Torsades de pointes, a polymorphic ventricular tachycardia associated with prolonged QT interval, is a well-known life-threatening arrhythmia, which has been found to be induced by various causes such as drugs, electrolyte imbalances, and severe bradycardia. Cisapride is a gastrointestinal prokinetic drug, which is widely used to treat gastroesophageal reflux disease or other functional gastrointestinal disorders. Cisapride can cause torsades de pointes and cases of torsedes de pointes induced by cisapride have been reported in other countries. Cases of torsades de pointes associated with antihistamine drugs have been reported in Korea, however, cisapride-induced torsades de pointes case has not been reported. We report a case of 31 year-old female patient who experienced repeated loss of consciousness due to cisapride-induced torsades de pointes.

Meningiomas are usually thought of as firm solid tumors and most standard references make no mention of cystic meningiomas. Although several cases of cystic meningioma have been reported in the literature and their neuroradiological features discussed, the rarity of this entity makes its preoperative diagnosis difficult. Recently, the authors encountered a case of cystic meningioma, which was thought as ependymal cyst or infarction, preoperatively. In this report the authors discussed its clinical, neuroradiological and pathological characteristics with brief reviews of the literature.

Black adenoma is known to be a rare variant of adrenal cortical adenoma containing characteristic abundant lipofuscin pigments in the cytoplasm. Almost all of them are nonfunctioning and only occasionally they are associated with Cushing's syndrome or primary hyperaldosteronism. We present two cases of black cortical adenoma of the adrenal gland associated with Cushing's syndrome in a 24-year-old woman and a 64-year-old man. This report dealt with clinical and pathologic presentation including ultrastructural identification of lipofuscin pigment.

Undifferentiated embryonal sarcoma(UES) is an uncommon malignant hepatic tumor that occurs in older children and young adults. Six cases of UES were reviewed to determine their characterisfic findings on sonography (US) (n=5), computed tomography (CT) (n=6), and angiography (n=2) US demonstrated a single large, echogenic mass with some anechoic spaces in each case. CT revealed a well demarcated hypodense mass with hyperdense septa of variable shape and thickness. Four cases showed enhancing peripheral rim and some solid portions at the periphery or adjacent to the septa. Follow-up CT scan in one case who had not been treated showed enhancing solid portion changed to hypodense areas. Angiography showed hypovascular or avascular mass. Familiarity with these radiological findings of UES of liver will be helpful in differential diagnosis of primary hepatic tumors of childhood.

PURPOSE: Results of bacterial cultures and susceptibility tests in the neonatal period have shown changes according to the year they were reported, and the increase in pathogenic organisms has become a topic of great interest. This study was carried out to determine the trend in the occurrence of microorganisms and susceptibility testing results conducted on neonates from the newborns hospitalized in the NICU at Hanyang University Hospital during the past 1- years. METHODS: The laboratory data of microorganisms isolated by culture of various specimens from the NICU from January, 1986 to December, 1995 were retrospectively analyzed. The study period was divided in two 5-year periods. RESULTS: 1) The total number of neonates in the first half was 6,392, and 8,364 in the second half. Of these, positive blood cultures were observed in 62(1.0%) and 166(2.0%), respectively. Isolation from other specimens totalled 834(450 in the first half and 384 in the second). 2) There was a total of 13 specimens other than blood, from which bacteria were isolated. There 380 urine specimens and 20 umbilical secretions in the first half and 167 bronchial secretion and 101 umbilical secretions in the second. 3) The microorganisms cultured in the first half included 215 Gram positive, 232 Gram negative organisms, and 3 fungi. Common organisms included 119 cases of E. coli(26.4%), 66 cases of MSSA(14.7%) and 64 cases of CNS(14.2%). In the second half, there were 182(47.4%) Gram positive, 192(50.0%) Gram negative organisms, and 10(2.6%) fungi. The most common organisms were MRSA(123 cases, 32%), E. cloacase(56 cases, 14.6%), CNS(29 cases, 7.6%), and Klebsiella (29 cases, 7.6%). 4) There were a decrease in organisms susceptible to Oxacillin, Gentamicin, Cephalothin, and Ampicillin in the second half. CONCLUSIONS: The changes in the NICU during the past 10 years include a great increase in MRSA, Enterobacter, and Candida, reflecting the increasing trend of nosocomial infections. Furthermore, there was an increase in resistance to Oxacillin and Gentamicin while there was good susceptibility to Vancomycin and Chloramphenicol even in the second half.

A microemulsion cyclosporine(Me-CsA) was developed and became available with more predictable whole blood CsA concentration and minimal inter- and intra-personal variability in daily dosage of CsA. We prospectively performed this study to assess 1) the ability of Me-CsA maintaining the adequate predefined therapeutic level and 2)long-term safety and tolerability of Me-CsA in kidney transplant recipients. A total of 123 renal transplant patients were enrolled on the Me-CsA group, who have been on Me-CsA as an initial main immunosuppressant since their transplantation. This group of patients were compared to 200 renal transplant patients on conventional cyclosporine(Con-CsA) as a historical control group(Con-CsA group). There were no differences in the methods of operation, induction immunosuppression, the strategies of maintenance immunosuppression and anti-rejection therapy between these two groups. The clinical status and laboratory values were monitored at 1,3,6,9, and 12 months after the kidney transplantation. There were no statistical differences in acute rejection episodes, serum creatinine level, and graft failure and survival rate between Con-CsA and Me-CsA groups. In this study, we could demonstrate the significant fluctuation of the mean values of daily dosage and whole blood trough level and their standard deviations of cyclosporine in Con-CsA group compare to those of Me-CsA group. We also could demonstrate early stabilization of CsA blood trough level in patients using Me-CsA. These results mean that Me-CsA has less interpersonal variations than Con-CsA. In conclusion, Me-CsA has more predictable pharmacodynamic characteristics than Con-CsA and comparable tolerability and safety to Con-CsA with no additional side effects.
