The thesis put forward a feasible method for the communication between teachers and students in the process of hospital personnel training based on comprehension.This approach named‘dialogue’,can adjust the conflict between teachers and students including‘the conflicts between teaching and studying’,‘distraction of teachers’and‘halfhearted students’,etc.Applying‘dialogue’method to resolve conflicts provided a new angle for better understanding of relationship between teachers and students.

Formation of coronary atherosclerotic plaques brings severe damage to human health.Macrophages play an important role during formation and development of plaques,which becomes an important marker for judging whether atherosclerotic plaques are stable or not.Therefore,recognition and quantification of macrophages become hotspot in current study.The present article made an overview on cunent frequently-used imaging techniques for de-tecting coronary plaques and macrophages inside them for now.

Objective To analyze and determine the clinical chatacteristics and treatment of suqerior mesenteric venous thrombosis.Methods We reviewed the medical records of all 32 patients who were diagnosed as with SMVT in Peking Friendship Hospital from 1985 to 2005.Results Thirty-two patients were diagnosted as SMVT,11 patients were with chronic superior mesenteric venous thrombosis(median age 46.6 years,ranging 32～78 years),without acut abdomen;22 cases were acute superior mesenteric venous thrombosis(median age 45.9 years,range 19～64 years).The main symptoms at the presentation were abdominal pain(100%),abdominal distension(100%),acute abdomen(100%),vomiting(68.2%,15/22),an fever(36.4%,8/22).Misdiagnosis rate 59.1%(13/22),death rate 31.8%(8/22).Conclusion The presentations of ASMVT have some relatively characteristics.The clinical diagnosis of ASMVT can be confirmed on the basis of abdominal CT and selective superior mesenteric arteriography in early phase.

Objective To compare the bronchial blocker and double-lumen tube for one-lung ventilation in the elderly patients undergoing minimally invasive direct coronary artery bypass (MIDCAB).Methods Thirty six patients of both sexes,aged 65-78 yr,with the left ventricular ejection fraction ≥ 45％,with body mass index ＜ 30 kg/m2,of ASA physical status Ⅱ or Ⅲ (NYHA Ⅰ-Ⅲ),scheduled for elective MIDCAB in the left thorax,were randomly divided into 2 groups (n =18 each):double-lumen endotracheal tube group (group D) and bronchial blocker group (group B).Anesthesia was induced with midazolam 0.05 mg/kg,etomidate 0.3 mg/kg,fentanyl 10μg/kg and cisatracurium 0.15-0.20 mg/kg.The patients were intubated with a left-sided double-lumen endotracheal tube 5 min later in group D.The patients were intubated with a single-lumen endotracheal tube 5 min later,and then Coopdech bronchial blocker was inserted into the primary bronchus in group B.The patients were mechanically ventilated.Before induction of anesthesia,at 2 min before intubation,immediately before and after intubation,and at 1 and 2 min after intubation,mean arterial pressure (MAP),heart rate (HR),and mean pulmonary arterial pressure (mPAP) were recorded and rate-pressure product (RPP) was calculated.The requirement for vasoactive drugs was recorded during induction of anesthesia.Lung collapse developed after the pleura was opened was also recorded.Surgical exposure was scored at the end of operation.Results Compared with group D,MAP,HR RPP and mPAP were significantly decreased after intubation,the requirement for nicardipine and esmolol was decreased,and no significant change was found in the requirement for atropine and metaraminol,rate of lung collapse and score of surgical exposure in group B.Conclusion Compared with doublelumen tube,bronchial blocker can provide sufficient exposure of the surgical filed,and intubation-induced fluctuation of hemodynamics is small in the elderly patients undergoing MIDCAB.

Objective:To study influence of type 2 diabetes mellitus (T2DM) on coronary artery lesion and plaque sta‐bility using optical coherence tomography (OCT) in patients with coronary artery disease (CAD) manifesting unsta‐ble angina pectoris (UAP) . Methods :A total of 200 patients diagnosed as CAD manifesting UAP were selected.According to complicated with diabetes mellitus or not ,they were divided into normal blood glucose group (n=98) and complicated T2DM group (T2DM group ,n=102). Coronary angiography (CAG) and OCT were used to eval‐uate coronary artery lesion ,assess plaque nature and measure the thickness of plaque fibrous cap in order to confirm plaque stability . Results:Compared with normal blood glucose group ,there were significant rise in percentages of multi-vessel coronary disease (48.98% vs. 78.43% ) ,severe artery stenosis (20.57% vs. 40.21% ) ,lipid plaques (25.00% vs. 39.84% ) and plaques with fibrous cap thickness <65μm (25.93% vs. 45.24% ) in T2DM group , P<0.05 or < 0.01 .Conclusion :Patients with coronary artery disease have more severe coronary artery lesion and high ‐ er incidence rate of multi‐vessel coronary disease and unstable plaque when complicated with type 2 diabetes mellitus .

Objective To explore the effects of psychological disorders on qualityoflife of elderly patients with non-erosive reflux disease (NERD) under maintenance treatment of proton pump inhibitors (PPIs). Methods A total of 76 elderly patients with NERD treated by proton pump inhibitors (maintenance treatment of Esomeprazole over half a year) were enrolled. They were classified into symptom group aged (66.5±8.2) years (n=35) and symptom group aged(68.2+6.9)years (n=41)according to reflux symptoms,with 20 healthy people aged(65.4 ±8.8)years as control.The subjects completed the questioniaires on psychological status including self-rating anxiety scale (SAS) and self-rating depression scale (SDS) and their quality of life was evaluated by gastroesophageal heath related quality-of-life instrument(GERD -HRQL) and medical outcomes study short form(SF-36). Results Scores of SAS and SDS were significantly higher in symptom group (54.9±6.2,57.9±8.6) than in symptomless(37.9±8.7,42.7±7.9) and health control group (38.9± 7.2,43.1 ± 8.9),all (P＜0.05),but there was no difference between symptomless and health control group.The rates of anxiety and depression in symptom group (51.4％,57.1％) were increased as compared with symptomless (12.2％,9.8％)and health control group(0.0％,5.0％).Scores of GERD HRQL in symptom group were higher than in symptomlcss group (21.4± 5.0 vs.4.5±2.2,t=19.76,P＜0.01)while the indexes such as body pain,vitality,social function,affection,mental and whole health in score of SF-36 besides physiology function were lower in symptom group than in symptomless and control groups (P＜0.05 or P＜0.01 ).Only the significant differences in mental and whole health of SF-36 were found between symptomless and control groups (P＜0.01).Correlation analysis revealed that SAS and SDS scores were positively correlated with scores of GERD-HRQL in symptom group(P＜0.01) and negatively with all dimensions of SF-36 but physical function (P＜ 0.05). Conclusions Most of elderly symptomatic patients with NERD treated by PPI are in psychological disorders which results in lower quality of life.
