2.Advances of vascular surgery in China.

This article reviewed the diagnosis and treatment of the main vascular diseases with surgical approach, especially the advance and challenge of endovascular therapy. Highlight of the basic studies on vascular surgery was briefly introduced. Also, the author proposes his recommendation on vascular surgeons training as well as the design and performance of multi-center prospective studies.

3.Relationship between executive function and depressive symptoms during the tower of London test in patients with major depressive disorder

Objective To investigate the relationship between executive function and depressive symptoms in patients with major depressive disorder( MDD) under the Tower of London Test ( TOL) . Meth-ods Thirty depression patients and thirty age?, gender?, education?matched normal controls participated in the study. All subjects received the Tower of London Test. The severity of depressive and anxiety symptoms were assessed by Hamilton Rating Scale for Depression ( HAMD) ,Hamilton Anxiety Rating Scale( HAMA) and Beck Depression Rating Scale( BDI) . Results ( 1) The numbers of correct response and total response of the TOL in MDD(9.1±5.1,12.1±5.3) were significantly lower than those of the control group(13.8±5.0, 17.3±3.9)(P<0.05).The response time of the TOL test in patients with MDD((10.4±2.8)s) was signifi?cantly longer than that of the control group((8.5±2.2)s)(P<0.05). (2)The scores of HAMD ,BDI and HAMA were negatively correlated with numbers of total response( r=-0.403,-0.544,-0.495,) in patients with MDD ( all P<0.05) . Conclusion The executive function of patients with MDD is impaired and nega?tively correlated with depressive and anxiety symptoms under the Tower of London Test.
5.Effects of Sanguis draxonis on wound healing in rat models of tissue-engineered skin transplantation

BACKGROUND:Increasing evidence suggests that Sanguis draxonis is of great significance for treating pressure sores, burns and ulcers.
OBJECTIVE:To observe the influences of Sanguis draxonis on vascular endothelial growth factor, epidermal growth factor, substance P and hydroxyproline in rats undergoing tissue-engineered skin transplantation and to verify its promotion of wound repair.
METHODS:The tissue-engineered skin transplantation model of rats were established. Rats in treatment groups were given external application, single oral use of 0.1 g/(kg·d) Sanguis draxonis and combined use, respectively. No treatment was given in control group. After continuous treatment for 7 days, the expression levels of vascular endothelial growth factor, epidermal growth factor, substance P and hydroxyproline in skin tissue were determined.
RESULTS AND CONCLUSION:Compared with the control group, the levels of vascular endothelial growth factor, epidermal growth factor, and hydroxyproline were significantly increased (P<0.05 or P<0.01), while substance P had no change in the treatment groups (P>0.05). These results demonstrate that Sanguis draxonis can promote tissue-engineered skin to repair skin wound by upregulating the expression levels of vascular endothelial growth factor, epidermal growth factor, and hydroxyproline.
6.Analysis of the related factors of local recurrence in patients with rectal cancer after Dixon operation

Objective To analyze the related factors of local recurrence in patients with rectal cancer after Dixon operation. Methods A retrospective analysis of clinical data of 100 patients from September 2005 to September 2007 in our hospital by TME standard surgicai treatment of low rectal cancer was carried out. Results Of 100 patients, the pre-sacral recurrence rate was 10. 0%, with gender, age, tumor size, tunor invasion lumen circumference,depth of invasion,lymph node metastasis and Duke's stage has nothing to do with the gross type,histological type and lumen,peritoneal tumor cell shedding,and as an independent prognostic factor. Of 90 patients without local recurrence,the 5-year survival rate was 76. 0% ,haff of the survival of 62 months;10 cases of local recurrence,5-year survival rate was 5. 0%, half of the survival of 24 months, suggested that local recurrence in patients influenced the prognosis. Conclusion The low local recurrence of rectal cancer related factors were gross type,histological type and the intestine, peritoneal tumor cells shed, seriously affect the prognosis of patients with local recurrence.
8.Surgical treatment of postoperative rebleeding in portal hypertension.

