1.Reliability and Validity of the Korean Version of the Perinatal Post-Traumatic Stress Disorder Questionnaire.

PURPOSE: The Perinatal Post-Traumatic Stress Disorder Questionnaire (PPQ) was designed to measure post-traumatic symptoms related to childbirth and symptoms during postnatal period. The purpose of this study was to develop a translated Korean version of the PPQ and to evaluate reliability and validity of the Korean PPQ. METHODS: Participants were 196 mothers at one to 18 months after giving childbirth and data were collected through e-mails. The PPQ was translated into Korean using translation guideline from World Health Organization. For this study Cronbach's alpha and split-half reliability were used to evaluate the reliability of the PPQ. Exploratory Factor Analysis (EFA), Confirmatory Factor Analysis (CFA), and known-group validity were conducted to examine construct validity. Correlations of the PPQ with Impact of Event Scale (IES), Beck Depression Inventory II (BDI-II), and Beck Anxiety Inventory (BAI) were used to test a criterion validity of the PPQ. RESULTS: Cronbach's alpha and Spearman-Brown split-half correlation coefficient were 0.91 and 0.77, respectively. EFA identified a 3-factor solution including arousal, avoidance, and intrusion factors and CFA revealed the strongest support for the 3-factor model. The correlations of the PPQ with IES, BDI-II, and BAI were .99, .60, and .72, respectively, pointing to criterion validity of a high level. CONCLUSION: The Korean version PPQ is a useful tool for screening and assessing mothers' experiencing emotional distress related to child birth and during the postnatal period. The PPQ also reflects Post Traumatic Stress Disorder's diagnostic standards well.

2.Aneurysmal Rupture of the Internal Carotid Artery in a Presumed Neurofibromatosis Type I Patient.

Aneurysm of the internal carotid artery is a rare disease and is known to be associated with congenital arterial anomalies such as neurofibromatosis type I (NF-I). NF-I is an autosomal dominant neurocutaneous disorder characterized by a variety of manifestations that involve the central and peripheral nervous systems, skin, vascular system, and skeleton. In particular, the involvement of vascular abnormalities in NF-I is well known. Any vessel may be affected by this condition, although the renal artery is most frequently involved. The vascular abnormality can be occlusive or an aneurysmal degenerative change. Therefore, symptomatic presentations might assume an indolent pathophysiologic course such as hypertension, or manifest as a catastrophic event such as arterial rupture that could result in sudden death. We report a rare autopsy case of an aneurysmal rupture of the internal carotid artery in a woman with suspected NF-I, who collapsed in her home.

3.Factors Affecting Nursing Intention for Patients with Emerging Infectious Diseases among Nurses in Hospitals Dedicated to COVID-19: A Focus on the Mediating Effects of Job Crafting

Purpose:
This study aimed to identify the relationship between job complexity, infection prevention environment, and nursing intention for patients with emerging infectious diseases, with a focus on the mediating effect of job crafting in hospitals dedicated to coronavirus disease 2019 (COVID-19).
Methods:
The study surveyed 202 nurses with at least 3 months of work experience who encountered COVID-19 patients at a dedicated COVID-19 hospital. The collected data were analyzed using IBM SPSS/WIN 25.0, and the mediating effect was analyzed through the SPSS PROCESS macro, using regression analysis.
Results:
It was demonstrated that job crafting fully mediated the relationship between job complexity and nursing intention for patients with emerging infectious diseases. Furthermore, this study showed that job crafting partially mediated the relationship between infection prevention environment and nursing intention for patients with emerging infectious diseases.
Conclusion
It was confirmed that job crafting of nurses in a hospital dedicated to COVID-19 was an important mediating factor in their nursing intention for patients with emerging infectious diseases, and this finding is expected to support efforts to further improve nursing intention for patients with emerging infectious diseases.
4.An Anion Site Change of the Glomerular Basement Membrane on Various Glomerular Diseases.

