To identify the factors influencing the charges per case of acute appendicitis and normal spontaneous vaginal delivery (NSVD), the personal data-base files and hospital-characteristics-reporting data files of Korea Medical Insurance Corporation were analyzed. One hundred and twenty-nine institutions were selected. The results of this study were as follows: 1. The differences of charges per case with respect to hospital ownership, location, and equipment levels were statistically significant. 2. The results of multiple regression analysis revealed that bed capacity was the most significant variable in both diseases. 3. Ownership was significant variable in acute appendicitis. In NSVD, ownership and hospital equipment level were statistically significant. In conclusion, bed capacity was statistically the most significant variable in the analysis of charges per case. And we thought that the results of this study would influence the policy of the hospital bed supply.

PURPOSE:Recent studies proposed that polyunsaturated fatty acids (PUFA) of the omega6 series (linoleic acid, LA) compared to the omega 3 series (linolenic acid, LNA) may be a potent promoters of adipogenesis during the gestation/lactation period. Increased ratio of LA/LNA may stimulate adipose tissue development during gestation/lactation period and may subsequently lead to chilhood obesity. The purpose of this study is to evaluate fatty acids composition of the breast milk in Korean lactating women and to analyze the relationship between LA/LNA and the body weight gain of the infants. METHODS:Fifty eight healthy postpartum lactating women and their healthy 58 breast feeding infants were studied. At 11.3+/-3.9 days of postpartum, breast milk were taken and analyzed by gas-liquid chromatography with one-step methylation method. RESULTS:The total fat contents in breast milk were 3.2+/-1.3%. The total fat contents in breast milk were not related to maternal BMI or Kaup index of infants. The percentage of PUFA was 24.9+/-6.0%. Among these PUFA, the percentage of LA and LNA was 20.3+/-5.8% and 2.2+/-0.8%, respectively. LA/ LNA ratio was 10.2+/-2.5. There were no significant correlations between LA/LNA ratio and infant weight gain during the first 2 months of life. CONCLUSION: Short term weight gain of infant was not related to the LA/LNA ratio in the breast milk. Further studies on the long term effects on fat percent and body weight of infants would be needed.

PURPOSE: To evaluate ocular findings of patients with acute cerebral infarction by analyzing fluorescein angiography (FAG) and optical coherence tomography (OCT). METHODS: We retrospectively reviewed the medical records of patients with acute cerebral infarction. FAG was used to analyze arm to retina time and arteriovenous (AV) transit time. The peripapillary retinal nerve fiber layer (pRNFL) was analyzed using OCT, and the data were compared with those of patients diagnosed with idiopathic epiretinal membrane (control group). RESULTS: Seventy-three patients were included in the patient group, and 56 participants were in the control group. In 27% of the subjects in the patient group, retinal abnormality was incidentally identified. Atrial fibrillation (p < 0.050) was the only systemic disease with a significantly higher incidence in the patient group. AV transit time (p < 0.050) showed a significant delay in the patient group, but there was no significant difference in the arm to retina time. pRNFL thickness did not significantly differ between the ipsilateral and contralateral locations of brain lesion. In addition, there was no significant difference in the subgroup analysis according to cerebral ischemic territory and no correlation between the severity of symptoms and the findings of ophthalmologic examination. CONCLUSIONS: Patients with acute cerebral infarction show delayed AV transit time in FAG, and about 27% of them have unrecognized retinal abnormalities.

Abrupt increase in the size of cervico-mediastinal tumor due to infection or spontaneous hemorrhage into cyst can induce severe tracheal compression and therefore sudden death. A 5 year old boy, who had a history of URI, had an enlarging cystic hygroma on the right side of the neck and anterior mediastinum. Under diagnosis of the cervico-mediastinal cystic hygroma, surgical removal was scheduled. After induction of anesthesia, intubation was done without any difficulty. A few minutes later, signs of partial airway obstruction were appeared. And within a very short period, total airway occlusion occurred. The tracheal tube was removed and manual ventilation was performed with positive airway pressure, but ineffective. We attempted to puncture cricothyroid membrane with 14 Gauge needle in order to ventilate manually. As soon as we puncture cricothyroid membrane, straw-colored fluid, not air, gushed out through a needle. After aspiration of about 200ml of cystic fluid, the obstructive signs disappeared and the patency of the airway was maintained. Intraoperatively, no more airway problems occured and vital signs were stable. And postoperatively, patient had no specific complications and discharged on the 7th day after operation.

