ObjectiveTo evaluate the adequacy of hemodialysis by online clearance monitoring (OCM) and Kt/V of correlation analysis.MethodsThe Kt/V values of 48 maintenance hemodialysis patients in the Department of Nephrology of General Hospital of PLA were accessed by both OCM of Germany BeiLang Dialog + hemodialysis machine and single-pool urea kinetic model. The Kt/V calculation value was also checked by the urea dynamics - room variable volume model. The results generated at different time point were compared, including at the real time, before and after appearing through blood by On - line Clearance Monitoring, and correlation analysis of Kt/V value was performed.ResultsThere was no significant difference on evaluating the level of Kt/v by these two methods. The relationship between the results of the two methods was significantly positive related.(r=0.539,P < 0.01).ConclusionsDuring hemodialysis, the OCM option provides an accurate tool for continuous on-line monitoring of urea clearance.

Objective To analyze the amount and allocation of pediatric healthcare resources in Chengdu, and to recommend on local pediatric healthcare resources shortages. Methods Pediatric healthcare resources data of Chengdu came from pediatric relevant data reported regularly at yearend by the counties and districts to healthcare administration of the city.Data from such reports were subject to statistics with various indexes and descriptive analysis.Analyzed in focus was the distribution in 2015 of pediatric healthcare resources of the city among medical institutions of various types, levels and properties,as well as causes for such shortage. Results Tertiary hospitals hold 62.3% of the pediatric beds and 64.2% of pediatricians, and provide around 70% of the medical workload for pediatric outpatients and inpatients, upon the majority of pediatricians with master degree and above, and senior ones of/above associate chief physician titles;Tertiary hospitals have 58.7% of the pediatric beds and 49.7% of pediatricians, yet the outpatients served by specialized hospitals were 5% above tertiary hospitals. Conclusions The imbalance and shortage in total of pediatric resources in Chengdu result from stable source of manpower supply, high professional risk exposure, low income, and long training duration among others.Such measures as a better pediatrician development system, greater incentives for pediatric development, and enhanced development pediatric service consortiums,as well as greater support for private specialized pediatric hospitals. Those measures combined can effectively alleviate the shortage of pediatric resources.

Objective To study interleukin-1 receptor(IL-1R) and connexin(Cx36) gene expression following Mg 2+-free-induced seizures in cultured developing neuron. Methods Rat embryo cortical neurons cultured for 6 and 17 days were exposed to Mg 2+-free media to induce seizure. At different time after Mg 2+-free treatment, real-time RT-PCR was used to detect IL-1R and Cx36 mRNA expression. Results 1. IL-1R mRNA expression transiently decreased after Mg 2+-free treatment in neurons cultured for 6 and 17 days in vitro. Then the levels of IL-1R mRNA expression recovered in neurons cultured for 6 days, but IL-1R mRNA expression were increased in neurons cultured for 17 days compared with control group and the peak was at 24 hours. 2. In neurons cultured for 6 days in vitro, Cx36 mRNA expression increased after Mg 2+-free treatment compared with control group, the peak was at 24 hours. But in neurons cultured for 17 days in vitro, Cx36 mRNA expression decreased at 6 hours after Mg 2+-free treatment compared with control group, the peak was at 24 hours. Conclusions IL-1R mRNA and Cx36 mRNA expression following Mg 2+-free-induced seizures are different between the neurons cultured for 6 and 17 days in vitro. This is possibly related to the different neuron injury between 6 and 17 days in vitro following seizures.

Objective In order to provide clinical evidence of reducing postoperative complications of procedure for prolapse and hemorrhoids and observe the effects of constipation on postoperative complications of procedure for prolapse and hemorrhoids. Methods One hundred and sixty cases of hemorrhoid were divided into two groups according to whether suffering from constipation:there were eihgty cases who were suffered from constipation in the experimental group and eighty cases who were not suffered from constipation in the control group. All patients' postoperative bleeding, anal edema, stool impaction and satisfaction were observed within 30 days and these data were compared and analysed by using SPSS 13.0 software. Results In the experimental group, the postoperative bleeding incidence was 37.5 % (30/80),the anal edema incidence was 31.3 % (25/80),the stool impaction inci dence was 8.6% (7/80),and the patients' satisfaction rate was 88.6% (71/80).In the control group, the postoperative bleeding incidence was 18.6% (15/80),the anal edema's incidence was 15.0% (12/80),the stool impaction incidence was 1.3% (1/80),and the patients' satisfaction rate was 97.5% (78/80).All the indexes above were different statistically in the two groups (P < 0.05).Conclusion constipation could increase complications and reduce the degree of satisfaction after procedure for prolapse and hemorrhoids.

Objective To observe the changes of serum potassium concentration in rectal cancer patients from the admission to before the operation,to provide clinical evidence for preventing serum potassium disorder during and after their operation. Methods Monitoring of the changes of serum potassium concentration of 40 cases of rectal cancer patients according to three time points including the first day of admission, before taking of cathartics and the morning before operation. They were divided into different groups according to their age(young, middle age and elderly group),gender(male and female group),diet(normal, decreased slightly and serious decline group) and course(long, middle and short course group) of disease. The serum potassium concentration was observed between each time point of each group and SPSS13.0 statistical software was used to analyze the changes above. Results The mean of all patients' serum potassium concentration was (4.09 ± 0.62) mmol/L on the first day of admission, was (3.83 ± 0.46) mmoL/L before taking of cathartics that was decreased compared to the former and there was significant difference (P < 0.01),it was (3.36 ±0.40) mmol/L on the morning of operation and had significant difference with the above two groups (P < 0.01).It hadn't significant difference between different gender and age groups at different time points (P > 0.05) except between the young group and the elderly group in the morning of the operation (P < 0.05).There were statistically significant differences between the different diet condition and different course at anytime point (P < 0.01).Conclusions The rectal cancer patients were possiblely in hypokalemia before operation, the causes might be relative to the long course of disease, the decreased diet, the aged and infirm, the diet structure change after admission and taking cathartics cleaning intestinal tract.

Objective To evaluate the value of CT perfusion imaging (CTP)for solitary pulmonary nodule (SPN)by Meta-analysis.Methods Literatures about SPN diagnosed by CTP were filtered.PubMed,EMBASE,CNKI,VIP and WANFANG databases were searched for the relevant articles. The retrieved studies were screened according to the criteria for diagnostic research published by the cochrane methods group on screening and diagnosis. The quality of the articles was accessed and the basic data in the articles was extracted.Review Manager 5.3 software was used to compare the blood volume (BV)among different nodules,to perform heterogeneity test and analyze publication bias. Results A total of 1 7 studies with 877 lesions were included in the study. The random effect model was used for the existence of heterogeneity. The result showed the BV value of malignant SPN was higher than that of benign ones. Conclusion The result indicates that there exists significant difference in BV between malignant and benign SPN.The BV value,as one of the hemodynamic parameters of CTP,can be used as the diagnostic basis of SPN.