We studied the ultrastructural alteration of glomerular anionic sites in 6 patients with minimal change nephrotic syndrome, 5 patients with membranous glomerulonephritis, 4 patients with focal segmental glomerulosclerosis, and 4 patients with IgA nephropathy by staining with polyethyleneimine (PEI) as a cationic probe. The control study was examined by using a nephrectomy specimen of non-glomerular disease which had no proteinuria. This method seems to selectively stain heparan sulphate in the basement membranes and has been widely used to evaluate changes in basement membrane charge in various human diseases as well as in experimental studies. The anionic sites in the lamina rara interna and lamina densa of normal glomerular basement membrane were always less numerous and less regularly distributed than those in the lamina rara externa. Characteristic common findings in these glomeruli showed a marked decrease of glomerular anionic sites in the regions with immune-complex deposits and normal distribution in the regions with focally those being absorbed and newly forming glomerular basement membrane. They were not detected in the gap of the basement membrane and on the area of the detached overlying epithelium using the PEI method. But the foot process fusion of epithelial cells seems not to influence the loss of anionic sites on the glomerular basement membrane.

5.Undescended Testis in the Complete Testicular Feminization Syndrome A report of three cases.

The testicular feminization is the most common type of male pseudohermaphroditism. Mutation of androgen receptor gene impairs normal sexual differentiation in genetic male (46, XY) and subsequently develops secondary female characteristics. We report three cases of complete testicular feminization syndrome with normal development of female external genitalia, but with blindly ended vagina and inguinal or pelvic masses. Microscopic examination of the testes and their adnexa had three components: 1) A testis composed of immature tubules, prominent Leydig cells and a spindle cell stroma resembling ovarian stroma with hamartomatous nodule or Sertoli cell adenoma, 2) a white, whorled, firm, smooth muscle body in the medial pole of the testis, and 3) a lateral adnexal cysts of varying sizes.

6.bcl-2 and p53 Protein Expression in Multiple Myeloma and Non-tumorous Plasma Cells A study related to survival.

The gene product of bcl-2 (B-cell leukemia/lymphoma-2) was suggested to suppress programmed cell death (apoptosis) of tumor cells and be involved in the development of multiple myeloma. However, the normal plasma cells also express the protein. It is unclear whether the expression of bcl-2 in multiple myeloma is of normal character or of regulatory adaptation in association with neoplastic transformation. p53 was also suggested to be involved in tumor progression since mutations on p53 were found in multiple myeloma. In order to find the relationship between the expression patterns of bcl-2 and p53 in tumor cells of multiple myeloma and non-neoplastic plasma cells, we examined 38 cases of multiple myeloma and 10 cases of nasal polyp immunohistochemically. Furthermore, expression of bcl-2 and p53, mitosis, clinical stage and infiltrative pattern of tumor cells in bone marrow were also evaluated in association with the survival of patients. By immunostaining with anti-bcl-2 and p53 monoclonal antibody, 37 out of 38 cases of multiple myeloma and all of 10 cases of nasal polyp were positive for bcl-2 but only 7 cases of multiple myeloma were positive for p53. Marked dysplasia, low percentage of bcl-2 expression, and increased mitoses were correlated with poor prognosis. Based on these observations, we suggest that bcl-2 and p53 are involved in tumorigenesis of multiple myeloma and the survival of patients would be influenced by dysplastic change, mitosis and degree of bcl-2 expression.

7.Atrophy of Multifidus Muscle on Low Back Pain Patients.

OBJECTIVE: To investigate the correlation of multifidus muscle atrophy on MRI findings with clinical findings in low back pain patients. METHOD: Medical records of 80 patients presenting with low back pain were retrospectively reviewed. Their MR images were visually analysed to know lumbar multifidus muscle atrophy, disc herniation, disc degeneration, spinal stenosis and nerve root compression. RESULTS: Multifidus muscle atrophy increased from the upper lumbar level to the most caudal intervertebral level. It was bilateral in the majority of the cases. Multifidus muscle atrophy was well correlated with patient's age, referred leg pain, and disc degeneration. However, duration of low back pain, disc herniation, spinal stenosis, nerve root compression, sex, weight, height and BMI had no correlation with multifidus muscle atrophy. CONCLUSION: Examination of multifidus muscle atrophy should be considered when assessing MR images of lumbar spine. It may help for further evaluation and planning the treatment modalities of low back pain.

