OBJECTIVES: To estimate the prevalence of edentulism according to the socio-demographic variables of Korean adults between 2007 and 2018 and to analyze the trends. METHODS: This study was conducted using raw data from the Korea National Health and Nutrition Examination Survey (KNHANES) from 2007 to 2018. Edentulism was defined as the absence of upper and lower teeth or the retained root of the missing teeth. To obtain the prevalence of edentulism, complex sample frequency analysis and regression analysis were performed according to the socio-demographic variables to represent the national population. The Korean population structure in 2005 was exploited as a standard population to calculate age-standardized edentulism. RESULTS: The prevalence of edentulism in 2016-2018 was 9.7% in the Korean elderly, and the change of age-standardized edentulism steadily declined over time from 2007, 12.8%. In particular, it decreased by approximately 20% in the period between 2016 and 2018 compared to that in 2007 for those in their 80s. The trend of the prevalence according to gender decreased significantly in women. According to the level of education, the greatest decrease was seen in the group with the lowest education, although the prevalence itself was higher than that seen in those who were more educated. CONCLUSIONS The prevalence of edentulism among the older Korean population has decreased over time. However, the concern is on those with lower education and men as these groups are still at higher risk for edentulism.

Many advances, including the development of antibiotics, the advent of diagnostic techniques and the use of nonsurgical drainage have decreased the mortality rate of pyogenic liver abscess. But, early prompt diagnosis and proper treatment is mandatory in gas-forming pyogenic liver abscess because it may run a fulminating course to death. Gas-forming pyogenic liver abscess mostly occurred to old diabetics. Symptoms of duration are shorter and the prognosis is poorer in the gas-forming than in the non-gas-forming liver abscess. We report three cases of gas-forming pyogenic liver abscess which occurred in old diabetic patients. They were successfully managed by parenteral antibiotics and early percutaneous drainage under sonographic guidance. Early and adequate drainage of pus by pigtail catheter may play a crucial role on the treatment of gas-forming liver abscess.

STATEMENT OF PROBLEM: The adjustable dental impression trays were made for being adjusted their width automatically along the width of dental arch. PURPOSE: The purpose of this study was to investigate the best retentive form of adjustable dental impression tray, and so to make it a more satisfactory product. MATERIALS AND METHODS: The eight pairs of adjustable trays were made of ABS(acrylonitrile butadiene styrene) with different distribution of holes and with or without the rim on the border area of them. The experiment was done with the horse-shoe shaped metal plate to pull out the set impression body from the tray, and the tray jig which was made for holding the tray on a lower part of Universal Testing achine(UTM, Zwick Z020, Zwick Co., Germany). After the impression in the tray was allowed to set four minutes, a tensile force was applied at right angles to the tray which had been previously seated on the jig. The force was applied to measure a maximum retentive force by use of a UTM at a constant strain rate of 100mm per minute. A 2-factor analysis of variance (p<.05) was used to determine whether differences existed among distribution of retentive holes and between rim existing and not. RESULTS: 1. The retentive force of the upper and lower resin tray with 2mm holes on the tray border was highest(25.83/24.98kg).(p<.05) 2. As the tray had more retentive holes, it was less retentive. 3. There was no significant difference in the retentive force of the varied hole intervals in the case of distributing all the area.(p>.05) 4. The rimless trays were more retentive generally, than the rimmed trays except 2 case: upper tray group-all area / 2 mm. intervals and lower tray group-margin only / 2 mm. intervals.(p<.05) 5. Most of the adjustable trays were showed higher retentive force than perforated metal tray except the lower group that perforated on the all area at intervals of 2 mm.

PURPOSE: Effectiveness of the performance of bystander cardiopulmonary resuscitation (CPR) has a decisive impact on the outcomes for out-of-hospital cardiac arrest patients who receive it. After initial CPR education, CPR performance for those trained declines over time. Public education in CPR is commonly recommended to be repeated every 2 years. In this study, we evaluated the willingness of those who have been CPR trained to perform CPR as impacted by their training experience. METHODS: The staff of a small firm received 2 hours of primary CPR education. Two years later, their CPR training was repeated in an identical manner. Participants answered a questionnaire regarding their willingness to perform bystander CPR. Those who answered that they wouldn't perform bystander CPR were asked to select their reason. We also inquired about what they viewed as the most difficult phase of CPR performance, and collected their opinions about the overall necessity and appropriate time period for re-education. RESULTS: Those who answered 'definitely yes' to 'willingness to perform to CPR' increased from 36.6% before primary CPR education to 74.2% afterward. But 2 years later and before re-education, only 30.1% answered 'definitely yes' to 'willingness to perform CPR'. Meanwhile, 41.2% and 29.4% answered that 'fear of legal liability' and 'fear of poor knowledge/performance' were the reasons why they would not perform bystander CPR. Ventilation was the most difficult stage in CPR identified by 34% of the participants, and 18.4% answered that chest compression was the most difficult stage in CPR. 93.6% answered that they should receive CPR re-education. CONCLUSION: The willingness to perform bystander CPR declined significantly after 2 years. Therefore routine public re-education for bystander CPR is necessary.

