2.A Case of Primary Mature Teratoma of the Rectum.

A mature teratoma is a tumor composed of normal derivatives of all three germ layers, and usually occurs in ovaries, testes, or mediastinum. Mature teratoma of the gastrointestinal tract occurs less frequently, and case reports of primary mature teratoma of the rectum have not been published much. Here, we report a 65-year-old woman patient presented with lower abdominal discomfort. Colonoscopy revealed a pedunculated polypoid tumor arising from the rectum with hairs on its surface, and endoscopic ultrasound revealed an exophytic pattern bulging from the serosa. The tumor was removed surgically and confirmed histologically as a benign, primary mature teratoma of the rectum.

3.A Comparative Study of Local versus General Anesthesia for Chronic Subdural Hematoma in Elderly Patients Over 60 Years.

OBJECTIVE: The purpose of this study is to compare the surgical and anesthetic complications of the local and general anesthesia in chronic subdural hematoma (CSDH) patients. METHODS: We retrospectively analyzed the medical record and brain CT of CSDH patients over 60-years-old, who were treated surgically in our institution between January 2005 and December 2012. RESULTS: One hundred six patients with CSDH were enrolled for this study and 61 patients had a burr hole craniostomy under the local anesthesia. In local anesthesia group, surgical complication was not increased than general anesthesia group. But in general anesthesia group, heart disease such as arrhythmia, acute myocardial infarction was relatively increased than local anesthesia group (p=0.04). And the hospitalization period of local anesthesia group was shorter than that of general anesthesia group (p=0.001). CONCLUSION: In this present study, there was no significant difference of surgical complications between the local and general anesthesia group. But the general anesthesia group had more cardiovascular complications and longer hospitalization periods. In conclusion, when we planned the burr hole craniostomy for the elderly patients with CSDH, local anesthesia should be considered more actively for postoperative prognosis.

4.The effect of thickness and deflection of orthodontic thermoplastic materials on its mechanical properties.

OBJECTIVE: The purposes of this study were to evaluate the force and stress depending on the type, deflection and thickness of the materials and to evaluate the mechanical properties of thermoplastic materials after repeated loading. METHODS: Four types of thermoplastic products were tested. Force until the deflections of 2.0 mm and the stress when the materials were restoring to its resting position were evaluated. The mechanical properties of thermoplastic materials evaluated after 5 repeated loading cycles. RESULTS: The interaction was observed between the thickness and the deflection (p < 0.05) from the regression equation. Thickness and amount of deflection rather than products and materials showed the largest effect on force and stress. In all products, at least 159 gf of force was required for more than 1.0 mm deflection or when materials with 1.0 mm thickness were deflected. The stress recorded was more than 19 gf/mm2. During repeated loading, each group showed significant difference on the force and the stress p < 0.01), 10 - 17% reduction of force and 4 - 7% reduction of stress in average. CONCLUSIONS: Proper thickness of thermoplastic materials and deflection level of tooth movement should be decided for the physiologic tooth movement. Force decay after repeated loading should be considered for the efficient tooth movement.

5.Isolates of Bacteria and Their Sensitivity to Antibiotics in Fungal Maxillary Sinusitis.

BACKGROUND AND OBJECTIVES: In patients with fungal sinusitis, purulent discharge from the maxillary sinus is frequently observed and it responds to antibiotic treatment. Even though fungal sinusitis is resolved after surgery, the purulent discharge continues in some cases. We aimed to identify the pathogens of the purulent discharge in fungal sinusitis and to obtain information for appropriate antibiotics through a sensitivity test. MATERIALS AND METHOD: Among 71 patients with fungal sinusitis of the maxillary sinus, purulent secretions were found in 44 patients (62%) during an endoscopic sinus surgery. Endoscope-guided collection of the maxillary sinus secretions were performed and specimens were sent to the laboratory for bacterial cultures and antibiotic sensitivity tests. RESULTS: Among 44 cases with fungal lesions and purulent secretion in the maxillary sinus, bacteria were isolated in 27 cases (61.4%). Gram (+) aerobes were recovered in 19 cases, and Gram (-) aerobes were recovered in 16 cases. Mixed infection by Gram (+) and Gram (-) bacteria were observed in 8 patients. Anaerobic bacteria were not isolated. Among the Gram (+) aerobes, S. aureus was the most common organism (8 cases), while Enterobacter aerogenes was the most common organism (7 cases) of the Gram (-) aerobes. In an antibiotic sensitivity test for Gram (+) bacteria, ciprofloxacin, vancomycin, and teicoplanin showed sensitivity over 85 %. For Gram (-) bacteria, ceftazidime, ciprofloxacin, ticarcillin, tobramycin, and imipenem showed sensitivity over 90%. CONCLUSION: In patients with fungal sinusitis, concomitant bacterial infections are identified in more than half of the cases. Thus, postoperative administration of appropriate antibiotics can be justified in the treatment of fungal sinusitis.