BACKGROUND: Tuffier's line, which connect the two iliac crests, is the often used landmark in determining the level for the needle insertion into lumbar intervertebral space for the spinal and/or epidural blocks. However, the level of Tuffier's line varies depending on the observer. METHODS: Two methods were used to measure the level of Tuffier's line was measured on a simple abdominal X-ray taken in the supine position (study 1). Second, the level of Tuffier's line was also measured on fluroscopy of students in the sitting position (study 2). RESULTS: Overall, the level of Tuffier's line was most frequently observed along the L4-5 intervertebral space and second most frequently observed along the lower one-third of L4. In study 1 using abdominal X-ray, the level of Tuffiers line was most frequently observed along the L4-5 intervertebral space, however, the second most frequently observed level varied depending on the ages of the volunteers. In study 2 using fluoroscopy, the level of Tuffier's line was most frequently observed along the L4-5 intervertebral space and second most frequently observed along the upper one-third of L5. CONCLUSIONS: The level of Tuffier's line was most frequently observed along the L4-5 intervertebral space both in the supine and sitting positions. Further, the level of Tuffier's line went higher with age for women but stayed constant independent of age for men in the supine position.

The authors reviewed 729 Pediatric inpatients in this Hospital during 3 years period from Jan. 1978. To Dec. 1980. The results were obtained as follow: 1. The male to female ratio was 2.2:1. 2. Distrstribution of cases as follows:; Infectious and parasitic diseases 39.5%, diases of the respiratory system 23.87% and diseases of the genitourinary system 10.29%, in turn, listed respecteively in decreasing order of frequency. 3. Major leading causes of hospitalization were gastroenteritis(11.11%) and pneumonia(11.11%) of all patients, bronchitis and bronchiolitis(5.76%) and glomerulonepihritstis(5.35%) in turn, listed respectively in decreasing order of frequency. 4. The highest age group was in the 2years-6years and 6years-12years, and the patients in these age group were occupied 23.87% respectively. 5. Seasonal distribution of patients showed high incidence in fall(Sept, Oct, and Nov.). 6. The distribution of duration of hospitalization as follow: Over 8days 26.75%, in 3days 16.87%, and 24 hours-48 hours 13.58% in order of frequency. 7. The mortality rate was 4.12%. Majou leading causes of deaths were pueumonia, sepsis and drug intoxicationin order of frequency.

PURPOSE: To evaluate the relation between ocular biometry and anthropometric parameters in Korean adults with cataracts METHODS: The preoperative medical data of 150 eyes (150 patients) who underwent cataract surgery from November 2015 to March 2016. Anthropometric parameters include height, weight and body mass index (BMI). Ocular biometry includes axial length, anterior chamber depth, corneal curvature, lens thickness and central corneal thickness. The relation between ocular biometry and anthropometric parameters was evaluated using linear regression analysis. RESULTS: The mean age was 67.44 ± 10.83 years. The mean height, weight and BMI were 157.24 ± 9.16 cm, 60.50 ± 10.18 kg and 24.47 ± 3.59 kg/m2. The mean axial length, anterior chamber depth, corneal curvature, lens thickness and central corneal thickness were 23.51 ± 0.80 mm, 3.23 ± 0.42 mm, 44.49 ± 1.33 diopter, 4.44 ± 0.42 mm and 534.64 ± 31.90 µm. The height showed a significant positive relationship with axial length and anterior chamber depth and a significant negative relationship with average corneal curvature. However, there was no significant relationship with the central corneal thickness or lens thickness. The weight showed significant positive relationship with axial length. BMI was not related to any anthropometric parameters. Age showed a significant positive relationship with average corneal curvature and lens thickness and a negative relationship with height and anterior chamber depth. CONCLUSIONS: As height increases, the axial length and the anterior chamber depth increase and the corneal curvature become flatter. As age increases, the anterior chamber depth becomes shallower, the corneal curvature becomes steeper and the lens thickness increases.

Plantar fibromatosis, otherwise known as Dupuytrens disease of the foot, is a relatively uncommon benign lesion. It is characterized by the replacement of elements of the plantar aponeurosis with fibrous tissue, which then slowly invades the skin and the deep structures. Generally an asymptomatic lesion, plantarfibrosis may occasionally cause foot pain. We recently experienced a case of painful plantar fibromatosis. On which a wide surgical excision was done. A one year postoperatively, follow-up examination revealed neither recurrence nor walking disability.
