2.Predictive Validity of the Braden Scale for Pressure Ulcer Risk: A Meta-analysis.

PURPOSE: The Braden Scale is one of the most intensively studied risk assessment scales used in identifying the risk of developing pressure sore. However, not all studies show that the predictive validity of this scale is sufficient. The purpose of this study was to evaluate the Braden Scale for predicting pressure ulcer development. METHODS: Articles published 1946 and 2013 from periodicals indexed in Ovid Medline, Embase, CINAHL, KoreaMed, NDSL and other databases were selected, using the following keywords: 'pressure ulcer'. The QUADAS-II was applied to assess the internal validity of the diagnostic studies. Selected studies were analyzed using meta-analysis with MetaDisc 1.4. RESULTS: Thirty-eight diagnostic studies with high methodological quality, involving 17,934 patients, were included. Results of the meta-analysis showed that the pooled sensitivity and specificity of the Braden Scale were 0.74 (95% CI: 0.72-0.76), 0.75 (95% CI: 0.74-0.76) respectively. However the predictive validity of the Braden Scale has limitation because there was high heterogeneity between studies. CONCLUSION: The Braden Scale's predictive validity of risk for pressure ulcer is interpreted as at a moderate level. However there is a limitation to the interpretation of the results, because of high heterogeneity among the studies.

5.Enzooticity of the dogs, the reservoir host of Thelazia callipaeda, in Korea.

The reservoir hosts of Thelazia callipaeda were examined. The eyes of the 76 dogs raised at farm, 78 military dogs (shepherds), 96 cattle, and 105 pigs were investigated for the presence of eyeworm. Among them, six worms of T. callipaeda were collected from two dogs raised at farm (2.7%), and 188 worms from 26 shepherds (33.5%). No worms were recovered from the cattle or pigs. These results suggest that the dogs, especially the military dogs are serving as a reservoir host of T. callipaeda. in Korea.

6.A comparison study of the effects of hypertrophied adenoid tissue on jaws morphology.

To investigate the association between the hypertrophy of adenoid and jaw morphology during growth, this paper was based on children patients with experimental adenoids (male-15 subjects at each bone age group, female-15 subjects at each bone group) and comparing them to data taken from a control group (male-15 subjects at each bone age group, female-15 subjects at each bone group) with normal respiratory function. The comparisons between the groups were done at each growth stage using cervical vertebrae maturation index(CVMI) of Hassel. The obtained results were as follows: 1. The differences in craniofacial morphology between experimental group and control group were appeared from CVMI 3 and CVMI 4 (aroud adolescent period) in males, and from CVMI 1 in females. 2. The mandibular position of experimental group was more inferior than control group. The difference appeared at adolescent period(male: at CVMI 4, female: at CVMI 5). 3. Experimental group had greater anterior facial height than control group. This difference seemed a relation with lower anterior facial height. The difference appears at CVMI 3(11.94+/-1.38 years old, at adolescent period) in male and at CVMI 1 in female. 4. The adenoid size of control male group was increased until CVMI 2(10.58+/-1.07 years old, just before adolescent growth peak) and then decreased, but in female the adenoid size was decreased from CVMI 1(6.92+/-0.53 years old).

8.Regulatory Effects of Nitric Oxide (NO) on Pancreatic Exocrine Secretion.

Recently nitric oxide (NO) is known as a bioactive molecule modulating secretory activity in various glandular tissues. Previously we have localized bNOS, a neuronal isoform of nitric oxide synthase, in the pancreatic tissue, particularly in the pancreatic islet of Langerhans and in the neurons of intrapancreatic ganglia. It implies that NO may play the important roles in regulation of pancreatic secretion by transmitting the neuronal signals from autonomic nervous system to endocrine and/or exocrine system of pancreas. We also revealed that NO is involved in regulation of insulin secretion and its synthesis. The present study was designed to elucidate the regulatory effect of NO on the pancreatic exocrine secretion by way of insulo-acinar axis. For the experiment, we observed modification of amylase secretion in the rats treated with N(G)-nitro-L-arginine-methyl ester (NAME), a potent NOS inhibitor. In addition, we observed the expression of clusterin which is known to be a protein associated with cell viability in order to assess the cytotoxic effect of NO. The present study showed that the intra-pancreatic NO is involved in regulation of amylase secretion of pancreatic acinar cells. Amylase immunoreactivity was significantly decreased at 60 and 90 min after NAME injection, although little change was seen during 30 min after treatment. However, the amylase immunoreaction was recovered toward the normal range at 120 min after NAME treatment. In electron-immunolabeling experiment, we observed the secretory granules with higher electron density, but less immunolabeling for amylase at 60~90 min after NAME treatment, while they restored normal feature and labeling density at 120 min. Clusterin expression increased along with the time course of experiment and demonstrated a highest level at 120 min after NAME injection. Taken together, the above results indicate that lowered level of NO induced by NAME treatment reduces amylase secretion of acinar tissue. It implies that increased level of NO in physiological range may stimulate pancreatic exocrine secretion.

9.Plantar Fibromatosis: A Case Report.

Plantar fibromatosis, otherwise known as Dupuytrens disease of the foot, is a relatively uncommon benign lesion. It is characterized by the replacement of elements of the plantar aponeurosis with fibrous tissue, which then slowly invades the skin and the deep structures. Generally an asymptomatic lesion, plantarfibrosis may occasionally cause foot pain. We recently experienced a case of painful plantar fibromatosis. On which a wide surgical excision was done. A one year postoperatively, follow-up examination revealed neither recurrence nor walking disability.

10.Treatment of Fracture of the Tibial Intercondylar Eminence with Arthroscopic Pull-Out Suture.

The purpose of this study is to demonstrate the effectiveness of arthrocopic treatment for the avulsion fracture of intercondylar eminence of the tibia and of the technique of arthroscopically assisted reduction and fixation through pull-out suture method. Between January 1995 and May 1997, three patients were underwent arthroscopic reduction and suture fixation for type 0 of avulsion fracture of intercondylar eminence of tibia and were followed up. The result of two patients were graded as normal and one as nearly normal by the criteria of the International Knee Documentation Committee(IKDC) rating scale. The advantages of this technique include no retained hardware and ability to treat comminuted fracture(type g ). We describe technique of arthroscopic reduction and suture fixasion for displaced avulsion fractures, including those with comminution of intercondylar eminence of tibia.
