1.Three Cases of Bee-sting Granuloma.

Most insects including the honeybee cause erythema and edema at the site of a sting in non-allergic individuals. This reaction usually resolves within a few days. However, in certain cases, the persistence of inciting materials in the cutaneous tissue may promote a local inflammatory foreign body reaction. Localized nodular lesions were seen in our two patients and erythemstous patches were present in one patient due to rarely encountered complications induced by retained sting materials. In these cases histopathology showed multiple foci of foreign body granuloma containing yellow refraetile material in the center surrounded by histiocytes and foreign body giant cells. The granulomatous skin lesions in these patients were treated with intralesional injections of triamcinolone acetonide and partial excisions of the nodular masses were performed.

5.The Effect of Intraluminal Oxygen Insufflation on the Oxygenation of Gut Mucosa in Hemorrhaged Cats.

BACKGROUND: This study was purposed to study the therapeutic value of intraluminal oxygen insufflation on the oxygenation of gut mucosa in a feline model subjected to a hemorrhagic shock followed by reperfusion. METHODS: Eighteen cats were divided into three groups : For group 1, 6 cats were subjected to sham operation as a control group ; for group 2, 6 cats were subjected to a hemorrhagic shock for 2 hours prior to reperfusion as a hemorrhagic shock(HS) group ; for group 3, 6 cats were subjected to a simultaneous insufflation of intraluminal oxygen, continued during the shock and reperfusion periods as a oxygenated hemorrhagic shock(OHS) group. Mesenteric PvO2, mesenteric venous arterial(v-a) lactate difference, mesenteric P(v-a)CO2 and mesenteric pH(a-v) were measured for every 30 minutes during the shock and reperfusion. RESULTS: The lactate(v-a) was increased during the shock. For group OHS, the lactate(v-a) returned to the baseline value after reperfusion, but for group HS, it did not return. The values of pH(v-a) and P(v-a)CO2 were increased during the shock and returned to the baseline value for group OHS, but for group HS, the increase was greater than group OHS(p<0.05), but not returned to the baseline value after reperfusion. For both pH(v-a) and P(v-a)CO2, there were statistically significant differences(p<0.05) between group HS and group OHS during the shock and reperfusion. CONCLUSIONS: We conclude that the insufflation of intraluminal oxygen improves the oxygenation of gut mucosa in the feline model undergone a hemorrhagic shock followed by reperfusion.

7.A Study on Diet Education of Participants in Diabetic Education at Public Health Center.

This study was carried out to see the effect of individualized diet education on the improvement of blood glucose level according to the changes of nutrient intake with the subjects of 25 people. Among 25 people who participated in diabetic education program held in Chilgok-gun public health center, Gyeongbuk, there were 15 diabetic patients(3 males and 12 females) and 10 healthy people(2 males and 8 females) and after the diet education, the results were as follows. As for clinical characteristics, the diabetic group tended to have slightly higher blood pressure than the control group, especially diastolic blood pressure of diabetic group was higher than the control group with a significant level(p<0.05). Blood glucose level measured 2 hours after the meal was higher in the diabetic group than the control group with significant level(p<0.0001). For the clinical characteristic changes before and after the diet education, the blood glucose level in the diabetic group reduced significantly(p<0.01). For the daily diet intake, intake of fat in the diabetic group increased significantly compared to the one before the education(p<0.05). When comparing the ratio of carbohydrate: protein: fat, it was changed from 72.9 : 16.2 : 12.9 before the education to 63.8 : 15.1 : 21.1 after the education. From the above results, it is necessary to perform the diet education which carries out in the health center for diabetic patients with individualized goal by stepwise way rather than as one time try, and by developing the diabetic education media which is appropriate for the patients.

8.Surface roughness and microleakage of class V composite restorations : Effect of surface sealing.

The purpose of this study was to compare the effect of surface sealing materials on microleakage and surface roughness in Class V composite restorations. Twenty five standardized Class V cavity preparations were made on the facial surface of human premolars and were randomly assigned to 5 groups. The teeth were restored with Z-250 after applying Single Bond. Following 7 days storage in distilled water at 37degrees C, the restorations were sealed as following systems : No sealing ; Single Bond Adhesive ; Biscover ; Fortify ; Optiguard. Then, toothbrush abrasion test was conducted using a wear testing machine. Surface roughness was measured by means of profilometer before and after toothbrushing and the results were statistically analysed by using a paired t-test and ANOVA. The bonded interfaces and the changes of surface roughness were examined by SEM. For microleakage test, specimens were stained in a 2% methylene blue solution, then longitudinally sectioned and analyzed for leakage at occlusal and cervical interfaces using stereomicroscope. The results were statistically analysed by using a Kruskal-Wallis and Mann-Whitney U test. Surface roughness was increasing in all groups after toothbrushing, but no statistically significant differences. In SEM observation, surface sealant was partially retained and partially detached in bonded interfaces. Especially, microgap was identified in cervical margins. In microleakage test, there was better seal in the enamel region and a significant difference between groups at occlusal margin. Control group and Single Bond group had significantly better marginal seal at enamel margin than cervical margin.

9.A study of adolescent smoking and drinking in Korea.

PURPOSE: Adolescence is a period of growth and development in body structure and physiologic, psychologic, and social functioning. Smoking and drinking in this period are very harmful and may lead to other forms of substance abuse. We surveyed the actual aspects of smoking and drinking among Korean adolescents and analyzed associated factors. METHODS: A survey was performed among adolescents using a questionnaire about smoking and drinking. Data were collected from students who visited pediatric clinics for routine school examinations from July to November 2007. RESULTS: A total of 2,546 adolescents (smoking 1,512; drinking 1,034) participated in the study. The overall smoking and drinking rates were 29.2% and 48.2%, respectively, and the mean age at the first instance of smoking and drinking was 13.8 and 14.1 years, respectively. Among drinking adolescents, 30.1% were offered a drink by their parents, although most adolescents were encouraged to smoke or drink by their friends. Dissatisfaction with family and school life, economic status, and school performance was associated with a higher smoking and drinking rate (P<0.001). Broken families were associated with a high rate of smoking, but peer satisfaction was not related to smoking or drinking. The frequency and amount of smoking and drinking in adolescents were as high as those in adults. Social sanctions seemed insufficient to keep adolescents from smoking and drinking. CONCLUSION: Adolescent smoking and drinking are highly influenced by family and school environments, so more active social sanctions are required, including parental involvement and legislation preventing adolescents from smoking and drinking.

10.Childhood Granulomatous Periorificial Dermatitis Improved by Dapsone.

Childhood granulomatous periorificial dermatitis is a distinctive granulomatous form of perioral dermatitis, characterized by a monomorphous, small papular eruption around the mouth, nose, and eyes. It occurs primarily in prepubertal children. We present a case of a 12-year-old boy with multiple, asymptomatic, erythematous papules on the perioral, perinasal and periorbital areas for 5 months. Histopathological examination revealed upper dermal and perifollicular granulomatous infiltrate. After using oral dapsone 25 mg daily for 7 weeks, the skin lesions were considerably improved. But, 3 months after discontinuence of dapsone, exacerbation of the skin lesions occurred.
