1.Clinical study on three different operation procedure for hysterectomy

Objective To investigate the effects of three procedures for hysterectomy.Methods The clinical characteristics of 370 cases who respectively received hysterectomy by total transabdominal hysterectomy(TAH,150 cases),transvaginal hysterectomy(TVH,120 cases),or laparoscopically assisted vaginal hysterectomy(LAVH,100 cases)were analyzed and their effects and outcomes and the expenses for medical treatment were compared from 2000 January to 2005 December.Results The average operation time of TAH,TVH and LAVH was(78?28) minutes,(102?35)minutes,(119?28)minutes respectively.The average operation time of TAH was significantly shorter than that of TVH and LAVH groups(P<0.01).The volume of hemorrhage during operation in TAH group was less than that of other two groups significantly[TAH(53?29)ml,TVH(110?35)ml,LAVH(93?50 )ml](P<0.01).The average time of bowel function recovery in TAH group[(42?7)hours]was significantly longer than that of TVH[(23?5)hours]or LAVH[(25?4) hours]groups(P<0.01).The average hospital stay after opera- tion in TAH[(8.9?1.1)days]group was significantly longer than that of TVH[(5.7?2.4)days]or LAVH [(6.3?1.9)days] groups(P<0.01).The average expenses for medical treatment in LAVH[(7273?938)yuans] group was significantly more than that of TAH[(5594?308)yuans]or TVH[(5450?167)yuans]groups(P<0.01).But the lesion and the complication after operation of three groups had no difference.Conclusion Different procedures for total hysterectomy have their own advantage and disadvantage.The operation procedure must take ac- count of the economy,echnique of medical treatment and the condition of the hospital.
2.Ethical Issues and Progress in Clinical Study of Dendritic Cell Tumor Vaccine

In recent years dendritic cell tumor vaccine has been of great interest in medical research.In this paper,the author introduces the theory of immunotherapy on the basis of dendritic cell the development of the research in dendritic cell tumor vaccine and the clinical effect.The conclusion is that the clinical application of dendritic cell tumor vaccine should follow the principle of medical ethics.
3.Effect of Dipyridamole Therapy on Kawasaki Disease with Glucose-6-Phosphate Dehydrogenase Deficiency in Children

Objective To evaluate the therapy and turnover of Kawasaki disease(KD)with glucose-6-phosphate dehydrogenase(G-6-PD)deficiency.Methods Six hundred and twenty-four patients with KD were selected including 32 patients who had G-6-PD defected.The same dose intravenous immunoglobulin(IVIG)was used in all 624 patients(2 g/kg),but used Dipyridamole in 32 patients had G-6-PD deficiency which replaced the role of acetylsalicylic acid(ASA)after acute period.The coronary artery of these patients were checked and followed-up through echocardiograph.The turnover of 32 patients with G-6-PD deficiency and 356 case selected randomly from all the KD patients were compared.SPSS 10.0 software was used to analyze the data.Results In 32 cases of KD with G-6-PD deficiency,4 children had coronary aneurysm(12.5%).After 6-12 months follow-up,the coronary lesions were recovered in 62.5% children,improved in 21.9% children and not improved in 15.6% childern,which were not significantly different from all the KD patients(Z=-1.604 P=1.09).Conclusions IVIG and Dipyridamole are feasible in treating KD with G-6-PD deficiency.J Appl Clin Pediatr,2009,24(1):26-27
4.Clinical study of prevention of oxaliplatin-induced acute neurotoxicity by calcium and magnesium infusions

Objective To evaluate the efficacy of calcium(Ca)and magnesium(Mg)infusions in pre- vention of oxaliplatin-induced acute neurotoxicity.Methods Fourty-two patients with advanced colorectal carcinoma were eligible for the study:21 were assigned to the Ca/Mg ann and 21 to the control arm.The Ca/ Mg arm and the control arm were comparable for patients'characteristics.Chemotherapy regimen were al- most the same in both arms.Chemotherapy regimen consisted of oxaliplatin 130 mg/m~2 on day 1,given as a 3-hour infusion in 500 ml of 5% glucose,concurrent with rahitrexed 3 mg/m~2 as a 15-minute intravenous (Ⅳ)infusion or calcium folinate(CF)200 mg?m~(-2)?d~(-1),days 1~5,5-Fluorouracil(5-Fu)500 mg?m~(-2)?d~(-1),days 1~5.Therapy was repeated every 3 weeks.The treatment consisted of Ca gluconate and Mg sulfate,1 g each, delivered i.v.over 15 min just before the oxaliplatin infusion and repeated at the same dose after the comple- tion of the oxaliplatin infusion.A specific neurotoxicity scale was used for oxaliplatin-related neurotoxicity. Results Ten patients(47.62%)had acute neurotoxicity in the Ca/Mg arm compared with 19 patients (90.48%)in the control arm(P0.05).Conclusion Ca/Mg infusions seem to reduce incidence and intensity of oxaliplatin-induced acute neurotoxicity,and they do not reduce the clinical activity of oxaliplatin,but dosage and administration schedule could be optimized.
5.Treatment with Chinese botulinum toxin type A in 16 cases of masticatory spasm