6.Effects of Physical Activity and Other Factors on Abdominal Obesity in Korean Middle-Aged Women: The 7th Korean National Health and Nutrition Examination Survey 2016–2017

Background:
Previous studies reported that waist circumference can be a more important risk factor of chronic diseases than body mass index.Abdominal obesity is known to be associated with physical activity and lifestyle. Recent studies have found a high prevalence of abdominal obesity and low physical activity levels in Korean women. The purpose of this study was to investigate the effects of physical activity and lifestyle on abdominal obesity in Korean middle-aged women.
Methods:
We collected data from the 7th Korea National Health and Nutrition Examination Surveys in 2016 and 2017, and 2,274 women aged 45–65 years were evaluated. Logistic regression analysis was used to examine the association between abdominal obesity and many factors after adjusting for confounding variables with significant correlations.
Results:
The results showed that the mean waist circumference was 80.20±9.05 cm and the prevalence of abdominal obesity was 28.8% in the population. The low physical activity group had the largest population (48.5%). Abdominal obesity was associated with moderate and high physical activity levels (odds ratio [OR], 0.791; 95% confidence interval [CI], 0.651–0.962; P=0.019), sedentary time (≥5 h; OR, 1.495; 95% CI, 1.173–1.906;P=0.001), and weekend sleep duration (<7 h/d; OR, 1.406; 95% CI, 1.127; P=0.003).
Conclusion
In the Korean middle-aged women, a statistically significant correlation was found between physical activity and abdominal obesity.
7.An Immunohistochemical Study of ErbB4 Receptor in Alzheimer's Disease Hippocampus.

Neuregulin-1 (NRG1) signaling participates in numerous neurodevelopmental processes. Although ErbB4, a key NRG1 receptor, is expressed in multiple regions in the adult animal brain, little is known about its expression in aged human brain. We show that ErbB4 immunoreactivity was shown regional difference in the hippocampus of age-matched control and that the distribution of these molecules was altered in Alzheimer's disease (AD) brains. Immunohistochemical characterization of the distribution of ErbB4 receptor in the hippocampus relative to pathology staging were performed in age-matched control (Braak stage I/II, n=5), early AD (Braak stage III/IV, n=5) and advanced AD(Braak stage V/VI, n=10). The intensity of ErbB4 immunoreactivity was higher in neurons of the CA2 than that in CA1 or CA3 in the age-matched control. Particularly, in the early AD, ErbB4 immunoreactivity was significantly increased in the apoptotic cells of the CA2 field. In the advanced AD, ErbB4 immunostaining was more intense in the apoptotic cell of the CA2 field. In the dentate gyrus (DG), ErbB4-positive granular cell density was gradually increased in proportion to the progression of pathology of AD brains. We have also found that ErbB4 immunostaining was increased in the nucleus, suggesting that the presenilin-dependent cleavage of ErbB4 generates the soluble ErbB4 ICD (intracellular domain) that translocalized to the nucleus. Together, these results provide the immunohistochemical analysis of ErbB4 receptor in the human hippocampus staged by the progression of pathology of AD.

8.Pilot study for the Psychometric Validation of the Sheffield Profile for Assessment and Referral to Care (SPARC) in Korean Cancer Patients