8.Evaluation of Intraosseous Pressure during Pelvic Percutaneous Cement Injection: An In Vitro Study in Swine

Background:
A minimally invasive procedure for symptomatic pelvic bone metastasis is a feasible option for advanced cancer patients, and bone cement injection plays an essential role. Pulmonary embolism caused by thrombus, fat, or tumor emboli is a major complication related to bone cement injection, and increasing intraosseous pressure is a predisposing factor. This study aimed to quantify the degree of pressure change in the pelvic bone during percutaneous bone cement injection and investigate whether there is a significant decrease in intraosseous pressure when a decompressive route is additionally established.
Methods:
Bone cement injection into the acetabulum of swine pelvises by simulating the actual surgical procedure in terms of the injection method, bone cement, and surgical instruments was performed while recording the intraosseous pressure. Twenty swine pelvises were used and grouped into a decompression group and a non-decompression group. Bone cement injection and pressure measurement were conducted in the same way in both groups, but an additional decompressive route was established for each pelvis in the decompression group. Continuous variables were compared using the Mann-Whitney test.
Results:
The mean amount of injected bone cement was 19.8 mL and 20.3 mL and the mean speed of bone cement injection was 0.14 mL/sec and 0.12 mL/sec in the decompression group and the non-decompression group, respectively. The mean peak intraosseous pressures was 10.5 kPa with decompression and 37.8 kPa without decompression, and the difference was statistically significant (p < 0.01).
Conclusions
Intraosseous pressure during bone cement injection into swine pelvises was similar to that during vertebroplasty or kyphoplasty. When the additional decompression route was established, the intraosseous pressure decreased to one third the level.

10.Analysis for Prognostic Factors of Postoperative Stage I Non-small Cell Lung Cancer.

OBJECTIVE: This study is to evaluate several histopathologic variables and molecular markers in patients with stage I NSCLC after complete tumor resection to assess their prognostic value. MATERIALS AND METHODS: Ninety one patients with stage I NSCLC, SCC or adenocarcinoma histologically, operated between March 1992 and December 2003 were evaluated. The postoperative 5-year survival rate was calculated with the Kaplan-Meier method. Comparison of the risk factors for recurrence was performed by Chi-square test. Multivariate analyses were performed using Cox proportional hazards model to identify independent prognostic factors. The criterion for significance was p<0.05. RESULTS: There were 73 men (80.2%) and 18 women (19.8%) a mean+/-SD age at diagnosis of 61.9+/-8.5 years (range, to years). A pulmonary resection and mediastinal lymph node dissection were done in all cases. A lobectomy was performed in 78 cases (93.4%, included with bilobectomy, 10 and sleeve lobectomy, 7) and a pneumonectomy in 6 cases (6.6%). The histologic types were SCC in 57 cases (62.6%) and adenocarcinoma in 34 cases (37.4%). There was no operative mortality and complications occured in 2 cases (1.1%). The overall 5-year survival was 59.8% and survival time did not depend on the type of operation or histologic type. Significant predictors of recurrence were stage Ib and worse differentation than moderate (p=0.003 and 0.037 each). The overall incidence of recurrence was 38.5% (local or regional 37%, distant 63%). Almost all recurrences (35 cases) occurred in T2 lesions. The distant organs that failed were, the brain in 8 patients, the liver in 6, the multiple lung metastasis in 5, the bone in 2, and extrapulmonary node in 1. We found significant differences between SCC and adenocarcinoma in sex predominence (p=0.001) and visceral pleural invasion (p=0.035). In immunohistochemical stain, the SCC showed higher CD44 expression (p=0.023) than adenocarcinoma and TTF-1 higher expression (p<0.05) on adenocarcinoma than SCC. Multivariate analysis showed that CD44 less expression than 10% (p=0.039) were significant independent prognostic factors of survival on patients with adenocarcinoma. CONCLUSIONS: We found that T2 lesion and poorly differentiated/undifferentiated tumor had significant value to predict the risk of recurrence. We suggest that CD44 antibody could be used as a prognostic value in patientsof resected stage I non small cell lun cancer
