1.Effect of prazosin on MMP-1 and TIMP-1 expression in atherosclerosis plaque of ApoE -/- mice model

Objective To observe the effects of prazosin on matrix metalloproteinase‐1(MMP‐1) and tissue inhibitor of met‐alloproteinase 1 (TIMP‐1) expression in atherosclerosis plaque of ApoE knock‐out(ApoE-/- ) mice model and to explore its anti‐atherosclerotic effect and mechanism .Methods Twenty‐four 8‐week‐old ApoE-/- mice were randomly divided into the normal diet group ,high‐fat diet group and prazosin group ,8 cases in each group .The normal diet group was fed by common fodder ,while the high fat group and prazosin group were fed by high fat diet ;on the basis of the high fat diet ,the prazosin group was started to con‐duct gavage of prazosin hydrochloride 1 mg/kg every day ,while the normal diet group and the high fat diet group were daily ga‐vaged by the same volume of normal saline .The abdominal aortic venous blood after 12 weeks in each group was collected for detec‐ting the blood lipid levels .The aorta arterial blood sample was collected for detecting MMP‐1 and TIMP‐1 expression levels by im‐munohistochemistry .Results Compared with the high fat diet group ,the levels of serum TG ,TC and LDL‐C in the prazosin group were significantly decreased ,and the HDL‐C level was increased ,the differences were statistically significant (P< 0 .01 or P<0 .05);the area of aorta arterial atherosclerotic plaque and intima thickness were significantly increased ,while prazosin could signif‐icantly inhibit the plaque formation and intima hyperplasia ;compared with the normal diet group ,the expression level of MMP‐1 protein in the high fat diet group and prazosin group was significantly increased ,while the TIMP‐1 protein expression level was de‐creased ,moreover the MMP‐1 protein expression level in the prazosin group was lower than that in the high fat diet group ,while the TIMP‐1 protein expression level was higher than that in the high fat diet group ,the differences were statistically significant (P<0 .05) .Conclusion Prazosin can decrease the level of TC and LDL‐C ,increase the HDL‐C level and has certain anti‐atherosclerotic effect ,its mechanism may be related with the decrease of the MMP‐1 level and the increase of the TIMP‐1 level in plaque .
2.The role of intraaortic balloon pumping in off-pump coronary artery bypass grafting in patients with a low ejection fraction

4.Significance of change in thyroid hormone in patients with heart failure

Thyroid hormone levels were fourd to be changed significantly in 93 patients with heart failure. Their T3 and FT3 decreased, while rT3 increased. The severer heart failure, the more evident change in thyroid hormone. The results suggest that the change in thyroid hormones might be an index in reflecting the severity of heart failure.
5.Quantitative evaluation of self-made Bushenpingchan formula combined with madopar for Parkinson disease

BACKGROUND: Levodopa substitutive therapy is essential and effective method for PD for a long time, but it can not delay the course of PD. Side effect is more and more and severe, for taking levodopa long term. It is benefit to patients to postpone the usage of levodopa or decrease the dosage of it.OBJECTIVE: To systematically investigate the curative effect of the selfmade Bushenpingchan formula combined with madopar on pathogenetic condition of PD patients after onset.DESIGN: Randomized controlled observation.SETTING: Senile Disease Research Center, Xuanwu Hospital, Capital University of Medical Sciences.PARTICIPANTS: Seventy patients diagnosed as having PD were recruited from the Xiyuan Hospital of Chinese Academy of Traditional Chinese Medicine between December 1999 and May 2002. Inclusive criteria: The patients must be diagnosed as PD finally; ②be at 1.5-4 grade of modified Hoehn & Yahr measuring scale; ③less than 80 years old; ④The Traditional Chinese Medicine (TCM) diagnosis was deficiency of liver and kidney. Exclusive criteria: ①secondary PD; ②accompanying with other sever central nervous system diseases. ③patients with severe diseases of heart,lung, kidney or multi-viscera-failure; ④mental sickness patients; ⑤drug and alcohol abuse; ⑥severe adverse effect.METHODS: Totally 70 patients were divided into two groups based on the proportion of one to one: There were 35 patients in treatment group containing 22 males and 13 females. The average age was (67.9±16.5)years. There were 35 patients in control group including 25 males and 10females. The average age was (65.5±16.5) years. There was no significant difference in their general condition. Patients in the control group received 62.5-500 mg madopar (250 mg in each pill), 2-4 times a day (to change the dose according to patients' condition). The patients in the treatment group were given with 62.5-500 mg madopar (250 mg in each pill), 2-4times a day orally and 200 mL self-made Bushenpingchan formula (twice a day) in addition, which was composed of 20 g tuber fleeceflower root, 1 g hairy deerhorn, 10 g tall gastrodia tuber, 15 g gambirplant, 12 g Chinese thorowax root, 15 g twolobed officinal magnolia bark, twice a day. The course of medication was 3 month in patients of the two groups. The patients were determined with unified Parkinson disease rating scale (UPDRS) before treatment and at weeks 2, 4, 8 and 12 after treatment, respectively. The symptoms in traditional Chinese medicine after treatment were measured. The dose of madopar before and after medication as well as the adverse effect were observed.MAIN OUTCOME MEASURES: Score of UPDRS before treatment and at weeks 2, 4, 8 and 12 after treatment in patients of the treatment group and control group; the dose of madopar before and after treatment and the adverse effect after treatment.RESULTS: Total effective rate was 69% and 51% in the treatment group and control group, respectively after treatment. There was no significant difference in therapeutic efficacy (x2=0.86, P=0.222 3). ①The score of UPDRS reduced significantly 8 weeks after treatment in the treatment group as compared with that before treatment (t=2.18 ,P < 0.05). ②The daily dosage decreased in the patients of the treatment group after treatment as compared with that before treatment, which had significant difference (t =2.862 8,P < 0.01 ). The daily dosage in the patients of control group had insignificant difference with that before treatment (t=2.320 3,P < 0.01 ). ③There were 2 cases with lightly dry mouth, nausea, dizziness,which could relieve two weeks later in the treatment group; 5 patients with nausea and 2 patients with light dizziness in control group, which could relieve. The blood pressure had no significant change in the two groups before and after treatment.CONCLUSION: The Chinese medicine Bushenpingchan formula combined with western medicine can decrease the UPDRS score in PD patients,can improve the syndromes in Chinese medicine.
6.Learning Curve of Minimally Invasive Direct Coronary Artery Bypass via Left Anterior Small Thoracotomy

