ObjectiveTo investigate the clinical value of routine para - aortic lymphadenectomy in patients with stage Ⅰ B1 and Ⅱ A2 cervical cancers treated by surgical intent.Methods240 patients with stage Ⅰ B1 and Ⅱ A2 squamous cervical cancer were randomly divided into tow groups( group A and B).120 patients (group A) underwent radical hysterectomy with systematic pelvic lymphadenectomy and paraaortic lymphadenectomy.Other 120 patients (group B) underwent radical hysterectomy with systematic pelvic lymphadenectomy,additional para-aortic lymphadenectomy were carried out when para-aortic lymph nodes were identified as suspicious by visualization and palpation.The operation time,amount of surgical bleeding,length of stay,complications,and metastasis rates of pelvic lymph node,metastasis rates of common iliac lymph node and metastasis rates of para-aortic lymph node were analyzed between two groups.ResultsThe clinical characteristics of the patients had no statistically significant difference between the two groups.The length of stay and the complications of group A were significantly higher than group B ( P ＜0.05 ).During the 240 patients,the metastasis rates of pelvic lymph node,common iliac lymph node and para-aortic lymph node were 27.9％,10.8％ and 5.4％.The metastasis rate of para-aortic lymph node in group A was statistically higher than that in group B( 8.3％ vs 2.5％,P ＜0.05),whereas there had no significant difference of the metastasis rates of pelvic lymph node and common iliac lymph node between the two groups.ConclusionsRoutine para-aortic lymphadenectomy can be helpful to accurately predict the status of the para-aortic lymph node in patients with stage Ⅰ B1 and Ⅱ A2 cervical cancer,but the length of stay and the complications of operation will increase.

Objective To determine the safety location of the bony tunnel in reconstruction of the coracoclavicular ligaments on the basis of digital characterization of the anatomy of the clavicle and coracoid process.Methods Unilateral shoulder spiral CT scan was conducted in 30 patients without injury to the clavicle or coracoid process.They were 15 men and 15 women,aged from 20 to 71 years old (average,49.3 years).Thirty 3D digital models of the clavicle and coracoid process were constructed using the CT scan data by Mimics13.0.Half of the models were of the left shoulder and half of the right.Anatomic measurements of the clavicle and coracoid process were carried out on these models.Virtual transclavicular-transcoracoid bony tunnels were established according to the anatomy of the conoid ligament.Parameters of these bony tunnels were measured before the safety location was calculated.Results The mean clavicular length was 147.70 ± 5.34 mm in males and 133.09 ± 6.61 mm in females; the distance between the lateral edge of the clavicle and the center of the conoid tuberosity (CCD) was 35.90 ± 3.16 mm in males and 30.48 ± 0.54 mm in females; the distance between the lateral edge of the clavicle and the center of the trapezoid tuberosity (CTD) was 22.68 ± 1.23 mm in males and 18.69 ± 1.65 mm in females,with significant differences between genders (P ＜ 0.05).There were no significant differences between male and female regarding the ratio of the CCD to the clavicular length,the ratio of CTD to the clavicular length,or the mean internal rotational angle of the coracoid process (P ＞ 0.05).According the attachments of the conoid ligament on the clavicle and coracoid,from the superior-posterior edge of the clavicle to the anterior midpoint of the coracoid basement,the bony tunnels were established.These tunnels nearly bisected the cross section of the clavicle and coracoid basement,35.23° ± 2.36° medially inclined to the sagittal section and 5.91° ± 2.14° posteriorly inclined to the coronal section of the body.Conclusions To ensure that the bony tunnel should pass through the center of the clavicle and coracoid,it should be drilled from the superior-posterior edge of the clavicle and located at the clavicular attachment of the conoid ligament,tilting about 35° medially and 6° posteriorly to aim at the anterior midpoint of the coracoid basement.

Aim To search the effective malaria mixed gene vaccine and to explore the immune response of vaccinated host. The recombinant pcDNA3-EBA175/HRP Ⅱ was injected alone or mixed with pcDNA3-Pfs25 of Plasmodium falciparum into mice by intramuscular route. The muscle of injected parts were treated previously, e.g. injecting 50μl of 0. 5％bupivacaine into the muscle of left latter leg, with 2mm depth. To observed the changes of IgG antibody value, the splenic lymphocyte proliferation, the ratio of CD4+/CD8+ subgroups and NK cell killing activity. After injecting recombinant pcDNA3-EBA175/HRP Ⅱ alone or mixed with pcDNA3-Pfs25 into mice by intramuscular route, it showed that sera IgG value increased, the splenic T lymphocyte proliferation stimulated by specific antigen of Plasmodium falciparum increased, the ratio of CD4+/CD8+ decreased and NK cell activity increased. Enhanced immune injection could improve host's immune reaction.It is suggested intramuscular injection is an effective immune route, and mice inoculated with coding two gene recombinant alone or that mixed with sexual stage gene recombinant could all induce increased humoral and cellular immune response, and NK cell activity.