Background: and Purpose Moderate-intensity statin plus ezetimibe versus high-intensity statin alone may provide a greater low-density lipoprotein cholesterol (LDL-C) reduction in patients with recent ischemic stroke. Methods: This randomized, open-label, controlled trial assigned patients with recent ischemic stroke <90 days to rosuvastatin/ezetimibe 10/10 mg once daily (ROS10/EZT10) or to rosuvastatin 20 mg once daily (ROS20). The primary endpoint was LDL-C reduction ≥50% from baseline at 90 days. Key secondary endpoints were LDL-C <70 mg/dL and multiple lipid goal achievement, and composite of major vascular events. Results: Of 584 randomized, 530 were included in the modified intention-to-treat analysis. The baseline LDL-C level was 130.2±34.7 mg/dL in the ROS10/EZT10 group and 131.0±33.9 mg/dL in the ROS20 group. The primary endpoint was achieved in 198 patients (72.5%) in the ROS10/EZT10 group and 148 (57.6%) in the ROS20 group (odds ratio [95% confidence interval], 1.944 [1.352–2.795]; P= 0.0003). LDL-C level <70 mg/dL was achieved in 80.2% and 65.4% in the ROS10/EZT10 and ROS20 groups (P=0.0001). Multiple lipid goal achievement rate was 71.1% and 53.7% in the ROS10/EZT10 and ROS20 groups (P<0.0001). Major vascular events occurred in 1 patient in the ROS10/EZT10 group and 9 in the ROS20 group (P=0.0091). The adverse event rates did not differ between the two groups. Conclusion Moderate-intensity rosuvastatin plus ezetimibe was superior to high-intensity rosuvastatin alone for intensive LDL-C reduction in patients with recent ischemic stroke. With the combination therapy, more than 70% of patients achieved LDL-C reduction ≥50% and 80% had an LDL-C <70 mg/dL at 90 days.

OBJECTIVES: Achalasia is primary esophageal motility disorder characterized by aperistalsis in body and incomplete lower esophageal sphincter(LES) relaxation. The aim of this study was to evaluate the parameters of successful balloon dilatation predicting long term outcome METHODS: 17 patients with primary esophageal achalasia from March 1995 to December 1996 were included in this study. 5 patients were performed re-balloon dilatation due to recurrence of symptoms. We used Rigiflex Achalasia Balloon with diameter of 30 and 35mm and balloon is dilated for 1 minute at a 5 minute interval. Esophageal manometry and scintigraphy before and after 1 month of balloon dilation were compared between groups with good long term results and relapsed RESULTS: 1) After balloon dilatation, LES pressure, basal esophageal pressure and retention rate were significantly decreased 2) After 6 months of dilatation, 12 patients were successfully treated and 5 patients were recurred 3) No significant pre-dilatation parameters were found to predict the recurrence 4) After 1 month of dilatation, patients with recurrence of symptoms showed significant higher LES pressure and retention rate 5) 2 patients showed recovery of the peristasis, and in these patients, amplitude of esophageal body contraction of pre- and post-balloon dilatation were significantly high CONCLUSION: After 1 month of dilatation, LES pressure and retention rate can predict the outcome after balloon dilatation and high amplitude of esophageal body contraction predict recovery of peristalsis and long term good result

Hemophagocytic lymphohistiocytosis (HLH) can develop following strong activation of the immune system and the cardinal symptoms are a prolonged fever, hematological abnormalities, hepatosplenomegaly, and hemophagocytosis. HLH can be classified as primary or secondary HLH, associated with infections, malignancy and autoimmune disease. There is no consensus on the primary treatment regimen in systemic lupus erythematosus (SLE)-associated HLH. We experienced a case of SLE-associated HLH in a previously healthy adult. She was initially treated with intravenous immunoglobulin, cyclosporine, and high-dose steroid, but had a poor clinical response. After intravenous etoposide, the patient stabilized and has been followed for 1 year without reactivation of the HLH or SLE.