Objective To investigate the learning curve of minimally invasive direct coronary artery bypass ( MIDCAB) via left anterior small thoracotomy. Methods We retrospectively reviewed results of a consecutive series of 80 patients underwent MIDCAB via left anterior small thoracotomy performed by a single surgeon.The patients were divided into 4 groups ( group A, B, C, D, n=20 in each group) according to the sequence of the operation.The left internal mammary artery ( LIMA) harvesting time, anastomosis time, total operative time, the drainage volume of the first day postoperation, mechanical ventilation time, hospital stays postoperation, in-hospital morbidity and mortality were compared.The learning curve was assessed by means of regression analysis with logarithmic curve fitting. Results The basic clinical characters were similar in all groups before the operation.The operations were accomplished successfully in all the 80 cases.No intraoperative conversion to sternotomy, death, or perioperative myocardial infarction happened.The LIMA-harvesting time in the group B, C, and D was significantly shorter than that in the group A [(53.7 ±19.2) min, (50.2 ±17.7) min, and (43.2 ±10.3) min vs.(77.0 ±30.0) min, P =0.001, 0.000, and 0.000].There was no difference between group B and C, B and D, C and D in the LIMA-harvesting time (P>0.05).The total operative time in the group B, C, and D was significantly shorter than in the group A [ (128.7 ±21.7) min, (129.0 ±33.3) min, and (112.2 ±14.5) min vs. (165.2 ±41.8) min, all P=0.000], without difference between the group B and C, B and D, C and D (P>0.05).The learning curve models were obtained: LIMA-harvesting time ( min) =113.77 -16.869 × ln ( operation number); total operative time (min)=220.281 -25.276 ×ln (operation number). Conclusion MIDCAB via left anterior small thoracotomy is safe and effective, with a learning curve of approximately 20 cases.

9.Construction and immunoscreening of cDNA library of Armillifer agkistrodontis nymphs

Objective To construct cDNA entry library and cDNA expression library of Armillifer agkistrodontis (A.) nymphs and make a preliminary immunoscreening for the cDNA expression library.Methods The nymphs were collected from the Kunming mice infected experimentally with A.agkistrodontis eggs and the total RNA were extracted from the nymphs using TRIzol Reagent.After purifying the mRNA,the synthesized cDNAs were cloned into the donor vector pDONR222 by BP reaction of Gateway technology and the recombinants were transformed into the DH10B cells by electroporation,the cDNA entry library was obtained.Next,the expression vector pDEST17 was ligated with entry clones by LR reaction,and the recombinants were transformed into the BL21 (DE3) cells.Hence,the cDNA expression library was constructed.Then,the expression library was immunoscreened with the mixed sera of mice infected with A.agkistrodontis,and the insertions of positive clones were sequenced.After that,the open reading frame(ORF) of positive slone sequence,the homology of the screened genes and their encoded proteins were analyzed by Finder and BLAST (basic local alignment search tool) program of National Center of Biotechnology Information(NCBI),and the discovered new genes were submitted into the GenBank.Besides,the physico-chemical properties,secondary structure and B cell epitopes of encoded proteins were also analyzed by bioinformatics software.Results The average titer and total clones of the cDNA entry library were 1.45 × 105 CFU/ml(colony-forming unit,CFU) and 1.74 × 106 CFU,respectively,and the range of fragment length of the inserted cDNA was between 0.2-4.0 kb,with an average of 1.4 kb.The total clones of cDNA expression library were 1.00 × 105 CFU,and the fragment length of the inserted cDNA was between 0.3-2.2 kb,with an average of 1.0 kb.Five positive clones,coded S1,S5,A1,D1 and F1,respectively,were obtained through preliminary immunoscreening.The sequence and homology of the five positive clones were sequenced and analyzed by BLAST program.No significant similarities were found in pentastomida species,which meant that they were all novel genes of A.agkistrodontis.The gene sequences were submitted to GenBank,with the accession number from JQ180451 to JQ180455.Also,results obtained by bioinformatics software showed that the predictive encoding proteins were all potential to be valuable recombinant diagnostic antigens.Conclusions The cDNA library of A.agkistrodontis nymphs is successfully constructed,and five new genes of A.agkistrodontis are discovered.The establishment of cDNA library and the discovery of the new genes will lay a foundation for further studying the gene functions and screening the immunodiagnostic antigens.