Objective To study the therapeutic efficacy of Chinese botulinum toxin type A (CBTX- A) in masticatory spasm patients.Methods 16 patients with masticatory spasm were treated with CBTX-A local injection.12 patients showed jaw clench with masseter and temporalis affected (type Ⅰ).Four showed jaw clench and deviation to one side with pterygoid muscle affected unilaterally or bilaterally (type Ⅱ).All patients showed paroxysmal clenched jaw with difficulty in opening their mouths.There were no other clinical manifestations.CT and MRI did not reveal any intracerebral abnormalities.The efficacy and adverse effects were observed.Results CBTX-A were injected into 16 patients,resulting in a significant improvement of symptoms in 13 cases (4 cases of unilateral type Ⅰ,7 of 8 cases bilateral type Ⅰ,2 of 4 type Ⅱ).The spasms ceased within 3-10 days after the injection,and the effects lasted for 8-26 weeks.Four patients were observed to have slight masseter weakness after the injections,which recovered within a few weeks.The benefit persisted after identical repeated injection.Conclusion CBTX-A injection is an effective and safe treatment for masticatory spasm.

9.Prokaryotic expression and purification of the efaA protein from Enterococus faecalis antigens

To prokaryotic express prokaryotically and to purify the efaA protein from Enterococus faecalis so as to provide the basis for the further study on the pathogenesis and clinical sero-diagnosis of endocarditis caused by E.faecalis, efaA gene of E.faecalis was amplified by PCR, the PCR-amplified product was digested with restriction enzymes and cloned into prokaryotic vector pET32a to construct the recombinant plasmid pET30a/efaA. This recombinant plasmid was confirmed by double enzyme digestion with BamhI and Xhol and then subjected to sequencing, and transformed to E.coli BL21 (DE3). Expression of the fusion protein was induced by IPTG, and analyzed by SDS-PAGE and Western blotting. The recombinant fusion protein was purified by His-binding affinity chromatography.It was shown that efaA gene of 943 bp in size was amplified from Enterococus faecalis and the recombinant plasmid pET30a,/ efaA was successfully constructed and expressed in E.coli BL21. The purified product was found to be 34 kDa in molecular weight as demonstrated by SDS-PAGE and Western blotting. It is evident that the efaA protein of E.faecalis can be successfully expressed and purified.
10.Effect of puerarin on blood pressure and serum lipid in a rat model of insulin resistance

AIM: To observe the effects of puerarin on blood pressure(BP),serum lipid in a rat model of insulin resistance and the mechanism.METHODS: Adult SD rats were maintained on high-fat-sugar-salt diet for 12 weeks.Puerarin was administered to the rats from 9th week for 4 weeks by intramuscular injection.BP was measured at the end of 0,8th,12th week.The levels of serum glucose,serum lipid,fasting serum insulin and the levels of MDA,TNF-?,plasma rennin,angiotensinⅡ(AngⅡ) and the activity of SOD were measured and the insulin-sensitivity index was also calculated at the end of the experiment.RESULTS: High and middle dosage of puerarin significantly decreased blood pressure,reduced the levels of serum lipid and AngⅡ,and also increased insulin-sensitivity index.The levels of MDA and TNF-? were significantly decreased by high dosage of puerarin.The activity of SOD was increased significantly.CONCLUSIONS: Puerarin possesses the effects of decreasing the blood pressure and serum lipid in a rat model of insulin resistance,which may be concerned with the changes of rennin-angiotensin system,the levels of oxygen-derived free radicals and TNF-?.
