6.The value of NT-proBNP paired with Cys-C in early diagnosis of the type 1 cardiorenal syndrome

Objective To study the value of plasma amino-terminal pro-B-type natriuretic peptide precursor (NT-proBNP) along with cystatin C (Cys-C) used together in early diagnosis of the type 1 cardiorenal syndrome (CRS) in patients with acute heart failure.Methods From January 2011 to January 2014,the data of 395 acute heart failure patients in Wenzhou People' s Hospital were retrospectively analyzed.Plasma concentrations of NT-proBNP and Cys-C of 395 patients with acute heart failure were measured by using PETIA and ECLIA methods at about 24 hours after admission.And the incidence of type 1 CRS syndrome was analyzed by statistics.ROC analyses were performed to evaluate the early diagnosing value of plasma NT-proBNP along with Cys-C in determining type 1 CRS and in discerning the optimal cut-off point for early diagnosing type 1 CRS patients.Results A total of 395 patients with acute heart failure aged from 29-96 years with (74.3 ± 13.0) years in mean,and 61.3% male were enrolled in this study.Of them,82 patients were suffered from CRS syndrome.Logistic regression analysis showed diabetes,chronic heart failure,Cys-C and NT-proBNP were independent risk factors for predicting type 1 CRS,and serum Cys-C level was the most important predictor.ROC analysis showed the areas under the curve (AUC) of plasma NT-proBNP and Cys-C for early diagnosing CRS were 0.835 (95% CI:0.785-0.886,P < 0.01) and 0.885 (95% CI:0.825-0.944,P <0.01),respectively.The value of plasma NT-proBNP alone in early diagnosing the type 1 CRS was limited with Youden index up to 0.483.When the NT-proBNP paired with Cys-C,the diagnostic reliability has been greatly improved,especially specificity with Youden index up to 0.717.Therefore,NT-proBNP doupled with Cys-C has great reference value in early diagnosing the type 1 CRS.Conclusion Plasma NT-proBNP level coupled with Cys-C level is a valuable biomarker for early diagnosing type 1 cardio-renal syndrome in patients with acute heart failure.If the levels of plasma NT-proBNP and Cys-C are detected simultaneously,the diagnostic reliability of CRS will be greatly improved,especially in the diagnostic specificity.
7.Value of Continuous Renal Replacement Therapy in the Treatment of the Type 1 Cardiorenal Syndrome.

Objective To evaluate the value of Continuous renal replacement therapy ( CRRT) in the treatment of the type 1 cardio-renal syndrome.Methods From June 2008 to August 2014,the type 1 cardiorenal syndrome patients receiving continuous renal replace-ment therapy in Wenzhou People′s Hospital ICU were retrospectively analyzed .Before and after treatment , the mean arterial pressure , central venous pressure, shock index, rapid shallow breathing index, oxygenation index, urine, plasma amino-terminal pro-B-type natriuretic peptide precursor , creatinine and bicarbonate and other clinical data were observed in order to understand CRRT ′s value in type 1 cardiorenal syndrome patients.Results Total 37 patients [aged from 27-88(67.9 ±18.7)years,51.4%male] with type 1 cardiore-nal syndrome were enrolled in this study .Statistics showed that after CRRT treatment , the mean arterial pressure , central venous pres-sure, shock index , rapid shallow breathing index , oxygenation index , urine, plasma amino-terminal pro-B-type natriuretic peptide precursor, creatinine and bicarbonate were significantly improved , and there was a significant difference (P<0.05).Conclusion For type 1 cardiorenal syndrome , especially critical patients with serious diuretic resistance , CRRT has great therapeutic value , even the only treatment option .
8.Effect of breviscapine on serum NSE, Ang-2 and IL-6 levels and its efficacy in patients with acute cerebral infarction