Purpose:
This study aimed to validate the Sheffield Profile for Assessment and Referral to Care (SPARC) as an effective tool for screening palliative care needs among Korean cancer patients.
Materials and Methods:
The English version of the SPARC was translated by four Korean oncologists and reconciled by a Korean language specialist and a medical oncologist fluent in English. After the first version of the Korean SPARC (K-SPARC) was developed, back-translation into English was performed by a professional translator and bilingual oncologist. The back-translated version was reviewed by the original author (S.H.A.), and modifications were made (ver. 2). The second version of the K-SPARC was tested against other questionnaires, including the Functional Assessment of Cancer Therapy-General (FACT-G) and the Edmonton Symptom Assessment System (ESAS).
Results:
Thirty patients were enrolled in the pilot trial. Fifteen were male, and the median age was 64.5 years. Six patients had an Eastern Cooperative Oncology Group performance status of 2 or more. All patients except one were receiving chemotherapy. Regarding internal consistency, the Cronbach’s α scores for physical symptoms, psychological issues, religious and spiritual issues, independency and activity, family and social issues, and treatment issues were 0.812, 0.804, 0.589, 0.843, 0.754, and 0.822, respectively. The correlation coefficients between the SPARC and FACT-G were 0.479 (p=0.007) for the physical domain and –0.130 (p=0.493) for the social domain.
Conclusion
This pilot study indicates that the K-SPARC could be a reliable tool to screen for palliative care needs among Korean cancer patients. A further study to validate our findings is ongoing.
9.The comparison of the relaxant effects of propofol, thiopental, ketamine, and etomidate on isolated rat uterine smooth muscle.

BACKGROUND: Intravenous hypnotics are used in pregnancy, labor and delivery. The aim of the present study was to investigate and compare the relaxant effects of propofol, thiopental, ketamine, and etomidate on isolated rat uterine smooth muscles. METHODS: Uterine smooth muscle preparations were obtained from non-pregnant female rats. The uterus of the rat was dissected and cut into 10 mm strips. The muscle strips were bathed in Krebs solution. After spontaneous uterine contractile activity had been accomplished, propofol, ketamine, thiopental, and etomidate in various concentrations were added cumulatively to the baths and resting tension, active tension, and frequency of contration were recorded at each concentration of agents. EC(5), EC(25), EC(50), EC(75), and EC(95) of each drug on active tension and frequency of contraction were calculated using a probit model. RESULTS: Propofol, thiopental, and etomidate reduced uterine contractions in a concentration-dependent manner. Ketamine concentrations of 10(-7) to 10(-5) M augmented uterine contractions but ketamine concentrations of 10(-4) to 10(-3) M attenuated uterine contractions. The EC(50)'s of propofol, thiopental, ketamine, and etomidate on active tension were 1.56 x 10(-5) M, 4.97 x 10(-5) M, 3.52 x 10(-4) M, and 2.73 x 10(-5) M, respectively. CONCLUSIONS: All four intravenous hypnotics relaxed the uterine smooth muscle of rats except for ketamine in low concentrations (10(-7) to 10(-5) M). Propofol had the greatest relaxant effects on isolated rat uterine smooth muscle among these hypnotics. It seems that ketamine is a suitable obstetric hypnotic agent for hypovolemic parturients and propofol is a useful hypnotic agent for uterine relaxation during pregnancy.

10.Schwannoma of the Orbit.

BACKGROUND: A schwannoma is a benign, slow-growing peripheral nerve sheath tumor that originates from Schwann cells. Orbital schwannomas are rare, accounting for only 1% of all orbital neoplasms. In this study, we retrospectively review orbital schwannomas and characterize clinical, radiologic, and histologic features of this rare entity. METHODS: A retrospective review was performed to identify patients with histologically confirmed orbital schwannoma, among a list of 437 patients who had visited our hospital with soft tissue masses within the orbit as the primary presentation between 2010 and 2014. Patient charts and medical records were reviewed for demographic information, relevant medical and family history, physical examination findings relating to ocular and extraocular sensorimotor function, operative details, postoperative complications, pathologic report, and recurrence. RESULTS: Five patients (5/437, 1.1%) were identified as having histologically confirmed orbital schwannoma and underwent complete excision. Both computed tomography (CT) and magnetic resonance imaging (MRI) studies were not consistent in predicting histologic diagnosis. There were no complications, and none of the patients experienced significant scar formation. In two cases, patients exhibited a mild postoperative numbness of the forehead, but the patients demonstrated full recovery of sensation within 3 months after the operation. None of the five patients have experienced recurrence. CONCLUSION: Orbital schwannomas are relatively rare tumors. Preoperative diagnosis is difficult because of its variable presentation and location. Appropriate early assessment of orbital tumors by CT or MRI and prompt management is warranted to prevent the development of severe complications. Therefore, orbital schwannomas should be considered in the differential diagnosis of slow-growing orbital masses.
