4.Hemodynamic analysis of subclavian steal syndrome in patients undergoing transcranial doppler examination

Objective To improve the theoretical basis for the typical hemodynamic changes in subclavian steal syndrome (SSS) patients underwent transcranial doppler (TCD) by detailed hemodynamic analysis.Methods Patients with SSS were examined by transcranial doppler and some patients performed tourniquet test.Flow spectrum,flow velocity and pulsatility index (PI) of vertebral artery,basilar artery and middle cerebral artery (MCA) were recorded.Results The flow spectrum of steal side vertebral artery showed that there were 14 cases with systolic notch,18 cases with systolic reverse flow and 3 cases with flow completely reversed.The flow spectrum showed high resistance spectrum in vertebral artery and basilar artery and ipsilateral basilar artery vortex or turbulence in 32 cases.The PI value was higher in contralateral vertebral artery and basilar artery than in anterior circulation (q =9.56,9.98,both P < 0.01),and there was no difference between contralateral vertebral artery and basilar artery (q=0.416,P>0.05).The tourniquet test were positive in all 22 patients.Conclusions TCD is a convenient,fast,accurate and non-invasive method for hemodynamic examination,which is the first choice for the diagnosis of SSS.
5.Locomotor model based on accumulate peripheral active time percent in ICR mice

Aim To establish a novel locomotor model based on accumulate peripheral active time percent(APATP)in ICR mice induced by pentobarbital sodium(PB)and diazepam.Methods Total distance, accumulate peripheral active time and accumulate peripheral time were aquired from video tracking system with computer, and APATP in all groups was fit and got correspondent parameters with Drugs Analysis System 2.0. Logarithm of area under the curve(lgAUC_(0-60)), minimum APATP(lgP_(min)), and half time of steady-state locomotor activity(T_(1/2α)) were ready for evaluation.Results APATP decreased with time increasing in all groups gradually, and PB 10 mg·kg~(-1) and PB 15 mg·kg~(-1) had similar tendency.PB 10 mg·kg~(-1) and PB 15 mg·kg~(-1) all decreased APATP significantly throughout the time course.APATP in all mice was fit to kinetics equation.Compared with norm group, PB 5 mg·kg~(-1) decreased three parameters slightly(all P >0.05), and PB 10 mg·kg~(-1) and PB 15 mg·kg~(-1) had similar tendency.PB 10 mg·kg~(-1) (all P <0.05) and PB 15 mg·kg~(-1) (P <0.01, P <0.01 and P >0.05)decreased parameters.Compared with PB 10 mg·kg~(-1) , PB 15 mg·kg~(-1) increased reversely(P >0.05, P <0.05 and P <0.05).lgAUC_(0-60) and lgPmin were both linar with total distance(r 2=1.0000 and r 2=0.9995, both P <0.01), T_(1/2α) also showed similar tendcency as well as total distance.Refering PB 10 mg·kg~(-1) as positive drugs and norm as negative control, diazepam 2 mg·kg~(-1) and 4 mg·kg~(-1) depressed all parameters significantly compared with norm group(all P <0.01) and were similar as PB 10 mg·kg~(-1) , which indicated that sedative effect of diazepam was the same as PB 10 mg·kg~(-1) .Conclusion Locomotor activity model based on APATP may be used to evaluate drug effects on lomocotor activity induced by sedative hypnotics.

9.Relation Analysis of Synovial Anti-Citrullinated Epitope Peptide Expression and Peptidyl Arginine Deiminase 4 Gene in the Rheumatoid Arthritis Patient

Objective To investigate the relation of synovial anti-citrullinated epitope peptide(CCP)expression and peptidyl arginine deiminase 4 (PADI4)gene in the rheumatoid arthritis patient.Methods From February 2013 to 2015 in Dongfeng General Hospital Affiliated to Hubei University,selected 110 patients with rheumatoid arthritis as the observation group, and selected 110 healthy people at the same period in this hospital for medical examination as the control group,both groups were given the synovial CCP expression positive rate and PADI4 gene expression detecting and correlation analysis.Results The synovial anti-CCP expression positive rates in the observation group and the control group were 70.9% and 9.1% re-spectively,and the observation group were significantly higher than the control group (P<0.05).The PADI4-104 genotype expression frequency compared between the observation group and control group,the difference were statistically significant (P<0.05).The G/G expression was more in the observation group,while the control group was more wit the C/G expres-sion.Pearson correlation analysis showed that in the observation group,synovial CCP positive expression were significant correlated to the expression of genotype PADI4-104 (r=0.344,P<0.05).Logistic regression analysis showed PADI4-104 genotype frequency were the main factors for the synovial CCP epitope expression (P<0.05).Conclusion The rheumatoid arthritis was more with synovial anti-CCP positive expression.There were also PADI4 polymorphism disorder expression, and clear correlation between the two.Thus impact the incidence of rheumatoid arthritis.
10.Learning Curve of Minimally Invasive Direct Coronary Artery Bypass via Left Anterior Small Thoracotomy

Objective To investigate the learning curve of minimally invasive direct coronary artery bypass ( MIDCAB) via left anterior small thoracotomy. Methods We retrospectively reviewed results of a consecutive series of 80 patients underwent MIDCAB via left anterior small thoracotomy performed by a single surgeon.The patients were divided into 4 groups ( group A, B, C, D, n=20 in each group) according to the sequence of the operation.The left internal mammary artery ( LIMA) harvesting time, anastomosis time, total operative time, the drainage volume of the first day postoperation, mechanical ventilation time, hospital stays postoperation, in-hospital morbidity and mortality were compared.The learning curve was assessed by means of regression analysis with logarithmic curve fitting. Results The basic clinical characters were similar in all groups before the operation.The operations were accomplished successfully in all the 80 cases.No intraoperative conversion to sternotomy, death, or perioperative myocardial infarction happened.The LIMA-harvesting time in the group B, C, and D was significantly shorter than that in the group A [(53.7 ±19.2) min, (50.2 ±17.7) min, and (43.2 ±10.3) min vs.(77.0 ±30.0) min, P =0.001, 0.000, and 0.000].There was no difference between group B and C, B and D, C and D in the LIMA-harvesting time (P>0.05).The total operative time in the group B, C, and D was significantly shorter than in the group A [ (128.7 ±21.7) min, (129.0 ±33.3) min, and (112.2 ±14.5) min vs. (165.2 ±41.8) min, all P=0.000], without difference between the group B and C, B and D, C and D (P>0.05).The learning curve models were obtained: LIMA-harvesting time ( min) =113.77 -16.869 × ln ( operation number); total operative time (min)=220.281 -25.276 ×ln (operation number). Conclusion MIDCAB via left anterior small thoracotomy is safe and effective, with a learning curve of approximately 20 cases.