ObjectiveTo evaluate the effect of different operative methods for treating rebleeding in patients with portal hypertension(PHT). MethodsThe clinical data of 66 patients with postoperative ebleeding out of 373 PHT cases during the last 30 years was retrospectively analyzed. Their first operations were splenectomy, portoazygous devascularization, various non-selective shunt and combined operation(devascularization combined with shunt), of which, the rebleeding rate were 26.67 %, 17.86 %, 14.58 % and 4.35 %, respectively. Among 55 cases receiving re-operation, there were 42 with distal esophago-fundusectomy, 11 mesocaval shunt(MCS)and 2 re-devascularization. The other 11 cases received nonoperative therapy. ResultsThe mortality, mean follow-up time, rebleeding rate and encephalopathy rate were 9.52% (4/42), 11 years,9.52% (4/42)and 14.29% (6/42)in distal esophago-fundusectomy group;0,7.5 years,0 and 9.09% (1/11)in MCS group. All 2 cases in re-devascularization group died within one postoperative month. All 11 cases in non-operative group were with portal hypertension gastropathy(PHG). ConelusionCombined operation may effectively reduce postoperative rebleeding rate in PHT cases and MCS is a relatively ideal method for treating rebleeding.
9.Progress on the diagnosis and treatment of colonic diverticulitis

Diverticulitis of colon,the inflammation of colonic diverticulars,is connected with dietary,age and disorder of colonic environment.Due to the different cultures between western contries and orient nations,the morbidity,predilection site,clinical manifestation and management are different in diverticulitis of colons.Computed tomography(CT)has been the key diagnosic method,and it is able to guide transfixion pins to treat diverticulitis of colon.After medical treatment,most of patients can be relaxed,and only someone need operations.
10.Effects of dexmedetomidine preconditioning on imbalance of nitric oxide/endothelin-1 and remote lung injury in patients with lower limb ischemia-reperfusion

Objective To investigate the effects of dexmedetomidine preconditioning on nitric oxide (NO)/endothelin (ET)-1 imbalance and remote lung injury induced by lower limb ischemia-reperfusion (LIR). Methods Sixty patients who scheduled for unilateral lower extremity surgery matched American Society of Anesthesiologists (ASA) Ⅰ-Ⅱ, were randomized into two groups:control group (R group, n=30) and dexmedetomidine preconditioning group (PD group, n=30). Lumbar plexus combined with sciatic nerve block was performed guided by a nerve stimulator in both groups. In group PD, dexmedetomidine intravenous infusion was started at a dose of 0.125 mL/kg (4 mg/L) for 10 minutes before using tourniquet, whereas group R received an equivalent volume of normal saline. Artery blood gas analysis, respiratory index and oxygenation index were measured, and NO, ET-1, interleukin-8 (IL-8) and malondialdehyde (MDA) concentrations were determined from plasma samples 10 minutes before tourniquet inflation (T0), 15 minutes (T1), 2 h (T2), 6 h (T3) and 24 h (T4) after tourniquet deflation. Results Compared with T0, RI was higher at T3 in group R and OI was lower at T2-4 (P<0.01). There were no significant differences in RI and OI of group PD between different time points (P>0.05). In R and PD groups, ET-1, IL-8 and MDA concentrations were increased, while NO level and NO/ET-1 ratio were significantly decreased after tourniquet deflation (P<0.05). Compared with group R, RI was lower and OI was higher at T3 in group PD (P<0.01). The levels of ET-1, IL-8, MDA, NO and NO/ET-1 ratio were significantly different after tourniquet deflation between group PD and group R (P<0.05). In group R, there was positive correlation between ET-1 and IL-8 levels with RI, and negative correlation between ET-1 and IL-8 levels with OI (P<0.01). There was positive correlation between NO level, NO/ET-1 ratio and RI level (P<0.01). Conclusion Lung function impairment induced by tourniquet application could be attenuated by dexmedetomidine preconditioning based on protecting endothelial cells and inhibiting lipid peroxidation.