Objective To observe the efficacy of breviscapine in the treatment of patients with acute cerebral infarction and its effect on serum neuron specific enolase(NSE), angiotensin-2(Ang-2) and interleukin-6(IL-6) levels.Methods 60 cases of acute cerebral infarction(ACI) patients from January 2014 to January 2015 in Sichuan Provincial People's Hospital were selected and randomly divided into two groups, 30 cases in each group.All patients were given conventional western medicine treatment , and the observation group were also treated with breviscapine.After 2 weeks, the degree of neural function defect scores and efficacy were compared and the serum levels of NSE , Ang-2 and IL-6 were detected by enzyme linked immunosorbent assay(ELISA) and compared pre-and post-treatment between two groups.Results The degree of neural function defect score post-treatment in observation group was significantly lower than that in control group ( P<0.05 ).The overall response in observation group was 27 cases (90.00%), which was significantly higher than 20 cases(66.67%) in control group(χ2 =4.81,P<0.05).The serum levels of Ang-2, IL-6 and NSE post-treatment in observation group were significantly lower than those in control group ( P<0.05 ) .Conclusion The curative effect of breviscapine in treatment of acute cerebral infarction is significantly, which could improve the cerebral microcirculation, protect the brain tissue, and its mechanism may be through reducing the serum levels of NSE, IL-6 and Ang-2.
9.Relative Analysis of Pseudomonas aeruginosa Strain in Hospital Infection

OBJECTIVE To investigate the relationship of Pseudomonas aeruginosa strain in hospital infection. METHODS ?-Lactamase gene,aminoglycoside modification enzymes(AMEs)gene,and resistant antiseptic(qacE△1-sul1)gene were detected to mutidrug resistant P.aeruginosa with polymerase chain reaction(PCR) and we conducted cluster analysis of the molecule marked drug resistance gene. RESULTS PCR showed the positive ratios of TEM,CARB and VIM genes were 24.32%,43.24%,and 5.4%,respectively,While SHV,CTX-M,OXA-10 group,PER,GES,VEB,DHA,and IMP genes were all negative,the deleted ratio of oprD_2 gene was 86.49%;the positive ratios of aac(6′)-Ⅰ,aac(6′)-Ⅱ,ant(3″)-Ⅰand ant(2″)-Ⅰgenes were 8.11%,43.24%,18.91%,and 40.54%,respectively and the positive ratio of qacE△1 gene was 51.35%,There were clone transmitted phenomenon. CONCLUSIONS Hospital P.aeruginosa infection can induce clone transmitted hospital infection and it has fulminant prevalence.
10.Efficacy and safety of mometasone furoate dry powder inhalation in treating mild and moderate asthma:a Meta-analysis

Objective To evaluate the efficacy and safety of mometasone furoate dry powder inhalation(MF DPI) in treating mild and moderate asthma .Methods The databases of PubMed ,EMBASE ,CINAHL were retrieved .The randomized ,controlled trials(RCT) on mometasone furoate dry powder inhalation in treating mild and moderate asthma were collected .The quality evalua‐tion and the data extraction were performed according to the Cochrane systematic evaluation method .The RevMan 5 .0 .2 software was adopted for conducting statistical analysis .Results A total of 9 RCT involving 1 795 patients with mild and moderate asthma were included .The meta‐analysis results showed that MF DPI 200 mcg/d improved FEV1(MD=0 .24 ,95% CI:0 .17 -0 .30) ,am‐PEF(MD= 25 .25 ,95% CI:8 .18 -42 .32) ,pmPEF(MD= 16 .00 ,95% CI:2 .10 -29 .90);MF DPI 400 mcg/d improved FEV1 (MD=0 .32 ,95% CI:0 .25-0 .39) ,amPEF(MD=36 .44 ,95% CI:23 .82-49 .05) ,pmPEF (MD=28 .50 ,95% CI:14 .11-42 .89) , which suggested that MF DPI 200 mcg/d and MF DPI 400 mcg/d improving FEV1 ,amPEF and pmPEF was higher than the place‐bo ;for reducing patient′s early morning dyspnea and albuterol dose ,MF DPI 200 mcg/d and MF DPI 400 mcg/d were superior to placebo .For reducing patient′s early morning wheezing ,MF DPI 200 mcg/d and MF DPI 400 mcg/d were not superior to placebo . For the occurrence of adverse events ,there was no statistical difference between MF DPI 200 mcg/d and MF DPI 400 mcg/d with placebo(P>0 .05) .Conclusion The existing evidence indicates that MF DPI has higher effect and safety in treating mild and mod‐erate asthma .
