts were randomly divided into 6 groups ( n = 8 each): Ⅰ control group (group C), Ⅱ low concentration sevoflurane group ( group S1 ), Ⅲ high concentration sevoflurane group (group S2 ), Ⅳ hypothermia group (group H), Ⅴ hypothermia + low concentration sevoflurane (group HS1 ) and Ⅵ hypothermia + high concentraion sevoflurane (group HS2 ).Group C received continous perfusion. Group S1and S2 received perfusion with K-H solution saturated with 2.4% and 4.8% sevoflurane at 37 ℃ for 30 min respectively. Group H received perfusion with K-H solution at 30℃ for 30 min. Group HS1 and HS2 received perfusion with K-H solution saturated with 2.4% and 4.8% sevoflurane at 30℃ respectively.MAPs of epicardium, mid-myocardium and endocardium of the left ventricle were recorded. MAP duration at 90%repolarization(MAPD90)and TDR were calculated. Early after-depolarization,delayed after-depolarization and arrhythmia were also recorded. Results Compared with group C, MAPD90 of the 3 layers of ventricle was significandy prolonged, the incidence of arrhythmia increased in group H (P<0.05). There was no significant difference in TDR among all groups ( P>0.05). There was no interaction between sevoflurane and hypothermia (P>0.05), and it only showed that MAPD90 was prolonged by hypothermia (P <0.05 ). Conclusion Hypothermia combined with sevoflurane exerts no significant effects on myocardial MAP and TDR of ventricles in rabbits, and sevoflurane decreases the incidence of hypothermia-induced arrhythmia through inhibiting the prolongation of MAPD90.
6.Effects of hypothermia combined with sevoflurane on expression of myocardial gap junction protein connexin 43 in isolated rabbit hearts

Objective To investigate the effects of hypothermia combined with sevoflurane on expression of myocardial gap junction protein connexin 43 (Cx43) in isolated rabbit hearts.Methods Healthy adult rabbits of both sexes,weighing 1.5-2.0 kg,were sacrificed after anesthetization.The hearts were rapidly excised and perfused in a langendorff apparatus with K-H solution saturated with 95% O2-5% CO2 at 37 ℃ in a langendorff apparatus.Forty isolated hearts were randomly divided into 4 groups (n =10 each):control group (group C),hypothermia group (group H) and hypothermia combined with sevoflurane group (group HS).At 15 min of equilibration,the perfusion with K-H solution was continued at 37 ℃ in group C,K-H solution saturated with 2.4% sevoflurane was perfused at 37 ℃ in group S,K-H solution was perfused at 30 ℃ in group H,and K-H solution saturated with 2.4% sevoflurane was perfused at 30 ℃ in group HS.At 30 min of perfusion,myocardial specimens were obtained from the anterior wall of the left ventricle for detection of the expression of Cx43 in myocardial cells (by immunohistochemistry and Western blot).Results Compared with group C,Cx43 expression was downregualted in group H (P < 0.05),and no significant change was found in Cx43 expression in S and HS groups (P > 0.05).Cx43 levels distributed mainly in the intercalated disc in S and HS groups,and in H group the distribution of Cx43 levels in the intercalated disc was less,but the percentages of lateralized Cx43 were increased.Conclusion Sevofurane can inhibit hypothermia-induced down-regulation and distribution disturbance of myocardial Cx43 expression,which may be the mechanism by which sevofurane inhibits hypothermia-induced arrhythmia.
7.Studies and prospects of the candidate virulence gene of familial medullary thyroid carcinoma

Familial non-medullary thyroid carcinoma (FNMTC) is de fi ned as the presence of two or more affected fi rst-degree relatives with non-medullary thyroid cancers without other known familial syndromes. FNMTC is one of the most inheritable forms of all cancers, with a high risk of a first-degree relative developing the disease. Compared with sporadic non-medullary thyroid carcinoma (NMTC), FNMTC presents at a younger age and is associated with a higher incidence of multifocal disease and metastasis. This in-creased aggressiveness has been hypothesized to translate into higher recurrence rates and decreased survival of patients with FNMTC. The genes involved in the pathogenesis of FNMTC are yet to be elucidated, although some recent studies identified several predisposi-tion loci with a high degree of genetic heterogeneity. Since 2005, next-generation sequencing (NGS) technologies have been developing as rapid, high-throughput, and cost-effective approaches to fulfill medical sciences and research demands. With the use of NGS, the un-derlying causative genes can be directly distinguished via systematic filtering, through which the identified gene variants are verified for novelty and functionality.
8.Influence of nursing intervention on patients with diabetes mellitus receiving intensive treatment of in-sunn pump

Objective To discuss the influence of nursing intervention on psychological state and glu-cose control of patients with diabetes mellitns before and after intensive treatment of insulin pump. Methods 120 patients who were going to receive intensive treatment of insulin pump were divided into the control group and the observation group with 60 patients in each group by simple random method.The two groups both re-ceivecl routine nursing. The observation group was given psychological intervention along with health education by application of progressive dominant content education method. The psychological state and glucose control of the two groups were compared by χ2 test and t test. Results The score of health knowledge and satisfaction degree with health education in the observation group were higher than those of the control group.The score of anxiety and depression in the observation group decreased more significantly and satisfaction rate of glucose control was higher than that of the control group after 14 days of insulin pump treatment. Conclusions Health education to patients with intensive treatment of insulin pump by application of progressive dominant content education method can make patients to master health knowledge easily.Psychological intervention can alleviate the negative emotion of these patients and facilitate better effect of insulin pump treatment.

10.Application of urodynamic study in 452 patients with benign prostatic hyperplasia

Objective To analyze the urodynamic data of the patients with benign prostatic hyperplasia(BPH)and provide evidence for the therapy.Methods 452 patients with BPH were selected.The mean age was(65.6 ±5.65)years(51 to 89 years).All patients had intact history documents which included the results of cystoscopy,urodynamics,doppler,DRE and PSA.Results 15 cases were not diagnosed with BOO.430 cases were diagnosed with BOO,among them,65 cases of bladder detrusor muscle weakness,bladder detrusor instability(detrusor,DI,instability)in 365 cases,3 cases were normal,and the other 4 cases had detrusor atony.In 445 patients the mean maximum urinary flow rate(8.32±3.15)ml/s,the average residual urine volume(87.68±79.46)ml,the average maximum flow rate when the detrusor pressure(62.32±7.54)cm H2O,maximum urethral closure pressure was(86.43±18.35)cmH2O,among which 19 cases were not suitable for surgery.At the end of the 426 patients underwent surgical treatment,postoperative follow-up of 3 to 18 months,the International Prostate Symptom Score(IPSS)from 0 to 5,the quality of life index(QOL)score of 0 to 1,Qmax 11 to 21 ml /s,residual urine(45.6±36.2)ml.There were significant differences between the preoperative and postoperative indexes(P<0.05).Conclusion Urodynamic tests can avoid the blindness in the surgery treatment of BPH.