Objective Examinating the efficacy and safety on prophylaxis of deep venous thrombosis(DVT) following arthroplasty using low-molecular-weight heparin (LMWH) in combination with intermittent pneumatic compression(IPC) starting with different time. Methods 156 patients undergoing primary arthroplasty from July 2003 to June 2005 were divided into four groups randomly. In the first group, 38 patients used LMWH preoperatively and IPC starting since anesthesia. In the second group, 42 patients did LMWH postoperatively and IPC starting since anesthesia. In the third group, 36 patients did preoperative LMWH and post-operative IPC. In the fourth group, 40 patients did post-operative LMWH and IPC. All patients were examined by colour duplex ultrasonography, valued the clinical symptom of DVT, recorded the volume of blood loss in perioperation and observed complication of hemorrhage. Results In the first group, two cases of DVT (5.26%) were found and the average perioperative volume of blood loss was (1030.8?282.0) ml. In the second group, two cases of DVT (4.76%) were found and the average volume of blood loss was (900.7?246.9) ml. In the third group, one case (2.78%) of DVT was found and the average volume of blood loss was (1040.3?288.5) ml. In the fourth group, nine cases of DVT (22.5%) were found and the average volume of blood loss was (852.2?295.4) ml. Comparing the incidence of DVT between each group, group 1, 2, 3 were lower than group 4(P﹤0.05). Comparing the volume of blood loss in perioperation, group 2 , 4 were less than group 1,3(P﹤0.05). Conclusion On the basis of this study, it is more effective and safer using LMWH staring with post-operative 10 h in combination with IPC starting with anesthesia on prophylaxis of DVT following arthroplasty.

Objective To investigate the degradation rate of profenofos in soil. Methods The residues of profenofos at different times after spraying on soil were determined by GC-FPD method. Results Detection limit of profenofos in soil was 0.04 mg/kg. The recovery rates and relative standard deviation obtained by adding standard assay were 95.2%～98.7% and 3.4%～4.5% respectively. According to the dose recommended, degradation of profenofos sprayed on soil was consistent with one range dynamics. The half-life of profenofos in soil was 1.09 days. At the 13th day after spraying on soil, 98.3% of profenofos had degraded. Conclusion This method was simple, rapid, accurate and precise. The degradation of profenofos was more rapid and profenofos contamination in soil was not a serious problem.

Objective To study the relationship between the changes of actin in cardiac muscle and skeletal myogen cells and postmortem interval in rats.Methods The changes of actin in cardiac muscle and skeletal myogen cells at different postmortem intervals were observed by immunohistochemical S-P method using anti-actin monoclonal antibody (HHF35)and analyzed by IBSA image analysis system.Results Within 54h postmortem,cardiac and skeletal muscles revealed depletion of HHF35 staining.The extent of HHF35-staining depletion increased with extension of postmortem interval.Statistical difference was significant in the data obtained by IBSA image analysis system (F cardiac muscle =588.27,F skeletal muscle =351.25,P

Seventeen physiological,biochemicaland morphological indexes consistently showed that Astragalus polysaccharide was able to enhance the myocardia contractility,attenuate the area of myocardial infarction, and reduce the degree of myocardial damage of myocardially infarcted dog heart in vivo.The mechanism might be related to the inhibition of Na+-Kr+ATPase activity & the anti-free-radical-damage action.

Objective To observe the clinical effect of preoperative acupuncture on hypercoagulability of elderly patients with intertrochanteric fracture. Methods Sixty elderly patients with intertrochanteric fracture were evenly randomized into control group and acupuncture group. Both groups were treated with external application of Shangke Huangshui gauze on the affected region, and additionally the acupuncture group was treated with acupuncture on bilateral acupoints of Huantiao, Xuehai, Liangqiu, Zusanli, Sanyinjiao, and Taichong. The treatment for the two groups was performed from the hospitalized day to the day before operation. The parameters of thrombelastograph (TEG) before and after treatment and the occurrence of deep venous thrombosis (DVT) were observed . Results After treatment, the difference of TEG parameters of the control group was insignificant as compared with those before treatment (P>0.05). In the acupuncture group, the coagulation reaction time(R) and clot formation time(K) were significantly prolonged(P < 0.05) , and coagulation angle alpha (α), maximal amplitude (MA)of thrombus and clot strength (G) were significantly decreased (P <0.05), the effect being superior to that in the control group(P<0.05). Till the day before operation , 3 cases from the control group suffered from DVT, but none of acupuncture group suffered from DVT, the difference between the two groups being insignificant(P > 0.05). Conclusion Preoperative application of acupuncture can improve the hypercoagulability of elderly patients after hip fracture.
